Effects of National Dental Screening on Dental Caries Experience, Treatment, and Cost in Children (국가 구강검진이 어린이의 치아우식 경험, 치료 및 비용에 미치는 영향)

  • This study aimed to evaluate the effects of national dental screening for children in Korea in the aspects of prevention of dental caries, the number of treatments related to dental caries, and cost of dental care. The study used the national cohort data on children health screening provided by National Health Insurance Service, and analyzed 32,937 1st dental screening, 22,608 2nd dental screening, 13,708 3rd dental screening, and 241,043 cases from the dental care of 27,944 children born in year 2008 and 2009. The decayed-filled teeth index and decayed-filled person rate, calculated from the results of the 2nd and 3rd dental screening, decreased as the number of preceding dental screening increased. Glass ionomer and amalgam restoration, pulp treatment of primary teeth and extraction of primary canine and molar were shown to decrease as the number of examined dental screening increased, and the total cost of dental care covered by national health insurance also decreased as the number of dental screening increased. In conclusion, national dental screening for children is an efficient and economical oral health care method that prevents dental caries and lowers the possibility of dental caries related treatment, thereby reducing cost of dental care.

A MORPHOMETRIC STUDY ON THE PRIMARY MOLARS AND PREFORMED STAINLESS STEEL CROWN (유구치 치관 및 기성금속관의 크기에 관한 계측학적 연구)

  • Data derived from odontometric studies are useful in many areas. Especially crown diameter of primary teeth is important in not only anthropology but also clinical dentistry. The purposes of this study were to compare diameters of primary molars of Korean children with those of preformed stainless steel crown, and examine racial characteristics of primary molars of Korean children. The mesiodistal and buccolingual diameters of primary molars were measured on dental casts taken from 235 children(male 105, female 131), with digital calipers. And the data were compared with those of preformed stainless steel crowns. The results were as follows; 1. No significant difference was observed between the right and left members of antimeric teeth and all primary molars of male were larger than those of female. 2. The diameters of primary molars of Korean are smaller than those of Austrailian aborigines and there was significant difference between diameters of primary molars of Korean and those of other populations. 3. There were size differences between diameters of maxillary, mandibular first molar and preformed stainless steel crown than other primary molars, relatively.

A SURVEY ON THE CARIES PREVALENCE OF PRESCHOOL CHILDREN IN A SMALL CITY (소도시 거주 미취학 아동의 치아우식 실태조사)

  • The purpose of this study was to investigate the caries prevalence and caries pattern of preschool children in a small city. Six hundred twenty four kindergarten children in Milyang city were examined for their caries experience of individual teeth and surfaces by dmf index. The obtained results were as follows. 1. The rate of children with caries experience(dmft rate) in primary teeth was 74.0% in 3-year-olds, 83.8% in 4-year-olds, and 90.6% in 5-year-olds. The mean number of decayed, missed, and filled primary teeth(dmft index) was 3.47 in 3-year-olds, 5.41 in 4-year-olds, and 6.01 in 5-year-olds. 2. The caries prevalence of children in this study was higher than those of other researches in past and in other cities. 3. The caries-experienced teeth in order in 5-year-olds were as follows : mandibular 2nd primary molar, mandibular 1st primary molar, maxillary 2nd primary molar, maxillary primary central incisor and maxillary 1st primary molar. 4. The pattern of dental caries development was different between the incisors and molars. The prevalent surface of caries was proximal surface in primary incisors, but occlusal surface in primary molars.

A study on Regional Economic Effects of the logistics complex in Incheon New Port (인천신항 배후물류단지의 지역경제 파급효과 연구)

  • This study estimated the regional economic effects by development of the logistics complex in Incheon New Port, applying the regional Input-Output tables in Incheon. And thereby, we tried to find some strategic implications on the priority to induce more effective industries for activated operation of the logistics complex in ports. According to the results, development of Incheon New Port will generate 2,579 billion KRWs(2.2 billion USDs) of production and 1,783 KRWs(1.5 billion USDs) of value-added annually. And we expect it to induce 38.8 thousand jobs, which is over the national average on the input-output table. By the business type, port logistics industry will contribute to develop regional economy more than any other ones in Incheon. We suggest business firms in port logistics industry to be induced to the logistics complex in Incheon New Port strategically. Until now, development of the logistics complex in Incheon New Port are planned to be postponed to 2015 when construction of the berth facilities, the access sea routes and the hinterland highways are completed. We suggest to invest more public fund, to induce the private capital, and then, to develop the logistics complex in Incheon New Port earlier in an appropriate scale.

Gambling Behavior, Gambling-related Problem, and The Satisfaction with Life among the Residents Who Live in the Location of Casinos (카지노 유치지역 주민의 도박참여 및 도박중독 실태와 삶의 만족도: 강원도 폐광지역을 중심으로)

  • The purpose of this study was to explore whether or not the increased availability of legalized gambling leads to increase in the prevalence of gambling-related problems, and the relationship between gambling behaviors and gambling-related problems and residents' perceptions of their own satisfaction with life among the residents who live at the location of casinos. Three casino communities, designated by a special law as run-down mining areas, were chosen for the survey research. One community consisted of two towns, Kohan and Sabuk in Chongsun county at which the casino is located. The other communities were Taeback city and other rural areas in Chongsun county. 559 samples from three communities completed self-administered questionnaires(gambling behavior participation, K-NODS, perceived impacts coward casino development, and scales of the satisfaction with life). Results were as follows; it was found that there was a positive relationship between gambling behaviors and gambling-related problems, and a negative relationship between gambling-related problems and the satisfaction with life. While residents who live Kohan and Sabuk community were likely to perceive negative impacts of the casino development, the residents in Taeback city were likely to perceive the casino development positively. It was found that the increased casino accessibility led co the increase in the gambling problems; the prevalence rates of pathological gambling in Kohan and Sabuk community were higher than both in Taebak city and in more rural areas in Chongsun county. The nagative impacts of legalized casino industry on community where the casino located were discussed.

A STUDY ON THE TRAUMATIC INJURY OF PATIENTS IN DEPARTMENT OF PEDIATRIC DENTISTRY, KANGNUNG NATIONAL UNIVERSITY DENTAL HOSPITAL (강릉대학교 치과병원 소아치과에 내원한 외상 환자에 대한 분석)

  • There is a growing tendency for injury and it is essential to know about the epidemiology of the injured teeth for proper and timely treatment. Through empirical data analysis of 120 children who came to pediatric dentistry of Kangnung National University Dental Hospital, this study purposes to give an understanding about the injury and analyzes the frequency by children's sex and age, the number of injured teeth according to the area in the mouth, the types of injury, causes and places of injury, the frequency occurred by month and hour, the position of injured teeth, and the elapse of time. The main findings of this study are summarized as follows: 1. The frequency analysis by sex shows that the rate of boys is more likely to be higher than girls(1.6:1). 2. The frequency analysis by age shows that the ratio of children between aged 2~4 and 8~10 are high. 3. The number of injured teeth is generally one(51.7%). 4. Periodontal tissue injury is the main cause for the primary teeth. In case of permanent teeth, the ratio of hard tissue injury which is much increased than the case of the primary teeth, is similar to that of periodontal tissue injury. 5. The main cause of injury is fall for both dentition; In case of permanent dentition, the ratio of injury by sports is increased. 6. The place of injury for primary teeth is mainly home(38.8%); Street and school for permanent teeth(42.5% and 35%, respectively). 7. The frequency by month shows that the injury is most frequently occurred in July. 8. The frequency analysis by hour shows that injury for primary teeth mostly happens in the morning; in the afternoon for permanent teeth. 9. The position of injured teeth according to the area in the mouth is mainly maxilla anterior in both case of primary and permanent teeth and especially the ratio of central incisors is high, 10. More than half(59.2%) of patients came to the hospital within one day and the seriously injured were likely to come within one day than the slightly injured.

TRAUMATIC INJURIES TO THE UPPER INCISORS IN PRIMARY AND MIXED DENTITION (유치열과 혼합치열기 어린이의 상악 절치부 외상)

  • In children, change in lifestyles such as an increase in participation of physical activities has increased the chance of receiving an injury, harming the child and their parents. This study was carried out to give basic information of traumatic injuries to the upper anterior teeth which can be used in prevention of future complications that may arise. From June, 2002 to May, 2003, of the 8,799 children in their primary and mixed dentition visiting Department of Pediatric Dentistry, College of Dentistry, Yonsei University 955 upper anterior teeth(primary 543, permanent 412) of 487 children(male 316, female 171) with traumatic injures to the upper incisors were studied. The results were as follows: 1. Trauma to the primary teeth prevailed at the age of 1-3 and to the permanent teeth at the age of 7. The male to female ratio was 1.85:1. 2. Trauma to the upper incisors for both the primary and permanent teeth occurred in the afternoon. Primary teeth were injured most frequently in the home(44.1%) and the permanent teeth outdoors, in the street(17.5%). 3. Both the primary and permanent teeth were most injured by fall-down injuries(39.8%, 12.9%), and next were collisions(22.2%, 6.0%). For the primary teeth, high fall, traffic accident, violence and sports are next in order, and for the permanent teeth, violence, sorts, traffic accidents were next. 4. Average number of 1.6 primary teeth and 2.8 permanent teeth were injured with the right central incisor most prevailing. 5. Crown fracture without pulpal exposure and subluxation injures were highest in frequency in both the primary and permanent teeth.

Epidemiology of central venous catheter related blood stream infections in pediatric patients (중심정맥 도관 관련 감염의 역학 고찰)

  • Purpose : The purpose of this study is to investigate the pathogens of central venous catheter-related blood stream infections and search for the association among the insertion site, the duration and the underlying conditions with the prevalence of central venous catheter-related blood stream infections under 15 years old. Methods : A retrospective study was performed from Jan, 2003 to Dec, 2003 in Severance Hospital on 112 patients who undertook central venous catheter insertions. Results : We examined 112 patients who undertook central venous catheter insertion. The mean age of patients was $4.77{\pm}4.12$ years old. Coagulase negative Staphylococci was the most common organism of central venous catheter-related blood stream infections accounting for 33.9 percent, followed by Eenterococcus faecium(9.3 percent), and Staphylococcus aureus(7.5 percent), The most common insertion site was the right femoral vein, followed by the right jugular vein and the left femoral vein. The mean insertion period was $14.17{\pm}12.00$ days. Conclusion : Central venous catheter-related blood stream infections were not only related to the underlying conditions, but also to the insertion site. We need to study the clinical importance of coagulase negative Staphylococci as it is part of the normal flora of the skin. In future, more studies are needed to take preventive measures and improve treatment methods.

THE DENTAL CARIES, CARIOGENIC MICROORGANISMS AND LEVELS OF SALIVARY IMMUNOGLOBULIN IN SUBJECTS WITH DOWN'S SYNDROME (다운증후군 환자의 치아우식증과 치아우식 원인균 및 타액내 면역항체의 연관성)

  • This study investigated the relationship between dental caries and various oral factors in subjects with Down's syndrome. We compared 25 subjects with Down's syndrome with 63 healthy control. The dental caries index and plaque index were examined, and the total salivary immunoglobulin A and S. mutans specific salivary immunoglobulin A concentration were measured using ELISA. The S. mutans counts, Lactobacillus counts and buffer capacity were measured with Dentocult test medium. The decayed and filled surface index of deciduous teeth in subjects with Down's syndrome was lower than in controls(p<0.001). The plaque index and total salivary immunoglobulin A concentration showed no difference, S. mutans specific salivary immunoglobulin A concentration and buffer capacity in subjects with Down's syndrome were lower than in controls(p<0.001). There was no significant difference between two groups in the S. mutans counts and Lactobacillus counts. In 9-11 year age group, S. mutans counts in subjects with Down's syndrome was lower than in controls(p<0.001) and S. mutans specific salivary immunoglobulin A concentration was lower(p<0.05). There was a high correlation among deciduous dental caries index and buffering capacity and S. mutans counts.

  • Recently, Patient Related Management(PRM) is focused to be increase the number of patients in hospitals. Therefore, the participants of this research are inpatient(104) and out patient(98) in a tertiery hospital in Seoul also, the research purpose is to analysis the relationship between hospital satisfaction and loyalty in various aspects by questionnaires. In the results, the relationship between hospital satisfaction and loyalty was very significant. Among various hospital satisfactions, the reservation satisfaction for out-patients and non-medical service satisfaction for in-patients were significant factors. In the demographic factors, the relationship between age and the hospital loyalty is significant. Therefore, we should make efforts to increase the patient's satisfaction.
