The Effect of Topical and Conduction Anesthesia by Phonophoresis with Lidocaine (Lidocaine 음파영동에 의한 표면마취 및 전달마취 효과)

  • This study conducted the following experiment to examine anesthetic effects of 500 kHz ultrasound with lidocaine. Clinical experiment was conducted to compare local anesthetic effects by ultrasound frequencies and drugs with 40 normal adults and it divided subjects into experimental group (I) applying only ultrasound, ointment group (II) applying only lidocaine, phonophoresis group of 1 MHz (III), phonophoresis group of 500 kHz lidocaine (IV) for pain threshold and nerve conduct experiments. Mechanical threshold was measured with von Frey monofilament, thermal pain threshold with halogen lamp and digital thermometer, action potential in median nerve with diagnostic electromyography before and after treatment, and at 30 min., 60 min., 90 min., and 120 min. after treatment. Results of this study can be summarized as follows. Topical anesthesia experiment showed that pain threshold of phonophoresis groups was significantly increased, but there were little differences in ultrasound frequency and drugs among phonophoresis groups. Conduction anesthesia experiment showed that phonophoresis group of 500 kHz using lidocaine had significant difference in pain threshold and sensory nerve conduction compared to ointment group and ultrasound group, but there was no great difference from other phonophresis groups and light nerve block effect was found. It was considered that application of 500 kHz ultrasound using lidocaine will be more effective in deep anesthesia or nerve block than 1 MHz ultrasound. However, researches considering various frequencies, intensities and application hours in low frequency areas including kHz ultrasound are needed to increase deep permeation of drugs.

Surgical Management for Chronic Atrial Fibrillation (만성 심방세동에 대한 외과적 치험 4례)

  • Atrial fibrillation is the most common of all cardiac arrhythmias. It is associated with significant morbidity and mortality and is frequently resistant to medical therapy. On the experimental and clinical study, the presence of macroreentrant circuits and the absence of either microreentrant circuits or evidence of atrial automaticity suggests that atrial fibrillation should be amenable to surgical ablation. The results of the maze III procedure are associated with a higher incidence of postoperative sinus rhythm, improved long-term sinus node function, fewer pacemaker requirements, less arrhythmia recurrence, and improved long-term atrial transport function. We had experienced 4 patients with chronic atrial fibrillation. For the first time, Hioki procedure had been performed in the first patient with ASD and atrial fibrillation, regular sinus rhythm showed on postoperative EKG, but junctional rhythm and bradycardia developed postoperative 3 years. The maze III procedure for the rest with mitral valvular disease and atrial fibrillation had been done, followed by regular sinus rhythm for 2 patients and atrial fibrillation for 1 patient, managed with amiodarone, on immediate postoperative state. Echocardiogram documented good contraction of right atrium and hardly contraction of left atrium for 2 patients with regular sinus rhythm postoperative 3 months.

  • The rock excavation work by doing blasting breaks the rock by using a shock pressure and gas pressure produced when explosive explodes and the shock wave by shock pressure propagated three-dimensionally from the exploding center is on the decrease notably to the distance, however, $0.5{\sim}20%$ of energy produced by blasting propagates into the ground outside a crack zone by the shape of an elastic wave, on the ground it appears as a ground vibration with a seismic amplitude and a seismic cycle, it is called a blasting vibration. on the other side, what propagated in the air is called a blasting sound. The blasting sound of both means the things which the shock sound within the range the audible frequency($20{\sim}20000Hz$) of the elastic wave in the air influences the response system of a human body, it doesn't harm physically to any structures but influences unreasonably a work accomplishment, such as a work discontinuance due to the outbreak of a public complaint by a mental pain, reduction of a blasting scale, etc.. So, this study is examined at about 20 sites on the installation time and method of soundproofing facilities for reduction of the sound accompanied with a tunnel blasting work.

Acoustic Characteristics of Mufflers with an Extended Inlet and Outlet (입출구가 연장된 동심형 소음기의 음향해석)

  • Cylindrical chamber silencers with an extended inlet and outlet are extensively used in many application fields to reduce the propagated noise in ducts. The basic attenuation effectiveness in the low frequency region can be explained by the reactive wave action inside the expansion chamber associated with the geometric configurations of the inlet and outlet locations, and the area expansion of the jacket. In this study. an acoustic analysis is carried out for a concentric extended pipe inserted into a simple expansion chamber. An algebraic equation is derived by using the eigenfunction expansion and orthogonality principle in which the acoustic pressures and particle velocities defined on each subdivided surface are expressed by the separable coordinates. By using the proposed analytical method, transmission losses are predicted for several configurations of the concentric extended systems and they agree very well with experimental results.

  • Sand ridges in the South Sea of Korea have been investigated to reveal the surface morphology and vortical variation in sediment physical property. Both field and laboratory methods such as high resolution subbottom profiling, side scan sonar, and physical property and sediment textural study on cored sediments were employed. Asymmetric sand waves are found on the ridge, No prominent difference in the sediment was recorded throughout the ridge area. Sediment sorting is generally poor due to interbedded mud. Average compressional wave velocity is 1753 m/sec for the sand dominated core section, Most of cores show two prominent facies: facies A and B. Facies A is defined as modern mud sediment and B is dominated by relict sand. Facies B is characterized by gravel, sand, and shell fragment, It is suggested that the facies B was developed in transgressive environment by tide and sporadic high energy events.

  • The performance of underwater acoustic(UWA) communication system is sensitive to the Inter-Symbol Interference(ISI) due to delay spread develop of multipath signal propagation. And due to limited frequency using acoustic wave, UWA is a low transmission rate. Thus, it is necessary technique of Space-time code, equalizer and channel code to improve transmission speed and eliminate ISI. In this paper, UWA communication system were analyzed by simulation using these techniques. In the result of simulation, the proposed Space-time code, Turbo code, and Zero forcing techniques is shown that improved performance than conventional UWA communication.

  • Recently, as a result of increasing concern about eco-friendly power, the demand for the power stations using environmentally friendly powers such as photovoltaic energy, wind force, tidal power, and tidal current has been increasing worldwide. Among these power stations tidal current power plant requires strong current generated by the topographic characteristics of the ocean floor. Uldolmok waterway producing very strong current is an ideal location for a tidal current power generation. However the occurrence of anthropogenic underwater noise generated by the tidal current power station may affect the marine environment. Therefore, it is necessary to evaluate the noise radiated from the station and predict the range influenced by the radiated noise. In this paper, the measurements of radiated noise spectrum level by the tidal current power station are presented, and the source level per unit area is estimated. Finally, the propagation properties of the radiated noise in the Uldolmok waterway is evaluated from the model simulation using the parabolic equation method, RAM.

  • It is important to predict accurately reverberation level, which is a limiting factor in underwater target detection. Recently, the studies have been expanded from monostatic sonar to bistatic sonar in which source and receivers are separated. To simulate the bistatic reverberation level, the computation processes for propagation, scattering strength, and scattering cross section are different from those in monostatic case and more complex computation processes are required. Although there have been many researches for bistatic reverberation, few studies have assessed the bistatic scattering cross section which is a key factor in simulate reverberation level. In this paper, a new method to estimate the bistatic scattering cross section is suggested, which uses the area of intersection between two circles. Finally, the reverberation levels simulated with the scattering cross section estimated using the method suggested in this paper are compared with those estimated using the methods previously suggested and those measured from an acoustic measurements conducted in May 2013.

Performance analysis of underwater acoustic communication based on beam diversity in deep water (심해에서의 빔 다이버시티를 이용한 수중음향통신 성능 분석)

  • Underwater communication performance is degraded by the influence of Inter-Symbol Interference (ISI) due to multipath. Passive time reversal processing is the most effective technique for mitigating multipath, and the diversity combining method can be used to improve its performance. This paper analyzed communication performance using the beam diversity combining method, which combines signals obtained through the beam steering to various angles. Directions of arrival were estimated through the beam-time migration, which, in turn, was estimated from probe signals received by a vertical line array. The performance was analyzed based on the number and type of combinations among the estimated angles. In this paper, the data obtained from the Biomimetic Long range Acoustic Communications 2018 (BLAC18) experiment, which was conducted in the East sea, ~50 km east of Pohang, in October 2018, were used for the analysis. The output Signal to Noise Ratio (SNR) was used as communication indicators.

Tracking of Internal Waves Observed by SAR in the Time Series of Temperature Profile Data (시계열 등온선 자료에서의 SAR로 관측된 내부파의 추적 연구)

  • An abundance of internal waves is observed by SAR in the Yellow Sea during summer. They are small scaled internal waves and are not relatively studied well compared to the ones in the East/South China Sea. These internal waves should be considered in the study of physio-biological properties of the Yellow Sea because the mixing of the stratified surface water caused by internal waves during summer is important for ocean biological environment, and they also affect the sediment transport and acoustic signal transmission in the continental shelf region. To understand the characteristics of internal waves, it is important to get the spatio-temporal information of internal waves simultaneously by executing in-situ measurements as well as the SAR observation. This study tracks the internal waves observed by SAR in the time series of temperature profile data by analyzing simultaneously acquired in-situ measurement data and RADARSAT SAR image on 29 May 2002.
