Causes of Under-staging in Patients with Gastric Cancer That was Proven to be Unresectable after a Laparotomy - Correlation with CT Findings (비절제 위암의 원인분석-전산화단층촬영(CT) 소견을 중심으로)

  • Purpose: The aim of this study was to investigate the causes of under-staging in patients with advanced gastric cancer that was proven to be unresectable after a laparotomy. Materials and Methods: We retrospectively analyzed 25 gastric cancer patients who had undergone a diagnostic laparotomy between 2001 and 2005. For the preoperative evaluation, spiral CT and multidetector-row CT were performed. We analyzed the clinicopathologic features of patients and compared the image findings and the results of surgery. The causes of under-staging were divided into 3 groups; patient factor, CT factor, and interpretation factor. Results: Grossly, there were 12 cases of Borrmann type-III tumors and 13 cases of Borrmann type-IV tumors. The most frequent histologic type was poorly differentiated adenocarcinomas (8 cases) and signet ring cell carcinomas (7 cases). There were 13 cases of adjacent organ invasion, and the pancreas was the most frequently invaded organ (9 cases). There were 17 cases of peritoneal metastasis, and 3 cases of distant lymph node metastasis. For the cause of under-staging, there were four cases of patient factor, 19 cases of interpretation factor, and 9 cases of CT factor. In three cases, the cause of under-staging could not be identified. Conclusion: CT interpretation factor was the most frequent cause of under-staging in the preoperative diagnosis with gastric cancer patients. Therefore, more cautious CT interpretation is necessary to avoid unnecessary laparotomies in gastric cancer patients.

Effects of high-fat diet induced obesity on tissue zinc concentrations and zinc transporter expressions in mice (고지방식이로 유도한 비만이 마우스의 조직 아연 농도와 아연수송체 발현에 미치는 영향)

  • Purpose: Obesity is often associated with disturbances in the mineral metabolism. The purpose of this study was to investigate the effects of high-fat diet-induced obesity on tissue zinc concentrations and zinc transporter expressions in mice. Methods: C57BL/6J male mice were fed either a control diet (10% energy from fat, control group) or a high-fat diet (45% energy from fat, obese group) for 15 weeks. The zinc concentrations in the serum, stool, and various tissues were measured by inductively coupled plasma (ICP)-atomic emission spectrophotometry or ICP-mass spectrophotometry. The levels of zinc transporter mRNAs in the liver, duodenum, and pancreas were measured by real-time RT-PCR. The levels of serum adipokines, such as leptin and IL-6, were determined. Results: The total body weight, adipose tissue weight, and hepatic TG and cholesterol concentrations were significantly higher in the obese group, as compared to the control group. The obese group had significantly higher levels of serum leptin and pro-inflammatory IL-6 concentrations, and had significantly lower levels of serum alkaline phosphatase activity. The zinc concentrations of the liver, kidney, duodenum, and pancreas were all significantly lower in the obese group than in the control group. On the other hand, the fecal zinc concentrations were significantly higher in the obese group than in the control group. The serum zinc concentrations were not significantly different between the two groups. The ZnT1 mRNA levels of the liver and the pancreas were significantly higher in the obese group, as compared to the control group. Hepatic Zip10 mRNA was also increased in the obese group. Conclusion: Our study findings suggest that obesity increases fecal zinc excretion and lowers the tissue zinc concentrations, which may be associated with alterations in the zinc transporter expressions.

Petrology of Host Body of Feldspar Deposits in Jechon Ganites (장석광상 모암인 제천반상화강암의 암석학적 특성)

  • Jecheon granite can be divided into two types; porphyritic granite (K-feldspar megacryst bearing) and medium-grained biotite granite. Porphyritic granite, host body of feldspar deposits, is 8${\sim}$11 km in diameter and about 80 $km^{2}$ in area. It mainly contains K-feldspar, plagioclase, biotite and quartz, and magnetite, zircon, sphene and apatite are accessary minerals. Enclosed minerals in K-feldspar megacryst with 3${\sim}$10 cm in diameter are hornblende, plagioclase, quartz, magnetite, apatite, sphene and zircon. Mafic enclaves mainly consisting of hornblende, plagioclase and quartz are frequently observed in porphrytic granite. Medium-grained biotite granite consists of K-feldspar, plagioclase, biotite and hornblende as main, and hematite, muscovite, apatite and zircon as accessary minerals. Core and rim An contents of plagioclase from porphyritic granite, medium biotite granite, K-feldspar megacryst, and mafic enclave are 36 and 21, 40 and 32, 37 and 32, and 43 and 36, respectively. $X_{Fe}$ values of hornblende are 0.57 at biotite granite, 0.51 at K-feldspar mehacryst and 0.45 at mafic enclave. $X_{Fe}$ values of biotite and hornblende are homogeneous without chemical zonation. K-feldspar megacryst shows end member of pure composition with exsolved thin lamellar pure albites. Characteristics of mineral compositions and petrography indicate porphyritic granite is igneous origin and medium-grained biotite granite comes from the same source of magma; biotite granite is initiated to solidly and from residual melt porphyritic granite can be formed. Possibly K-feldspar megacrysts are formde under H$_{2}$O undersaturation condition and near K-feldspar solidus curve temperature; growth rate is faster than nucleation rate. Mafic enclaves are thought to be mingled mafic magma in felsic magma, which is formed from compositional stratigraphy. Estimated equilibrium temperature and pressure for medium-grained biotite granite are about $800^{\circ}C$ and 4.83${\sim}$5.27 Kb, respectively.

  • 적혈구(赤血球) 수명의 측정에는 $^{51}Cr$-표지적혈구법(標識赤血球法)이 임상적(臨床的)으로 이용(利用)되고 있으며 이는 이론상(理論上) steady state 즉(卽) 측정기간(測定期間)동안 순환(循環) $^{51}Cr$량(量)-적혈구량(赤血球量)이 일정(一定)할 때에 한(限)하여 유효(有效)하며 unsteady state 때는 true red cell survival을 알기 위하여서는 측정치에 영향을 주는 요인(要因)에 대하여 각각(各各) 교정(校正)해 줄 필요(必要)가 있다. 이 요인(要因)중에 특히 실혈(失血)로 인(因)한 영향에 관(關)하여는 계통적인 연구(硏究)가 적다. 이에 저자(著者)들은 $^{51}Cr$표지적혈구법(標識赤血球法)을 이용(利用)하여 실혈(失血)이 적혈구(赤血球) 수명측정(測定)에 미치는 영향을 인체(人體)에서 실험 관찰하여 몇가지 성적을 얻었다. 연구대상(硏究對象)은 총(總) 56명(名)의 청장년(靑壯年)으로 급성실혈군(急性失血群)과 만성실혈군(慢性失血群)으로 구분(區分)하여 급성실혈군(急性失血群)은 위장출혈등(胃腸出血等)이 없는 2대(代)의 의대생(醫大生)으로 $^{51}Cr$표지적혈구법(標識赤血球法)을 사용하여 적혈구(赤血球) 수명을 측정하는 동안($10{\sim}14$ 일간(日間)) 1일당(日當) 10ml(6명(名)), 25ml(4명(名)), 50ml(4명(名)), 75ml(4명(名)), 100ml(6명(名))를 각각(各各) 사혈(瀉血)한 군(群)과 10일간(日間) 1,000ml를 사혈한 군(群) 즉 200ml씩 5회(回)(4명(名)), 500ml씩 2회(回)(4명(名))로 세분(細分)하였으며 만성실혈군(慢性失血群)은 직업적인 공혈자(供血者)로 반복사혈로 생긴 9명(名)의 빈혈자와 십이지장충증(十二指腸蟲症)에 감염(感染)되어 구충(驅蟲)한 중등도(中等度)의 철결핍성 빈혈환자 7명(名)으로 나누어 관찰하였다. 측정(測定) 방법(方法)으로는 Gray 및 Sterling법(法)을 개설한 방법(方法)으로 $^{51}Cr$표지적혈구(標識赤血球)의 계측시료(計測試料)로서 전혈(全血) 및 적혈구(赤血球)를 사용(使用)하였다. 실험(實驗)성적은 1. 1일당(日當) 실혈량(失血量)이 증가(增加)할수록 적혈구(赤血球)수명($T\frac{1}{2}$)은 짧아짐을 알 수 있었다. 즉(卽) 1일당(日當) $20{\sim}50ml$ 사혈군에서는 $T\frac{1}{2}$이 현저히 짧아지는 rapid phase을 나타내고 1일당(日當) 50ml이상(以上) 사혈군에서는 짧아지는 정도(程度)가 완만한 slow phase을 나타낸다(Fig. 6). 2. 1일량(日量) 10ml 및 25ml식(式) 사혈한 군(群)의 적혈구수명(赤血球壽命)을 측정(測定)하는데 있어 적혈구(赤血球)를 사용하였을 때에는 $T\frac{1}{2}$측 정치에 유의한 차(差)가 없었으며 이 범위 내에서는 Hct., Hb. 및 혈청철치(血淸鐵値)도 역시 유의한 차(差)가 없었다. 3. 1일량(日量) 50ml 및 75ml, 100ml씩 사혈한 군(群)에서는 적혈구(赤血球)만을 사용(使用)하였을 때와 전혈(全血)을 시료(試料)로 하였을 때 사이에 $T\frac{1}{2}$의 측정치에 유의한 차(差)가 있었으며 이 때는 Hct., Hb. 및 혈청철치(血淸鐵値)에도 변화(變化)가 있었다. 즉(卽), 전혈(全血)을 사용한 적혈구(赤血球) 수명($T\frac{1}{2}$)의 측정치가 적혈구(赤血球)만를 사용(使用)한 적혈구(赤血球) 수명($T\frac{1}{2}$)의 측정치 보다 짧았다. 4. 일정(一定)기간(10 일(日)) 사혈의 총량(1000ml)이 같을 매는 200ml를 5회(回) 사혈한 군(群)이나 500ml를 2회(回) 사혈한 군(群) 사이에 적혈구(赤血球) 수명($T\frac{1}{2}$)에 유의(有義)한 차(差)를 볼 수 없었다. 5. 직업적 공혈자의 반복사혈로 인(因)한 만성(慢性) 빈혈환자 9명(名)에서의 $^{51}Cr$적혈구(赤血球)수명($T\frac{1}{2}$) 측정치는 평균(平均) 19.2일(日)로 짧아져 있으나 적혈구수명측정전후(赤血球壽命測定前後)에 충분(充分)한 철제(鐵劑)를 투여(投與)하여 Hct., Hb. 및 혈청철치(血淸鐵値)를 증가(增加)시켰으며 이때 볼 수 있었든 Hct치(値)를 규준(規準)하여 교정한 적혈구(赤血球)수명($T\frac{1}{2}$)은 거의 정상(正常)범위 안에 있어(27.6일(日)) 이러한 인자(因子)를 고려하지 않으면 잘못 이해할 수가 있다. 6. 구충자충(鉤蟲仔蟲)을 구충한 7명(名)의 중등도(中等度) 철(鐵)결핍성 빈혈환자에서의 적혈구(赤血球)수명($T\frac{1}{2}$) 측정치는 25일(日)$\sim$31일(日)로 평균(平均) 28일(日)이었으며, 이때 장 출혈량은 1일(日) $1.0{\sim}3.5ml$이었다. 단시일내의 급성실혈시에는 이와같은 소량의 실혈(失血)도 적혈구(赤血球)수명($T\frac{1}{2}$) 측정치에 영향을 보여 줌을 알 수 있었다. 따라서 이러한 정도의 실혈은 실험오차에 기인하는 것인지 아니면 장기 출혈에서는 이러한 소량의 실혈이 적혈구(赤血球)수명($T\frac{1}{2}$) 측정에 영향을 미치지 않는 것인지는 아직 확실히 말할 수 없다. 8. $^{51}Cr$-표지적혈구(標識赤血球)로 측정한 적혈구(赤血球)수명($T\frac{1}{2}$)은 측정시의 실혈량(失血量)에 큰 영향을 받음을 알 수 있으며 저자(著者)들은 $^{51}Cr$표지적혈구(標識赤血球)를 이용(利用)한 적혈구(赤血球) 수명 측정때 검사기간중 실혈량이 적혈구수명치(赤血球壽命値)에 미치는 관계를 상술(上述)한 실험치(實驗値)를 기초(基礎)로 하여 다음과 같을 교정식(校正式)을 고찰(考察)해 보았다. $^{51}Cr\;T\frac{1}{2}=17.0e^{-0.0495}+18.4e^{-0.000924x}$ 단(但) X : 1일(日) 실혈량(失血量)(단위(單位) ml)

Study on Phase Relation and Synthesis of Pyrochlore in the System of Ca-Ce-Zr-Ti-O (Ca-Ce-Zr-Ti-O System에서의 파이로클로어 합성 및 상관계에 대한 연구)

  • Pyrochlore is known as one of the most promising materials for the immobilization of radionuclide in high level waste. This study included the synthesis, phase relation and characteristics of $pyrochlore(CaCeZr_xTi_{2-x}O_{7,\;x=0.2\~2.0)$ in the system of Ca-Ce-Zr-Ti-O. Using the CPS(Cold pressing and sintering) method, the mixtures of $CaCO3_,\;CeO_2,\;ZrO_2\;and\;TiO_2$ oxides were pressed, and sintered at $1100\~1600^{\circ}C$ for 20 hours. The optimal synthetic conditions at various compositions were differed from 1300 to $1600^{\circ}C$ Even in the optimal temperatures, pyrochlore or fluorite coexisted with minor amount of perovskite, $CeO_2\;or\;Ce_{0.75}Zr_{0.25}O_2$. It was confirmed that pyrochlore and fluorite structures were stable at $x\leq0.6\;and\;x\geq1.0$, respectively. Especially, the compositions of pyrochlore or fluorite showed non-stoichiometric compositions in that contents of Ca and Ti were more deficient and those of Zr and Ce were more excess than batch compositions with the increase of x value. These characteristics stemmed from the behavior of elements occupied at eight- and six-coordinated site, and then caused the coexistence of perovskite, $CeO_2\;or\;Ce_{0.75}Zr_{0.25}O_2$ along with pyrochlore or fluorite.

  • 효과적인 저수지 수질관리를 위해서는 신뢰도 높은 수리 및 수질모델이 필요하며, 이러한 모델의 성능은 다양한 수문사상에 대하여 적용함으로써 검증할 수 있다. CE-QUAL-W2 모델(이후 W2)은 횡방향 평균 2차원 수리 수질 해석 모델로써 수체의 길이에 비해 폭이 상대적으로 좁고 수심이 깊은 우리나라 대부분의 저수지 지형에 적합한 모델이다. 본 연구의 목적은 기존 연구에서 가뭄년인 2001년과 평수년인 2004년 수문사상에 대하여 보정한 대청호 W2 부영양화모델을 최근 평수년인 2006년과 가뭄년인 2008년을 대상으로 검증하는데 있다. 모델의 검증은 물수지, 수온성층 구조 변화, 부영양화 해석에 중점을 두었으며, 실측자료와 모의결과의 적합성 비교 평가는 결정계수값$(R^2)$, AME(absolute mean error)와 RMSE(root mean square error)를 이용하였다. 저수지 물수지의 적합성을 검증하기 위하여 모의수위와 실측수위를 비교한 결과, $R^2$값이 2006년과 2008년에 각각 0.9945, 0.9972로 나타나 신뢰도가 높은 것으로 확인되었다. 계절별 성층구조 변화 모의 성능을 검증하기 위해 회남수역과 댐 앞 지점에서 수심별 수온의 모의값과 실측값을 비교하였다. 2006년의 경우 모델은 홍수기 동안 안정적으로 수온 성층현상을 모의하였으나, 댐 앞 지점에서 수온 약층이 형성된 구간에서 실측값과 다소 편차를 보였으며, 오차크기는 AME가 $0.561\sim2.088^{\circ}C$, RMSE는 $0.797\sim2.762^{\circ}C$범위였다. 반면, 가뭄년인 2008년에는 전 기간에 걸쳐 모두 안정적으로 저수지 수온 성층현상을 모의하였으며, 오차크기는 AME $0.413\sim1.162^{\circ}C$, RMSE $0.546\sim1.415^{\circ}C$ 범위였다. 조류의 생산성이 높은 표층에서 T-N, T-P 및 Chl-a 농도 모의결과를 장계교, 대정리, 회남대교, 댐 앞, 추동취수탑 및 문의취수탑에서 시계열로 실측값과 비교 검증한 결과, T-N과 T-P는 2006년과 2008년 모두 모든 비교 지점에서 모의값과 실측값의 시계열 변동이 매우 잘 일치하였으며, 홍수기 이외 기간에는 큰 변동 폭을 보이지 않았다. 그러나 기존 연구에서 확인된 바와 같이 7월 이후부터 T-P 모의값이 실측값을 과대 산정하는 경향을 보였는데, 그 원인은 산소 결핍상태에서 저니층에서 용출되는 철(Fe) 또는 망간(Mn)과 같은 이온 성분이 인과 흡착하여 침전되는 기작이 모의과정에 적절히 반영되지 않은 것이 원인으로 판단된다. 조류(Chl-a)농도의 경우, 2006년과 2008년에 모든 지점에서 모델은 조류의 발생과 시계열 변화를 적절히 모의하였으나, 2008년 1월부터 8월까지 댐 앞과 추동 및 문의취수탑에서는 모의값이 실측값을 과대 산정하는 경향을 보였다. 이는 해마다 그리고 계절별로 우점하는 조류 종이 상이한 반면, 모델에서는 이에 대한 매개변수를 적절히 고려하지 못한 것이 원인으로 판단된다.

  • An experiment was carried out to study the effects of Zn application on the rice plant of IR-667 and Jinheung varieties grown under the conditions of poor drainage and low available Zn. The results obtained were an follows; 1. The poor drained condition of the paddy field was more favorable for Zn utilization by the rice plants than the well drained condition. Also Zn application in the form of Zinc chloride $(ZnCl_2)$ seemed to significantly increased the rice yield of both IR-667 and Jinheung. 2. Zn content of IR-667 was much higher than that of Jinheung. The concentration of Zn in IR-667 rice plant treated by Zn fertilizer was about 200 ppm while that of control plot (not received Zn fertilizer) only 30 ppm. This result suggests that Zn application might improve the yield under the condition of poor drained soil when Zn concentration of IR-667 rice plant is around 30 ppm. 3. Absorption of Zn-65 by the rice plant was greater in the plants grown without Zn fertilizer than those with Zn fertilizer application, At harvest, the higher amount of Zn-65 was accumulated in the gram than in the straw. 4. Zn fertilization increased the grain yield in the soil of Paju, Dalsung, Chilg ok and Chungwon with low available Zn.

  • (6R)-5,6,7,8-tetrahydrobiopterin ($6-BH_4$) cofactor is essential for various process, and is present in probably every cell or tissue of higher organism. $6-BH_4$ is required lot various enzyme activities, and for less defined functions at the cellular level. And it is well known about the antioxidant effects as a non-protein compound. Recently, scientists proposed another roles for $6-BH_4$ in melanogenesis. $6-BH_4$ is a well known tyrosinase inhibitor. In this study, we found that methyl-$BH_4$ and $6-BH_4$ have antioxidant activities and inhibitory activity for melanin synthesis. These pterin compounds were not toxic in HaCaT and B16F10 cells and showed scavenging activity against DPPH radicals. We also showed that pterin compounds decreased protein levels of tyrosinase and TRP-1. In a clinical test, pterin compounds showed the significant skin whiteining effect after treatment for 3 weeks. Furthermore pterin compounds significantly suppressed the UVB-induced expression of $PGE_2$ and IL-6 genes induced UVB In HaCaT and inhibited UVB-induced melanogenesis in B16F10 cells. These results showed the effect of pterin compounds as a cosmeceutical ingredient.

A Novel PHKA1 Mutation in a Patient with Glycogen Storage Disease Type IXD (당원 축적병 9D (GSD9D) 환자의 신규 PHKA1 돌연변이)

  • Distal myopathy is a clinically and genetically heterogeneous group of degenerative diseases of the distal muscle. Glycogen storage disease type IXD (GSD9D) is a metabolic distal myopathy characterized by muscle deficiency of phosphorylase kinase, a key regulatory enzyme in glycogen metabolism. Affected individuals may develop muscle weakness, degeneration, and cramps, as well as abnormal muscle pain and stiffness after exercise. It has been reported that mutations in the PHKA1 gene which encodes the alpha subunit of muscle phosphorylase kinase cause GSD9D. In this study, we examined a Korean GSD9D family with a c.3314T>C (p.I1105T) mutation in the PHKA1 gene. This mutation has not been previously reported in any mutation database nor was it found in 500 healthy controls. The mutation region is well conserved in various other species, and in silico analysis predicts that it is likely to be pathogenic. To date, only seven mutations in the PHKA1 gene have been documented, and this is the first report of Korean GSD9D patients. This study also describes and compares the clinical symptoms and pathological conditions of previously reported cases and these Korean patients. We believe that our findings will be useful for the molecular diagnosis of GSD9D.

  • Objective : To evaluate the relationship between serum leptin concentration and obesity indices in a rural population. Methods : Two hundred and nine subjects were randomly sampled from 1036 residents who had participated in the survey. Their obesity indices were measured by bioelectrical impedance analysis and anthropometry. Serum leptin concentrations were measured by immunoradiometric assay. Results : Serum leptin concentrations were positively correlated with body mass index, percent of body fat, fat mass, waist circumference, and hip circumference. Serum leptin concentrations were higher in obese or overweighted subjects than normal or thin subjects(8.5[0.9-31.4] compared with $2.7[0.1-12.0]{\mu}g/L$, p<0.01). Serum leptin concentrations were higher in women than men, even after an adjustment to body mass index$(\beta=4.183,\;p<0.01)$. According to body composition, serum leptin concentrations were more related with waist circumference in men, hip circumference in women. Conclusions : Serum leptin concentrations are positively correlated with obesity indices. Serum leptin concentrations are higher in obese subjects, in women, and more related with peripheral obesity. These findings support the hypothesis that human obesity is associated with leptin-resistance rather than leptin-deficiency.
