Association between False Positive, False Negative, High-Background Cases and Humidity in One Lambda LABTypeTM HLA-DR Typing (One Lambda LABTypeTM을 이용한 HLA- DR Typing시 나타나는 위양성과 위음성, High-Background 사례와 검사실 습도의 관계분석)

  • Human leukocyte antigen (HLA) typing was performed in the diagnostic immunology laboratory of the Seoul National University Hospital. Among 611 HLA-DR tests, specific bead reactions suspected of being false positive and false negative in Lot 20 reagents were found. Therefore, we aimed to identify the factors causing cut-off corrections by examining cases where cut-off corrections were not made for 533 test results and cases where cut-off corrections were made for 78 cases after the cut-off corrections of specific beads. Frequency analysis was conducted to verify the demographic characteristics, and descriptive statistics were used to assess the humidity in the laboratory as a variable. Cross-tabulation was done to examine the association between cut-off corrections and demographic characteristics. Independent samples t-tests were conducted to verify the difference in humidity based on cut-off corrections. Finally, logistic regression analysis was conducted to examine the relationship between humidity levels and the rate of cut-off corrections, and results showed as the humidity level in the laboratory highs, the number of cut-off corrections decreased by a factor of 0.986. This suggests cut-off corrections rate increases when the humidity lowers. Therefore, it indicates that humidity in the laboratory is also a factor that affects HLA typing results.

Evaluation of Usefulness for $Monototal^{TM}$ in Non Small Cell Lung Cancer (비소세포성폐암 환자의 $Monototal^{TM}$ 유용성 평가)

  • Purpose: Cancerous death rate 1 place from 2008 year domestic is the lung cancer. The body count which is caused by in the lung cancer every year is increasing. At this lung cancer is the most non small cell lung cancer. $Monototal^{TM}$ test is short reaction time. it is known as the experiment where the example standard of living is high about non small cell lung cancer. This is the study evaluate usefulness of $Monototal^{TM}$ kit. Materials and Methods: $Monototal^{TM}$ were measured using IRMA kit, using 40 CEA positive patients sample which are diagnosed NSCLC, 15 SCC positive patients sample which are diagnosed NSCLC, 40 Cyfra 21-1 positive sample, 20 negative sample in Seoul national university from March to April, 2009. Results: According to result of the $Monototal^{TM}$ test, which is benignancy rate 87.5% in CEA positive patients sample, 93.3% SCC positive patients sample and 100% Cyfra 21-1 positive sample Conclusions: We recommend that using of $Monototal^{TM}$ parallel with different tumor markers. It was useful that diagnosis and convalescence presumption of Non small cell lung cancer.

  • This study is intended to compare the performances of sound source localization methods used for stable automobile control by improving voice recognition rate in automobile environment and suggest how to improve their performances. Generally, sound source location estimation methods employ the TDOA algorithm, and there are two ways for it; one is to use a cross correlation function in the time domain, and the other is GCC-PHAT calculated in the frequency domain. Among these ways, GCC-PHAT is known to have stronger characteristics against echo and noise than the cross correlation function. This study compared the performances of the two methods above in automobile environment full of echo and vibration noise and suggested the use of a median filter additionally. We found that median filter helps both estimation methods have good performances and variance values to be decreased. According to the experimental results, there is almost no difference in the two methods' performances in the experiment using voice; however, using the signal of a song, GCC-PHAT is 10% more excellent than the cross correlation function in terms of the recognition rate. Also, when the median filter was added, the cross correlation function's recognition rate could be improved up to 11%. And in regarding to variance values, both methods showed stable performances.

Clinical Utility of Bronchial Washing PCR for IS6110 and Amplicor for the Rapid Diagnosis of Active Pulmonary Tuberculosis in Smear Negative Patients (객담도말 음성인 폐결핵환자의 기관지세척액에서 Amplicor PCR과 IS61110 PCR의 임상적 유용성에 관한 비교 연구)

  • Background : There is a well recognized interlaboratory variation in the results using the polymerase chain reaction(PCR) to detect the IS6110 sequence. The clinical utility of a commercially developed PCR test(Amplicor) in bronchial washings for detecting pulmonary tuberculosis in smear negative patients was evaluated. The sensitivity and specificity of Amplicor was compared with that of an in-house PCR test used for detecting the IS6110 sequence of Mycobacterium tuberculosis(M.tbc) in the bronchial washing fluid. Methods : 66 patients whose sputum smear for M. tbc were negative or who could not produce any sputum were recruited from January 1999 to July 1999. They all had a bronchoscopy performed to determine if there were signs of hemoptysis, patients who could not cough up sputum, lung lesion that exclude pulmonary tuberculosis. Pulmonary tuberculosis was diagnosed on the basis of a positive culture or a response to anti-tuberculosis therapy. Results : 19 patients with tuberculosis were identified and samples from 16 patients were later confirmed by culture. Bronchial washing for Amplicor PCR revealed a sensitivity, specificity, positive and negative predictive values of 94.7%, 97.9%, 94.7%, 97.9%, respectively. Using IS6110 based PCR, the sensitivity, specificity, positive and negative predictive values were of 73.7%, 87.2%, 70%, 89.1% respectively. Conclusion : Bronchial washing for Amplicor PCR proved to be more useful than IS6110 based PCR in rapidly diagnosing smear negative pulmonary pulmoary tuberculosis in patients where tuberculosis was likely to be differential and rapid diagnosis was essential for optimal treatment.

Clustered Microcysts Detected on Breast US in Asymptomatic Women (무증상 여성의 유방초음파에서 발견된 군집 미세낭종)

  • Purpose To investigate the incidence, outcomes, and imaging characteristics of clustered microcysts detected on breast US in asymptomatic women, and suggest appropriate management guidelines. Materials and Methods We identified and reviewed the lesions recorded as "clustered microcysts" on breast US performed in asymptomatic women between August 2014 and December 2019. The final diagnosis was based on pathology and imaging follow-up results for at least 12 months. Results The incidence was 1.5% and 100 patients with 117 lesions were included. Among 117 lesions, 3 (2.6%), 2 (1.7%), and 112 (95.7%) were malignant, high-risk benign, and benign lesions, respectively. The malignant lesions included two cases of ductal carcinoma in situ and one invasive ductal carcinoma. Two of them were assessed as category 4, showing mammographic suspicious microcalcifications and internal vascularity on Doppler US. The remainder was a false negative case and showed echo pattern change on the 12-month follow-up US. Conclusion The incidence of clustered microcysts on breast US in asymptomatic women was 1.5% and malignancy rate was 2.6% (3 of 117). Knowledge of outcomes and imaging features of benign and malignant clustered microcysts may be helpful for radiologists, thereby aiding categorization and management recommendations.

Comparison of Three Third-Generation Anti-HCV Enzyme Immunoassay Tests (세가지 효소면역측정 시약을 이용한 C형 간염 바이러스 항체 검사의 비교)

  • The aim of this study was to evaluate domestic enzyme immunoassay(EIA) kit 'LG RCD 3.0' (LG) for the detection of antibody to hepatitis C virus(anti-HCV) in comparision with Axsym HCV version 3.0(Axsym), Cobas Core anti-HCV EIA(Cobas). Cobas kit shows better clear distinction between positive and negative by signal/cutoff ratio(S/C), but it also reveal relatively high false positive rate. The concordance rate of test results between LG and Axsym was 96.2%, between LG and Cobas was 95.5%, and total agreement between three EIA kit was 93.9%. LG were relative poor distinction between positive and negative results, but it could be applied clinically as a screening tool for hepatitis C in general population. The SIC of one false negative result by LG was 0.91, and false positive were less than 4.0, therefore we concluded it is necessary to confirm by immunoblotting assay when SIC were between 0.8 and 4.0.

Phonotaxis of the African Mole Cricket, Gryllotalpa africana Palisot de Beauvois (땅강아지의 주음성에 관한 연구)

  • Phonotaxis of the African mole cricket, Gryllotalpa africana palisot de Beauvois, was investigated in 1990 and 1992 at the agronomy Experiment Station of Korea Ginseng & Tobacco Research Institute in Hwaseong-gun, Kyonggi-do, Male adults produced calling sounds (calling songs) through the openings of subsurface burrows. Intensities of the sound were 77-80 dB at 15 cm above the openings. When tape recordings of male calling songs were broadcasted outdoors at 105-110 dB by two horn speakers installed at the center of a 1.4 m diameter-funnel, flying adults were attracted for 1.5 hours from about 30 minutes after sunset. Among attracted adults, 14.3-16.9% landed in the funnel, and 65.7-74.7% landed on the ground within 2m form the sound source. Females were 66.7-74.3%, which seemed to be due to the sex ratio of the population in the field. Adults landing in the funnel and at the distance of within 2m from the center of the funnel were tend to be a little more than those attracted to a blacklight trap.

  • Skip lesion is not uncommon feature in osteosarcoma and considered to be importantly associated with poor prognosis factor, and thus, should be excised with the main mass. The accurate pre-operative evaluation of the intramedullary extent of osteosarcoma is essential, because it determines the level of bone resection. Among the reliable detection methods, bone scan has a drawback of high rate of false negative results and regional MRI has a difficulty to cover the whole involved lesions without clinical suspicion. The authors report a case of osteosarcoma of the distal femur with a proximal skip lesion that was not detected by either regional MR imaging or by bone scan, but which was visualized by FDG-PET/CT.

The Analysis of COVID-19 Pooled-Testing Systems with False Negatives Using a Queueing Model (대기행렬을 이용한 위음성률이 있는 코로나 취합검사 시스템의 분석)

  • COVID-19 has been spreading all around the world, and threatening global health. In this situation, identifying and isolating infected individuals rapidly has been one of the most important measures to contain the epidemic. However, the standard diagnosis procedure with RT-PCR (Reverse Transcriptase Polymerase Chain Reaction) is costly and time-consuming. For this reason, pooled testing for COVID-19 has been proposed from the early stage of the COVID-19 pandemic to reduce the cost and time of identifying the COVID-19 infection. For pooled testing, how many samples are tested in group is the most significant factor to the performance of the test system. When the arrivals of test requirements and the test time are stochastic, batch-service queueing models have been utilized for the analysis of pooled-testing systems. However, most of them do not consider the false-negative test results of pooled testing in their performance analysis. For the COVID-19 RT-PCR test, there is a small but certain possibility of false-negative test results, and the group-test size affects not only the time and cost of pooled testing, but also the false-negative rate of pooled testing, which is a significant concern to public health authorities. In this study, we analyze the performance of COVID-19 pooled-testing systems with false-negative test results. To do this, we first formulate the COVID-19 pooled-testing systems with false negatives as a batch-service queuing model, and then obtain the performance measures such as the expected number of test requirements in the system, the expected number of RP-PCR tests for a test sample, the false-negative group-test rate, and the total cost per unit time, using the queueing analysis. We also present a numerical example to demonstrate the applicability of our analysis, and draw a couple of implications for COVID-19 pooled testing.

Gastroduodenoscopic Findings and Effect of Therapy of Helicobacter pylori Infection in Children (소아 Helicobacter pylori 감염의 상부 위장관 내시경 소견 및 치료 효과)

  • Purpose: Helicobacter pylori infection is known to be associated with acute or chronic abdominal pain and upper gastrointestinal bleeding in children. This study was performed to analyze the gastroduodenoscopic findings and the efficacy of triple therapy with omeprazole, amoxicillin and clarithromycin between one and two weeks of duration in children with H. pylori infection. Methods: We have assessed retrospectively 60 patients presented with acute or chronic abdominal pain or upper gastrointestinal bleeding. H. pylori infection was confirmed by endoscopic biopsy and rapid urease test. Out of 60 patients, 30 patients were treated with a combination of omeprazole, amoxicillin, and clarithromycin for one week, and the other 30 patients were treated for two weeks with the same medication. Efficacy of treatment was assessed 4 weeks after the termination of treatment by using the $^{13}C$ urea breath test. Results: The 60 patients with the complaint of diffuse abdominal pain, epigastric pain, vomiting or hematemesis were included in this study. One-week treatment group (group I) consisted of 30 patients (14 male, 16 female) with mean age of $11.6{\pm}2.67years$. Two-week treatment group (group II) consisted of 30 patients (11 male, 19 female) with mean age of $10.7{\pm}4.17years$. In group I, H pylori were eradicated in 26 out of 30 patients (86.7%). In group II, H. pylori were eradicated in 26 out of 30 children (86.7%). Both groups did $^{13}C$ urea breath test after 4 weeks after termination of the triple therapy. The eradication rates were same in both groups as 86.7%, 26 out of 30 patients in each group. The results of endoscopy were nodular gastritis 26 (43.3%), erosive gastritis 10 (16.7%), hemorrhagic gastritis 7 (11.7%), gastric ulcer 2 (3.3%) and normal finding 15 (25.0%). Conclusion: In this study, the nodular gastritis was most common endoscopic findings with H. pylori positive patients. The eradication rate of H. pylori with omeprazole, amoxicillin and clarithromycin was 86.7% and it would be highly effective as primary treatment with no significant differences in the eradication rate between one-week and two-week treatment groups. However, we should need more long-term follow-up data.
