산모 집중치료실에 입원한 고위험 임부의 건강관리 요구: 혼합적 연구방법 적용 (Health-care Needs of High-risk Pregnant Women Hospitalized in Maternal-Fetal Intensive Care Units: A Mixed-methods Design)

  • Purpose: To identify the characteristics and health-care needs of high-risk pregnant women in maternal-fetal intensive care units (MFICU). Methods: A mixed-methods design was adopted. Data were collected from 78 high-risk pregnant women admitted to the MFICU. Qualitative data included ten participants' experiences with hospitalization and childbirth, which were analyzed using mixed content analysis. Quantitative data were analyzed using at-test and one-way ANOVA testing. Results: The average score for pregnancy and childbirth health-care needs was 3.54 points. Average score by area was before-admission health care (3.70), health care of baby (3.67), health of childbirth (3.61), postpartum health (3.51), and pregnancy health care during hospitalization (3.48). Qualitative results showed diverse feelings and experiences of high-risk pregnant women and their need for health care, which was expressed in three themes and 11 sub-themes. Conclusion: Nurses should recognize high-risk mothers' feelings and needs for pregnancy and childbirth-focused health care to help patients accept their vulnerability and cope positively.

산욕기 산모의 임신시 정서상태, 분만준비도, 산후 사회적 지지도 및 산후 우울감과의 관계연구 (A Study on the Relationship among Prenatal Emotional Status, Preparation for Delivery, Postpartum Social Support and Postpartum Blues)

  • Purpose: This study was to examine the degree of prenatal emotional status, preparation for delivery, postpartum social support, and postpartum blues and to investigate the relationship of the above variables in postpartum women. Method: The subjects were 131 postpartum women. Selection criteria were women who were 2 to 8 weeks post delivery. After Informed consent was obtained, they were asked to fill out a self-administered questionnaire. The instrument was a Postnatal Depression & Anxiety check list. Result: The mean age of the subjects was 29.3 years, and 63.4% of subjects delivered their children by vaginal birth. 18.3 percent had felt depressed during their pregnancy. The mean score of the postpartum blues was 19.2. 61.8 percent of the subjects had postpartum blues and 8.4 percent experienced postpartum depression. Monthly income was negatively correlated to postpartum blues (r=-.189, p<.05), but the emotional status during pregnancy had a significant and positive relationship. However, preparation for delivery had no significant correlation with postpartum blues. While husband support and social support had a tendency to have a negative correlation. Conclusion: Further research is needed to identify the factors that affect the emotional status during pregnancy with various scales. Also, intervention programs to increase emotional support for pregnancy and husband support are needed.

예비 신혼부부의 수태 전 건강관리 인식수준과 생식 건강증진 행위에 관한 연구 (Study on Awareness of Preconception Care and Reproductive Health Behaviors in Pre-honeymooners)

  • Purpose: The purpose of this study was 1) to examine levels of awareness on preconception care and pregnancy, and reproductive health promoting behavior among pre-honeymooners; and 2) to explore the relationship between awareness levels of preconception care and pregnancy, and reproductive health promoting behavior. Methods: This study was a correlation design with a total of 134 participants (67 couples of pre-honeymooners). Data were collected with questionnaire Results: Mean score of self-perception of awareness of preconception care and pregnancy was 2.82 out of 4. Content awareness level was 6.83 out of 13, and reproductive health promoting behavior score was 3.02 out of 4, indicating low to mid-level. Women reported higher scores in all variables than those in men. Self-perception awareness and content awareness of care and pregnancy had a weak positive correlation (r=.18, p=.038). Reproductive health promoting behavior was positively related to self-perception awareness of care and pregnancy (r=.33, p<.001) and content awareness of care and pregnancy (r=.23, p=.008). Conclusion: Based on this results, a variety of nursing intervention strategies may need to promote reproductive health behavior such as raising awareness of the preconception care and pregnancy.

임부의 식이섭취 수준과 산전우울의 관련성 (Associations of Dietary Intake Levels with Ante-natal Depression in Pregnant Women)

  • Purpose: This study was conducted to identify the association of dietary intake levels with ante-natal depression among pregnant women. Methods: Secondary data analysis was done using Can Pro software (version 3.0) to evaluate the diet intake levels with 24 hour recall diary method with 130 pregnant women visiting a general hospital. Statistical analysis included descriptive statistics, t-test, Mann-Whitney U test and adjusted logistic regression using the SPSS/WIN program. Results: Whole dietary intake levels of pregnant women were insufficient according to Recommended Nutrient Intake (RI) with the except of phosphorus intake. There were significant differences in RI by gestation period and high risk pregnancies. Non-depressed women showed more intake in niacin (z=1.33, p=.018) and zinc (t=3.99, p=.048) than depressed women did. Niacin was a significant determinant of ante-natal depression (Exp (B)=5.88, p=.47, 95% CI [1.02~23.83]). Conclusion: Dietary intake assessment would be necessary during pregnancies for the screening of ante-natal depression. For ante-natal depression care, a tailored dietary intervention should be applied for women with insufficient nutrient intake.

고위험 임부 배우자의 스트레스와 대처양상 (Stress and Coping Style of High-risk Pregnant Women's Spouses)

  • Purpose: This study determined the level of stress and type of coping style of spouses with high-risk pregnant women. Methods: Subjects were 102 spouses with high-risk pregnant women at 6 hospitals in Seoul and Gyeonggi province from January to August, 2009. The tools for this study were stress scale and coping scale. The data were analyzed by t-test, ANOVA, Scheffe's test and Pearson's correlation coefficient. Results: The average score of subjects stress was 2.18 and coping score was 2.46 point out of 4.00. The highest score of stress categories was 2.44 point in emotional problems and the lowest score was 1.72 in communication and support resources. The stress level was significant differences according to length of marriage, number of hospital visits, satisfaction with marriage relationship, hospitalization days and gestational age respectively. Spouses tended to use an active coping style (2.60) rather than a passive coping style (2.31). There were significant differences according to mother's age and gestational age in active coping and educational level in passive coping. Conclusion: Considering the level of stress coping style and characteristics of high-risk pregnant women's spouses, a nursing intervention should be provided to help them alleviate stress and to encourage active coping.

고령 초산모와 비고령 초산모의 임신영양지식, 식습관 및 신생아 건강상태 비교 (Comparing Nutrition Knowledge, Eating Habits, and Neonatal Health Status of Primipara for Pregnant Women of Advanced Maternal Age Compared to Those of Younger Ages)

  • Purpose: The aim of this study was to examine differences in nutrition knowledge, eating habits during pregnancy, and neonatal health status between primipara for pregnant women of advanced maternal age in comparison to those under the age of 35. Methods: This study used a comparative survey design. Data were collected through self-report questionnaires and patients medical records. A total of 127 participants, mothers after delivery were recruited from metropolitan city B. Results: Primipara in advanced maternal age (n=32) reported significantly higher scores of eating habits (Z=-2.96, p=.003) than younger ages (n=95). There were no significant differences in scores of pregnancy nutrition knowledge (Z=-0.44, p=.660), duration of gestation (Z=-0.28, p=.778), neonatal birth height (Z=-0.10, p=.924), neonatal birth weight (Z=-0.28, p=.777), Apgar score 1 minute (Z=-0.53, p=.599) and 5 minutes (Z=-0.23, p=.816) between two groups. Conclusion: It concludes that age is not the obstacle to the best nutritional status of women and their newborns.

임부 부부의 성태도와 부부친밀도가 성만족도에 미치는 영향: 자기효과와 상대방효과 (Impact of Sexual Attitude and Marital Intimacy on Sexual Satisfaction in Pregnant Couples: An Application of the Actor-Partner Interdependence Model)

  • Purpose: The purpose of this study was to investigate actor and partner effects of sexual attitude and marital intimacy on sexual satisfaction in pregnant couples. Methods: Data were collected from 176 pairs of the pregnant couples visiting for prenatal care at hospitals from June 18 to September 24, 2016. The collected data were analyzed by paired t-test and Pearson's correlation coefficients using SPSS 18.0 and interdependent effect (Actor-Partner Interdependence Model analysis) through AMOS 18.0. Results: The sexual attitude and marital intimacy of the pregnant woman did not have a partner effect on the sexual satisfaction of her husband, respectively (${\beta}=.12$, p=.141), (${\beta}=.01$, p=.938). The sexual attitude of the husband had a partner effect on the sexual satisfaction of the pregnant woman (${\beta}=.13$, p=.021), but the marital intimacy of the husband did not show a partner effect (${\beta}=.07$, p=.202). Conclusion: Study suggests that the sexual attitude and marital intimacy of pregnant couples should be considered as factors when developing an intervention to improve sexual satisfaction in couples. Moreover, pregnant couples should participate in intervention together because the sexual satisfaction has conceptual view of interdependence in two-person relationships.

독신모의 임신 경험: 벼랑 끝으로 내몰림 (Experiences of Single Pregnant Mothers)

  • Purpose: This study was done to assess the personal experiences of the coping process during pregnancy for single mothers. Methods: The participants were 17 single mothers who had stayed in a social welfare facility. Data was collected with an in-depth unstructured interview. Data analysis was done by the grounded theory method. Results: One-hundred twelve concepts and 49 sub-categories were confirmed in the analysis. The sub-categories were grouped into 19 categories; escape from a miserable family, wrong meeting, openness of sex, defenseless state of pregnancy, inevitable result of pregnancy, heartbreak by herself, closure, isolation, difficult situation of being alone, stigma, supporting & protecting, helplessness, seeking, empowering, feeling of loss, conflict, facing issues, assuring a fresh start and becoming-mature. "Being driven over the edge of a cliff" was the key phenomenon which the single mothers experienced during the process of pregnancy. Conclusion: The above results will help nurses assessing single pregnancy mothers' needs and developing a nursing intervention program for supporting them. Therefore, nurses will be able to stop them from "being driven over the edge of cliff". A more vigorous nursing intervention is suggested for the research of the vulnerable classes of medical health care including single pregnant mothers.

수태 전 건강관리에 대한 젠더 간 인식 차이 (Gender Differences in Awareness of Preconception Care and Pregnancy)

  • Purpose: This study was conducted to identify awareness levels in regard to preconception care and gender differences targeting single undergraduate students in their 20s. Methods: The study participants were 867 single undergraduate students aged 20~29 (523 women, 344 men). This was a descriptive research, in which awareness levels of preconception care and pregnancy were identified the through questionnaire surveys. Results: Gender differences in the awareness of reproductive care before conception were found. Generally, women's awareness of issues related to preconception care and pregnancy was higher than that of men. Women recognized risk factors in pregnancy ($x^2$=22.85, p<.001) and positive behaviors ($x^2$=10.91, p=.012) better than men. Women's awareness of preconception care was significantly higher than that of men (t=8.37, p<.001). The awareness levels of the students who completed a reproduction related class (t=3.16, p=.002) and the students who knew about folic acid (t=-10.78, p<.001) in preconception care were statistically significantly higher. Conclusion: The results indicate that the ways to educate and alert students to major risk factors before pregnancy and other reproductive care content before conception is to provide education both single women and single men.

산전우울의 영향요인 (Influencing Factors on Antenatal Depression)

  • Purpose: This study examined the influencing factors on antenatal depression among pregnant women. Methods: This was a cross sectional descriptive study with 255 pregnant women who visited a general hospital in a metropolitan city for their regularly scheduled check-up. Measurement tools employed were the Korean version of Beck Depression Inventory (BDI), the food habits, and the Pittsburg Sleep Quality Index (PSQI). Socio-demographic variables and the status of high risk pregnancy were identified. Influencing factors on antenatal depression were identified using a stepwise multiple regression analysis. Results: The mean score of antenatal depression was $7.2{\pm}5.0$; 18.4% with mild depression, 5.9% had moderate depression, with 0.8% identified with severe depression on BDI scale. Influencing factors on antenatal depression accounted for 47.8% of the total variance which consisted of quality of sleep, marital satisfaction, food habits, gestation periods, sexual satisfaction, high risk pregnancy, and age. Conclusion: Findings show that antenatal depression should be monitored on a regular basis during early pregnancy and in high risk pregnancy if possible, and quality of sleep and food habits should be incorporated in the management of antenatal depression.
