New Role and Implications of Japanese Clinical Laboratory Technologists through Team Medical Care (팀 의료에서 일본 임상병리사의 새로운 역할과 시사점)

  • When it comes to 'Team Medical Care' in Japan, clinical laboratory technologists have actively engaged in various kinds of teams, such as a diabetes team, a nutrition support team, an infection control team and a medical practice support team. Overall, with continual changes in the medical environment, clinical laboratory technologists are recently being required to not only conduct phlebotomy, specimen collection, laboratory testing, and electrocardiography, but also get actively involved in 'medical practice assistance or support' in collaboration with medical doctors and nurses. Therefore, it is anticipated that resident clinical laboratory technologists in wards or emergency rooms in Korea will be better able to contribute to improving medical quality and securing medical safety by functioning as a link to the clinical laboratory, while medical doctors and nurses will have a reduced burden of work and can dedicate themselves to better patient care.

  • 미래의 정보시스템에서는 온토로지가 핵심적인 역할을 할 것이다. 최근 의학분야에서도 임상의 지식관리와 의학정보검색의 효율화를 위해 온토로지에 대한 연구가 많이 이루어지고 있다. 본 연구는 의학 분야에서 개발된 온토로지를 비교 분석하여 특징을 파악하고 의학정보시스템의 활성화를 위한 온토로지 구축의 방향을 제시하고자 하였다.

  • 술후 통증관리를 위해 많은 새로운 약물과 기술이 도입되고 특히 최근에는 intraspinal narcotics나 PCA방법의 많은 발달에 의해 많은 발전을 가져왔다. 그러나 통증자체가 신체적으로 정신적으로 여러 가지 복합적인 인자가 개제되어 있기때문에 사용되는 약물의 임상약리의 이해와 계속적인 새로운 기술의 개발이 적절한 통증관리의 방침을 수립할 수 있을 것이다.

  • 본 연구는 치위생과 재학생들의 교외 임상실습과정에서 느끼는 스트레스를 파악하고 이를 관리하는 프로그램을 개발하는데 기초자료로 사용하고자 실시하였다. 대구 경북지역 치위생과 재학생을 대상으로 자기기입식 설문조사를 실시하여 전공만족도와 실습기관의 유형이 임상실습 스트레스에 영향을 미치는 것으로 나타났다. 향후 치과환경에 학생들의 스트레스를 대처할 수 있는 대처방안의 개발이 필요할 것이라 사료된다.

The Attitude of Patient Safety and Patient Safety Management Activity in Nursing Students (간호대학생의 환자안전에 대한 태도와 환자안전관리활동)

  • This study is descriptive research to investigate level of the attitude of patient safety and patient safety management activity(PSMA) and identify influencing factors of PSMA in nursing students. The participants were 241 4th grade nursing students who had clinical practice over a year from 3 nursing colleges in G city and J province. Data were collected with structured questionnaires from March 27th to April 17th, 2015 and analyzed using descriptive statistics, t-test, one way ANOVA, Pearson's correlation and multiple regression with SPSS 21.0 program. As a result, the level of attitude of patient safety was 3.68 score and PSMA was 4.03 score. In PSMA, there were significant differences in practice period(p=.020) and practice satisfaction(p=.026). There was a positive correlation(p<.001) among attitude of patient safety and PSMA. The most effective factor on PSMA was practice satisfaction, followed by attitude of patient safety. Based on the findings of this study, suggest that to develop the systematic education program considering effective factors of PSMA for nursing students.

  • The aim of this study was to analyze in depth the standardized Clinical dental hygiene curriculum of the Fones School in the United States. We investigated the clinical dental hygiene curriculum in 2015~2016 including title, credit, hours, contents, goals, competencies, and evaluation. We obtained the course syllabus and data related to each subject, for each grade, from the professors and students at the university. The goals and competencies, of the clinical dental hygiene program, which were based on the goals of the Fones School and the mission of the University of Bridgeport, were developed in accordance with the dental hygienist practice standards proposed by the American Dental Hygienists Association. The curriculum consisted of theory to teach proper dental hygiene care procedures and incorporated practical exercises that modeled an actual clinical setting. The students had to document the procedures performed for each client/patient and improve their clinical competency through discussion with the professors. Dental hygiene care should be provided for children, adolescents, adults, elderly, and patients, which includes patients with moderate or severe periodontal status. Students were evaluated by a paper test or case study presentation and their clinical evaluation was based on their clinical competency. In particular, professors evaluated students on a rotational basis, so they could evaluate the level of achievement of clinical competency of all students and find ways to improve any weaknesses. Therefore, the current study suggested that clinical dental hygiene program in Korea could be improved if based on the curriculum of Fones School in the United States.

Job Titles of Medical Technologist Managers in a Hierarchical System and Team System (계층제와 팀제에서 임상병리사 관리자 명칭)

  • In the 2000's, due to a change in hospital management strategy, the organizational structure shifted from a hierarchical system to a team system. While the hierarchical system is characterized by being activity centered, job title linked, and vertically managed, the team system is characterized by being competency centered, job title segregated, and horizontally managed. The job titles of medical technologist manager was surveyed three times in 1997, 2007, and 2017. It has been confirmed through staff members working at 24 hospitals in more than 500 beds in the metropolitan area. The results of job titles follow are as follow: "Team Leader; Part Leader" 14/24 (59%), "Chief Technologist; Area Head Technologist" 7/24 (29%), and "Chief" 3/24 (12%). The present authors propose an alternative name based on the team system to refine the three job titles currently used by medical technologists. First, the Chief Technologist is unclear if it refers to the Technologist General Manager or Technologist Manager. The Chief Technologist should be changed to "Team Leader". Second, given that Area Head Technologist or Section Chief are on the same position as Head Nurse, we suggest that Area Head Technologist or Section Chief should be changed to "Part Leader". Third, while the organization regulation is marked merely as Department of Laboratory Medicine according to the hierarchical system, it is marked as Laboratory Medicine Team according to the team system. Medical technologists come to have more belongingness, feeling of solidarity, and intimacy under the team system.

The Influence Factors on Nursing Practice by Clinical Nurses (in the COVID-19 Situation) (코로나19 상황에서 간호사의 간호업무수행 영향요인)

  • The purpose of this study was to identify the factors affecting nursing practice of clinical nurses in the COVID-19 situation and provide basic data that could improve nursing practice in the pandemic situation. For this descriptive research, a survey was conducted to collect data from 119 clinical nurses in G City for six weeks from June 15 to July 20, 2022. Descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression were used. The results showed that infection control practice (r=.428, p<.001) and resilience (r=.324, p<.001) were statistically significantly correlated with nursing practice. As a factor affecting nursing practice of clinical nurses, infection control practice (B=.529, p<.001) had an explanatory power of 18.3%. This result is thought to be due to the fact that COVID-19 infection control acted as a new burden on nursing work in the COVID-19 pandemic, and recovery elasticity and job stress did not have an influence. It is however necessary to help clinical nurses relieve job stress and perform high-quality nursing through regular counseling for clinical nurses suffering severe job stress.

Survey on the Relationship between the Number of Medical Technologists and the Medical Test Count (검사건수에 따른 임상병리사의 인원편성에 관한 설문조사 분석)

  • This study examined the relationship between the number of medical technologists and the medical test count. Data was obtained from 441 medical technologists in a hospital through a self-reported questionnaire. The Pearson correlation test, student's t-test, ANOVA or descriptive statistics were performed for data analysis. The distribution of medical technologist according to the size of hospitals was small 5.8, medium 14.9, large 25.8, and super 45.4. The analysis demonstrated a relationship between the number of medical technologists and the number of medical tests in the field, number of clinical tests per MT, and number of optimal medical test per MT according to the hospital size (P<0.001). The average time for quality control by the department at a higher hospital was less than two hours. In terms of the satisfaction of salary, work environment, test accomplishment, and welfare service, the dissatisfaction of medical technologists in small and medium hospitals was higher than those in large and super hospitals. Overall, a focus on intensifying systemic supplementation and improving the condition of medical technologists is needed to provide reliable data for medical examinations in medical areas.
