The Efficiency of Objective Structured Clinical Examination (OSCE) as a Method of Clinical Clerkship in the Korean Ophthalmology & Otolaryngology Department (한방안이비인후과에서 임상실습 한 방법으로서 Objective Structured Clinical Examination (OSCE)의 효용성)

  • Objectives : 본 연구를 통하여 한방안이비인후과 임상실습에서 Objective Structured Clinical Examination (OSCE) 수업의 효용성을 알아보고자 한다. Methods : 2014학년도 한의과대학 본과 4학년 학생을 대상으로 안이비인후과 임상실습의 한 방법으로 OSCE를 시행한 후, 학생들은 OSCE 수업 만족도에 관한 설문지를 작성하고 레지던트는 학생들을 평가하였다. OSCE 평가 성적과 기타 임상실습 및 필기시험 성적과의 연관성을 분석하여 OSCE 성적의 학점 부여 타당도를 살펴보았다. Results : 만족도 관련 설문 결과 총 평균점수가 4.34점이 나왔다. OSCE성적과 기타 임상실습 및 필기시험 성적과의 상관성을 pearson correlation을 통하여 살펴본 결과, OSCE성적은 임상실습 성적 및 (r=0.210, p=0.056), 필기시험 성적과 통계적으로 유의한 상관성이 없었다 (r=0.064, p=0.567). Conclusions : 위의 결과로부터 임상실습에서 OSCE 시행은 학생들의 학습 만족도를 높인다는 것을 알 수 있었다. 임상실습에서 효율적으로 OSCE를 활용하는 방안을 모색하기 위해 더 많은 연구가 필요할 것으로 생각된다.

The Assessment of Reporting Interventions in Randomized Controlled Trials of Acupuncture according to the STRICTA Recommendation (STRICTA 권고안에 따른 침 효과에 대한 무작위 대조군 임상 시험 논문의 개업 보고에 대한 평가)

  • 목 적 : Pubmed에서 검색된 침의 효과에 대한 무작위 대조군 임상시험논문에서 STRICTA 권고안의 반영률을 알아보고자 본 연구를 계획하였다 . 방 법 : Pubmed에서 2003 년 7 월 1 일부터 2004 년 6 월 30 일까지 발간된 침 무작위 대조군 임상시험 논문을 검색 한 후 수작업으로 침의 효과에 대해 무작위 대조군 임상시험을 실시한 36 개의 논문을 채택하였다 .대상논문들의 저널, 대상 질병에 대해 조사하였으며 Jadad scale 접수를 매기고 STRICT A 권고안의 반영률을 ‘STRICTA 접수’를 매겨서 알아보았다. 논문이 SCI, SCIE, 무등재된 저널 중 어디에 실려 있는가에 따라 ANOVA 를 사용하여 Jadad 점 수 및 STRICTA 점수에 차이가 있는지 알아보았으며 단순상관 분석을 통해 STIRCT A 점수와 Jadad scale 접수의 관계를 살펴보았다. 결 과 : Jadad scale 접수의 평균은 2.6점 이었으며 STRICTA 접수의 평균은 11.0점 이었다. 36편의 침 무작위 임상시험논문에서 다음의 6가지 항목 - 문현적 근거, 자침 깊이, 함께 처치한 내용, 시술자 배경, 참가자의 맹검여부. 대조군 선택을 정당화 할 수 있는 근거-의 기술율은 50% 미만이었다. SCI, SOE, 무둥재된 것에 따라 논문들의 Jadad 접수 평균은 통계적으로 유의한 차이를 보였으나 STRICTA 점수 평균에서는 유의한 차이를 보이지 않았다. 한편 STRICTA 점수와 Jadad scale 점수 간에는 아무런 관계가 없었다. 결 론 :36 편의 침 임상시험 논문은 STRICTA 권고안의 많은 부분을 받아들이고 있었으며 STRICTA 권고안을 받아들이는 정도는 Jadad 점수와는상관이 없었다.

  • Technological advances in contemporary medicine has discovered the causes of countless diseases and recorded a noticeable medical performance. As technology develops, the role of hospitals is expanding to include disease prevention of inpatients, on top of their fundamental role of treating diseases. Recently, hospitals are becoming more influential as they create environments to provide comfort and stability to patients. In this regard, contemporary hospitals are increasingly shifting their focus to create a patient-centric environment as well as develop into humanistic establishments. The same goes to dentists, as well. Since inpatients often have fear and frustration over treatment, hospitals should figure out the environmental factors that are more effective and relaxing for patients and design medical services to provide them. The patients' movement and spaces during their treatment were categorized by stages and collected for gazing information using eye tracking. It analyzed users' gaze information according to Heatmap analysis of distribution and frequency and was determined the presence or absence of stimuli on the components of space. This research is an advanced research to study and enhance treatment environment based on the analysis of patients' gazes. It attempted to create an opportunity to get closer to patient-centric environment by understanding the stimulants and obstacles and controlling the background settings.

Proposals on Basic Data Based on Comparison of Changes in Clinical Laboratory Technologists' National Examination and Job Definition: Focused on Korea, Japan, and Taiwan (임상병리사국가시험 및 직무의 변천 비교를 중심으로 한 기초자료 제안: 한국, 일본, 대만을 중심으로)

  • This study examined the transition process of clinical laboratory technologists' national examination and job definition in Korea and compared the differences in the national examination between Korea and neighboring countries, such as Japan and Taiwan. In Korea, the number of questions made for it was 200 (1965), 200 (1977), 300 (1982), 250 (1992), 330 (2006), and 280 (2015). The practice of clinical physiology is important for real-time monitoring, given the characteristics of physiological testing. On the other hand, there are conflicts between other occupations in the working area. Clinical molecular biology needs to be established as a new major subject considering the diagnostic importance of molecular biological tests and the speed of science and technology development. Clinical laboratory operations provide policy and guidance recommendations to technologist staff. The proposed clinical laboratory technologists' national examination comprises major subjects: clinical biochemistry, clinical hematology, clinical transfusionology, clinical immunology, clinical microbiology, clinical molecular biology, clinical histology, clinical cytology, clinical physiology, and clinical laboratory operations. In addition, this study proposes the job definition of clinical laboratory technologists, revising various chemical or physiological testing to biomedical or physiological testing required for medical practice.

Effects of Work Environment, Job Satisfaction, and Organizational Commitment on Turnover Intention of Clinical Research Coordinator Nurses (임상시험코디네이터 간호사의 근무환경, 직무만족, 조직몰입이 이직의도에 미치는 영향)

  • This was a descriptive study aiming to verify the effects of work environment, job satisfaction, and organizational commitment on the turnover intention of Clinical Research Coordinator(CRC) nurses. In the results of this study, the organizational commitment had significant effects on the turnover intention of CRC nurses. On the other hand, the work environment and job satisfaction had no statistically significant effects on the turnover intention of CRC nurses. Based on the results of this study, the main discussions were drawn as follows. In order to increase the organizational commitment, it would be necessary to have the institutional improvement like the increase of opportunity to participate in the hospital operation, administration, and the establishment of affiliated institution for improving a sense of belonging. It would be also necessary for each hospital to establish the standard of welfare, wage system for the improvement of work environment. If the organizational commitment of CRC nurses who are the actual performers, supporters of researches is ignored, it is unavoidable that the performance and quality of clinical tests would be declined. Therefore, there should be some changes, so that CRC nurses could play a pivotal role in the whole process of clinical test, by changing the employment status from contract position to more stable employment status, to increase their commitment to organization and work continuity.

  • Phase I clinical trials (Dose Finding Studies) are the first step in administering new drugs developed through animal experiments or in vitro experiments to humans. An important area of interest in designing Phase I clinical trials is determining the dose that provides the greatest efficacy and acceptable safe dose to the patient. In this paper, we propose a method to determine the maximum tolerated dose considering efficacy and safety using Biased coin design and stopping rule. The proposed method is compared with existing methods through simulation.
