Analysis of Mammography for Patient with Breast Cancer (유방암 환자를 대상으로 한 유방영상검사 분석)

  • The purpose of this study is to evaluate the diagnostic accuracy of breast cancer from mammography images and to investigate the characteristics of patients who was diagnosed as a breast cancer. The subjects for this study were 85 patients who underwent breast imaging test with screen-film method and investigated from March 2010 to October 2010. The average age of the subjects is 51.9 years old. The 43.5% of onsets were located at the upper outer quadrant (UOQ). In terms of the age group, group for 40-49 ages were highest 37.6 (32/85)%. As for breast composition, fatty and dense breast were 30.6% (26/85) and 69.4% (59/85) respectively. The average age of the patients with fatty breast is 62.4 years old, whereas those with dense breast was 46.5 years old. The false negative rate of mammography images was 27.1% (23/85) and the sensitivity of mammography was 72.9% (62/85). The false negative rate of the patients with dense breast was 32.2% (19/59). the sensitivity of them was 67.8% (40/59).

  • In this paper, we used MOM-based BIM(Born Iterative Method) algorithm to implement the inverse scattering for the detection of cancer. We adopted two-dimensional breast structure, integral equations and two-dimensional Green's function is solved with MoM(Method of Moment) to analyzing electromagnetic scattering phenomena. In addition, verifying the calculation of developed inverse scattering algorithm and analyzing medical applicability and limitations of the algorithm.

Detection Efficiency of Microcalcification using Computer Aided Diagnosis in the Breast Ultrasonography Images (컴퓨터보조진단을 이용한 유방 초음파영상에서의 미세석회화 검출 효율)

  • Digital Mammography makes it possible to reproduce the entire breast image. And it is used to detect microcalcification and mass which are the most important point of view of nonpalpable early breast cancer, so it has been used as the primary screening test of breast disease. It is reported that microcalcification of breast lesion is important in diagnosis of early breast cancer. In this study, six types of texture features algorithms are used to detect microcalcification on breast US images and the study has analyzed recognition rate of lesion between normal US images and other US images which microcalification is seen. As a result of the experiment, Computer aided diagnosis recognition rate that distinguishes mammography and breast US disease was considerably high 70~98%. The average contrast and entropy parameters were low in ROC analysis, but sensitivity and specificity of four types parameters were over 90%. Therefore it is possible to detect microcalcification on US images. If not only six types of texture features algorithms but also the research of additional parameter algorithm is being continually proceeded and basis of practical use on CAD is being prepared, it can be a important meaning as pre-reading. Also, it is considered very useful things for early diagnosis of breast cancer.

$^{99m}Tc$-Tetrofosmin Scintimammography in Suspected Breast Cancer Patients: Comparison with $^{99m}Tc$-MIBI (유방암이 의심되는 환자에서 $^{99m}Tc$-Tetrofosmin을 이용한 유방스캔: $^{99m}Tc$-MIBI와 비교)

  • Purpose: The aim of this study was to investigate the diagnostic role of $^{99m}Tc$-Tetrofosmin in detection of breast cancer and compared with that of $^{99m}Tc$-MIBI. Material and Methods: Forty-eight patients with a clinically palpable mass or abnormal mammographic or ultrasonographic findings had $^{99m}Tc-MIBI\;and\;^{99m}Tc$-Tetrofosmin scintimammographies after intravenous injection of 925 MBq of radiopharmaceuticals. The scintimammographs were correlated with histopathologic findings. Results: Thirty-three patients were diagnosed with breast cancer and 15 patients with benign breast diseases. The numbers of true positive, true negative, false positive, and false negative cases of $^{99m}Tc$-MIBI scintimammography were 29, 10, 5, and 4 respectively. The sensitivity, specificity, positive predictive value, and negative predictive value of $^{99m}Tc$-MIBI scintimammographies were 87.8%, 66.7%, 85.3%, and 71.4% respectively. The numbers of true positive, true negative, false positive, and false negative cases of $^{99m}Tc$-Tetrofosmin were 31,10, 5, and 2 respectively. The sensitivity, specificity, positive predictive value, negative predictive value of $^{99m}Tc$-Tetrofosmin were 93.9%, 66.7%, 86.1%, and 73.3% respectively. One patient was false negative in both $^{99m}Tc-MIBI\;and\;^{99m}Tc$-Tetrofosmin acintimammographies and its size was 0.5 cm. Conclusion: $^{99m}Tc-Tetrofosmin\;and\;^{99m}Tc-MIBI$ were non-invasive and useful in detection of breast cancer and $^{99m}Tc$-Tetrofosmin was comparable to the $^{99m}Tc$-MIBI in detection of primary breast cancer.

  • As the number of cancer patients and its death rate increases, this study is to understand the occurrences in cancer patients in Busan area and to provide active help for treatment, prevention and management of cancer patients. It was investigated and analyzed based on 4462 patients who were diagnosed with cancer and received radiation therapy from Jan. 2011 to Dec. 2012 at tertiary medical institution in Busan area. The seven main cancer types were breast cancer, cervical cancer, lung cancer, colon rectal cancer, brain tumor, laryngopharyngeal cancer and liver cancer. The most common type of cancer was breast cancer which takes up 38.1% of cancer patients stood out from other cancer types. Breast cancer is that shows increasing trend and in need of appropriate countermeasures and managements for decreasing cancer risk. Therefore the causes and prevention of breast cancer analysis, an appropriate management program is required.

The National Survey of Breast Cancer Treatment Pattern in Korea (1998): The Use of Breast-Conserving Treatment (1998년도 우리나라 유방암 치료 현황 조사: 유방보존술 적용 실태를 중심으로)

  • Purpose: In order to improve the proper use of radiotherapy and breast-conserving treatment (BCT) in the management of breast cancer, current status of breast cancer treatment in Korea was surveyed nationwide and the use of BCT were evaluated. Materials and Methods: Patients characteristics and treatment pattern of 1048 breast cancer patients from 27 institutions diagnosed between January, 1998 and June, 1998 were analyzed. The incidence of receiving BCT was analyzed according to the stage, age, geography, type of hospital, and the availability of radiotherapy facility. Results: Radical mastectomy was peformed in 64.8$\%$ of total patients and 26$\%$ of patients received breast- conserving surgery (BCS). The proportions of patients receiving BCT were 47.5$\%$ in stage 0, 54.4$\%$ in stage I, and 20.3$\%$ in stage II, Some of the patients (6.6$\%$ of stage I, 10.1$\%$ of stage II and 66.7$\%$ of stage III) not received radiotherapy after BCS. Only 45$\%$ of stage III patients received post-operative radiotherapy after radical mastectomy. The proportion of patients receiving BCT was different according to the geography and availability of radiotherapy facilities. Conclusion: Radiotherapy was not fully used in the management of breast cancer, even in the patients received breast-conserving surgery. The proportion of the patients who received BCT was lower than the report of western countries. To improve the application of proper management of breast cancer, every efforts such as a training of physicians, public education, and improving accessibility of radiotherapy facilities should be done. The factors predicting receipt of BCT were accessibility of radiotherapy facility and geography. Also, periodic survey like current research is warranted.

  • Carcinoma of the axillary tail of Spence is a rare type of breast cancer that develops at a specific anatomical position in the breast, with an incidence of approximately 0.3%. It should be differentiated from axillary soft tissue tumor, axillary ectopic breast cancer, and lymph node metastases of breast and other primary cancers. Here, we report a case of carcinoma of the axillary tail of Spence in a 47-year-old patient who visited our clinic with a lower axillary mass and was diagnosed based on mammography, US, CT, and MRI findings.

A Qualitative Study of Health Beliefs and Health Behaviors among Korean Breast Cancer Survivors (한국 여성 유방암 생존자의 건강신념과 건강행위에 대한 질적 연구)

  • The purpose of this study was to explore 1) the illness experiences, 2) health behaviors (causal thoughts), and 3) health beliefs (health behavior changes after cancer diagnosis) among Korean breast cancer survivors using focus groups, which is a form of qualitative research methods. Thus, this study intended to understand how the illness experiences and health beliefs are associated with health behaviors. Three focus groups composed of 16 breast cancer survivors living in Seoul and GyeongGi were conducted with 5 to 6 people in each group. For illness experiences, health beliefs, and health behaviors of breast cancer survivors, some major themes related to physical, behavioral, psychological, and social domains were identified. Understanding health beliefs and health behaviors for Korean breast cancer survivors will be an important step toward developing social work interventions for enhancing health promotion.

Risk Factors for Depression and Anxiety among Breast Cancer Survivors in Their 40s (40대 유방암 생존자의 우울·불안 위험요인)

  • This study was performed to examine the prevalence of and risk factors for depression and anxiety among breast cancer survivors in their 40s. Completed questionnaires were collected from 609 breast cancer survivors in their 40s who agreed to participate the study. The mean scores of CES-D and GAD-7 were 16.35(SD=9.24) and 4.25(SD=4.17), respectively. Nearly 47.7% of the participants had depression and 10.3% had anxiety. The mean score of pain severity was 1.91(SD=1.60) and 10.9% of the participants reported more than moderate pain. The final model in the hierarchical regression analysis showed that pain interference, unemployment, the type of live-in partner, and past psychiatric disease were the significant risk factors for depression, and pain interference, unemployment and past psychiatric disease for anxiety. These results show the prevalence of depression and anxiety among breast cancer survivors in their 40s is high and suggest appropriate psychosocial intervention should be provided for high risk groups based on those risk factors.
