  • urpose : The renin-angiotensin system (RAS) has been demonstrated to play a major role in regulating blood pressure. Therefore, components of the RAS are likely candidate genes that may predispose an individual to essential hypertension and cardiovascular complications. Among them, the M235T polymorphism of the angiotensinogen gene has been speculated to be associated with elevated circulating angiotensinogen concentrations and essential hypertension. This study aimed to analyze the angiotensinogen M235T polymorphism in hypertensive adolescents and investigate its relationship with cardiovascular risks. Methods : Forty Korean hypertensive adolescents (aged 16-17, systolic $BP{\geq}140 mmHg$ and/or diastolic $BP{\geq}90 mmHg$) and fifty seven normal adolescents were included. Obesity index (OI), body mass index (BMI) were calculated. BP was measured by oscillometric methods in resting state. Polymerase chain reaction (PCR) technique was performed on DNA from the hypertensives subjects to analyze the M235T polymorphism. Serum homocysteine, insulin, renin, aldosterone and angiotensin converting enzyme (ACE) were tested according to each genotype. The carotid intima-media thickness (IMT) and carotid artery diameter, Pulse wave velocity (PWV) and ankle-brachial index (ABI) were measured according to each genotype. Results : Genotype frequencies of T/T, M/T and M/M were 62.5%, 35.0%, 2.5%, respectively in hypertensive adolescents. The results were not significantly different compared to control group. Serum insulin, renin levels, BMI and OI were significantly higher in thoses with the M/M genotype as compared to those with the T/T of M/T genotype. Conclusion : This study showed that the M235T polymorphism was not associated with essential hypertension or any cardiovascular risks. Further clinical research is required to ascertain the relationship between this polymorphism and cardiovascular complications in Korean hypertensive adolescents.

  • The gallbladder(GB) polyps have malignant potential and nowadays are increasing due to the extended use of ultrasonography. Even though there are various reports on the risk of GB polyps, there is few study on the relationship between the presence of metabolic syndrome including serum biochemistry, lifestyle and the development of gallbladder polyps in korean population. The aim of this study is to establish a relationship between the presence of metabolic syndrome including serum biochemistry and the development of gallbladder polyps in korean population. This study conducted on 151 men and 167 women who visited a musculoskeletal hospital in Gyeonggido to have an ultrasound scan of the abdomen from November 2, 2012 to January 31, 2014. They are randomly selected to evaluate the prevalence risk factors were analyzed by comparing various variables between the GB polyp positive and negative. The collected data were analyzed using the personal computer program SPSS version 20.0 for windows. The mean size of the GB polyps was 3.92mm in diameter. Size of GB polyps were less than 10mm in diameter. As to frequency in the people with the GB polyps, the number of patients with the GB polyps was 184(57.9%): the number of metabolic syndrome of them was 114(26.4%); these results showed that risk of the GB polyps got higher at patients with metabolic syndrome. As to the risk of the GB polyps, people with the GB polyps had significantly higher risk than those without the GB polyps, as follows: The risk was significantly 1.061 times higher by weight; 1.186 by BMI; 1.060 by systolic blood pressure; 1.030 by diastolic blood pressure; 1,009 by Total cholesterol; 2.736 by metabolic syndrome; 1.011 by blood sugar on an empty stomach; 1.064 by ESR. The risk factors of GB polyps were found to be weight, BMI, total cholesterol, Triglycerol, Glucose, systolic blood pressure, diastolic blood pressure, ESR and metabolic syndrome. This study has a possibility to be used an indicator to identify the GB polyps, and in the future larger-scale study will have to be conducted to objectify it.

  • The objective of this study was to evaluate associations of dietary sodium (Na) intake with salt-related dietary behaviors of 218 university students (95 men; 123 women) living in Gyeonggi area. Dish frequency questionnaire (DFQ) was used to identify salt-related dietary behaviors and to determine Na intakes. In men, systolic & diastolic blood pressures, Na intakes and DFQ-15 scores were significantly higher than in women. High-salt intake group (HS), classified by DFQ-15, had higher scores of high-salt dietary attitude and more Na intakes than low-salt intake group (LS). HS took protein foods and had balanced diets less frequently than LS (p<0.05). HS had fried dishes & fatty meats, and added salt to dishes more frequently (p<0.05). HS and LS had differences in preference of soy-boiled and Chinese or Japanese foods, in intake frequency of bean-paste soup, in use of soy sauce with fried food or raw fish, and in salt addition to dishes at the table (p<0.05). HS, classified by Na intakes, had high-salt dietary attitudes such as preference of seasoned rice & soy-boiled foods and habitual addition of soy sauce or salt to dishes at the table. The subjects using food labels when purchasing had better salt-related attitudes & behaviors, and lower DFQ-15 scores & Na intakes than the non-users (p<0.01). Self-assessed HS (SHS) had worse salt-related attitudes and behaviors (p<0.05). Male self-assessed LS (SLS) had higher Na intakes, which indicated that self-assessment of salt preference did not actually reflect Na intake. In summary, male university students belonged to a high-risk group of salt intakes, and HS preferred soy-boiled foods or fatty dishes, frequently added salt to dishes and rarely had balanced diets. These results suggest that nutrition education programs for university students should include fundamental dietetics and a balanced diet, in addition to a low-Na diet.

  • This study was conducted to investigate the effect of sodium intake on serum lipids and glucose. 20 young adult women were fed the diets containing 290.5 mEq (high-Na diet) and 51.3 mEq (low-Na diets) Na for 6 days, respectively. Serum lipids, glucose, and other parameters after high-Na diet and low-Na diet were compared. The results would be summarized as follows. The mean age, body weight, height, and blood pressure of the subjects were 22.9$\pm$2.5 years,54.7+6.6 kg, 160.0$\pm$4.8 cm, 110.3$\pm$7.7/67.5$\pm$19.7 mmHg, respectively. Body weight, BMI, and diastolic blood pressure were significantly higher at the end of high-Na diet than of low-Na diet (p<0.001 p<0.001, p<0.05). However, there were not significantly different in height and systolic blood pressure between high- and low-Na diet. Serum cholesterol, LDL-cholesterol, and HDL-cholesterol were not significantly different with Na intakes. Serum triglyceride was significantly higher at the end of high-Na diet than of low- Na diet (p<0.05) Serum apo A-I was significantly decreased in low-Na diet, while apo B was increased (p<0.001, p<0.001). Thrombin time and prothrombin time, blood aggregation time were significantly faster following low-Na diet (p<0.001, p<0.05). There was not significantly different in serum glucose between high- and low-Na diet. However, serum insulin was significantly higher following low-Na diet (p<0.01). It is concluded that diastolic blood pressure, serum triglyceride, serum apo A-I, blood aggregation time were decreased in low-Na diet, while serum apo B and serum insulin were increased. These results suggest that Na-restricted diet affects not only blood Pressure but other biochemical parameters in blood. Therefore, for the patients who need restricted Na diet, it would be suggested that various biochemical changes should be carefully considered along with dietary Na manipulation.

  • The aim of the present study was undertaken to investigate the association between diagnostic indices of metabolic syndrome(MetS) with carotid intima-media thickness using ultrasonography. The participants in the study were 315 male employees without carotid atherosclerosis and other cardiovascular disease. This study was approved by the Institutional Review Board of Occupational Safety and Health Research Institute. Written informed consent for the participants in this study was obtained from all individuals. Anthropometric parameters and biochemical characteristics were done using each specific equipments and the NCEP-ATP III criteria were used to define MetS. They were examined by B-mode ultrasound to measure the carotid intima-media thickness(carotid IMT) at the near and far walls of common carotid and bifurcation(bulb). The mean carotid IMT was $0.739{\pm}0.137\;mm$ and it's thickness significantly increased with the increase in age. Also, amounts of systolic and diastolic blood pressure, triglyceride and fasting glucose were significantly increased with the increase in age. Carotid IMT were significantly correlated with BMI(r=0.170, p=0.004), systolic(r=0.148, p=0.011) and diastolic blood pressure(r=0.123, p=0.036) and HDL-cholesterol(r=-0.164, p=0.005). On multiple logistic regression analysis for the diagnostic indices of MetS, carotid IMT were significantly associated with blood pressure(OR=4.220, p<0.01) and MetS(OR=1.301, p<0.05). The results indicate that blood pressure and MetS are important risk factors for carotid atherosclerosis.

  • A cross-sectional analysis of blood pressure (BP) changes and the current prevalence and distribution of hypertension among age groups in Korea were conducted. Systolic (SBP), diastolic (DBP), and pulse (PP) pressures were evaluated. Residents visiting a local Community Health Center for a health check-up and/or participating in the Health Promotion Program were included for the analyses. A total of 6,570 subjects (2,809 men, 3,761 women) were divided into five age groups: 30-39, 40-49, 50-59, 60-69, and $70\leq$ years. Comparisons were made between sex and among age groups. SBP and DBP were continuously elevated, while men showed higher levels than women until their mid-60s; then, the trend was reversed. DBP in men became elevated from their 30s, reaching the highest in the 40s then slowly decreasing. DBP in women increased up to their 40s and 50s, then reached a plateau. PP increased rapidly from the 50s in both sexes. The major contributor of age-related BP elevation may be large artery stiffness in men, particularly from their 50s, while the blood pressure elevation of women may relate to diverse factors including large artery stiffness, stroke volume, and ventricular ejection rate. The rapid elevation of PP after the 50s in both men and women could represent a risk of cardiovascular or coronary mortality, particularly with increasing age. The current data showed a higher rate of hypertension in both sexes compared to previous reports. When those with both high-normal BP and hypertension are looked at in combination, BP management of the target population should be drastically elevated.
