A Case of Tuberculosis-associated Hemophagocytic Syndrome during Antituberculosis Medication for Tuberculous Pericarditis (결핵성 심막염으로 항결핵약을 복용하던 중 발생한 혈구 탐식증후군 1예)

  • A 63-year old woman was admitted to our hospital for an evaluation of thrombocytopenia. She had been diagnosed with tuberculous pericarditis three months earlier in a local clinic and treated with anti-tuberculosis medication. Two months later, thrombocytopenia developed. The medication was subsequently stopped because it was suspected that the anti-tuberculosis medication, particularly rifampin, might have caused the severe platelet reduction. However, the thrombocytopenia was more aggravated. A bone marrow biopsy was performed, which showed moderate amounts of histiocytes with active hemophagocytosis. This finding strongly suggested that the critical thrombocytopenia had been caused by hemophagocytic syndrome, not by the side effects of the anti-tuberculosis medication. Furthermore, the development of hemophagocytosis might have been due to an uncontrolled tuberculosis infection and its associated aberrant immunity. Therefore, she was started with both standard anti-tuberculosis medication and chemotherapy using etoposide plus steroid. One month after the initiation of treatment, the thrombocytopenia had gradually improved and she was discharged in a tolerable condition. At the third month of the follow-up, her platelet level and ferritin, the activity marker of hemophagocytic syndrome, was within the normal range.

Estimation of Slime Thickness of Bored Piles by Using Borehole Electrical Resistivity Method (시추공 전기비저항 기법을 활용한 현장타설말뚝의 슬라임층 두께 평가)

  • The slime, deposited in the bored pile due to falling soil particle, reduces the bearing capacity of bored pile and thus the stability of construction also decreases. The weight pendulum and iron have been used for estimating the slime thickness based on the subjective judgment and thus the previous method has a limitation of reliability. The objective of this paper is to suggest the method for estimating the slime thickness by using characteristics of electrical resistivity as scientific method. The temperature-compensation resistivity probe (TRP), which has a conical shape and the diameter of 35.7mm, is applied to the measurement of the electrical resistivity in the borehole during penetration. The field tests are carried out for estimating the slime thickness in the application site of bored pile. The slime thickness is calculated through the difference between excavation depth of borehole and measured data. Furthermore, the laboratory tests are also conducted for investigating effects of casing, time elapsing and relative density by using the specimen of slime. The laboratory test supporting the suggested method is reasonable for determining the slime depth. The paper suggests that the electrical resistivity method may be a useful method for detecting slime thickness and the method is expected to be applicable to various sites of bored piles.

Mechanism of Decrease in Lung Injury by Low Dose of Endotoxin During Hyperoxia in the Rats (저용량의 내독소가 쥐에서 고농도의 산소에 의한 급성폐손상을 경감시키는 기전)

  • 배 경 : 쥐를 고농도의 산소에 60시간 이상 노출시켰을 때 급성 폐손상이 유발되지만 내독소를 저용량으로 투여시에는 이러한 폐손상이 경감된다고 알려져 있으나 그 기전에 대하여는 확실히 밝혀지지 않고 있다. 산화질소(nitric oxide, NO)는 내독소나 염증성 사이토카인(cytokine) 등의 자극에 의해서 폐내 여러 염증세포에서 만들어지며 이 산화질소는 경우에 따라 우리 몸에 이롭거나 해로운 양면성을 지니고 있다. 저자들은 쥐에서 고농도의 산소에 의한 폐손상이 저농도의 내독소 투여로 경감되는 기전에, 산화질소가 중요한 역할을 하는지 또는 황산화효소나 다른 항염증성 사이토카인이 중요한 역할을 하는지를 규명하고자 하였다. 방 법 : 총 120마리의 쥐 (Sprague-Dawley rat)를 24마리씩 5군으로 나누어 대조군은 실내 공기를, 고농도 산소군은 100%의 산소를 100%의 산소를 60시간 투여하였고 내독소군은 100% 산소 투여시 2일간 저용량의 내독소를 투여하였다. 다른 두 군은 산화질소 합성 억제물인 aminoguanidine(AG)과 N-nitro-L-arginine methyl ester (L-NAME)를 각각 2일간 고농도 산소와 내독소에 더하여 투여하였다. 각각의 군에서 폐손상의 정도와 사망률을 관찰하고 superoxide dismutase(SOD), catalase, nitric oxide, IL-6, IL-11을 기관지폐포세척액에서 측정하고, 고농도산소 투여군의 폐조직에서 iNOS synthase rnRNA의 발현을 비교하였다. 결 과: 1. 100%의 산소에 60시간 노출시켰을 때 쥐의 사망률은 8.3% 이었고 내독소 투여군은 4.2%, NAME 투여군이 37.5%, AG 투여군이 25%로 산화질소 합성 억제제에 의하여 사망률의 증가가 관찰되었다. 2. 폐의 손상 정도를 나타내는 폐의 wet/dry 중량비와 늑막액도 100%의 산소에 노출된 군에서 증가되었고 내독소 투여에 의하여 감소되었으며 NAME나 AG 투여군에서는 오히려 증가되었다. 3. 이러한 내독소에 의한 폐손상 억제효과가 항산화효소인 SOD나 catalase, 또는 protective cytokine인 IL-6나 IL-11등의 증가와 관련이 있는지를 관찰하였으나 이들 모두에서 유의한 변화를 관찰하지 못하였다. 4. 산화질소는 100% 산소에 노출시킨 군에서도 증가하였으나 내독소 투여군에서 유의하게 더욱 증가하였고 이는 L-NAME 나 aminoguanidine의 투여시 감소하였다. 5. iNOS mRNA의 발현도 내독소 투여군에서 유의하게 증가하였다. 결 론 : 쥐의 고농소 산소 투여에 의한 폐손상은 저용량의 내독소 투여로 경감되며, 이는 주로 내독소 투여에 의한 iNOS mRNA의 발현을 유도하여 생성된 산화질소의 증가에 기인하는 것으로 생각된다.

A Case of Erdheim-Chester Disease Who Has Policythemia Vera (진성 적혈구증다증 환자에서 발현한 Erdheim-Chester Disease 1예)

  • Erdheim-Chester disease (ECD) is a rare disease that is characterized by multi-organ involvement of foamy histiocytes. It causes systemic inflammation, and also demonstrates various clinical manifestations and has a poor prognosis. We encountered a case of ECD in a patient that had been treated for underlying polycythemia vera. As far as we know, this is the first reported case worldwide where ECD developed in association with polycythemia vera. A 59-year-old man visited our hospital due to pleuric pain at the right side of the chest. Pleural tissue that was obtained following a thoracoscopic biopsy showed non-Langerhan's cell histiocytosis, suggesting the presence of ECD. The histiocytes stained positively for CD68, but were negative for S-100 and CD1a. The patient also complained of pain at both hips and the right shoulder area. An X-ray and magnetic resonance image demonstrated that the lesion showed sclerosis and osteolysis in both the proximal femur and right humerus. Treatment was started with predinisolone, and subsequently cyclophosphamide was added. ECD is a very rare multi-systemic disease, and its cause and therapeutic options have not yet been defined. ECD has a poor prognosis. Therefore, we believe that additional case studies are needed prior to the determination of a novel therapy for ECD.

Void Ratio Evaluation of Unsaturated Soils by Compressional and Shear Waves (압축파와 전단파를 이용한 불포화토의 간극비 산정)

  • Soils are commonly unsaturated in the near surface. The stiffness of soils is affected by the amount of air and water. The objective of this study is to evaluate the porosity of the unsaturated soils by using the elastic waves including compressional and shear waves. The elastic waves are measured at different degrees of saturation by controlling the matric suction. Thus, the unsaturated soils are characterized at different levels of the matric suction. Shear and compressional waves are measured by using the bender elements and the piezo disk elements, respectively. Both transducers are installed on the walls of the rectangular cell. The unsaturated soils are prepared by using uniform size sands and silts. Test results show that both compressional and shear wave velocities change according to the matric suction. The elastic modulus, the shear modulus, and the Poisson's ratio are estimated based on the measured elastic wave velocities. In addition, the void ratio of the unsaturated soils estimated using elastic wave velocities matches well with the volume based void ratio. This study demonstrates that the elastic waves can be effectively used for the characterization of unsaturated soils.

A Study of Theoretical Methods for Estimating Void Ratio Based on the Elastic Wave Velocities (탄성파 속도를 이용한 간극비 산출 식의 고찰)

  • The void ratio is an important parameter for reflecting the soil behavior including physical property, compressibility, and relative density. The void ratio can be obtained by laboratory test with extracted soil samples. However, the specimen has a possibility to be easily disturbed due to the stress relief when extracting, vibration during transportation, and error in experimental process. Thus, the theoretical equations have been suggested for obtaing the void ratio based on the elastic wave velocities. The objective of this paper is to verify the accuracy of the proposed analytical solution through the error norm. The paper covers the theoretical methods of Wood, Gassmann and Foti. The elastic wave velocity is determined by the Field Velocity Probe in the southern part of Korean Peninsular. And the rest parameters are assumed based on the reference values. The Gassmann method shows the high reliability on determining the void ratio. The error norm is also analyzed as substitution of every parameter. The results show every equation has various characteristics. Thus, this paper may be widely applied for obtaining the void ratio according to the field condition.

  • Electrical resistivity is applied for understanding details about layers and obtaining basic properties of soils to various measurement devices. The objective of this study is development of TRP(Temperature-compensated Resistivity Probe), analysis about effects of temperature changes during cone penetration test, and observation of characteristics of cone penetration. In order to observation of temperature changes according to a diameter difference of resistivity cone probe, the cone which has wedge type cone tip is made to two types, 2mm and 5mm. Temperature sensor is attached at 15mm below from cone tip because of an electrical interference with elecrical resistance probe. Delectrical connector is used to prevent electric disturbance between motor type penetrating machine and electrical resistivity cone probe. Application tests are carried out in acrylic cell whose diameter is 30cm with uniform Jumunjin sand according to densification caused by blows. The test results indicate that the temperature is increased uniformly during penetration and a tendency, characteristics of cone penetration, is discovered during altering state of soils. This study suggests that the temperature effects and characteristics of penetration should be considered in penetrating tests in order to conduct an accurate ground investigation using TRP(Temperature-compensated Resistivity Probe).

Cone Resistivity Penetrometer for Detecting Thin-Layered Soils (협재층 탐지를 위한 선단비저항 콘)

  • The thin-layered sand seam in clay affects the soil behavior. Although the standard cone penetrometer (A: $10cm^2$) have been used to evaluate the thin-layered soil, the smaller diameter cone penetrometer have been commonly recommended because of the high resolution. The purpose of this study is the development and application of the Cone Resistivity Penetrometer (CRP), which detects qc, fs, and electrical resistivity at cone tip for the evaluation of thin layered soils. Two sizes of the CRP are developed for the laboratory and field test. The projected areas of CRP for the laboratory and field tests are $0.78cm^2$ (d: 1.0 cm) and $1.76cm^2$ (d: 1.5 cm), repectively. The length of friction sleeve is designed in consideration of ratio of the projected area to the friction sleeve area. The application tests are carried out by using the artificially prepared thin-layered soils in the laboratory. In addition, the field tests are conducted at the depth of 6 to 15 m in Kwangyang. In the laboratory test, the measured electrical resistivity and cone tip resistance detect the soil layers. Moreover, in the field test the CRP investigates the three thin-layered soils. This study suggests that the CRP may be a useful tool for detecting thin-layered in soft soils.

Evaluation of Preconsolidation Stress Considering Small-Strain Shear Wave Velocity (미소변형 전단파 속도를 고려한 설행압밀하중 산정)

  • Preconsolidation stress is one of the important design parameters in soft soils because the behavior of saturated soft soils changes dramatically at the preconsolidation stress. For the estimation of preconsolidation stress, the global vertical settlement without considering micro strain behavior has been considered. The purpose of this study is to propose and verify a new method called the "shear wave velocity method" for determination of the preconsolidation stress reflecting on particle behavior at the small-strain. In this study, the undisturbed soft soils obtained at Busan, Incheon and Gwangyang in Korea were used. The oedometer cell incoporated with the bender elements is used for the consolidation tests under the $K_0$ condition. The preconsolidation stress determined by the proposed method is compared with that estimated by Casagrande (e-log p'), Sridharan (log (1+e)-log p'), and Onitsuka (In(1+e)-log p') methods. This study suggests that the shear wave velocity method may determine simply the preconsolidation stress with considering the small-strain behavior.

Blade Type Field Vs Probe for Evaluation of Soft Soils (연약지반 평가를 위한 블레이드 타입 현장 전단파 속도 프로브)

  • The assessment of shear wave velocity($V_s$) in soft soils is extremely difficult due to the soil disturbances during sampling and field access. After a ring type field $V_s$ probe(FVP) has been developed, it has been applied at the southern coastal area of the Korean peninsular. This study presents the upgraded FVP "blade type FVP", which minimizes soil disturbance during penetration. Design concerns of the blade type FVP include the tip shape, soil disturbance, transducers, protection of the cables, and the electromagnetic coupling between transducers and cables. The cross-talking between cables is removed by grouping and extra grounding of the cables. The shear wave velocity of the FVP is simply calculated by using the travel distance and the first arrival time. The large calibration chamber tests are carried out to investigate the disturbance effect due to the penetration of FVP blade and the validity of the shear waves measured by the FVP. The blade type FVP is tested in soils up to 30m in depth. The shear wave velocity is measured every 10cm. This study suggests that the upgraded blade type FVP may be an effective device for measuring the shear wave velocity with minimized soil disturbance in the field.
