Splenic Irradiation in Chronic Myeologenous Leukemia (만성골수성백혈병 15예의 비장조사)

  • Radiation therapy was the treatment of choice for CML in the past, in the form of SI or radioactive phosphorus. Its use has been replaced to a large extent by various chemotherapeutic agents. Recently SI in CML has been used, both to relieve painful splenomegaly and to take advantage of an indirect effect of SI on unirradiated bone marrow. We have treated 15 CML cases who had a huge spleen during chemotherapy or even after chemotherapy by 6 MV linear accelerator during the past two years at the Division of Radiation Therapy, Kang Nam St. Mary's Hospital, Catholic College. Response to SI has been rated according to the scoring system of Roger W. Byhardt, et al. which evaluated the splenic and hematologic response as well as the response of disease-related systems. According to this scoring system, most patients demonstrated a significant relief of splenomegaly along with improvement of hemogram. And we observed the change of Karnofsky Performance Status after SI, and survival after a confirmative diagnosis and SI.

Synergistic Anti-Tumor Effect by the Combination of Cyclophosphamide and Dendritic Cell Vaccination in Murine Tumor Model that CEA Expressing (CEA 발현 마우스 종양모델에서 Cyclophosphamide와 수지상세포 백신의 병합치료에 의한 상승적인 항종양 효과)

  • Carcinoembryonic antigen (CEA) is an oncofetal antigen primarily detected in the peripheral blood of cancer patients, particularly in those with colorectal cancer. CEA is considered a valuable target for antigen-specific immunotherapy. In this study, we induced the anti-tumor immunity for CEA through the administration of a dendritic cell (DC) vaccine. However, there was a limitation in inducing tumor regression in the DC vaccinated mice. To enhance the efficacy of anti-tumor immunity in MC38/CEA2 tumor-bearing mice, we evaluated the effects of DC vaccine in combination with cyclophosphamide (CYP). Administration of CYP 100 mg/kg in mice resulted in significant inhibition of tumor growth in the 2-day tumor model, whereas a lower inhibition of tumor growth was seen in the 10-day tumor model. Therefore, the 10-day tumor model was selected for testing chemo-immunotherapy. The combined CYP and DC vaccine not only increased tumor antigen-specific immune responses but also induced synergistic anti-tumor immunity. Furthermore, the adverse effects of CYP such as weight loss and immunosuppression by regulatory T cells and myeloid-derived suppressor cells showed a significant reduction in the combined chemo-immunotherapy treatment compared with CYP alone. Our data suggest that chemoimmunotherapy with the DC vaccine may offer a new therapeutic strategy to induce a potent anti-tumor effect and reduce the adverse effects of chemotherapy.

  • 약물 투여 후 약효의 출현은 흡수, 분포 등의 약동학적 단계(pharmacokinetic phase)를 거쳐 수용체 부위에서 약물과 수용체 사이의 약력학적(pharmacodynamic) 상호작용에 의해 나타난다. 따라서 약물요법 시 약효나 독성 발현의 큰 개인차는 약동학적 또는 약력학적 개인차에 의해 나타나며, 많은 약물에서 약효의 개인차는 흔히 약동학적 차이가 주역할을 함이 알려져 있고, 약물의 특성에 따라 약물의 대사 및 배설과 관련 있는 신장, 간장 및 심장 질환자에서는 현저한 약동학적 변화로 용법 조정이 필수적으로 요구되는 경우가 많다. 약동학적 개인차는 안전역이 좁은 약물에서 더욱 문제가 되며 이러한 약물의 적정 요법을 위해서는 수용체 부위와 평형을 이루고 있는 혈장약물농도를 적정 유지하는 것이 약동학적 개체차를 배제할 수 있는 한 방안이다. 근자에 이르러 체액내 약물 및 이의 대사물을 측정하는 분석 화학의 발전과, 임상 약물 동태학(clinical pharmacokinetics)의 도입 등으로 개개 환자에서의 적정 약물요법을 위한 TDM (Therapeutic Drug Monitoring)이 구미에서는 이미 보편화되어 있다 또한 국내에서도 이러한 새로운 의료분야의 연구 및 임상응용의 필요성이 점차 증가하고 있으므로, 이러한 시대적 요구에 발맞추어 본 지면을 통하여 임상약리학적 지식을 이용한 혈장 농도 데이터의 분석을 통하여 개개인에서의 적정 약물요법을 제시하는 TDM에 관하여 소개하고자 한다.

  • This study was performed to estimate the clinical signs and changes of biochemical parameters in rats with hepatic injury induced by the administration of 2, 2'-azobis(2- amidinopropane)dihydrochloride (AAPH) . Minor behavioral change, brittleness of skin hair and decreased volume of water and feed intake were observed in ra% 2 hours after AAPH administration compared to control group. Concentration of serum albumin showed lower than that of control group. However, concentration of total bilirubin has shown higher than that of control group. As time goes on, . serum LDH activity increased significantly compared to control group, but senun CPK activity did not change compared to control group. Passive hemanglutination of $\alpha $-fetoprotein was negative in all the treaDent groups and control group.

  • The purpose of this study is to find terms that can give identity to the major and occupation of clinical laboratory technologist (also known as medical technologist). The term clinical pathology includes all branches of pathology, namely anatomical pathology, chemical pathology, hematology, microbiology, and all respective subspecialties. Unfortunately, several countries exclude anatomical pathology from the term clinical pathology, a problem that gets compounded when the title is translated into languages other than English. Clinical pathology (US, UK) is a medical specialty. Similar terms are laboratory medicine (Germany, Poland), medical/clinical biology (France, Netherlands) or clinical analysis (Spain). Depending on the person questioned, medical technology is defined slightly differently by individuals, companies, and institutions. The definition also depends on the language in which the question is asked. Medical technology can be translated to define clinical laboratory technology, allied health sciences, medical equipment, biomedical engineering, and health technology. The terms 'clinical pathology technology and pathological technology' are not used in allied health sciences. The names of 'medical technology·medical technologist' can be replaced by 'biomedical laboratory science·biomedical laboratory technologist' or 'clinical laboratory analysis·clinical laboratory analyst'. In this study, it is proposed to change the name of academic and occupation to 'medical biology·medical biology technologist' that combines the term biomedical.

  • An adaptive seamless Phase II/III clinical trial design enables a reduction in the sample size (in comparison to a conventional design) that also shortens the clinical development time. It is also very effective in clinical trials since it can have higher statistical power than Phase III alone. In this study, we use extensive simulation studies to compare several multiple hypothesis testing methods that can help select the best doses in a Phase II study along with several methods to combine p-values of the Phase II and Phase III study.

Juvenile Nephropathy in a Jindo Dog (진도견에서 발생한 소아 신증)

  • An 1-year-old intact male Jindo dog weighing 20 kg was referred with 7-day history of vomiting, anorexia, and lethargy. The dog was diagnosed with chronic kidney disease (CKD) based on clinical signs, urinalysis, serum biochemistry, radiology and abdominal ultrasonography. Ten days of conservative therapy was given, but there were no signs of improvement. The patient was euthanized and necropsy was conducted. Renal histopathology was consistent with juvenile nephropathy and this is the first report of juvenile nephropathy in a Jindo dog.

  • This study aims to analyze chemotherapy medication safety management system and formulate efficient methods to solve problems in the medication safety practices through application of 6 sigma techniques. From the results of analysis conducted, targeting nurses, such 3 factors as process-related factor, nurse-related factor and environment-related factor were identified as problems of the chemotherapy medication safety management. Through analyzing the prior knowledge about chemotherapy mediation safety and level of performance of nurses, the educational performance to complement deficiencies were selected as the final improvement plan, and the improvement activities were completed through drawing out management plans that specify management methods and countermeasures in the event of problems.
