이 연구는 선주, 선원노동조합, 정부기관, 선원교육기관 등 선원유관단체들의 선원보건의료 실태와 선내 응급의료지원 실태 등에 대한 인식도 조사를 실시하여 의료사각지대인 선내의료 및 선원 건강관리 여건을 개선하기 위한 방향을 제안하고, 향후 선내 의료지원제도 개선방향에 대한 기초자료 제시와 개선점을 찾는 것을 목적으로 한다.

2006년 정부에서 암정복 10개년 계획을 발표하면서 추진전략의 하나로 암환자 재활 및 완화의료 지원 강화가 포함되었다. 2008년부터 '말기암환자 전문의료기관 지정고시'를 제정하여 인력, 시설, 장비기준을 충족할 경우 완화의료전문기관으로 지정하여 사업평가 및 운영비를 지원하고 있다. 2010년 암관리법이 개정 공포되면서 '완화의료제도' 관련규정 또한 강화되었으며, 2011년 대상자, 사업범위, 인력기준, 시설기준 등이 포함된 시행령 및 시행규칙을 개정 공포함에 따라 말기암환자 완화의료 전문기관에서는 이 규정에 따라 평가 및 운영되고 있다. 말기암환자는 호스피스완화의료의 일부이지만 현재 정부의 제도안에서 시행되는 법령이기에 호스피스완화의료기관 운영에 도움이 되기를 바라며 암관리법 중 완화의료관련 규정을 소개하고자 한다.

This study evaluates the degree of the inequality of medical care expenditure and private health insurance benefits and the relation with household income inequality in korea health care system. This study used the 2014 korea Health Panel survey, and study method is Gini coefficient. The main results are as follow. First, average household income in 1st income quartile is 6,290,000won and 10st income quartile is 101,930,000won. And Gini coefficient of Korea household income is 0.3756. In other words, family income inequality is quite serious. Second, the Gini coefficient of the public institution supported medical care expenditure, such as health insurance and public assistance, is 0.0761, and the Gini coefficient of the expenditure of transportation fee and medical materials etc that don't supported is 0878. The inequality in medical care expenditure in public health care system and without public support aren't serious all. Third, Gini coefficient in excluding household medical care expenditure from household income slightly increased. That is, the medical care expenditure of our country household is the factor of aggravating the inequality of household income. Fourth, Gini coefficient of private health insurance benefits is 0.0927. Therefore, the ineqality in private insurance benefits is low. In addition, the Gini coefficient of the sum of private insurance benefits and household income is 0.3672. it decrease from Gini coefficient(0.3756) of household's. Private health insurance perform the functions somewhat weaken household income inequality. However, it is very little improvement.

의료기기 산업은 시장규모와 생산규모에서 미국이 전 세계의 $40\%$ 이상을 차지하고 있어 국내외 모든 의료기기 제조업체에서는 미국시장진출을 목표로 꾸준히 노력하고 있는 실정이다. 그러나 의료기기는 인체에 직접 적용된다는 특성으로 인해 각 국가의 정부는 저마다 다른 엄격한 기준으로 자국내 시장진출을 규제하고 있으며 엄격한 관리제도를 유지하고 있다. 국내 최신의 전자 의료기기는 필수적으로 의료기기를 제어하기 위한 의료용 소프트웨어를 필요로 하므로 의료기기 산업의 발전은 의료용 소프트웨어 산업의 발전에 직결된다고 할 수 있다. 그러나 의료기기의 경우에는 인$\cdot$허가를 위한 제도 및 절차가 갖추어져 활용되고 있는 반면, 의료용 소프트웨어의 경우에는 적합한 평가기준이 마련되어 있지 못하여 제도 및 체계구축의 필요성이 대두되고 있다. 본 논문에서는 국제표준(ISO/IEC 12119, ISO/IEC 9126을 바탕으로 평가메트릭을 구축하여 의료용 소프트웨어의 정량적인 품질을 측정, 평가하여 품질측정의 결과를 토대로 품질을 인증하는 의료응용 소프트웨어 시험인증 지원도구를 설계 및 구현하였다.

병원은 다양한 의료서비스를 제공하기 위해 의료기관 별 운영 전략에 따라 각기 다른 방식으로 운영된다. 특히 종합의료시설은 중증도가 높은 환자를 대상으로 수술이나 입원 등의 의료서비스를 효율적으로 제공하고자 그에 따른 운영 전략이 상이하다. 이러한 병원의 운영방식에 의해 결정되는 건축 역시, 건립 시기, 유형, 중증도에 따른 병원의 규모 등에 따라 시설 수준의 차이가 나타나며 이는 최근 정부에서 요구되는 제도 및 정책의 수용 여부를 결정짓는 중요한 요인이 되기도 한다. 최근 의료법은 기존 의료시설뿐만 아니라 신규 의료시설에 대한 설치 및 운영기준이 강화 추세에 있고, 서울시 또한 공공의료 확충을 위해 감염관리시설이나 필수의료시설 설치를 위한 용적률 인센티브 정책을 추진 중이다. 병원 운영 환경이 상이함에 따라 종합의료시설 설치 및 운영에 대한 인센티브제도의 적용 기준을 일괄적으로 적용하기에는 어렵다. 그러나 공익을 위해서 종합의료시설 인센티브제도를 지속 가능하게 운영하기 위한 객관적이고 합리적인 기준은 반드시 필요하다. 공공의료 기능 확충을 위한 서울시 종합의료시설 지구단위계획 수립·운영 기준은 공공과 민간이 모두 만족하는 의료환경 구축을 위해 매우 깊은 고민이 필요했다. 본 논설은 서울시 공공필요의료시설 확충 제도의 주요 내용과 공공필요의료시설 설치 및 운영 기준에 관해 소개하고자 한다. 특히 기준의 주요 내용을 정립하게 된 배경과 이유 등을 설명하여 본 제도의 의의를 정확히 전달하고 향후 보완해야 할 부분들을 검토하고자 한다.

호스피스-완화의료 사업에 대한 정부 차원의 지원이 점점 가시화되고 있다. 사업지원금을 제공하는 것과 함께 건강보험수가에 반영하기 위한 작업이 속속 진행되고 있으며, 이와 관련된 법률의 정비에 대해서도 관심이 높다. 이같은 의료제도의 변화 속에 호스피스 사업이 제대로 자리를 잡아가기를 기원하면서, 현재 논의되고 있는 사안에 대한 검토와 함께, 향후 부딪히게 될 문제점에 대하여 정리해 보고자 한다.

한국의 제도현실을 감안해보면, 호스피스 환자관리에 대한 표준지침 마련, 적정수가와 의료수송체계 관련 법률의 제정, 정부의 예산지원, 재가 암환자 관리사업의 활성화, 호스피스ㆍ완화의료 서비스 교육ㆍ홍보의 활성화 등에 대한 방안의 강구가 절실히 요청된다.

In this paper, occurrence tendency of the sick and wounded on board ship is analyzed. The medical management systems according to shipping companies are investigated. Furthermore, comparative analysis with the medical support system for seafarers in advanced country is carried out.

This study was performed to investigate health care system satisfaction and reform need using the data from the '2019 Health Care Experience Survey'. For 8,349 data with experience in medical use, health care system satisfaction and reform need level was analyzed by t-testing and ANOVA by characteristics of the study subjects, and multiple regression was conducted. Research has shown that health care users' recognition of the health care system is relatively low compared to reliability and satisfaction. It is necessary to promote policies and health care systems for senior citizens, low education levels. Since the reliability has the biggest impact on the satisfaction of the health care system, government should establish policies that they can trust, and in the process, they should gather opinions from the public and secure credibility through social consensus. Medical users were sympathetic to the need for reforms in the health care system, and felt the need to support vulnerable areas and vulnerable groups the most.
