3-methyl-crotonyl-CoA carboxylase deficiency 환자의 임상경과와 분자유전학적 특성

  • MCG는 3-methylcrotonyl-CoA의 결핍으로 발생하는 선천성 leucine 대사 장애이다. 무증상에서 간질지속증 등의 다양한 임상양상을 보이며 주로 신생아대사이상선별검사에서 의심되어, 소변 유기산 검사를 통해 3- hydroxyisovaleric acid의 증가의 소견이 보인다. 치료는 leucine 제한 식이와 L-carnitine의 복용 등의 식이요법이 있다. 서울아산병원에서 MCG로 확진된 9가계 11명의 환자를 대상으로 임상상과 분자 유전학적 특성을 조사하였다. 9명은 신생아 대사이상검사로 발견되었으며, 나머지 2명은 가족검사를 통해서 진단되었다. 총 2-10세($2.6{\pm}1.96$년)까지의 관찰 기간 동안 모든 환자는 정상 발달을 보였으며, 신경학적 이상이나 대사불균형의 이상소견은 보이지 않았다. 총 18개의 대립유전자 중 17개에서 돌연변이를 발견하였으며, p.D280Y 돌연변이가 66.7%의 대립유전자에서 확인되어, 한국인 MCG에서 흔한 돌연변이임을 알 수 있었다. 또한 p.S342K, p.P459S, p.552S, p.Q496H, p.T556A 등 대부분의 돌연변이가 이전에 보고된 바가 없던 돌연변이로 한국인 MCG환자의 유전학적 특성이 다른 민족과 다름을 시사한다.

  • 임상용 모니터의 출력값 중 가장 큰 영향을 주는 것이 휘도이다. 최근 컬러 모니터의 임상용 사용으로 인해 출력값의 보정은 매우 중요하게 관리되고 있다. 색보정 변위의 특성화가 휘도와 동일한 인자로 영향을 주는지 분석하기 위해 사용된 특성화 보정 모델은 GOG모델과 S-curve모델 이다. 두 개의 모델을 통해 모니터의 휘도는 장치의 출력과 직접적인 관계가 있다는 것을 증명하고 동일한 출력에 입력값을 가질 수 있도록 구성하고자 한다. 동일한 출력을 디스플레이 할 경우 동일한 출력값을 얻어야 한다. 그러나 모니터에 따라 서로 다른 출력값을 산출한다. 컬러모니터의 경우는 출력의 안정성에 휘도뿐만 아니라, 색감(R,G,B) 변화가 큰 역할을 수행한다. 본 연구에서는 분석인자 중 대조도에 영향을 주는 인자를 역추적하여 특성화 보정 S-curve모델을 LCD에 적용하였다. 이는 GOG모델을 적용한 CRT와 동일한 결과를 얻었다. 따라서 인자는 CIEXYZ의 결과값과 동일한 인자를 구할 수 있다.

A Study on Changes of Confidence in Core Basic Nursing Skill by Each Period of Clinical Practicum of Nursing Students (간호대학생의 임상실습 시기별 핵심기본간호술 수행자신감의 변화양상)

  • The purpose of this longitudinal study was to investigate confidence in core basic nursing skill each period of clinical practicum and discover the changes, depending on characteristics of periods. The data was collected for 94 nursing students attending a university located in J-do from Mar. 9 to Dec. 16 2016 for 4 times in total. For data analysis, the frequency analysis, descriptive statistics and paired t-test were used. The research findings showed that confidence in core basic nursing skill by each period of clinical practicum was lowest before clinical practicum in general but gradually increased depending on lapse of time and experience of clinical practicum. However, it decreased in all items after having first clinical practicum in the $2^{nd}$ semester, compared to after first clinical practicum in the $1^{st}$ semester. In addition, the difference of confidence in core basic nursing skill between clinical practicum periods was higher after the first clinical practicum in the $1^{st}$ semester and after clinical practicum at the end of school year, compared to before clinical practicum and after the first clinical practicum in the $2^{nd}$ semester respectively. Therefore, this study suggested that it's necessary to have efficient and systematic clinical practicum education strategy, reflecting the characteristics of periods in clinical practicum in order to improve the confidence in core basic nursing skill of nursing students.

Estimation of Growing Stock and Carbon Stock based on Components of Forest Type Map: The case of Kangwon Province (임상도 특성에 따른 임목축적 및 탄소저장량 추정: 강원도를 중심으로)

  • This research aimed to provide a method to estimate growing stock and carbon stock using the characteristics of forest type map such as the age-class, DBH class and crown density class. We transformed the growing stock data of national forest inventory (mainly Kangwon-do province) onto those of time when the forest type map was established. We developed a simulation model for the growing stock using the transformed data and the characteristics of forest type map by the quantification method I. By comparing partial correlation coefficient, we found that quantification of growing stock was largely affected by age-class followed by crown density class, forest type and DBH class. The growing stock, was estimated as minimum in the broadleaved forest with age-class II, DBH class 'Small', and crown density class 'Low' as $20.0m^3/ha$, whereas showed maximum value in the coniferous forest with age-class VI, DBH class 'Large', and crown density class 'High' as $305.0m^3/ha$. The growing stock for coniferous, broadleaved, and mixed forest were estimated as $30.5{\sim}305.0m^3/ha$, $20.0{\sim}200.4m^3/ha$, and $23.8{\sim}238.1m^3/ha$, respectively. When we compared the carbon stock by forest type, the carbon stock by age class based on growing stock was maximum when DBH class was 'Large' and crown density class was 'High' regardless of forest type. This estimation of growing stock by using characteristic of forest type can be used to estimate the changes in growing stock and carbon stock resulting from deforestation or natural disaster. In addition, we hope it provide a useful advice when forest officials and policy makers have to make decisions in regard to forest management.

분할영상의 계층적 구조를 이용한 주제도 갱신방법

  • 임상도는 항공사진을 판독하여 얻어진 산림에 관한 정보를 지형도(1/25,000)에 도화 작성한 도면으로 전국 산림조사와 연계하여 10년을 주기로 순환제작 되며, 현재 제 4차 수치임상도가 제작 중에 있다 임상도는 여러 산림관련 주제도 중 가장 많이 활용되는 도면으로 산림 분야뿐만 아니라 다른 분야에서도 널리 활용되고 있다. 그러나 10년을 주기로 제작되므로 부분적으로 현실과 부합하지 않는 내용이 포함되어 있어 각종 계획수립 및 활용에 장애요인으로 작용하고 있다. 따라서 실제 임상정보를 획득할 수 있도록 지속적인 갱신이 필요하다. 그러나 임상도의 부분적 갱신을 위하여 별도의 항공사진을 촬영하는 것은 현실적인 어려움이 있으며, 최근 고해상도 위성영상이 활용 가능하게 됨에 따라 임상도의 갱신에도 활용될 수 있을 것으로 기대되고 있다. 본 연구에서는 고해상도 위성영상인 IKONOS를 이용하여 수치임상도를 갱신하는 방법을 제시하였다. 연구대상지는 제 4차 임상도의 수치화가 완료된 전라북도 완주 지역으로 1:25000 지형도의 도엽명 대아와 읍내의 일부지역이다. 영상자료는 2001년 8월 18일에 촬영된 IKONOS Multispectral 자료를 이용하였다. 영상의 기하보정을 위하여 RPC Model과 1:25000 수치지형도로부터 만들어진 DEM을 사용하였다. 기하보정된 영상을 이용하여 영상분할(Segmentation)을 실시하여 서로 중복되지 않는 동질한 지역으로 구분하였다. 이때 기존의 수치임상도를 Super-Object로 사용하여 영상을 분할할 때 형성될 수 있는 가장 큰 Segment로 제한하였으며 Super-Object의 경계를 벗어나지 않는 보다 작은 Sub-Object를 만들도록 하여 분할영상의 계층적 구조를 형성하였다. 어느 한 임상내에서 변화가 발생하면 변화가 발생한 지역은 변화가 발생하지 않은 지역과 서로 다른 분광특성을 나타내므로 별도의 Segment를 형성하게 된다. 따라서 임상도의 경계선으로부터 획득된 Super-Object의 분광반사 값과 그 안에서 형성된 Sub-Object의 분광반사값의 차이를 이용하여 임상도의 갱신을 위한 변화지역을 탐지하였다.

Study on Meta-cognition, Self-efficiency, Clinical competence of nursing students (간호대학생의 메타인지, 자기효능감, 임상수행능력의 융복합적 영향)

  • This study examined the influence of meta-cognition, self-efficiency and clinical competence in nursing students. A convenience sample of 153 subjects were recruited from fourth year nursing students, and the survey was conducted from September 1st to 30th 2015. The data were collected by questionnaires and analyzed SPSS/Win 18.0 program, using descriptive statistics, Pearson's correlation coefficient, t-test, one-way ANOVA and hierarchical regression. Significant positive correlation among the meta-cognition, self-efficiency and clinical competence were found. The regression model explained 27% of the clinical competence. The meta-cognition and self-efficiency for the nursing students were factors influencing clinical competence. Therefor, it is necessary develop an educational program to improve the meta-cognition and self-efficiency of nursing students.

  • The purpose of this study was to identify experiences of core basic nursing skills(CBNS), self-confidence during clinical practice and clinical competence in 208 nursing students attending nursing department and understand which factor, either experiences of CBNS or self-confidence during clinical practice has an impact on clinical competence. Our research findings are as follows. There was a positive correlation between experiences of CBNS, self-confidence during clinical practice and clinical competence. The factor that had the biggest impact on clinical competence was self-confidence during clinical practice and the explanation power of both factors was 31.2%, based on multiple regression analysis. Based on these findings, nursing educate need to develop a curriculum to increase self-confidence during clinical practice in students.

  • The purpose of this study was to investigate the satisfaction of clinical practice for university students in the physical therapy department. 163 students participated in the study. The subjects completed an online survey that consisted of 36 clinical practice satisfaction questions and 5 questions of general characteristics. The collected data was analyzed by using frequency analysis, independent t-test, one-way ANOVA and Scheffe post hoc analyses with a significant level of 0.05. The students' satisfaction of clinical practice was 3.84 in average on a scale of 5. The students' satisfaction was the highest for the item 'Satisfaction after clinical practice', and the lowest for the item 'Internal conflict in clinical practice' and 'Evaluation of clinical practice'. The students' satisfaction of clinical practice showed significant differences according to the satisfaction of the major (p<.001), however, no significant difference according to gender, grade, training period and practice institution.

  • In order to examine the characteristics having influence on the satisfaction with clinical practice for the effective clinical practice, this study analyzed the satisfaction with clinical practice in accordance with the characteristics of major practice conditions, targeting 228 students of Dental Hygienics with experience in clinical practice in Daegu and Gyeongbuk region. In case of 4-year major practical conditions, high satisfaction with major, when students could choose the hospital for practice, and when there were lots of dental hygienists in the hospital for practice, the satisfaction with clinical practice was high(p<.05). In case of 4-year course, high satisfaction with major, and when there were lots of dental hygienists in the hospital for practice, the satisfaction with clinical practice evaluation was high(p<.05). In case of 4-year major practical conditions, high satisfaction with major, dental hospital, when students could choose the hospital for practice, and when there were lots of dental hygienists in the hospital for practice, the satisfaction with interpersonal relation was high(p<.05). In case of 4-year course, high satisfaction with major, and when students and school could choose the hospital for practice, the satisfaction after practice was high(p<.05).

The Factors Influence of Clinical Competency of Core Basic Nursing Skills During the Clinical Practice in Nursing Students (간호대학생의 임상실습 중 핵심기본간호술 수행자신감에 영향을 미치는 요인)

  • This study was a descriptive study conducted to identify factors that had an influence on clinical competency of core basic nursing skills(CBNS) during the clinical practice in nursing students. Data were collected from 3rd and 4th grade 190 nursing students in G city using self-report questionnaire, and analyzed using descriptive statistics, the t-test, one way ANOVA, multiple regression with SPSS 23.0 program. This study found that the number of experience CBNS below 5 was 51.1%, and the number of observation CBNS over 11 was 78.4%. Clinical competency according to general characteristics, clinical practice satisfaction and the level of CBNS experience increased with male, increase age and grade, and major subject satisfaction and clinical practice satisfaction, especially, as the number of experience CBNS increased, clinical competency increased(p<0.001). But clinical competency did not differ according to the number of observation CBNS(p=0.463). The factors affecting the clinical competency of nursing students were age, grade, clinical practice satisfaction and the number of experience CBNS. Therefore, it is important to increase the opportunity to directly experience the various CBNS in clinical practice of nursing students in the nursing education curriculum, and it is necessary to find ways to actively use simulation education.
