An Evaluation of the Rapid Antimicrobial Susceptibility Test by VITEK MS and VITEK 2 Systems in Blood Culture (혈액배양에서 VITEK MS와 VITEK 2 System을 이용한 신속 항생제 감수성 시험의 유용성 평가)

  • The results of rapid antimicrobial susceptibility test (AST) in blood cultures were obtained by inoculating the bacteria directly into the VITEK MS and the VITEK 2 systems without subculturing in the blood culture positive medium. The obtained results were compared with the results using a standard method to evaluate their reliability and accuracy. The direct AST results in blood culture positive specimens were 97.9% (1,936/1,978), consistent with the standard AST results. Gram-positive bacteria showed a concordance rate of 97.2% (1,051/1,081), a very major error rate of 0.5% (5/1,081), a major error rate of 0.1% (1/1,081), and a minor error rate of 2.2% (24/1,081). Staphylococcus epidermidis was the main cause of discordance, and gentamicin (N=9) and fusidic acid (N=8) showed high errors. The overall concordance rate and minor error among the Gram-negative bacteria were 98.6% (885/897) and 1.4% (12/897), respectively. Escherichia coli and Pseudomonas aeruginosa were the major causative bacteria of Gram-negative bacteria. Among them, amoxicillin/clavulanic acid (N=3) showed high error. Direct AST met the CLSI criteria and shortened the reporting time by 24 hours; however, we found that there was a need to perform an addition test via disk diffusion for antimicrobials with very large errors. These results suggest that the method of direct AST in blood culture positive medium may be very useful in efficiently treating patients.

  • 본 논문에서는 '그룹화된 패리티를 갖는 소수 라운드 로빈({{{{ { PRP}_{gp } }}}}:Prime Round Robiin with Grouped Parties)' 방식에 대한 기존의 조합 모델을 이용한 신뢰도 분석 시에 문제가 됐던 결함복구율을 고려하지 못한 모델링에 대해서 미르코프 모델을 이용한 신뢰도 모델링을 바탕으로 결함복구율을 고려한 신뢰도를 계산한다. 또한 산출된 신뢰도를 근거로 반최적화된(semi-optimal) 패리트 그룹 나누기 알고리즘을 도출하고 동시에 두 개의 결함에 대한 분석을 수행한다. 마르코프 모델을 이용한 신뢰도 모델링을 통해서 결함발생율만을 고려한 경우에 신뢰도가 기존의 조합 모델의 신뢰도와 거의 일치하고 결함발생율과 결함복구율을 동시에 고려한 경우에 신뢰도가 결함발생율만을 고려했을 경우보다 더높다는 것을 보인다. 반최적화된 패리티 그룹 나누기 알고리즘을 사용할 경우에, 동시에 두 개의 결함에 대한 분석을 통해서 약 30% 이상의 경우에 대해서 저장된 패리티 정보를 이용한 복구가 가능하다.

Tuberculin Skin Test and QuantiFERON-TB Gold Assay before and after Treatment for Latent Tuberculosis Infection among Health Care Workers in Local Tertiary Hospital (일개 병원의 의료인에서 투베르쿨린 검사와 QuantiFERON-TB Gold 검사를 이용한 잠복결핵의 진단과 치료 전후의 변화)

  • The QuantiFERON-TB Gold assay and tuberculin skin test (TST) have been useful test for diagnosing latent tuberculosis infections (LTBI). However, there are few reports on the efficacy of the QuantiFERON-TB Gold assay and TST in evaluating the response after the treatment of LTBI. This study examined the changes in the TST and QuantiFERON-TB Gold assay before and after a treatment for latent tuberculosis in health care workers (HCWs) at a local tertiary hospital. Methods: A cohort of volunteers working as nurses and doctors who underwent a TST and QuantiFERON-TB Gold assay was established. The volunteers positive for the QuantiFERON-TB Gold assay had been treated with 3 months of isoniazid (INH) and rifampin (RFP). After completing treatment, the TST and QuantiFERON-TB Gold assay were repeated. Results: Of the 48 participants (14 doctors, 34 nurses, M: F=11:37, mean $age=29.9{\pm}5.5$ years, mean employment $period=74.9{\pm}64.3$ months), 19 (39.6%) tested positive to the TST (mean induration=$19.1{\pm}9.7mm$) and 8 (16.7%) were QuantiFERON-TB Gold assay. Among them, one had active pulmonary tuberculosis. Seven volunteers were consistently positive to both the TST and QuantiFERON-TB Gold assay after being medicated with INH and RFP for 3 months. Conclusion: TST and QuantiFERON-TB Gold assay are unsuitable for evaluating the treatment response of LTBI because they were consistently positive both before and after the anti-tuberculosis medication.

Evaluation of the Automated Immunohematology Analyzer DAYMATE M (혈액은행 자동화 검사장비 DAYMATE M의 수행능 평가)

  • Background: An automated immunohematology analyzer, DAYMATE M (DAY Medical, Switzerland), has been recently developed. The potential of this analyzer to improve test results has been evaluated. Methods: A total of 300 blood samples from Seoul St. Mary's hospital and Incheon St. Mary's hospital were tested for ABO and RhD typing. In addition, 336 antibody screening test (AST) samples and 82 patients treated with hematopoietic stem cell transplantation (HSCT) were included. AST results by DAYMATE M were compared with those obtained by a manual method using DS-Screening II (Bio-Rad Laboratories, Switzerland) and red blood cells from Selectogen (Ortho-Clinical diagnostics Inc., USA). Results: Of the 300 patients enrolled, 87, 73, 79, and 61 had type A, B, O, and AB blood, respectively. The concordance rate was 99.9% for cell typing and 97.0% for serum typing. One discordant case was classified as type B instead of AB, and six discordant serum-typing cases were type A, but classified as type AB. Among the 336 AST samples, the concordance rate was 93.2%. From 136 positive cases, six were discordant. Within the 82 HSCT-treated patients, the concordance rate for ABO blood typing was 92.2%. Among the six discordant cases, DAYMATE M typed four cases as donor type where the standard method typed them as the recipient blood type. Conclusions: The DAYMATE M automated immunohematology analyzer performs reliably for ABO and RhD typing, as well as for ASTs and on samples from patients treated with HSCT.

  • 일반적으로 얼굴인식 시스템은 영상에서 추출한 Feature와 DB 상의 Feature를 비교하는 구조를 가지고 있다. 하지만 원하는 Class의 Feature만 보고 DB 상에서 일치하는 Class의 위치를 특정하는 것은 불가능하기에 DB 상의 모든 Feature와 비교하는 절차가 필요하다. DB 크기가 커짐에 따라 처리시간과 메모리상의 문제가 발생하는데, 이 논문에서는 이를 해결하기 위한 Deep Hashing 모델을 제안한다. Softmax 기반의 Loss를 이용하여 학습하였고, 8-bits의 해시를 추출하였을 때 53%의 Feature 일치율을 보였으며, 이를 사용할 경우 DB 평균 대조군을 23% 이하로 줄이는 효과를 볼 수 있을 것으로 추정한다.

  • 1990년대 후반까지 줄곧 광고 시장 점유율 1위를 차지하던 신문광고가 2001년을 기점으로 광고 시장 점유율의 지속적인 하락세를 보이고 있다. 이는 인터넷과 같은 경쟁적인 뉴미디어가 급격히 성장하는 데 그 원인이 있을 뿐만 아니라, 신문의 광고효과를 산출하는 과학적 방법론이 부족하다는 데에도 그 원인이 있다. 이러한 문제의식에서 출발하여, 본 연구에서는 안구운동 추적(eyetracking)을 활용하여 신문광고의 노출효과를 측정했다. 이를 위하여, 기존 신문 구독자의 인구통계학적 분포에 따라 추출된 50명의 표본을 대상으로, 신문의 면내 광고별 체류시간을 헤드 마운트형 안구운동 추적기(head-mounted eyetracker)를 활용하여 측정했다. 실험 결과, 데이터의 95%가 기존의 주의-기억 단계를 따르는 순차적인 정보처리모델과 스타치 구독률 조사(starch readership reports) 결과와 안구운동 추적 결과와 일치했다. 하지만, 5%의 데이터가 주의와 기억 단계의 차이에 기인한 이월 효과(carry-out)로 순차적인 정보처리모델과 일치하지 않는 것으로 나타났다. 즉 5%가량이 광고를 보지 않았음에도 봤다고 표시했다. 또한, 기존에 스타치 구독률 조사와 같은 재인 측정 방법을 통하여 간접적으로 측정하였던 기존의 신문 광고 노출율(보통 30%)에 비하여, 직접 측정한 결과 60%에 달하는 광고 노출율을 보였다. 이는 기존의 인식과 달리, 스타치 구독률 조사가 단순 노출 정도가 아니라, 응답자의 선택적인 기억 결과를 측정한 것임을 보여준다. 이로써, 안구운동 추적을 활용한 노출효과측정은 기존에 스타치 구독률 조사에서 놓치고 있는 부분을 보완할 수 있는 기법임을 보여준다. 또한 이 기법에 의하여 도출된 노출율은 60%에 달하여 기존의 약30%로 제시된 연구결과들이 실제로 측정기법의 한계로 인하여, 기억이 가지고 있는 왜곡을 그대로 반영한 노출율에 관한 자료를 쓰는 것으로 나타났다. 본 연구에서 제안된 노출효과 측정방법론은 기존의 왜곡된 데이터를 보완하고, 신문 광고 시장의 과학적인 측정기법을 발전시키는 데 의미 있는 기여를 할 것으로 기대한다.

Analytical Solutions for Predicting Movement Rate of Submerged Mound (수중둔덕의 이동율 예측을 위한 해석해)

  • Analytical solutions to predict the movement rate of submerged mound are derived using the convection coefficient and the joint distribution function of wave heights and periods. Assuming that the sediment is moved onshore due to the velocity asymmetry of Stokes' second order nonlinear wave theory, the micro-scale bedload transport equation is applied to the sediment conservation. The nonlinear convection-diffusion equation can then be obtained which governs the migration of submerged mound. The movement rate decreases exponentially with increasing the water depth, but the movement rate tends to increase as the spectral width parameter, $ u$ increases. In comparison of the analytical solution with the measured data, it is found that the analytical solution overestimates the movement rate. However, the agreement between the analytical solution and the measured data is encouraging since this over-estimation may be due to the inaccuracy of input data and the limitation of sediment transport model. In particular, the movement rates with respect to the water depth predicted by the analytical solution are in very good agreement with the estimated result using the discritization technique with the hindcast wave data.

Automatic Interpretation of Epileptogenic Zones in F-18-FDG Brain PET using Artificial Neural Network (인공신경회로망을 이용한 F-18-FDG 뇌 PET의 간질원인병소 자동해석)

  • For the objective interpretation of cerebral metabolic patterns in epilepsy patients, we developed computer-aided classifier using artificial neural network. We studied interictal brain FDG PET scans of 257 epilepsy patients who were diagnosed as normal(n=64), L TLE (n=112), or R TLE (n=81) by visual interpretation. Automatically segmented volume of interest (VOI) was used to reliably extract the features representing patterns of cerebral metabolism. All images were spatially normalized to MNI standard PET template and smoothed with 16mm FWHM Gaussian kernel using SPM96. Mean count in cerebral region was normalized. The VOls for 34 cerebral regions were previously defined on the standard template and 17 different counts of mirrored regions to hemispheric midline were extracted from spatially normalized images. A three-layer feed-forward error back-propagation neural network classifier with 7 input nodes and 3 output nodes was used. The network was trained to interpret metabolic patterns and produce identical diagnoses with those of expert viewers. The performance of the neural network was optimized by testing with 5~40 nodes in hidden layer. Randomly selected 40 images from each group were used to train the network and the remainders were used to test the learned network. The optimized neural network gave a maximum agreement rate of 80.3% with expert viewers. It used 20 hidden nodes and was trained for 1508 epochs. Also, neural network gave agreement rates of 75~80% with 10 or 30 nodes in hidden layer. We conclude that artificial neural network performed as well as human experts and could be potentially useful as clinical decision support tool for the localization of epileptogenic zones.

  • This paper presented a rate-distortion estimation method for effective mode decision in H.264/AVC. In this approach, in order to decide a mode, laplacian distribution modeling of DCT coefficients is utilized, which do not need to such process as quantization, entropy coding. From the simulation results, proposed a method showed that rate-distortion between proposed scheme and practical value was almost the same and performed 0.02dB of PSNR gain.

  • 최근 항공우주산업을 비롯한 자동차, 선박, 철강, 건설, 전기, 국방, 금속산업 등 산업의 현장에서 첨단소재들을 널리 사용하고 있으며, 특히 비자성 금속재료에 대한 도전율 특성의 정밀분석이 요구되고 있다. 따라서 본 연구는 비자성 재료의 전기 도전율에 대해 널리 사용되고 있는 3가지의 측정방법을 통하여 정확한 측정과 함께 비교 분석하였으며, 이 측정 방법에 따라 평가된 도전율 측정결과는 0.5 %이내의 정확도에서 일치하고 있는 것으로 나타났다.
