Development of e-learning Education Programs for Social Workers in Hospice and Palliative Care (호스피스완화의료 사회복지사 e-learning 교육과정 개발)

  • Education is essential for specialists in charge terminal care of cancer patients. In the second 10-year plan to conquer cancer under the Cancer Control Act, the Korean government announced a plan to train more hospice and palliative care (HPC) specialists based on quantitative HPC expansion. Specifically, the government aims to develop e-learning programs to foster social workers in HPC, following those offered to doctors and nurses. In HPC, social workers have served a vital role in helping patients overcome psychological and social issues. As professionals, they have carried out their responsibilities and played their part in the field to help HPC to take root and be institutionalized in Korea. To date, it has been difficult to obtain practical knowledge and skills for social workers due to the lack of systematic training program. Development of an e-learning program for social workers, as proposed in this study, should strengthen social workers in charge of terminally-ill cancer patients in terms of their identity, expertise, and practical skills in clinical setting and improve their access to education. We also hope the program to be further developed by the government by introducing an education system that offers refresher courses to guarantee social workers' continued expertise through.

The meaning and challenges of the introduction of the Customized Care Services for Older Adults (노인맞춤돌봄서비스의 도입 의미와 과제)

  • The South Korean government has introduced a new Customized Care Service for Older Adults by eliminating the six existing care services for them since 2020. It is a significant change of care service system for older adults and the service would play a key role in providing preventive services for them. The aim of the study is to examine the meaning and tasks of the introduction of the Customized Care Services for Older Adults in South Korea by using the framework of Gilbert and Terrell. The study found that the new service was designed under the government's policy contexts of the establishments of public-based social service providers and the emphasis of community care, and it has brought about a number of significant institutional changes of care system in terms of allocation, benefits, delivery, and finance. The institutional coverage of the service was increased to lower 70% of older adults and a large number of kinds of health and social care services would be provided for them. The sphere of living was set in each local area and the new service providers are entirely in charge of providing the services in each sphere. The financing system was changed to support service providers by providing government subsidies. Although a number of positive results would be expected to occur for the elderly, it is likely that some challenging tasks would happen in the fields as follows: the inadequate coverage, the vagueness of the application of self-care principle and the decrease in the amounts of direct care services, the inappropriate area of service provision, and the supplier-oriented provision of services rather than user-oriented.

Material Hardship of the Poor Families in Korea: The Distributions and Determinants of Material Hardship (빈곤층의 물질적 궁핍, 실태와 영향요인 분석)

  • Material hardship measures have been used to supplement the traditional income-based poverty measures. Recently poverty researchers have increasingly used measure of material hardship to examine the well-being of low income families. Measuring the material hardship might be useful to a better understanding of the multi-dimensions of the poverty in Korea. Using the data of the Korea Welfare Panel Study(the fourth wave), I examine incidences of material hardship across quintiles of the income distributions and the factors that might affect the experience of material hardship among the poor families. Major findings are as follows. Descriptive results show that nearly one-fifth of all families had experienced at least one of the five material hardships in the year. Those in the bottom quintiles and the poor families are more likely to experience material hardship than the other quintiles and non-poor families. But, incidences of the material hardship in the middle income quintiles and low income families are not a few(18 percent and 37 percent). Logistic regression results show that family-consumption related variables, income other economic resources(asset and public assistance), and household's employment status affect the experience of material hardship among the poor families. But, material hardship among the poor families is not significantly related with family income. These results indicate that material hardship measures are the useful indicators to understanding the multi-dimensions of the poverty in Korea. And they suggest that an extensive reform of the public assistance policy is necessary to relieve the material hardships of the poor in Korea.

A Study on the Motivation to Write Signing Advance Medical Directives (사전연명의료의향서 작성 동기에 관한 연구)

  • This study examined the patient's advantage and respect self-decision to protect human dignity and values, who are on the pathway of the hospice palliative care and death process. The study subjects were the elderly who had signed the advance medical directives at C Christian religious facilities in S region. The survey period was started on July 1st, 2019 and lasted for ten days. The study resulted in four topics and eight sub-topics on the motivation for preparing advanced medical directives. The four topics were 'for children', 'fear of pain', 'want to clear my life', 'felt the necessity', and the sub-topics were 'don't want to make a burden to children', 'don't want to make a worry of medical expenses', 'fear of pain', 'have experience of taking caring of painful death process', 'relaxed mind', 'importance of self-decision', 'have known it before but now decided', and 'it is the new information and decided'. This research is meaningful in that it can form the basis for improving well-dying education programs for the good death of the elderly and supplementing effective systems for preparing advance medical directives.

A Study on Awareness of the Dental Health Insurance Coverage (치과 건강보험에 관한 인식 조사)

  • The purpose of this study was to examine people's awareness of health insurance in a bid to help improve the management of dental health insurance coverage. The subjects in this study were 1,036 people who included experts in that field and medical consumers. The findings of the study were as follows: 1. In regard to the demographic characteristics of the expert group including gender and age, the female experts outnumbered the males, as the former accounted for 84.7 percent. And the experts in their 20s made up the largest age group, followed by the 30-39 age group and those in their 40s. As to the demographic characteristics of the medical consumers, the rates of the men and women stood at 49.8 percent and 50.2 percent respectively, which were similar. By age, the largest number of the medical consumers were in their 20s, followed by in their 30s and in their 40s. 2. Concerning opinions on the procurement of finance for health insurance, many respondents in both groups agreed that the health insurance coverage should be enlarged by securing more finance, and that items involved in the coverage should be prioritized again. Regarding a means of securing financial resources, the experts placed the most emphasis on extended budgetary support from the government, followed by establishing a social security system and budget compilation by local governments. The medical consumers attached the most importance to extended budgetary backing from the government, followed by budget compilation by local governments and determining contribution according to income level. 3. As for general view of the dental health insurance, the experts believed that the amounts of copayment and self-payment by the insured were appropriate(2.47), and they found the number of benefitted items to be proper(2.29) as well. The medical consumers thought that the benefit percentage of the dental health insurance in the overall health insurance budget was appropriate(2.26), and they also considered the number of benefitted items adequate(2.16). But this group didn't give a lot of marks to the appropriateness of those things. 4. As to perception about the dental health insurance, both groups took a similar view of it. Specifically, they felt the strong need for a dental health insurance system. In terms of satisfaction level, both groups were dissatisfied with it, and there was a strong tendency for them to be discontented with the health insurance coverage.

  • We are confronted by increase in old people due to the improvement in medical science, public hygiene and socioeconimic status in 20th century. But our medical security system for old people dees not meet the need for medical service of old people. Current medical insurance system restricts term and extent in allowance although the characteristics of the disease of the aged people need medical care of Bong duration and high cost. And in the medicaid system the speciality of the aged people is not recognized and the budget of the government is scanty. In addition many old people to our country are in economic distress due to low income. But the government authority does not give sufficient consideration for eld people in law, policy and budget. To improve social security system for old people it is necessary to increase the budget for the security of old people, to enhance the traditional respect for the aged, to improve medical security system by improving the accessibility to medical service and by expanding the allowance of medical insurance, and to expand the public welfare institutions. And these are roles for all the family. the society and the nation as well as the aged people themselves.

Health Medical Center Utilization Pattern and Its Related Factors among the Rural Inhabitants (농촌지역(農村地域) 주민(住民)들의 보건의료원(保健醫療院) 이용양상(利用樣相)과 관련요인(關聯要因))

  • This study was conducted to assess health medical center utilization pattern and its related factors among the rural inhabitants for the purpose of contribution to establishment of health medical center institutions. A questionnaire survey was carried out for object of 3,754 population of three primary school and three middle school student's parents (total 832 household) in Kyungbook Ulchin Gun rural area from 24 to 28 September, 1990. The summarized result are as follows, Respondents are 60.3% in male, 39.7% in female and 30-40s 81.3% in age, high school graduates 40.3% in education level and a regional medical insurance scheme in 44.1% in forms of health insurance. Recognition for health medical center was showed higher according to high educational, high income level, and short distance for location of health medical center of respondents (p < 0.01). Recognition for health medical center services was showed higher about care of medicaid in medical treatment services and higher preventive vaccination in health prevention services by respondents. Utilization rates of health medical center by out-patient care and preventive care service were 11.1 and 4.5 per 100 persons by year, but admission utilization rate was 34.6 per 10.000 persons by year. Motivations of health medical center utilization were showed a good care(45.7%), a good drugs(45.2%), and nearby health medical center(42.9%). In comparison health service levels of health medical center with general clinic was better (16.3%), similar(38.7%), 7(19.0%), and worse(19.0%) in view of health medical center utilizators. Inconvinience about health medical center utilized was the most higher longtime waiting, the next was limited utilization times. Transportation utilited were on foot(55%), by bus(35.5%), and so on. As mentioned above, there are many inhabitants who less understanding and less acknowledgement about health medical center and even mistake health center for health medical center. Therefore, there must be more information about health medical center. For higher utilization of health medical center, there must be considered expansion of health equipment, facilities, accomplishment with reinforcement of health staffs and efficiency management.

  • This paper conducts a comparative analysis of the KORUS FTA and the TPP Agreement to assess the current state of affairs in international trade rules for the pharmaceutical industry. Intellectual property rights as well as public health related regulations have evolved to strengthen the position of innovator drug companies. In particular, the TPP Agreement which adopted data exclusivity for biologics for the first time, will set the standard for the future. Apart from this however, the TPP Agreement has not gone further than the KORUS FTA and in some respects, even contains greater policy flexibilities and provisions for market access than the KORUS FTA. Korea should take advantage of such differences when and if she must engage in negotiations to join the TPP Agreement or a renegotiation of the KORUS FTA.

Preparation Characterizations for old Age of the Baby Boomers (베이비붐 세대의 노후준비 특성분석)

  • This study analyzed the preparation characteristics for old age of the baby boomers by the framework of the multi-pillar pension system. Analysis results are as follows. First, multi-pillar pension system's subscription rates of baby boomers was public pension 59%, private pension 11.5% and retirement pension 1.5%. The baby boomers isn't ready for old age life. Second, women and people with the low level of education are less prepared for old age. Third, people in a bad health state are less prepared for old age. Forth, low-income people are less prepared for old age. We must support baby boomers' preparations for old age by establishing income security system for old age. We must establish public pension support policy for the people of the low level of education and economic hierarchy, women, bad health status people, and must introduce universal old-age allowance policy for guaranteeing the minimum income of baby boomers.

  • Korean Longitudinal Study of Ageing(KLoSA)', launched in 2006, has developed a research network with 'Health and Retirement Study(HRS, US),' Studies on Health and Retirement in Europe(SHARE, EU),' and 'English Longitudinal Study of Ageing(ELSA, UK)'and tried to construct a mega data library. The KLoSA team has designed their research with focusing on the following three points, in order to use KLoSA for international comparative studies. First, KLoSA shares the key research areas such as family structure, health, employment, income, asset, subjective expectations with the comparable studies. Second, KLoSA team has developed the instrument to reflect the Korean culture and institutions. They considered the institutional characteristics in public pension and health care system and in employment practices. They also counted the cultural conventions in family life such as financial management and care exchange, Finally, KLoSA tries to capture the psychological characteristics of the Korean elderly by making the measurement scales more understandable.
