Recently, many studies have introduced artificial intelligence systems in the surgical process to reduce the incidence and mortality of complications in patients. Bleeding is a major cause of operative mortality and complications. However, there have been few studies conducted on detecting bleeding in surgical videos. To advance the development of deep learning models for detecting intraoperative hemorrhage, three models have been trained and compared; such as, YOLOv5, RetinaNet50, and RetinaNet101. We collected 1,016 bleeding images extracted from five surgical videos. The ground truths were labeled based on agreement from two specialists. To train and evaluate models, we divided the datasets into training data, validation data, and test data. For training, 812 images (80%) were selected from the dataset. Another 102 images (10%) were used for evaluation and the remaining 102 images (10%) were used as the evaluation data. The three main metrics used to evaluate performance are precision, recall, and false positive per image (FPPI). Based on the evaluation metrics, RetinaNet101 achieved the best detection results out of the three models (Precision rate of 0.99±0.01, Recall rate of 0.93±0.02, and FPPI of 0.01±0.01). The information on the bleeding detected in surgical videos can be quickly transmitted to the operating room, improving patient outcomes.
This study presents a new hybrid fNIRS-EEG system to meet the demand for a lightweight and low-cost sleep pattern monitoring device. For multiple-channel configuration, a six-channel electroencephalogram (EEG) and a functional near-infrared spectroscopy (fNIRS) system with eight photodiodes (PD) and four dual-wavelength LEDs are designed. To enhance the convenience of signal measurement, the device is miniaturized into a patch-like form, enabling simultaneous measurement on the forehead. Due to its fully integrated functionality, the developed system is advantageous for performing sleep stage classification with high-temporal and spatial resolution data. This can be realized by utilizing a two-dimensional (2D) brain activation map based on the concentration changes in oxyhemoglobin and deoxyhemoglobin during sleep stage transitions. For the system verification, the phantom model with known optical properties was tested at first, and then the sleep experiment for a human subject was conducted. The experimental results show that the developed system qualifies as a portable hybrid fNIRS-EEG sleep pattern monitoring device.
Functional brain imaging techniques have been used to diagnose psychiatric disorders such as dementia, depression, and autism. Recently, these techniques have also been actively used to study hearing loss. The present study reviewed the application of the functional brain imaging techniques in auditory research, especially those focusing on hearing loss, over the past decade. EEG, fMRI, fNIRS, MEG, and PET have been utilized in auditory research, and the number of research studies using these techniques has been increasing. In particular, fMRI and EEG were the most frequently used technique in auditory research. EEG studies mostly used event-related designs to analyze the direct relationship between stimulus and the related response, and in fMRI studies, resting-state functional connectivity and block designs were utilized to analyze alterations in brain functionality in hearing-related areas. In terms of age, while studies involving children mainly focused on congenital and pre- and post-lingual hearing loss to analyze developmental characteristics with and without hearing loss, those involving adults focused on age-related hearing loss to investigate changes in the characteristics of the brain based on the presence of hearing loss and the use of a hearing device. Overall, ranging from EEG to PET, various functional brain imaging techniques have been used in auditory research, but it is difficult to perform a comprehensive analysis due to the lack of consistency in experimental designs, analysis methods, and participant characteristics. Thus, it is necessary to develop standardized research protocols to obtain high-quality clinical and research evidence.
We developed a device that integrates digital otoscope and stethoscope for telemedicine. The integrated device was utilized for the collection of tympanic membrane images and cardiac auscultation data. Data accumulated on the platform server can support real-time diagnosis of heart and eardrum diseases using artificial intelligence. Public data from Kaggle were used for deep learning. After comparing with various deep learning models, the MobileNetV2 model showed superior performance in analyzing tympanic membrane data, and the VGG16 model excelled in analyzing cardiac data. The classification algorithm achieved an accuracy of 89.9% for eardrums data and 100% for heart sound data. These results demonstrate the possibility of diagnosing diseases without the limitations of time and space by using this platform.
Liver cancer is one of the highest incidents in the world, and the mortality rate is the second most common disease after lung cancer. The purpose of this study is to evaluate the diagnostic ability of deep learning in the classification of malignant and benign tumors in CT images of patients with liver tumors. We also tried to identify the best data processing methods and deep learning models for classifying malignant and benign tumors in the liver. In this study, CT data were collected from 92 patients (benign liver tumors: 44, malignant liver tumors: 48) at the Gil Medical Center. The CT data of each patient were used for cross-sectional images of 3,024 liver tumors. In AlexNet and VggNet, the average of the overall accuracy at each image size was calculated: the average of the overall accuracy of the $200{\times}200$ image size is 69.58% (AlexNet), 69.4% (VggNet), $150{\times}150$ image size is 71.54%, 67%, $100{\times}100$ image size is 68.79%, 66.2%. In conclusion, the overall accuracy of each does not exceed 80%, so it does not have a high level of accuracy. In addition, the average accuracy in benign was 90.3% and the accuracy in malignant was 46.2%, which is a significant difference between benign and malignant. Also, the time it takes for AlexNet to learn is about 1.6 times faster than VggNet but statistically no different (p > 0.05). Since both models are less than 90% of the overall accuracy, more research and development are needed, such as learning the liver tumor data using a new model, or the process of pre-processing the data images in other methods. In the future, it will be useful to use specialists for image reading using deep learning.
뇌신경계 인터벤션 시술은 장시간의 시술로 인해 피부의 수포, 탈모, 홍반 등의 방사선 피폭으로 인한 위해가 빈번히 보고되고 있다. 인체공학적으로 제작된 Bismuth (원자번호 83;Bi) 차폐체를 뇌혈관계 인터벤션 시술에 적용함으로써 의료방사선 피폭으로부터 두피 및 수정체의 방사선 피폭을 최소화하고자 하였다. 측정 부위는 4부위로 후두부(9 points), 양쪽 측두부(12 points), 양쪽 수정체부(6 points), 코 끝부(6 points)이며, 측정 소자는 광자극 형광 선량계(Optically Stimulated Luminescence Dosimeter: OSLD)를 각 지점(points)에 측정기를 부착 후 자체 제작된 Bismuth차폐 기구를 사용 전(A그룹)과 후(B그룹)를 측정한 후 피부표면선량(entrance surface dose)을 비교 분석하였다. A 그룹(Bismuth unshield)과 B 그룹(Bismuth shield)의 피부선량 평균은 A 그룹은 92.44 mGy였고, B 그룹은 67.55 mGy로 측정되었다. A 그룹에 비해 B 그룹에서 평균 26.92% 감소되었다. 후두부의 피부선량 평균은 A 그룹(9 point)은 146.08 mGy, B 그룹(9 point)은 103.23 mGy로 측정되었고 A 그룹에 비해 B 그룹에서 평균 29.32 % 감소하였다. 측두부의 피부선량 평균은 A 그룹(6 point)은 101.90 mGy, B 그룹(6 point)은 72.69 mGy로 측정되었고 A 그룹에 비해 B 그룹에서 평균 28.67% 감소하였다. 수정체부의 피부선량 평균은 A 그룹(3 point)은 27.51 mGy, B 그룹(3 point)은 21.39 mGy로 측정되었고 A 그룹에 비해 B 그룹에서 평균 22.26% 감소하였다. Bismuth 차폐체의 사용은 뇌혈관 중재적 시술 후 나타날 수 있는 일시적 탈모 및 기타 확률적 영향에 따른 방사선 장해를 감소시킬 수 있는 대안이 될 것으로 사료된다.
본 연구는 머리운동 제어 시스템에 대한 생체 물리학적인 모델링과 시뮬레이션을 통하여 모델의 동특성을 조사하였으며, 생체운동(머리-안구)의 상호 관계를 비교하였다. 결과를 요약하면 다음과 같다. 1) 시스템을 구성하고 있는 매개변수의 변화가 출력에 미치는 영향을 조사하기 위해 감도해석법을 써서 감도계수를 구한 결과, 입력 제어신호의 펄스높이(PH), 동근의 펄스폭(PW1), 길항근의 펄스폭(PW2)이, 시스템의 출력특성을 결정하는 가장 중요한 매개변수 임을 알 수 있었다. 2) 비 선형 제어모델의 선형화를 통하여 선형모델의 응답특성과 크나큰 차이가 없음을 알았다. 3) Main-Sequence도를 작성하여 실험 데이터와 비교한 결과, 거의 일치함으로서 모델의 타당성을 입증하였다. 4) 머리운동이 시간최적으로 응답하기 위해서는 bang-bang 제어법칙이 적용되어야 함을 알았다. 5) 머리운동이 목표점에 도달하는 순간에는 길항근의 역제통 펄스가 가해 짐으로서, 길항근이 궤적의 마지막 부분을 지배함을 알았다. 6) 머리-안구운동의 main-sequence도를 비교 함으로서 상호관계를 규명하였다. 앞으로는 이 모델링 법을 개선확장하면, 비행시 파일럿의 생체 시뮬레이션, 헬멧 조준 사격 시스템등의 항공공학, 생체의용공학 연구 및 제어입력을 생체신호로 하는 로봇틱스 연구에 본 연구 방법은 유용하리라 생각된다.
Cephalosporium 배양시 균체의 형태학적 측면을 고려한 cephalosporin C 생합성에 대한 모델식을 제안하였다. 제한기질로 glucose와 methionine을 고려한 double-substrate 모델을 설정하였는데 여기서 glucose는 균체의 증식 정도를, methionine은 균체의 증식속도를 제어하게 된다. Cephalosporin C의 생산은 균체의 형태학적 분화와 밀접한 관계가 있다. 한편 cephalosporin C의 생산성을 증대시키기 위해 모델식을 유가식 배양에 응용하였다.
스트레스를 측정하는 방법에 대한 연구는 끊임없이 진행되고 있다. 이토록 연구가 계속 진행되고 있다는 것은 우리의 삶에 항상 존재하면서도 해결하기 쉽지 않은 문제이기 때문이다. 스트레스에 대한 개념 정립이 아직 확실하지 않고, 다양한 해석을 하고 있기 때문에 접 근방법도 다양하다. 그리고 개인에 따른 스트레스 반응의 차이가 많고, 여러 가지 요인들이 관여하기 때문이다. 본 논문은 심리적 영향과 생체적 영향을 통합해서 지수로 나타내어 스트레스에 대한 요인을 분석하고 해석 할 수 있는데 도움이 될 수 있는 변형 지수 알고리즘을 소개한다. 통계 자료와 특정 집단에서 이용되고 있는 측정도구를 중심으로 스트레스의 정도를 지수로 표현하는 알고리즘을 연구하였다.
스트레스 정도를 측정하기 위한 다양한 도구들이 많이 사용되고 있다. 스트레스 측정에는 심리측정과 생체측정이 있다. 어느 한쪽만을 고려하였을 경우 주관적이거나 객관적 결여가 발생한다. 그 문제점을 보완하기 위해 두 측정을 혼합한 새로운 스트레스 지수가 필요하다. 개인적 특성에 따라 측정 결과도 다양하게 나타나지만, 현재 사용되고 있는 측정도구를 바탕으로 한 일반적인 경우를 고려하여 연구하였다. 심리와 생체 측정 도구를 통해서 얻어지는 지수를 이용하여 통합적인 스트레스 지수를 얻는다. 그래서 임의로 4가지의 스트레스 측정도구를 사용한다. 각 측정도구에서 나온 결과에 대한 멀티지수가 나타나는데, 그 지수들을 각각 좌표평면에 나타내어 스트레스에 대한 균형성과 편향성을 분석하고 진단한다.
