The Study on the Social Expenditure of Medical Care and Medical Expenditure by Smoking (흡연에 의한 의료이용 및 의료비지출에 따른 사회적비용에 관한 연구)

  • In this study, only 2,877 men, 2,614 men (44.6%), and 143 women (2.3%) were selected as the subjects who were over 18 years old in response to the health consciousness of the Korean medical panel data in 2012. Emergency of smokers through medical use and medical expenditure data by smoking. The social costs were estimated through medical expenses according to the number of hospitalization and outpatient medical use. The social cost was calculated by summing the social expenditure on health care costs, insurer (corporation) costs, copayment, non - salary, and productivity costs by adopting the social perspective established by the health economist Rice (1968). The rate of annual emergency medical use by smoking status is 7.5% for smokers per 100 people, 9.8 times for use, and 809,003 won for social expenses. The annual rate of hospitalization per 100,000 population by smoking status was 9.6% for smokers per 100 population, 9 times for use, The social cost is 706,870 won. Annual smoking rate by smoking status was 68.6% for smoking, 9 cases for annual medical use,

  • RNA 정량검사는 혈중 바이러스 농도를 측정하는 검사로, 감염인의 투약과 치료에 필수적인 검사로써 감염인이라면 $3{\sim}6$개월 간격으로 정기적으로 받는 검사이다. 그동안 국가에서 시행해 왔던 RNA 정량검사가 2009년 7월 1일자로 민간 의료기관에서 시행된다. RNA 정량검사를 민간 의료기관에서 실시하게 됨에 따라 감염인의 진료비 부담이 증가하게 된다. 병원에 따라서는 감염인의 비급여 본인부담금을 고려하여 특진료를 가산하지 않는 병원도 있으나 이는 극히 일부 병원에 불과한 실정이다.

Differences of Cancer Patient's Health Care Utilizations between Medical Aid Program and National Health Insurance in the Elderly (노인 암환자의 건강보험과 의료급여 이용차이 분석)

  • This study to analyze differences of cancer patient's health utilizations in medical aid program and national health insurance by analysing health insurance claims data, and identify effects of health care systems. The majors results of the research were as follows. First, cancer patients in medical aid program more used total medical expenditures than in national health insurance mostly by many outpatient visits and long term hospitalization. Second, results of multiple regression, cancer patients in medical aid program more used total expenditures and inpatient expenditures. But, outpatient expenditures weren't different, cancer patients in medical aid program more visited medical institutions and hospitalized long term periods than in national health insurance. Therefore, it is too early to conclude that moral hazard is in health utilizations of medical aid program, because cancer patients in medical aid program many use in benefits for many nonbenefit burdens.

  • Objectives : The purpose of this study was to analyze the effect of outpatient cost sharing reduction on health care utilization of children under 6 years old. Methods : The data in this analysis was the health insurance claims data between August 2006 and July 2008. The study group was divided into two age groups, namely 0-5 years old and 6-10 years old. This study evaluated the impact of policy change on office visits per person and expenditure per visit. In order to do so, the double difference analysis is used. Results: The results showed that outpatient cost-sharing reduction has never really had a huge impact on office visits per person and expenditure per visit. Conclusions: This study showed that the outpatient cost sharing reduction for children under 6 years old policy is not working. Therefore, cost sharing of National Health Insurance by income class is needed.

A Study on the Insurance Contribution and Health Care Utilization of the Regional Medical Insurance Scheme (1개 군지역 의료보험제도에서의 보험료 부담수준별 병.의원 의료이용에 관한 연구)

  • This study was conducted to assess the equity in the regional insurance scheme through analysis of the computerized data from one regional insurance society and National Federation of Medical Insurance. We analysed the insurance contribution and benefit by the classes based on total and income-related contribution per household. The major findings of this study are as follows : 1. The average proportion of income-related contribution among the total was 39.2% and the upper classes show higher proportion of the income-related contribution. 2. The upper classes show higher health care utilization rate than the lower classes. It suggests that the lower classes have relatively large unmet medical needs. 3. The analysis through the Lorenz curve reveals that there exists transference of contributions from the upper to lower classes. But the cumulative percentage of insurance benefit is smaller than that of the number of the insured. It implies that regional medical insurance scheme in Korea has still some inequity in the context of social security principles.

  • This study tried to find the relationship between household income level and medical expense to household income ratio. For data analysis, it used 2010 and 2011 yearly data beta version of Korea Health Panel, co-managed by Korea Institute for Health and Social Affairs and National Health Insurance Corporation. To find out how the effect of independent variable changes in 2010 and 2011, the interaction effect between year and independent variable was examined, and separating the factors that showed interaction effect into each year, linear regression analysis was conducted using generalized estimating equations method. As a result of reviewing the factors that were related to medical expense to household income ratio among the people who used medical services, it was found that the higher the household income level, the lower the medical expense. It indicates that policy measures are needed to lessen the medical burden of low-income families.

  • Objectives: The purpose of this study was to analyze whether there are differences in medical expenses according to medical security type in the use of medical services with high disease burden such as coronary intervention. Methods: Chi-square test and covariance analysis(ANCOVA) were conducted to identify the differences in the characteristics and costs according to medical security type of 1,904 patients who underwent coronary intervention in a university hospital from 2011 to 2012. Hierarchical regression analysis was conducted to determine whether the cost affects medical expenses. Results: In the medical aid group, the proportion of women, those without a job, those without a spouse, and those who received hemodialysis was high, length of stay was high, patients using the emergency room and those who died was high. The medical aid patients were significantly higher in the non-benefit medical expenses, optional medical expenses, physician and admission, meals, medications and injections. National health insurance patients were significantly higher in procedure. The medical security type was found to be significant as a variable affecting the medical expenses. Conclusions: Provision of medical expenses should be managed in advance by providing prevention and education services for the vulnerable, and care services in the region should be provided to suppress the occurrence of medical expenses due to the increase in the number of days spent. In addition, it is necessary to support medical expenses to prevent unsatisfactory medical services from occurring for non-benefit and optional care.

  • 최근 저출산 문제로 인한 노동인구의 감소로 노동력이 감소하며 인건비가 증대되고, 의료 기술의 발달과 고령화로 인해 병실의 수용 인원이 증가했다. 특히 중환자, 거동이 불편한 환자, 노인 환자와 같은 간병인의 도움과 의료 종사자의 지속적인 진료의 필요성이 증대되고 있다. 환자의 상태를 일정 시간마다 확인해야 하는 불편함, 간병인 고용의 필요로 기본적인 병원비와 인건비가 증가되고, 의료업계 종사자들의 부담도 늘어나는 추세이다. 따라서 이러한 문제를 해결하기 위해 중앙 관제실에서 환자의 상태를 실시간으로 확인하고 자동으로 병실을 관리하는 시스템을 도입하고자 한다. 정보를 실시간으로 전달함으로써 의료진의 부담을 줄이고 위기 상황에 즉각적으로 대응하며, 자동 병실 환경관리와 환자 편의를 위한 의료 환경을 지원하는 시스템을 제안한다.

Utilization of Medical Assistance Patients in Nursing Hospital (의료급여환자의 요양병원 이용에 관한 연구)

  • The purpose of this study is to analyze the use of hospital, hospitalization, medical service, discharge and power of medical care patients who are concerned about moral hazard. We conducted focus group interview with 3 medical care patients and their families and 5 workers who had worked for more than 4 years in a nursing hospital. The main results and implications are as follows. First, admission to nursing hospitals was mostly based on the linkage between the medical institutions and the competition to attract the patients rather than the choice of the patients. Second, the main cause of the long-term hospitalization of medical assistance patients was the lack of social protection measures such as absences of residence and care giver, although there are factors that cause moral hazard such as low self-pay. Third, most of the patients were in need of treatment, but they were admitted to the hospital even though their needs were not higher than those of the health insurance patients. Fourth, the rehabilitation service is the mainstay of the medical service of the nursing hospital, and the roles of nursing staff and care givers are important. Fifth, medical care patients are paying medical expenses for nursing hospitals due to cost of living and family support, but they are exempted from the hospital expenses or the burden of their own expenses in the hospital. Sixth, public institutions and social welfare institutions have not managed continuously since commissioning patients to nursing hospitals and have neglected the connection with community services after discharge.

  • The determinansts of expenditure spent on medical care and the ratio of medical care spending to consumption expenditure were investigated using the 2002 House Income and Expenditure Survey by the Korea National Statistical Office, which consisted of a sample of 918 elderly households. There were significant differences in expenditure on medical care and the ratio of medical care spending to consumption expenditure between elderly and nonelderly households. Age, education, overspending were significant factors that determine the expenditure on medical care and the ratio of medical care spending to consumption expenditure. Overspending is the most important factor related to expenditure on medical care and the ratio of medical care spending to consumption expenditure.
