Platelet Serotonin Level and Psychological Features of the Suicidal Attempters (자살시도 환자의 혈소판내 세로토닌 농도와 심리학적 특성에 관한 연구)

  • Objectives : Significant progress has been made in understanding psychosocial, psychological, and environmental factors associated with suicide. However it is only recently that attention has been paid to the understanding of the neurobiology of suicide. The aim of this study was to examine the relationship between platelet serotonin level and suicidal behavior and psychological features of the suicidal attempters. Methods : After a suicidal attempt platelet serotonin level was measured from 21 patients and compared it with those from depression patients and normal controls. Also MMPI, HAM-D, Barratt impulsiveness scale(BIS) were done to evaluate their psychological features. Results : 1) There was no significant difference in sex ratio of the suicidal attempters. 2) There was no significant difference in platelet serotonin levels among three groups. 3) The analysis of MMPI revealed that the scores of D, Hs, Pt in clinical scales were significantly higher in suicide patients and scores of D, Pa, Si were in depression patients. 4) The HAM-D score was significantly higher in depression and suicide patients, especially in depression patients. 5) The analysis of Barratt impulsiveness scale revealed that the scores of nonplanning, motor and cognitive impulsiveness scale were significantly higher in suicide patients. Conclusions : There was no significant difference in platelet serotonin levels among three groups. However the analysis of psychological features revealed significant differences. Therefore we concluded that psychological examinations are benefit to evaluate the suicidal tendency.

Differences of Personality Characteristics According to the Suicide Symptoms in Patients with Depression (우울증 환자에서 자살 증상 유무에 따른 성격 특성의 차이에 관한 연구)

  • Objectives : The purpose of this study was to examine personality traits associated with suicidal symptoms (with history of suicide attempt or suicidal idea) in depressed patients. Methods : A sample 186 patients diagnosed with major depressive disorder was divided into two groups : suicidal group (with history of suicide attempt or suicidal idea, n=70) and non-suicidal group (without history of suicide attempt or suicidal idea, n=116). NEO Personality Assessment System were used to evaluate personality trait. Results : Neuroticism was high in suicidal group (p=0.041). In multivariate logistic regression, Neuroticism was also a significant influence on suicidal symptoms (Odds Ratio=1.04, 95% Confidence Interval 1.01-1.07, p=0.0145). Conclusions : High Neuroticism is a useful information to identify individuals at high risk of suicide in depressive patients.

Heart Rate Variability and Lipid Profile in Patients with Major Depressive Disorder (주요우울장애 환자에서의 심박변이도와 혈중 지질 농도와의 연관성)

  • Objectives: The analysis of heart rate variability (HRV) is a useful non-invasive tool to investigate the autonomic nerve function. Previous studies on the relationship between HRV and depression have been reported controversial results. Similarly, the correlation between the serum lipids and depression is debatable. The purpose of this study is to examine the relationship between heart rate variability, lipid profile and depression. Methods: A total of 42 patients with major depressive disorder (MDD) and 32 age and sex-matched normal subjects who had no previous history of major medical and mental illnesses were recruited for this study. A structured-interview was used to assess the general characteristics and psychiatric illness. HRV measures were assessed by time-domain and frequency-domain analyses. Psychological symptoms were measured using the Hamilton rating scale for anxiety (HAM-A), Hamilton rating scale for depression (HAM-D). In addition, the evaluation for lipid profile was performed by blood test. Results: In serum lipid profile test, MDD group showed higher cholesterol ($197.68{\pm}42.94$ mg/dL vs. $176.85{\pm}34.68$ mg/dL, p=0.044), TG ($139.45{\pm}92.54$ mg/dL vs. $91.4{\pm}65.68$ mg/dL, p=0.018), LDL ($130.03{\pm}33.18$ vs. $106.62{\pm}27.08$, p=0.004) level than normal control group. In HRV time domain analyses, the standard deviation of the NN interval (SDNN) was decreased in MDD group than normal control group, but was not significant ($32.82{\pm}14.33$ ms vs. $40.36{\pm}21.40$ms, p=0.078). ApEn (Approximate Entrophy) was significantly increased in MDD group than normal control group ($1.13{\pm}0.11$ vs. $0.91{\pm}0.18$, p<0.001). ApEn was correlated with LDL level (r=0.277, p=0.028), HAM-D scores (r=0.534, p<0.001) and HAM-A scores (r=0.470, p<0.001). Conclusions: MDD patients showed increased ApEn, one of the HRV measurement. And this ApEn was correlated with LDL, HAM-D and HAM-A scores. In this study, the analysis of ApEn would be a useful test of MDD.

Risk Factors for Depression, Anxiety, and Stress in Patients with Polycystic Ovary Syndrome (다낭난소증후군 환자에서의 우울, 불안, 스트레스를 유발하는 위험 인자)

  • The aim of this study was to evaluate anxiety, depression and stress in women with polycystic ovary syndrome(PCOS) and to investigate the risk factors related to psychological difficulties. Sixty women with PCOS were evaluated for level of psychological stress using Beck depression inventory(BDI) and Depression anxiety stress scale(DASS) questionnaire. Serum antimullerian hormone, total testosterone, lutenizing hormone, follicle stimulating hormone, estradiol, lipid profile and 75g oral glucose tolerance test were measured. Thirty healthy women served as the control. Fifty two women with PCOS and 29 healthy women completed a questionnaire. Women with depression who scored >13 by BDI and >10 by DASS were 38.5 %, women with anxiety who scored >8 by DASS were 23.1 %, and women with stress who scored >15 by DASS were 30.8 %, which were significantly higher than control. In PCOS women, total testosterone, LH and AMH were significantly correlated with depression and stress. Weight, body mass index and waist-hip ratio were also significantly correlated with depression. In women diagnosed as diabetes and hyperlipidemia, depression and stress were significantly prevalent. Women with PCOS seemed to be more vulnerable to depression, anxiety and stress. Early diagnosis and management should be considered.

Risk Factors for Depression of Patients with Tuberculosis in Tuberculosis Specialty Hospital (결핵전문병원에 입원한 결핵환자의 우울증위험인자)

  • Objectives : This study aimed to investigate the risk factors of depression for patients with tuberculosis(TB). Methods : A total of 57 patients with TB were recruited. All participants completed the Becks Depression Inventory-II for evaluating depressive symptoms. The risk factor for depression was analyzed by binary logistic regression analysis. Nomogram was performed for probability of depression. Results : Low body mass index(BMI, OR 0.801, 95% CI 0.65, 0.98), interruption of treatment for TB(OR 5.908, 95% CI 1.19, 29.41), past history of depression(OR 24.653, 95% CI 1.99, 308.44) were associated with increased risk for depression. The calibration curve for predicting probability of survival showed a good agreement between the nomogram and actual observation(Original C-index=0.789, bias corrected C-index=0.754). Conclusions : The result of the present study indicate that low BMI, interruption of treatment for TB, and past history of depression were risk factors for depression in patients with TB. The psychiatric intervention may be needed to prevent depression if the patients with TB have risk factor during treatment for TB.

Association of depression with chewing problems in Koreans : A cross-sectional study using the Korea National Health and Nutrition Examination Survey 2016 (한국인에서 씹기 불편감과 우울증의 연관성: 2016 국민건강영양조사를 이용한 단면 연구)

  • Purpose: This study was designed to analyze the relationship between the presence and severity of depression and chewing problems (CPs) in a representative sample of the general population. Materials and Methods: Health surveys and examinations were conducted on a nationally representative sample (n = 8150) of Korean was conducted. CPs was determined by a simple survey response concerning "Do you feel uncomfortable about chewing your food because of problems with your mouth such as teeth, dentures and gums?" Depression was defined as individuals with a total score ≥ 10 on the Patient Health Questionnaire (PHQ)-9 survey. Data regarding demographics, socioeconomic history and comorbid health conditions were used to analyze adjusted logistic regression models. Results: In the Korean population, the prevalence of depression was significantly greater in individuals with CP (17.2%) than in those without CP (10.2%). On multivariate logistic regression analysis, the presence of depression was significantly associated with CPs (adjusted odd ratio [aOR]: 1.90, P < 0.001). The risk of CPs increased with increasing severity of depression as follows: severe depression (OR: 2.62, P < 0.001), moderately severe depression (OR: 2.19, P < 0.001). Conclusion: The presence of depression was significantly associated with CPs, especially in severely depressed individuals. Depression screening should be considered in treating CP patients.

THE DEGREE OF EGO IMPAIRMENT IN ADOLESCENT PSYCHIATRIC PATIENT GROUPS (청소년 정신과 환자군을 대상으로 한 자아기능의 장애정도 비교 - Rorschach검사의 자아손상지표를 중심으로 -)

  • Objective:This study was designed to examine the validity of the Ego Impairment Index(EII) in adolescent psychiatric patients(depression, depressive-conduct disorder, schizophrenia), and to explore the validity of the depressive-conduct disorder as a diagnostic entity. Method:19 depressives, 13 depressive-conduct disordered, and 10 schizophrenics with the age from 12 to 18 were selected based on DSM-IV and ICD-10 criteria, and their responses of the Rorschach and MMPI were analyzed. The EII was empirically developed by Perry and Viglione(1991) in order to assess the degree of ego impairment, and was derived from the Rorschach test. Results:It was founded that a single factor, ego impairment, was derived from the principal component analysis and explained 57.18% of total variances. The degree of ego impairment was grater in schizophrenics than in the other two groups, but there were no difference between depressives and depressive-conducts. Conclusions:These results offered support for the use EII as an empirical means of assessing the degree of ego impairment in adolescent patients. And these results suggest that depressive-conduct disorder group is more similar to the depressive group, implying that depressive-conduct disorder might correspond to so called masked depression. The clinical implication and limitation of present study were discussed.

EEG Classification for depression patients using decision tree and possibilistic support vector machines (뇌파의 의사 결정 트리 분석과 가능성 기반 서포트 벡터 머신 분석을 통한 우울증 환자의 분류)

  • Depression is the most common and widespread mood disorder. About 20% of the population might suffer a major, incapacitating episode of depression during their lifetime. This disorder can be classified into two types: major depressive disorders and bipolar disorder. Since pharmaceutical treatments are different according to types of depression disorders, correct and fast classification is quite critical for depression patients. Yet, classical statistical method, such as minnesota multiphasic personality inventory (MMPI), have some difficulties in applying to depression patients, because the patients suffer from concentration. We used electroencephalogram (EEG) analysis method fer classification of depression. We extracted nonlinearity of information flows between channels and estimated approximate entropy (ApEn) for the EEG at each channel. Using these attributes, we applied two types of data mining classification methods: decision tree and possibilistic support vector machines (PSVM). We found that decision tree showed 85.19% accuracy and PSVM exhibited 77.78% accuracy for classification of depression, 30 patients with major depressive disorder and 24 patients having bipolar disorder.

Influence of Depression and Eating Disorder on College Adjustment in Female Freshmen (일개대학 여자신입생의 우울, 섭식장애가 대학생활적응에 미치는 영향)

  • The study was conducted to identify the influence of depression and eating disorder on college adjustment in female freshmen. Two hundred twenty one subjects completed a structured questionnaire in March to April 2016. Data were analyzed using IBM SPSS 21.0 program. The results of this study were as follows: college adjustment in female freshmen was 106.14, depression was 10.22, bulimia nervosa was 61.84, anorexia nervosa was 10.46. Depression (r=-0.409, p<.001) and bulimia nervosa(r=-0.133, p<.048) were negatively related with college adjustment and bulimia nervosa(r=0.391, p<.001) and anorexia nervosa(r=0.302, p<.001) were positively related with depression. Female freshmen with high depression and eating disorder had lower college adjustment. The results of this study suggest that it needs to do a screening test for depression and eating disorders, predict maladaptation and help their college adjustment in freshmen.

Association between Depression and Sociodemographic Factors of Elderly Welfare Facility Users in a City (일도시 노인복지시설 이용자들에서 우울증과 인구사회학적 요인과의 연관성)

  • Objectives : As the prevalence of elderly depression increases, it becomes urgent problem to provide preventive and management measures. But in practice, it is difficult to detect depression in early stage. The purpose of this study is to evaluate the association between sociodemographic factors and depression in elderly welfare facility users in a city. Methods : In this research, the severity of depression and sociodemographic factors(gender, age, education, co-morbid disease, housing type, number of children, number of family members living with the subjects) was evaluated through PHQ-9 and interview. Using the data, the associations of depression and sociodemographic factors were analyzed. Based on PHQ-9 depression cut-off value(10 points), the subjects were divided into two groups, and the difference of the variables between groups were analyzed statistically. Results : 'Own house'(YES/NO), education(less than elementary school graduation/more than elementary school graduation) were sociodemographic factors which showed significance difference in mean PHQ-9 scores. Group with Over PHQ-9 10 points showed less 'having their own house' and less education level than group with less PHQ-9 10 points. Conclusions : With this research, it is expected that the risk factors for the elderly depression can be understood and the measures for early detection and invention of elderly depression would be provided.
