응급실 중증도 분류 간호사의 업무 경험: 질적 연구 (Triage Nurses' Work Experiences in Emergency Department: A Qualitative Research)

  • Purpose: The purpose of the study was to describe work experiences of triage nurses working at emergency departments in Korea. Methods: A qualitative descriptive research method was used. Twenty-one nurses of emergency department participated in the first interview and 16 of them participated in the second interview. Data were collected through in-depth interviews and field notes. Qualitative content analysis was utilized to analyze the data. Results: Forty-eight codes, 12 categories, and six themes emerged from the data analysis: caring on the frontline, burden as an initial evaluator, handling things alone, lack of others' understanding, becoming an expert, and needs for improvement. Participating triage nurses faced many challenges and difficult situations in emergency departments. They felt burden and loneliness as initial evaluator. Also, they felt pride in their work and struggled to become a professional. Conclusion: This study is the first qualitative study that has focused on exploring the experiences of triage nurses in emergency departments in Korea. The results provide description and in-depth understanding of working experiences of triage nurses. The results provide valuable fundamental data for regulation, clinical practice, education, and research for triage nurses in Korean emergency departments.

직장 내 괴롭힘에 대한 간호단위 관리자의 대응 양상: Q 방법론적 접근 (Response Patterns of Nursing Unit Managers regarding Workplace Bullying: A Q Methodology Approach)

  • Purpose: The purpose of this study was to identify the response patterns of nursing unit managers regarding workplace bullying. Methods: Q methodology was used to identify the response patterns. Thirty-six Q samples were selected from the Q population of 210 that included literature reviews and in-depth interviews with clinical nurses and nursing managers. Participants were 30 nursing unit managers who had experience managing workplace bullying and they classified the Q samples into a normal distribution frame measured on a nine-point scale. The data were analyzed using the PC-QUANL program. Results: Five types of response patterns were identified: (1) sympathetic-understanding acceleration, (2) harmonious-team approach, (3) preventive-organizational management, (4) passive observation, and (5) leading-active intervention. The preventive-organizational management type was most frequently used by the nursing unit managers. Conclusion: The results of this study indicated that nursing unit managers attempted to prevent and solve workplace bullying in various ways. Therefore, it is necessary to develop and conduct leadership training and intervention programs that appropriately address the response patterns of nursing unit managers, such as those identified in this study.

응급실 간호사의 감염관리 조직문화, 감염관리 피로도와 코로나19 감염관리 수행도와의 상관관계 (Correlation Among Organizational Culture, Fatigue for Infection Control, and Infection Control Compliance of COVID-19 Among Emergency Nurses)

  • Purpose: The purpose of this study was to investigate the levels and the relationship of emergency nurses' organizational culture and fatigue for infection control, and infection control compliance of COVID-19. Methods: Data was collected from 114 nurses caring for patients suspected of COVID-19 at two emergency medical centers in Gyeonggi-do and Seoul June 18-July 23, 2021. Results: Emergency nurses caring for patients suspected of COVID-19 suffered from high fatigue for infection control. The mean scores of the organizational culture and fatigue for infection control, and infection control compliance of COVID-19 were 51.80 ± 8.37 of 70 points, 78.46± 12.28 of 100 points and 71.02± 7.84 of 80 points, respectively. The higher infection control compliance of COVID-19 is significantly related to the higher organizational culture for infection control (r= .42, p< .001). Conclusion: In the COVID-19 pandemic, the formation of a positive organizational culture for infection control may be a priority as a major strategy to improve the infection control compliance of emergency nurses. It is also necessary to manage the high level of fatigue for infection control among emergency nurses.

응급실 간호사의 중증도분류 전문역량에 영향을 미치는 요인 (Factors Influencing Professional Competencies in Triage Nurses Working in Emergency Departments)

  • Purpose: This study was conducted to identify the professional competency of nurses working in emergency medical institutions that use the Korean Triage Acuity Scale (KTAS), and to identify factors that affect them. Methods: This study collected data from 105 nurses working in emergency medical institutions from June to August 2020. For data analysis, descriptive analysis, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression were performed using the SPSS 25.0 program. Results: As for the professional competency in triage, the higher the self-efficacy (β= 0.58, p< .001), the more experience they have in triage-related education (β= 0.30, p< .001), 2-4 years of clinical experience in emergency department (β= 0.19, p= .002), in case of triage alone (β= 0.24, p< .001), the higher the level of education a nurse has (β= 0.19, p= .003), the higher the professional competency in triage. These variables explained professional competency in a total of 64.2% of the participants (F = 38.30, p< .001). Conclusion: To improve nurses' professional competence in triage, introducing manpower expansion, financial support, and the provision of appropriate places is suggested. In addition, it is necessary to repeatedly provide educational opportunities in an environment similar to actual clinical practice by developing various scenarios and introducing simulations and web-based formats.

응급실 간호사의 중증도 분류 역량 측정도구 개발 (Development of a Triage Competency Scale for Emergency Nurses)

  • Purpose: This study aimed to develop a triage competency scale (TCS) for emergency nurses, and to evaluate its validity and reliability. Methods: Preliminary items were derived based on the attributes and indicators elicited from a concept analysis study on triage competency. Ten experts assessed whether the preliminary items belonged to the construct factor and determined the appropriateness of each item. A revised questionnaire was administered to 250 nurses in 18 emergency departments to evaluate the reliability and validity of the scale. Data analysis comprised item analysis, confirmatory factor analysis, contrasted group validity, and criterion-related validity, including criterion-related validity of the problem solving method using video scenarios. Results: The item analysis and confirmatory factor analysis yielded 5 factors with 30 items; the fit index of the derived model was good (${\chi}^2/df=2.46$, Root Mean squared Residual=.04, Root Mean Squared Error of Approximation=.08). Additionally, contrasted group validity was assessed. Participants were classified as novice, advanced beginner, competent, and proficient, and significant differences were observed in the mean score for each group (F=6.02, p=.001). With reference to criterion-related validity, there was a positive correlation between scores on the TCS and the Clinical Decision Making in Nursing Scale (r=.48, p<.001). Further, the total score on the problem solving method using video scenarios was positively correlated with the TCS score (r=.13, p=.04). The Cronbach's ${\alpha}$ of the final model was .91. Conclusion: Our TCS is useful for the objective assessment of triage competency among emergency nurses and the evaluation of triage education programs.

국내 응급전문간호사의 업무 수행빈도, 중요성, 자율성 및 지각된 수행능력 (Task Analysis of Emergency Nurse Practitioners: Frequency, Importance, Autonomy, and Perceived Competence to Perform)

  • Purpose: The purpose of the study was to identify the frequency and importance of task performance and the perceived competence to perform of the emergency nurse practitioners (ENPs) certified in Korea. Method: This cross-sectional study was conducted with 41 ENPs certified by March 2012. The questionnaire to identify frequency, importance, autonomy, and perceived competence to perform 131 typical ENP task elements was developed on the basis of the standardized role of an ENP defined by the Korean Accreditation Board of Nursing Education. The data were analyzed using descriptive statistics. Results: The professional nursing practice showed the highest scores for performance frequency, task importance, autonomy, and perceived competence to perform as compared to other job categories. Clinical decision making was the most frequently performed duty. Data collection was the duty with highest levels of importance, autonomy, and perceived competence for the ENPs. However, the ENPs reported that research was the duty with the lowest levels of frequency, importance, autonomy, and perceived competence Conclusion: ENPs in Korea perform various tasks. In order to improve their position in the field, it is necessary to clarify their job description and enhance their competence to perform evidencebased professional nursing practices.

응급실과 일반병동 간호사의 밤번 근무시 Circadian rhythm과 피로도 비교 (The Difference in Circadian Rhythm and Level of Fatigue Between ER and General Ward Nurse)

  • The purpose of this study is to find out the difference in circadian rhythm and level of fatigue between ER and General Ward Nurse. This study was done through analyzing body temperature, heart rate and major electrolytes such as $Na^+$ and $K^+$ concentration on urinalysis, which are related to factors of circadian rhythm and level of fatigue. The subjects, 17 nurses from Emergency Room and 25 nurses from general ward, were selected from a General Hospital in Taegu City on August 8 to 23, 1998. And the data were analyzed by proportion, t-test, chi-square and Pearson's Correlation, ANCOVA, using SAS program. The results of this study were shown as follows: First, The difference of Maximal and minimal heart rate was significant between ER group and general ward group. However, body temperature difference wasn't notable. Second, Urine $Na^+$ concentration on urinalysis was lower and $K^+$ concentration on urinalysis was higher in ER group than general ward group significantly. Third, Fatigue level was higher in ER group than general ward group significantly. Fourth, through urine concentration, significant relations in circadian rhythm of ER nurse group and general ward group and their fatigue level were found. Urine $Na^+$ concentration on urinalysis of ER nurse was significant related with their fatigue level. In conclusion, the effect of fatigue on circadian rhythm is greater in ER group than general ward group during night shift. This study could be a help in improvement of nurse's health and understanding the effect of fatigue on health.

응급실 간호사의 법의간호 교육에 대한 인식과 요구도 (Emergency Department Nurses' Recognition of and Educational Needs for Forensics Nursing Education)

  • Purpose: The purpose of this survey was to investigate emergency department nurses' recognition of forensics nursing and their educational needs for forensics nursing education. Methods: Data were collected via questionnaires from 167 emergency department nurses who were working in 7 university hospitals. Nurses' experiences of trauma or accidents and their recognition of and educational needs for forensic nursing education were assessed. Results: All subjects reported having suffered bodily injury due to falling or serious traffic accidents; 92.2% reported having suffered bodily injury caused by suicide attempts; 91.6% reported having experienced physical violence or abuse; 76.0% reported having experienced sexual assault or abuse; 68.9% reported having experienced some difficulties during their nursing care due to lack of forensics knowledge; and 88.6% reported never having been trained in forensics nursing. The educational needs score for "forensics nursing" was 3.61; the needs score for "abuse- and violence-related education" was 3.65; the needs score for "incident data collection related education" was 3.47; and the needs score for "forensics theory related education" was 3.34. Conclusion: The findings of the study underscore a strong need to develop an educational program on forensics nursing for emergency department nurses.

응급실 내 의사와 간호사가 인식하는 병원 내 응급구조사의 업무인식도 (Emergency Medical Participants' recognition of The Emergency Medical Technicians' Job Awareness)

  • 본 연구는 대전,충남 소재 대학병원에 근무하는 응급의료종사자 122명을 대상으로 병원 내 응급구조사를 조사하여 응급구조사의 역할과 법적 업무규정의 현실적 제고 및 병원 내 응급구조사의 업무에 관한 인식을 보다 명확히 하기 위한 기초자료를 제공 하고자 연구를 실시하였다. 연구 대상자들의 병원 내 응급구조사의 업무인식을 살펴본 결과, 전체 40문항에 대해 $3.23{\pm}0.67$점으로 나타나 병원 내 응급구조사에 대한 업무인식정도는 중간 보다 다소 높은 것으로 나타났다. 영역별로는 외상처치가 가장 높게 나타났으며, 약물투여가 가장 낮았다. 응급의료종사자가 응급구조사의 업무를 바르게 인식하게 하기 위해서는 응급구조사가 고유 업무와 역할을 충실히 해 낼 수 있어야 할 것이다. 그러므로 보다 명확한 교육과정이 이루어져야 하며 보다 명확한 업무가 법적으로 규정되어져야 한다. 응급구조사에 대한 홍보와 지속적인 모니터링이 필요할 것으로 사료된다.

회복력 중이론을 기반으로 한 폭력대처 프로그램이 응급실 간호사의 회복력, 폭력대처, 간호역량과 소진에 미치는 효과 (The Effects of Violence Coping Program Based on Middle-Range Theory of Resilience on Emergency Room Nurses' Resilience, Violence Coping, Nursing Competency and Burnout)

  • Purpose: The purpose of this study was to examine the effects of a violence coping program (VCP) based on Polk's middle-range theory of resilience on nursing competency, resilience, burnout, and the ability to cope with violence in nurses working in emergency rooms. Methods: A quasi-experimental study, with a nonequivalent control group and a pretest-posttest design, was conducted. Participants were 36 nurses who worked in emergency rooms and had experienced violence; 18 nurses from D hospital and 18 nurses from C hospital were assigned to the experimental and control groups, respectively. The experimental group received the VCP twice per week for 8 weeks. Results: Levels of resilience, F=59.41, p<.001, active coping behavior, ${\chi}^2=33.09$, p<.001, and nursing competency, F=59.41 p<.001, increased significantly and levels of passive coping behavior, ${\chi}^2=22.92$, p<.001, and burnout, F=52.74, p<.001, decreased significantly in the experimental group. Conclusion: The results suggest that the VCP could be an effective strategy for reducing burnout and improving resilience, active coping behavior, and nursing competency. Therefore, it would be a useful intervention for improving the quality of nursing care provided in emergency rooms.
