A Study on the Recognition of Home Care Services by nurses, doctors and patients. (일 종합병원 의사, 간호사 및 입원환자의 가정간호사업에 대한 인식 조사)

  • Purpose: To provide the data for improving home health care through investigating the recognition of home care services and the possibility of providing it by nurses, doctors and patients. Method: The subjects were 167 nurses and 71 doctors who were working at a general hospital in Seoul and 72 patients who were admitted to that hospital. Data were collected through questionnaire surveys from November 29 to December 17 in 2004. Result: Home care services were recognized by 70.7% of nurses, 64.8% of doctors and 58.4% of patients. The percentages of agreement regarding the necessity of home care services were 70.7% of nurses, 47.9% of doctors and 86.1% of patients. The percentages of agreement to refer the patients to home care when they only want to receive it, were 58.1% of nurses and 57.7% of doctors. There were significantly higher in nurses than doctors between the differences of possible or impossible percentages in agreements about each items in the test-related services, medication-related services and treatment-related services. Conclusion: The above findings indicated that more specific and continuous educations and advertisements are needed to enlarge home care services in general hospitals.

The Relationship of National University Hospital Inpatient's Perceived Quality, Satisfaction, and Customer Loyalty (국립대학병원 입원환자가 느끼는 의료서비스 질, 만족도, 고객 충성 도간의 관련성 분석)

  • The purpose of this study is to identify the nature of the inpatient service quality of national university hospital, and based on that, to examine the relationship of hospital inpatient's perceived quality, overall satisfaction, customer loyalty(intention of revisiting, intention of oral transmitting). To carry out these objectives, first we analyzed the dimensions of inpatient care service quality using SERVQUAL scale. The SERVQUAL scale is based on the gap theory, that is, the difference of patients' expectations and the actually received medical care service in hospital. On the basis of this theory, we measured the inpatient's perceived service quality, overall patient satisfaction and customer loyalty. Data were collected by self-administered questionnaires at a 809 bed national university hospital. These questionnaires measuring the service quality were distributed to 400 inpatients. The data samples are 347 cases in final. The response rate was 86.8%. Firstly, to categorize inpatient service quality in hospital, the factor analysis was performed on 48 items. The reliability and validity of these items was evaluated. Finally to explore the relationship of service quality, overall satisfaction, and customer loyalty, the multiple regression and logistic regression analysis are used. This study shows firstly, the dimension of inpatient service quality was categorized into 7 dimensions, that is, kindness, medical service, nurse caring, environment, facilities, appropriateness and access. Secondly, the reliability and validity of inpatient service quality items was satisfied. Thirdly, as a result of multiple regression analysis, the effect of inpatient's perceived service quality, especially, nurse caring(P<0.01), environment (P<0.01), facilities, appropriateness and access variables(P<0.05), on overall satisfaction was statistically significant. Lastly, in case of the effect on customer loyalty as a intension of oral transmitting, medical service(P<0.05), environment(P<0.01) and overall satisfaction(P<0.01) are statistically significant. Also, in case of intension of revisiting, medical service, environment, access, and overall satisfaction variables are significant factors. In conclusion, to maintain the satisfaction and customer loyalty on national university hospitals, the efforts to improve the inpatient service quality, especially, environment, medical service, and access factors might be needed.

A Study on the Types of Public Hospitals in the Region by Cluster Analysis (군집분석을 통한 지역거점공공병원의 유형화)

  • This study selected indicators that can represent the characteristics of general hospitals, including local medical centers and Red Cross hospitals, which are representative public health institutions, and analyzed clusters. And we present to benchmark in each cluster. According to the analysis, 276 general hospitals were classified into 13 clusters, and local medical centers and Red Cross hospitals were classified into clusters between 1 and 7 of the total 13 clusters because of their small size. Local medical centers and Red Cross hospitals, selected as excellent hospitals in each cluster, showed significant differences in management performance despite similar regional environment and medical performance, and among them, surgical consultation and internal medical care rates, inpatient and outpatient rates. In order for local medical centers and Red Cross hospitals to play their role as secondary acute hospitals in the region, inpatient care services and surgical functions must be activated.

Differences in Social and Clinical Characteristics between Readmission and Dehospitalization in Long-Term Inpatients with Schizophrenia (장기 재원 조현병 환자에서 재입원 혹은 탈원에 따른 사회적 및 임상적 특징 차이)

  • Objectives: Dehospitalization of long-term inpatients with schizophrenia can be difficult because of complex factors such as chronic symptoms and low family and social support. The aim of this study was to identify factors related to dehospitalization and readmission of long-term inpatients with schizophrenia. Methods: This retrospective study reviewed the medical records of patients discharged from a psychiatric hospital in Yongin, South Korea, from February 1, 2016, to July 31, 2017. Patients who were hospitalized for over 3 months were divided into two groups: readmission (n=47) and dehospitalization (n=55). Differences in sociodemographic and clinical factors were analyzed between the two groups. Results: Regarding sociodemographic characteristics, female sex, familylessness, discharge to nursing homes, and discharge after symptom improvement were more prevalent in the dehospitalization group, whereas male sex, having a sibling as next of kin, and discharge because of other problems were more prevalent in the readmission group. Among clinical characteristics, hospital stay was longer in the readmission group. Conclusion: In this study, patients without a family showed a tendency to not be readmitted when they were discharged to nursing homes after symptom improvement. Expansion of social welfare support may encourage dehospitalization of long-term inpatients with schizophrenia.

  • This study aims to further strengthen the medical expertise to offer specialized medical care specialization strategies to gain a competitive edge through the customer segmentation of the Regional Public Hospital. Investigation period was selected to study the inpatients 26,658 people January to December 2013. The method of analysis are Cluster analysis and Decision Tree Analysis. In conclusion, female, age over 60, and diseases in musculoskeletal system and connective tissue were commonly selected as identifiers of the target market of Regional Public Hospital. Customers in this target market are loyal to specialized medical service and keeping continuous relationship with these customers through communication and monitoring of results of provided medical service would be important because the effect of word of mouth propagated to other group of customers having equivalent scale of consumption is expected. And the concentration of the scope of medical service of Regional Public Hospital and the collaboration and mutual reliance of medical service under the strategic alliance with other institutions and private hospitals are also needed.

Subjective Attitudes towards Terminal Patients of Nursing Students with Clinical Practice Experience: Application of Q Methodology (임상실습을 경험한 간호대학생의 말기환자에 대한 주관적 태도: Q방법론 적용)

  • Purpose: This study was conducted to identify subjective attitudes towards terminal patients in nursing students who had clinical practice. The types of subjective attitude were classified by applying Q methodology. According to those types, basic reference data for the development of educational programs were provided. Methods: Thirty-four final Q samples were selected, and Q classification with a nine-point scale was performed with P samples of 43 nursing students. A key factor analysis was conducted with the collected data using the PC QUANAL program. Results: Nursing students' attitudes towards terminal patients were grouped into three types. The total variable was 49.96%. Students with Type 1 ("wish for life-sustaining medical treatment") thought that terminal patients accurately understood their medical condition and wanted to prolong their lives. Others with Type 2 ("need for service and support") believed that a multidisciplinary nursing system needs to be established to help terminal patients prepare for death. Students with Type 3 ("awareness and acceptance of death") thought that terminal patients wanted to die with dignity at a hospice unit. Conclusion: This study analyzed various types of attitude towards terminal patients, as perceived by nursing students with clinical training experience. Development of educational programs for each attitude type analyzed in this study could contribute to systematic training programs for nursing students caring for terminal patients.

The needs of nursing home services for frail elderly people receiving home care nursing services (가정간호서비스 이용 노인환자의 간호요양원(nursing home) 필요 예측에 관한 연구)

  • The purpose of this study was to estimate the population requiring nursing home services for frail elders who received home care nursing services. This study identified the need of nursing home services and the proportion of elders over 60 years of age requiring nursing home services according to the admission eligibility including items with intensity of home care nursing services and the criteria for screening of patients requiring home care nursing services. In this study, survey research design was conducted. A total of 49 home care nurses were collected. They were asked to assess the need of nursing home services based on items with intensity of home care nursing services and the criteria for screening of patients requiring home care nursing services. Using secondary data analysis from the survey on 'The Evaluation of Home Care Nursing Service Program in 1994 in Korea', the proportion of elders requiring nursing home services was estimated. The estimated numbers and proportion of frail elders requiring nursing home services were 169 (86.2%) aged persons among community dwelling elders based on admission eligibility. However, the proportion of aged persons was 68% using the only criteria for screening of patients requiring for screening home care nursing services.

Influences of Satisfaction of Mental Disease Patient's Protector about Medical Service on Utilizing Hospital (정신과환자의 입원진료서비스에 대한 보호자의 만족도가 병원재이용에 미치는 영향)

  • This study was conducted to understand the influences or satisfaction of mental disease patient's carer about medical service on utilizing hospital, for 202 mental disease patients who are in 19 hospitals from March 2, to 31, 2000 with questionnaire. Factor for understanding satisfaction of patient's carer includes ability of medical professional, view of facility, kindness of steps, convenience of medical procedure, cleanness of environment and convenience of traffic. The data collected were analyzed by SPSS program and the results of this study are summarized as follows; 1. There are not significantly difference bet wren general character and whole satisfaction of patient's carer. 2. Each factor has the positive relation with whole satisfaction of patient's carer about medical service. 3. Whole satisfaction of patient's carer have the significant positive relation with utilizing hospital($\beta$=.527, p=.000), this means that the higher whole satisfaction of patient's carer, the higher their tendance of utilizing hospital. 4. Knowledge for mental disease of patient's carer appear the positive results in view of medical care. In conclusion, the policy has to be developed in order to prevent the stop of utilizing hospital of patient and their carer. But there is a limit to except mental disease patients in this study.

Evaluation of Pharmacists Medication Teaching Service for Hospitalized Cancer Patients (입원한 환자에 대한 약사의 복약지도 서비스 평가)

  • The purpose of this study was to evaluate the patient education provided by the pharmacist for cancer patients receiving chemotherapy in the hospital. One time patient medication teaching including verbal instruction and written materials were provided by a pharmacist for cancer patients receiving chemotherapy on the first or second day of hospitalization. After providing medication teaching by a pharmacist a written survey was performed in order to measure the patient's satisfaction with the medication teaching and to evaluate the effectiveness of the patient medication teaching. This one-time patient medication teaching by a pharmacist was provided for 44 solid and hematological cancer patients (23 male, 21 female). The results of 27 written surveys completed by the cancer patients revealed that almost all cancer patients $(96.3\%)$ felt that medication teaching is a must in order to understand and accept the chemotherapy by cancer patients. In addition, almost all patients $(92.6\%)$ stated that they were extremely satisfied with the medication teaching provided by the pharmacist. The levels of understandings on the chemotherapy.

The impact of admission decided by patients or their family on the length of stay and medical expenses of the elderly (노인 환자 및 보호자에 의한 입원서비스 결정 여부가 재원일수와 진료비에 미치는 영향)

  • Purposes: This study aims to investigate the changes in the length of stay and medical expenses by the impact of the admission decided by patients or their family. Methodology: The analysis was conducted using data from elder patients aged 65 or older who have used admission services more than once from year 6(2012) to year 12(2017) of the Korean Medical Panel Data(KOWEPS). The statistical analysis was performed through the Chi-square test, mean difference analysis, and generalized estimation equation analysis. Findings: Compared to the patients who used admission services decided by doctors, those who used admission services decided by patients or their family had a longer length of stay and a lower daily medical expense. Practical Implications: Inappropriate admission decided by patients or their family can increase the risk of hospital-acquired infections, impairing the health of elderly patients, and has a negative impact on the efficiency of health resources. Therefore, it is necessary to prevent and properly manage inappropriate admission decided by patients or their family
