We report a 2-month-old boy who presented with severe hyponatremia and hyperkalemia secondary to ureteropelvic junction(VPJ) obstruction. By prenatal ultrasonography at 19 weeks of gestation, severe hydronephrosis was found which was confirmed postnatally Pyeloplasty was done on the 45th day of life, and fifteen days after pyeloplasty, non-bilious vomiting, decreased activity and dehydration developed. Severe hyponatremia and hyperkalemia were observed, as a result of elevated serum aldosterone and plasma renin activity. The anterior posterior pelvic diameter(APPD) and Society for Fetal Urology(SFU) grade measured showed no interval change before and after pyeloplasty. Pseudohypoaldosteronism was diagnosed, and 2M NaCl was administrated orally for 7 days. The electrolyte imbalance was corrected, and 8 weeks later, the elevated levels of aldosterone and plasma renin activity were normalized. The left hydronephrosis was improved at 5 months of age. We hereby report a transient pseudohypoaldosteronism secondary to UPJ obstruction with a review of the literature.
Asan Medical Center Children's Hospital, University of Ulsan College of Medicine, Seoul, Korea Hyperammonemia in the newborn often leads to severe fatal illness associated with hyperammonemic encephalopathy. Transient hyperammonemia in newborns (THAN) is characterized by self-limiting, transient hyperammonemia during the neonatal period. THAN may have favorable long-term outcomes if it is diagnosed early and appropriately managed. However, severe hyperammonemia can develop even in newborns with THAN, which may require emergent management. Here we report a case of THAN with severe hyperammonemia during the neonatal period that was successfully treated with continuous renal replacement therapy and nitrogen-scavenging medications. Our patient went on to develop normally and has not re-experienced a hyperammonemic episode until 9 months of age without the administration of a protein restricted diet or medications.

This study aims to analyze modern hairstyle and fashion in the collections in the late 1990s, based on the concepts and characteristics of Minimalism, a buzzword of the art field in the 1960s. This study used qualitative research method, in other words, presenting an analysis framework by studying domestic and foreign books and dissertations on Minimalism and then applying the Internet or visual image to the analysis framework. The result indicates that Minimalism design in modem hairstyle and fashion has four characteristics: simplicity, unity, repetition and spatiality.
사람들은 자신이 선호하는 스타일의 의복을 착용하기를 원한다. 하지만 다수의 사람들이 자신의 체형 부위별로 본인이 생각하기에 만족하지 못하는 경우가 많아 실제로는 선호하는 스타일이 아닌 자신의 체형을 커버하는 디자인의 의복을 착용하는 경우가 많다. 따라서 본 연구에서는 정장류 착용 경험이 많고 사회 활동을 많이 하는 성인 여성을 대상으로 신체인지도 및 만족도에 따른 아이템(재킷, 팬츠, 스커트)별 선호스타일과 착용스타일에 어떠한 차이점이 있는가를 살펴보고, 이러한 차이가 나타나는 요인을 분석해 보고자 한다. (중략)

Carboxyethylgermanium sesquioxide(Ge-132)를 투여용량(300,600, 900mg/kg)과 투여일을 변화시켜 정상마우스와 cyclophosphamide(CY)로 처리한 마우스에 경구투여한 후, Ge-132가 CY 에 의해 변화된 마우스의 면역기능에 미치는 효과를 실험하였다. SRBC 항원 주사전, 동시 또는 후에 Ge-132를 투여시 SRBC에 대한 적혈구 응집소가와 비장세포의 용혈반 형성세포(PFC)수가 항원 주사일과 관계없이 용량의존적으로 증가되어 체액성 면역반응을 강화시켰으며, 마크로파지의 탐식능도 항진시켰다. 그러나 DNFB에 대한 접촉성 지연형 과민반응은 DNFB 감작일과 투여용량에 관계없이 Ge-132투여로 억제되었다. CY를 투여한 마우스에 Ge-132를 병용 투여한 군이 CY 단독 투여군에 비하여 적혈구 응집소가와 PFC수 및 혈중 탄소입자의 제거율이 현저하게 증가되어, CY로 억제된 체액성 면역반응과 마크로파지의 탐식능이 항진된 것으로 나타났다. DNFB 감작 전 CY 투여로 증폭된 접촉성 지연형 과민반응이 Ge-132 병용투여로 감소되어 CY로 유도된 면역독성 반응에 대한 Ge-132의 저지효과를 시사해 주었다.
Purpose: Transient neurological deterioration (TND) is one of the complications after extracranial-intracranial bypass surgery, and it has been assumed to be caused by postoperative transient hyperperfusion. This study was performed to evaluate the relationship between TND and preoperative and postoperative cerebral perfusion status on brain perfusion SPECT following superficial temporal artery - middle cerebral artery (STA-MCA) anastomosis surgery. Materials and Methods: A total of 60 STA-MCA anastomosis surgeries of 56 patients (mean age: $50{\pm}16$ yrs; M:F=29:27; atherosclerotic disease: 33, moyamoya disease: 27) which were done between September 2003 and July 2006 were enrolled. The resting cerebral perfusion and cerebral vascular reserve (CVR) after acetazolamide challenge were measured before and 10 days after surgery using 99mTc-ethylcysteinate dimer (ECD) SPECT. Moreover, the cerebral perfusion was measured on the third postoperative day. With the use of the statistical parametric mapping and probabilistic brain atlas, the counts for the middle cerebral artery (MCA) territory were calculated for each image, and statistical analyses were performed. Results: In 6 of 60 cases (10%), TND occurred after surgery. In all patients, the preoperative cerebral perfusion of affected MCA territory was significantly lower than that of contralateral side (p=0.002). The cerebral perfusion on the third and tenth day after surgery was significantly higher than preoperative cerebral perfusion (p=0.001, p=0.02). In TND patients, basal cerebral perfusion and CVR on preoperative SPECT were significantly lower than those of non-TND patients (p=0.01, p=0.05). Further, the increases in cerebral perfusion on the third day after surgery were significant higher than those in other patients (p=0.008). In patients with TND, the cerebral perfusion ratio of affected side to contralateral side on third postoperative day was significantly higher than that of other patients (p=0.002). However, there was no significant difference of the cerebral perfusion ratio on preoperative and tenth postoperative day between patients with TND and other patients. Conclusion: In patients with TND, relative and moderate hyperperfusion was observed in affected side after bypass surgery. These finding may help to understand the pathophysiology of TND.

We report the case of a 30-year-old man who was presented with transient calcification on the graft shortly after anterior cruciate ligament(ACL) reconstruction using a autogenous bone patellar tendon. The patient underwent ACL reconstruction with two incisional technique and six month later, calcific density was seen radiologically around the graft. On postoperative 13 months follow-up radiographic films, the calcific density disappeared. After two months of operation, Lachman and pivot shift test were negative and one millimeter side to side difference was detected in KT-1000 with 20 Ibs strength. But 6 months after the reconstruction, mild anterior instability was detected with the calcific density around the grafted tendon. However the anterior stability was recovered according to the disappearance of calcific density.
Transient hyperammonemia in a newborn is an overwhelming disease manifested by hyperammonemic coma. The majority of affected newborns are premature and have mild respiratory syndrome. The diagnosis may be difficult to determine. This metabolic disorder is primarily characterized by severe hyperammonemia in the postnatal period, coma, absence of abnormal organic aciduria and normal activity of the enzymes of the urea cycle. Hyperammonemic coma may develop within 2-3 days of life, although its etiology is unknown. Laboratory studies reveal marked hyperammonemia (>$4,000{\mu}mol/L$). The degree of neurologic impairment and developmental delay in this disorder depends on the duration of hyperammonemic coma. Moreover, the infant may succumb to the disease if treatment is not started immediately and continued vigorously. Hyperammonemic coma as a medical emergency requires dialysis therapy. Here, we report a case of severe transient hyperammonemia in a preterm infant (35 week of gestation) presented with respiratory distress, seizure, and deep coma within 48 hours and required ventilatory assistance and marked elevated plasma ammonia levels. He survived with aggressive therapy including peritoneal dialysis, and was followed 2 years later without sequelae.

본 연구의 목적은 (1) 가정에서의 소비행동을 자원활용적 측면에서 탐색해 봄으로써 소비의 사용/처분 효율성 개념을 제품군 별로 명료화 하고 (2) 사용/처분 효율성과 인구통 계적 특성 환경보전에 대한 태도와의 차이를 실증하기 위한 것이다 연구대상은 서울시에 거 주하는 주부 400명이며 최종분석에 사용된 자료수는 357부이다 연구에 따르면 사용/처분 효 율성 개념은 상품군(식품, 의류 가전제품) 에 따라 다르게 규정지어야만 함을 알 수 있었다 또한 사용/처분 효율성과 몇몇 인구통계적 변수와는 관련이 있음을 보여주었고 환경보전 태 도는 사용효율성과 정적 관계를 보여주었으나 처분효율성과는 일과성 있는 관계를 보여주지 는 못했다 이러한 연구결과는 소비자 교육 및 재활용정책에 적극 활용할 수 있으리라고 본다.
