A Clinical Analysis to Study Effectiveness of Korean Medicine for Medial Collateral Ligament Injury of the Knee (무릎 내측측부인대 손상의 한의학적 치료에 대한 국내외 임상연구 동향 분석)

  • Objectives This study aimed to review clinical studies on traditional Korean medicine treatment for medial collateral ligament injury of the knee. Methods Clinical studies on Korean traditional medicine treatment of medial collateral ligament injury were conducted. We used five Korean online databases (OASIS, KISS, RISS, DBPia, and ScienceOn) and three foreign databases (PubMed, Cochrane Library, and CNKI). Out of 99 studies that were found, we excluded repeated articles, studies that were not related to Korean medicine, and those not relevant to the topic of the study. Results Ten randomized controlled trials and 20 case studies were selected. Eight traditional Korean medicine treatments, including acupuncture, herbal medicine, chuna, and herbal ointment, were used in these studies. The most commonly used treatment was found to be acupuncture. Conclusions Our study showed that traditional Korean medicine for medial collateral ligament injuries was effective. However, there were some limitations. Further clinical studies and randomized controlled clinical trials are needed for more evidence on Korean traditional medicine.

  • Syndesmotic injuries are found frequently in clinical practice, and they remain controversial because of the variety of diagnostic techniques and management options. Bony avulsions or malleolar fractures are commonly associated with syndesmotic disruptions. Even unstable isolated syndesmosis injuries are associated with a latent or frank tibiofibular diastasis and should not be ignored in the early phase. A relevant instability of the syndesmosis with diastasis results from collateral ligaments tears and requires operative stabilization. The treatment involves an anatomic reduction of the distal tibiofibular articulations followed by stable fixation. Syndesmotic transfixation screws or suture button implants are being proposed as a means of fixation. Recently, suture button fixation has shown more favorable outcomes, but the outcomes can still be controversial. Syndesmotic malreduction can lead to hardware failure, adhesions, heterotopic ossification, tibiofibular synostosis, chronic instability, and posttraumatic arthritis. In particular, the correct diagnosis and evidence-based treatment options for unstable syndesmotic injury should be considered.

  • Carpal tunnel syndrome (CTS) caused by pseudogout is an uncommon disease. The authors report a 65-year-old female who complained of sudden pain and neurological symptoms on her left hand. Surgical decompression was performed. In the histologic study, a calcium pyrophosphate dihydrate crystal deposit was confirmed. Her pain and neurological symptoms were relieved. Because CTS caused by pseudogout is rare, it is difficult to differentiate it from other diseases. This paper reports an uncommon case of CTS caused by pseudogout.

  • This study was performed to provide the dress form for high school girls' clothing construction which can reflect the characteristics of their body. Two hundred and forty seven high school girls aged from 15 In 17 were directly measured. The results obtained from this study were as follows ; 1. High school girls' somatotype was similar to that of young adult women. They had the features of well developed upper trunk and well developed breadth, but less developed depth. 2. The correlation between length item and the other items was small enough to interpret that each item was independent. There was a Positive correlation between depth item and girth item. And, Observed correlation between weight and Rohrer's index and breadth, depth and girth item was much larger than expected, which showed that weight and Rohrer's index influenced on fatness factor. 3. Through factor analysis, six factors were extracted from anthropometric measurements. The first factor was representing upper body volumn which expressed the characteristics of somatotype better than any other factors, the second factor was breadth, the third factor was size, the fourth factor was length, the fifth was back shape and the sixth factor was shoulder shape. 4. For development of dress form for high school girls, directly anthoropometric measurements were used. The dress form constructed through this study represented the characteristics of high school girls, therefore this would be very useful to manufacture clothing for them in the future.

Effects of Cervi Parvum Cornu on the Cell Cycle Regulation in Human Periodontal Ligament Cells (녹용이 치주인대세포의 세포주기조절에 미치는 영향)

  • Cervi Parvum Cornu(CPC) is that the young horn of deer family and has been traditionally used as a medicine in Eastern. The purpose of present study was to investigate the effects of CPC on cell cycle progression and its molecular mechanism in human periodontal ligament cells (HPOLC). In cell proliferation assay, 1 ng/ml, 10 ng/ml, 100 ng/ml, 1 ㎍/ml and 10 ㎍/ml of CPC were used, all treatment groups increased the cell growth. Maximal cell proliferation was observed in cells exposed to 100 ng/ml of CPC at 4 day, and 10 ng/ml and 100 ng/ml of CPC at 6 days. S phase was increased and G1 phase was decreased in the group treated with 100 ng/ml of CPC in cell cycle analysis. The protein levels of cyclin D1 were not changed, but the levels of cyclin E, cdk 2, cdk 4 and cdk 6 were increased. The protein levels of p21, pRb were decreased as compared to that of control group, but the levels of p53 was not changed in the cells both treated with CPC Md untreated. These results suggested that CPC increases the cell proliferation and cell cycle progression in HPDLC, which is linked to an increased cellular levels of cyclin E, cdk 2, cdk 4 and cdk 6, and decreased the levels of p53, p21.

Alteration of Trabecular Bone Microarchitecure at Tibial Epiphysis due to Knee Joint Instability by Anterior Cruciate Ligament Rupture: Difference between Medial and Lateral Part (전방십자인대 손상으로 인한 슬관절 불안정성에 따른 경골 골단 해면골 미세구조 변화 : 내방과 외방에서의 해면골 미세구조 패턴 변화)

  • Knee joint instability by anterior cruciate ligament(ACL) rupture is allowing the abnormal loading condition at the tibial epiphysis locally, resulting in producing locally different bone bruise. The study examined difference between local alteration patterns of trabecular bone microarchitecture at medial and lateral parts of the tibial epiphysis by ACL rupture. Fourteen SD rats were divided into Control(CON; n = 7) and Anterior Cruciate Ligament Transection(ACLT; n = 7) groups. The tibial joints were then scanned by in vivo ${\mu}$-CT at 0, 4, and 8 weeks post-surgery. The results showed that alteration pattern on trabecular bone microarchitecture at medial part was significantly higher than that at lateral part of the tibial epiphysis in ACLT group from 0 to 8 weeks(P < 0.05). Tb.Th and Tb.Sp distributions were well corresponded with differences between aforementioned trabecular bone microarchitectural alteration pattens at medial and lateral parts of the tibial epiphysis in ACLT group from 0 to 8 weeks(P < 0.05). These findings suggest that the alteration patterns of trabecular bone microarchitecture should be locally and periodically considered, particularly with respect to the prediction of bone fracture risk by ACL rupture. Improved understanding of the alteration patterns at medial and lateral trabecular bone microarchitectures at the tibial epiphysis may assist in developing more targeted treatment interventions for knee joint instability secondary to ACL rupture.

  • Mastication is the process to help digestion by chewing or grinding food. Masticatory system consists of maxilla, mandible, temporomandibular joints, ligaments, dentitions, and musculatures. Assessing the bite force can be one of the methods to estimate the masticatory system. Bite force is influenced by facial morphology, age, sex, periodontal status, temporomandibular joint disorder and dental condition, and so forth. In general, higher maximum bite force is seen in those who have a square-shaped face and in male rather than female. In addition, bite force tends to be increased by age 20, maintained constantly until age 40 - 50, and then decreased. Periodontal disease is known as a causative factor for decreased bite force while temporomandibular disorder (TMD) remains controversial as to whether it affects the force. The status of teeth is considered as an important factor to determine the maximum bite force.

Effects of Knee Brace on the Anterior Cruciate Ligament Injury Risk Factors during Spike Take Off in Female Volleyball Players (여자 배구 선수들의 스파이크 도약 시 무릎보호대가 전방십자인대 부상위험 요인에 미치는 영향)

  • In volleyball, the most common injuries are anterior cruciate ligament (ACL) tears. For this reason, volleyball players frequently use knee brace as prophylactic and rehabilitation measures. The purpose of the study was to investigate the effects of knee brace on anterior cruciate ligament injuries risk factors during spike take off in female volleyball players. Fifteen female volleyball players were recruited and performed randomly spike take off with and without knee brace. Kinematics and ground reaction data were collected to estimate the anterior cruciate ligament injuries risk factors. The ACL risk factors are knee maximum flexion angle, thigh maximum adduction angle, thigh maximum internal rotation angle, shank maximum abduction angle, shank maximum external rotation angle, knee maximum extension moment and knee maximum abduction moment. Data were analyzed with paired samples t-test with Bonfferoni collection. Female volleyball players with knee brace had no significant results in knee maximum flexion angle, thigh maximum adduction angle, thigh maximum internal rotation angle, shank maximum abduction angle and shank maximum external rotation angle compare to without knee brace. Female volleyball players, however, with knee brace showed more reduced knee maximum extension moment and knee maximal abduction moment than without knee brace. In conclusion, Female volleyball players with knee brace reduced anterior cruciate ligament stress.

  • The maxillary protraction headgear has been widely used in the treatment of skeletal Class III children with maxillary deficiency. A variety of treatment objectives which allow dentoalveolar movements may be established, but when only maxillary protraction without dentoalveolar movement is needed, one of the limitations in maxillary protraction with conventional tooth-borne anchorage is the loss of dental anchorage. This is because a bone remodeling occurs not only at circummaxillary sutures but also within the periodontal tissues. During protraction treatment in the mixed dentition phase, in older children or for the patient with multiple congenitally missing teeth, it is not uncommon to observe undesirable mesial movement of maxillary teeth. Such a side effect can be eliminated or minimized using absolute anchorage such as skeletal anchorage. The purpose of this case report is to introduce a new technique of the maxillary protraction headgear treatment using surgical miniplates.

Usefulness of Anteromedial Portal for Femoral Tunneling in Anterior Cruciate Ligament Reconstruction (전방십자인대 재건술시 대퇴골 터널에 있어 전내측 삽입구의 유용성)

  • Purpose: Recent development and advances in the arthroscopic surgical techniques for anterior cruciate ligament(ACL) reconstruction have led to the ideal location for more oblique anatomic point of the femur from 10 to 10:30 o'clock(in the right knee) and from 2 to 1:30 o'clock(in the left knee) in the frontal plane. This study was performed to compare the operative methods and the radiologic results of the femoral tunnels made through the tibial tunnel(trans-tibial approach) and the anteromedial portal. Materials and Methods: From January 2003 to May 2004, on hundred reconstructions of ACL were performed. Group I(the femoral tunnel made through the tibial tunnel) consisted of 50 cases and group I(the femoral tunnel made through the anteromedial portal) consisted of 50 cases. The operative methods and the radiographic results of the femoral tunnels were compared. Results: Femoral tunnel was made more easily at more oblique anatomic point in group II than in group I. In group II, better visual field was achieved at the angle of 100? flexion of the knee joint, the risks of the posterior cortical breakage and the tunnel-graft mismatching were reduced more, and the divergence of femoral interference screw from the radiograph decreased more than in group I(p<0.05). The angle between the femoral tunnel and the longitudinal axis of ACL increased in group II. Conclusion: Anteromedial portal technique was useful for femoral tunneling toward 10 to 10:30 o'clock(in the right knee) and 2 to 1:30 o'clock(in the left knee) in ACL reconstruction.
