  • This study is a methodological study that explains the procedure for verifying effectiveness in developing practical guidelines for cognitive programs suitable for dementia patients. Based on the development of evidence-based new clinical practice guidelines, a preliminary recommendation for the domestic dementia patient care guidelines was developed. The practical guidelines consisted of the final four types, and the content validity score of the configuration items was 0.87 to 1 point. In the sub-categories of field applicability, appropriateness score was 3.95 to 4.34 points, applicability score was 3.57 to 4.27 points, and predicted effect score was 3.84 to 4.22 points. Through the examination of the content validity and field applicability of experts, it was confirmed that the practical guidelines developed in this study can be used as the basis for establishing an intervention plan for dementia cognitive program managers engaged in clinical practice. In future studies should further facilitate the development of evidence-based treatment guidelines to select appropriate treatment activities for dementia patients.

  • Since 1990, descriptive studies about the concept of client advocacy have been published in the health journals. However, empirical studies of the concept are still lacking. There are not enough bases to apply the concept of client advocacy in nursing practice. The purpose of this paper is to encourage empirical research about client advocacy by clarification of the concept and to discusses application of the theory in child healthcare. The literature was reviewed that empirical studies on client advocacy conducted after the year 2000. The changing trends in the client advocacy concept was examined; it was changed from the philosophical concept to nursing action and changed from individual advocacy to collective advocacy, with the rise of the self advocacy concept. The research trends on client advocacy evolves from descriptive study to quasi-experimental study and instrument development study, with diversification and expansion of research methods, advocates, clients and settings. The advocacy role of the nurse in child healthcare is significant due to the child's lack of self determination ability. In ill child care, the application of individual advocacy is potent, while in healthy child care, collective advocacy, policy advocacy, and social advocacy is effective.

Data were collected through questionnaires administered to 170 nurses working in a general hospital in Gwangju city and Jeollanam-do province from March 24 to May 27, 2021. There were significant differences in clinical competence according to age, marital status, and educational level. Clinical competence was significantly correlated with nursing professionalism (r=.600, p<.001), critical-thinking disposition (r=.568, p<.001), communication skills (r=.658, p<.001), and nursing practice environment (r=.353, p<.001). Factors influencing clinical competence were communication skills, nursing professionalism, critical-thinking disposition, and age, which explained 54.0% of the total variance. It is necessary to develop educational programs to improve the communication skills, nursing professionalism, and critical-thinking disposition of nurses.
