  • 한국간호 I. 간호행정제도 및 법규의 변천 1. 간호행정기구 한국의 의료행정은 현대의학의 도입과 더불어 1894년 6월 내부에 위생국을 둔다는 규정이 발표됨으로서 시작되었고 1901년 관제개편에 따라 위생국에 보건과와 의무과가 설치되었다. 그 후 1945년 12월 보건후생부 간호사업국내에 보건간호과 기관간호과 조산 간호과가 설치되었고 각 도에는 간호 사업계가 설치되었다. 1948년부터는 보건사회부 의정국내의 조산간호과가 간호 사업과로 개편되었다. 그후 1970년 간호사업과가 간호사업담당관제로 개편되었으며 1981년부터는 간호사업담당관제도 폐지되어 보건국의 가족보건과나 의정국 지역의료과에서 한 명의 간호원이 참여하는 정도로 간호행정기구가 점차 축소되었다. 2. 간호법규 1)면허 1962년 개정된 의료법에 의하여 간호원의 면허는 문교부장관이 인가한 간호교육기관 졸업자중 간호원 국가시험 합격자로 규정하였다. 조산원의 경우는 1914년 처음으로 조산원 면허등록을 시작하였으며 현재는 간호원 면허를 받은 자로서 보건사회부장관이 지정하는 교육병원에서 1년의 정규 교육과정을 필한자로 그 자격을 명시하고 있다. 2) 자격시험 자격시험은 1916년부터 실시되었으며 1920년 이후는 각 도에서 관할 실시하였다. 그 후 1962년부터는 보건사회부장관의 주도하에 국가시험을 시행하였으며 현재는 국립 보건원에서 간호원을 포함한 의료업자의 국가 시험을 주관하고 있다. 3)간호수가 간호원 조산원에 관한 간호수가 관계 규칙은 1911년에 발표된 것으로 간호원의 경우 출장 시에 출장비와 간호료를 환자가 지불하도록 하는 것이었다. 1925년 5월까지는 각 지역별로 간호수가에 차이가 있었으나 동년 6월부터 수가규정이 전국적으로 통일되었다. 그 후 1953년부터는 국공립병원 간호원들에게도 다른 일반 공직자와 같이 직급을 보함으로서 간호직에 대한 보수가 통일되었으며 1971년부터는 간호직 수당이 제정되었다. II. 간호사업의 분야별 발전 1. 임상간호제도의 발전 1)초기의 임상간호 한국에 서양의학을 기초로 설립된 최초의 병원은 1885년 의사 Allen에 의한 왕립병원이다. 그 후 정부에 의하여 1894년 군부병원이 설립되었고, 1899년 내부병원이 1904년에 적십자 병원이 설립되었다. 당시에 이루어진 현대간호는 일본인 간호원들에 의해 전해진 일본식 간호와 선교사 간호원들에 의해 전해진 서구식 간호방식이 있었는데 이 두 간호방법은 문화적 배경이나 사회적 인습에 의한 많은 차이점을 볼 수 있었다. 2) 일제하의 임상간호 이 당시에 이루어진 일본식 간호방법을 보면 간호원들의 주업무가 환자를 위한 간호보다도 의사 보조에 더 치중한 것을 볼 수 있다. 한편 선교계 병원에서는 입원환자에 대해서는 간호원들이 전인간호를 실시하였으며 병원당국과 의사들의 협조로 많은 간호사업의 발전을 가져올수 있었다. 3) 광복 이후의 임상간호 6.25 동란 후 한국에는 병원이 계속 늘어나 현재 20Bed 이상의 전국의 병원수가 431개소이고 이중 80Bed 이상의 종합병원이 148개소나 된다. 각 병원의 간호사업은 간호사업과 또는 간호사업부의 행정체제로 운영되고 있으며 최근에는 간호과정(Nursing Process)의 개념을 도입하여 문제중심 간호기록인 POMR(Problem Oriented Medical Record) 방법을 시도하고 있다. 또한 면허간호원은 매년 중앙회에서 실시하는 보수교육을 10시간 이상 받고 있다.

Study on Clinical Patient-oriented Education System for Medical Organizations (의료기관에서의 환자 중심 교육 시스템 연구)

  • Recent healthcare studies evaluate the effective impacts of a patient educational information intervention in hospitals on adherence to communicating knowledge, life quality, and therapeutic response in patients treated with highly active therapy in medical organizations. Therefore, the aim of this study is to design and implement the effective and systematic frameworks by providing clinical patient-oriented education system(CPES), which enhances health promotion and medical cures. CPES initiatives accelerate positive financial returns with network-driven educational system. To validate the CPES, the author analyzes the efficiency and effectiveness using statistical survey works. With this outcome, it is compared with previous system. One of the findings is that the system can improve compliance with treatment right dietetics and give patients self-manage their conditions in S hospital. Patients' acknowledge of their condition and its treatment process can be improved through the educational systems.

Analysis of Survey of Interdisciplinary Unification of Biomedical Laboratory Science (임상병리학과 학제단일화에 대한 설문조사 분석)

  • Opinions regarding interdisciplinary unification of medical laboratory science were analyzed by an online questionnaire distributed to 255 professors and 4,000 hospital practitioners in January of 2018. The recovery rate was 79 (30.9%) for professors and 1,368 (34.2%) for hospital practitioners. In the perception survey on the duality of interdisciplinary, both the professors and the hospital practitioners reported that they felt interdisciplinary integration is necessary. The prerequisite of four-year integration was the establishment of institute of accreditation and standardization of curriculum, and the requirements for the four-year integration were considered important for adjustment of student quota and standardization of curriculum. In the four-year integrated approach, the opinion that only universities that passed the accreditation evaluation should operate a four-year system was highest among professors and hospital practitioners. The optimum capacity of university students was less than 40 in cases of 4-year integration. In conclusion, the above results suggest that the professors and hospital practitioners realize 4-year interdisciplinary integration is necessary to produce a competent medical technologist, and the institute of accreditation, standardization of curriculum, and student quota adjustment should be presupposed for 4-year integration.

The Role of Perfusionists during the COVID-19 Pandemic and Clinical Laboratory Technologists: Comparison of Training Systems in Japan and the United States (코로나19를 통해서 바라본 체외순환사의 역할과 임상병리사: 일본과 미국의 양성체계를 비교)

  • In 2022, Korea reported a total of 224 perfusionists, employing nurses or clinical laboratory technologists (also known as medical technologists) to provide hospital self-education or reliable education. In 2021, the total number of perfusionists in Japan was 2,100, mainly supported by clinical engineering technologists. During the same period, the number of perfusionists reported in the United States was 4,212, who had received training in the master's program, post-bachelor certificate program, and bachelor's program. Most personnel in the USA were graduates of healthcare sciences or life sciences. Perfusionists must be knowledgeable in heart anatomy, physiology, pharmacology, pathology, hemodynamics, laboratory analysis, and quality assurance, as well as techniques to operate the cardiopulmonary bypass machine (heart-lung bypass machine). These are jobs similarly handled by clinical laboratory technologists. The importance of perfusionists became more prominent during two major crises: the MERS-CoV and COVID-19 pandemic. Currently, perfusionists play a significant role in the rapidly expanding field of extracorporeal membrane oxygenation (ECMO) and extracorporeal circulation during cardiac surgery. Results of the current study indicate that hospitals offering cardiac surgery and infectious disease hospitalization need to be institutionalized to secure a certain number of qualified perfusionists. In the future, we look forward to establishing a perfusion technology association under the Korean Society for Clinical Laboratory Physiology to provide academic exchanges.

Validation of the Effectiveness of Education for Obtaining Consent in Clinical Study (임상연구 동의서 교육 시행의 효용성 검증)

  • Purpose: To validate the effectiveness of obtaining consent education on errors in the consent process and to develop the education program for researchers. Methods: From February 2019 to February 2022, a 30-minute, 1:1 face-to-face consent education developed using the ADDIE model was conducted on 78 nurses as principal investigators. An informed consent audit tool, which includes 6 items developed by Asan Medical Center Human Research Protection Center, was used to analyze errors in obtaining informed consent process. Data analysis was performed using the SPSS ver. 25.0, and the Mann-Whitney U-test and χ2-test were utilized to verify the difference in errors between the experimental and control groups. Results: The participants consisted of 42 in the experimental group and 36 in the control group, with no statistically significant difference between the 2 groups. Both 2 groups showed the highest frequency of documentation errors, followed by format errors, errors related to a suitability of investigator, participant, or participant's legally acceptable representative, witness and confidentiality issues. After education, there was a significant decrease in both format errors (p=0.002) and documentation errors (p<0.001) in the experimental group. The proportion of participants without any errors in all items was higher in the experimental group (35.7%) compared to the control group (5.6%), and this difference was statistically significant (p=0.001). Conclusion: The obtaining consent education program was found to be effective in reducing informed consent errors. This study emphasizes the importance of education, suggesting the need for its expansion and accessibility, as well as the necessity for all researchers conducting clinical studies to receive the obtaining consent education.

  • Quality control in medical is getting very important issue so that the importance of CDS(Clinical Decision Support) System has been increased. Local clinics as well as big hospitals are required to implement the CDS System. But the cost and complexity of CDS system implementation is so high since many different activities including knowledge authoring, software development, and integrating the legacy system are necessary. In this paper, we suggest the CDS system architecture to be sharable and interoperable and evaluate the availability and efficiency of this architecture.

The Necessity for End-of-Life Care Education: A Preliminary Analysis with Interns at Two University Hospitals (임종돌봄에 대한 교육의 필요성: 2개 대학병원의 인턴을 대상으로 예비 분석)

  • Purpose: This study was performed to explore the current state of end-of-life (EoL) care education provided to new interns at two university hospitals. Methods: A questionnaire was given to incoming interns (N=64). The levels of acquired knowledge and experience of clinical observation were measured. Seven areas for self-assessment questions were identified and used to analyze the interns' attitudes towards EoL-related education and practice. Results: On average, participants learned five elements (nine in total) from EoL-related classes and two (seven in total) from clinical observation. The most frequently educated element was how to deliver bad news (96.9%) in the classroom setting and how to control physical symptoms (56.5%) in clinical observation. Less than 20% received training on EoL care communication, including discussion of advanced directives. Compared with participants who had no EoL training, those who had EoL training showed positive attitudes in all seven categories regarding overall satisfaction, interest and preparedness in relation to EoL-care classes and practice. Conclusion: Although interns are responsible for caring of dying patients, their EoL training in classroom and clinical settings was very insufficient. Further research should be conducted to establish an education system that provides sufficient knowledge and training on EoL care.

A study on the Participation Motivation of Clinical Nurses in Job Training (임상간호사의 융복합적 직무교육 참여동기에 관한 연구)

  • This study aimed to identify nurses' participation motivation, its influence factors, and their job training need. A survey was conducted on 345 nurses of general hospitals in Gyeonggi-do. Data was collected from October 5th to 18th, 2016 and was analyzed through t-test, ANOVA, Bonferroni post-test, and multiple regressions using SPSS 21.0. Participation motivation was high in 'expertise capability improvement and development' and low in personal gain and job stability. Job training need was the highest in nosocomial infection management and CPR and was the lowest in hospice and rehabilitation nursing. Participation motivation had significant differences depending on age, marital status, educational level, and clinical experience, and was influenced by the job training need of professional nursing and medical knowledge for disease treatment. Therefore, it is necessary to plan medical educational programs to enhance job training effectiveness, establish a strategy to increase participation motivation; and expand various job training support.

  • Purpose: The purpose of this study was to identify role through job analysis (duties and tasks) of nurses who work as clinical nurses with education and clinical nurse with patient coordination within the hospital. Methods: The DACUM committee was organized with 8 nurses each, clinical nurses with education and clinical nurses with coordination. The committee derived the duties and tasks of the two groups of clinical nurses from their actual work in the hospital through a DACUM workshop. Validity of the derived duties and tasks was tested by 12 nurses at each of 3 hospital. Results: For clinical nurse with education, 8 duties and 45 tasks were identified, which included patient education and consultation, patient advocacy, management of the therapeutic process, direct nursing care, activities work directed at improvement, management of health promotion events, administration work, and self-improvement. For clinical nurse with coordination, 10 duties and 54 tasks were identified, and included client consultation, client education, direct nursing care, coordination of therapeutic flow, employee education, public relations with the community, planning and operation of meetings, administration work, work directed at improvement, and self-improvement. Conclusion: The results of this study contribute to not only the work of the participants but also basic data for human resource management in the hospital.

Factors Influencing on Clinical Decision Making Ability of Nurses in Long-term Care Hospitals (요양병원 간호사의 임상의사결정능력에 미치는 영향요인)

  • The purpose of this study was to identify factors influencing the clinical decision making ability of nurses in long-term care hospitals. Data were collected from 174 nurses working in long-term care hospitals in G-do from October 11 to December 10, 2021. The data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation and stepwise multiple regression through the SPSS 23.0. As a result of the study, the clinical decision making ability was positively correlated with critical thinking disposition(r=.494, p<.001) and nursing work environment(r=.451, p<.001). Critical thinking disposition and nursing work environment explained 33.1% of clinical decision making ability. In order to improve the clinical decision making ability, which is the core competency of nurses, education to increase critical thinking disposition and the creation of a desirable nursing work environment are necessary. In future research, research to identify various variables affecting the clinical decision making ability of nursing hospital nurses, and research for the development and effectiveness of programs to improve clinical decision making ability are suggested.
