The Effects of Reminiscence Therapy Using Mind Map to Improve Cognitive Function, Depression Index, Quality of Life for Elderly Women Suspected Of Dementia. (마인드맵을 활용한 회상치료가 여성 치매 의심 노인의 인지기능, 우울 지수 및 삶의 질에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of reminiscence therapy using mind map to improve cognitive function, depression index, quality of life for old woman with dementia. Method : The study were 14 patients who were diagnosed with a woman suspected of dementia. They were randomly assigned to Study group(N=7) and control group(N=7). All patients received only to Study groups. reminiscence therapy using mind map was composed to 10 sessions, 40 minutes per sessions, 2 times a week, for 5 weeks. For result analysis, descriptive statistic, Wilcoxon signed rank test, and Mann-Whitney U test were used. The evaluation tools were Mini-Mental State Examination Korean Version (MMSE-K), Korean Version of Beck Depression Inventory (K-BDI), Korean Version of the World Health Organization Quality of Life Scale Abbreviated Version (WHOQOL-BREF). Result : There was a significant difference in cognitive function, depression index, and quality of life in the within group after intervention, and there was a significant difference in cognitive function in the between group comparison. Conclusion : According to the results of study, an easing effect was confirmed regarding reminiscence therapy using mind map for an old woman with dementia. using reminiscence therapy using mind map when applied to the improvement of cognitive function, depression index, quality of life.

Associations between Vasomotor Symptoms and Depression, Stress and Quality of Life in Midlife Women (중년여성에서의 혈관운동증상과 우울, 스트레스, 삶의 질과의 상관성)

  • Objectives : The aim of this study was to evaluate the associations between vasomotor symptoms and factors such as sociodemographics, health behaviors, medical condition, depression, stress, anxiety, attitude toward menopause, and quality of life. Methods : We conducted a cross-sectional study in peri- and post-menopausal women enrolled by the Korean Association of Health Promotion. Subjects submitted self-report questionnaires about vasomotor symptoms and other clinical symptoms. Associations between vasomotor symptoms and clinical variables were analyzed using stepwise multiple regression analyses. Results : 1951 women completed self-report questionnaires and 1022 women were enrolled in the study. The prevalence of vasomotor symptoms in peri- and post-menopausal women was 63.9%. Variables showing significant differences between subjects with vasomotor symptoms and subjects without them were score of Beck Depression Inventory, Brief Encounter Psychosocial Instrument-Korean Version, proportions of clinically significant depression(Beck Depression Inventory ${\geq}16$), Menopausal rating scale, attitude towards menopause, the 4 subscales of World Health Organization Quality of Life-BREF(Physical health, psychological, social relationships, environment), and a History of Premenstrual syndrome/Premenstrual dysphoric disorder. Stepwise multiple regression analyses indicated that Beck Depression Inventory, Brief Encounter Psychosocial Instrument-Korean Version, Menopausal Rating Scale, and the Psychological subscale of World Health Organization Quality of Life -BREF show associations with vasomotor symptoms. Conclusions : Menopausal vasomotor symptoms are associated with various psychological factors, especially with depression. Midlife women suffering vasomotor symptoms should therefore be screened for depression. Future prospective studies where clinical subjects are diagnosed using structured interviews, focusing on the causal relationship between depression and vasomotor symptoms are necessary.

Effect of Mechanical Thermal Massage Inducing Gradual Spinal Segmentation on the Improvement of Pain (단계적 척추 분절운동을 유도하는 기계식 온열 마사지가 통증 개선에 미치는 영향)

  • In this study, we tried to confirm whether the mechanical sequential elevation method of the body pressure measuring bed actually induces segmental motion for each part of the spine. To this end, a lateral X-ray examination was performed, and it was confirmed that the sequential pressure device induces a step-wise segmentation of the spine by mechanically lifting each part of the spine vertically. Then, pain, walking ability, and depression scale were measured and analyzed in subjects who were aware of back pain. VAS(p<0.05) and ODI(p<0.05) for 10 days tended to decrease in average after bed use. In the gait ability test(p<0.05), as the number of times of bed use increased, the moving time in the test decreased and the moving distance increased. In addition, GSDDF(p<0.05) decreased after bed use. As a result, it was confirmed that the spinal segmentation caused by the heat and acupressure provided by the bed affected gait and depression as well as pain relief.

  • The disc internal disruption syndrome is not well known to us, but the following hypothesis is widely accepted in clinical practice. The disc internal disruption syndrome may develop intractable back pain with aggrevated of pain, loss of spinal motion with any physical exercise, leg pain, loss of energy, marked weight loss, and profound depression. The patient with this syndrome will be found to have normal plain roentgenograms, myelograms, CT scans, results of blood examination and neurologic findings. For these reasons, this syndrome was frequently diagnosed by abnormal discographic findings. We had experience with two cases of disc internal disruption syndrome with clinical, roentgenographic and discographic evaluations. Thus We present these cases with a brief review of the concerned literature.

Clinical Characteristics in Patients for Mental Disability Assessment Following Traumatic Brain Injury -Comparison between Patients with and without Abnormal Brain Imaging- (두부외상 후 장애평가 환자들의 임상적 특징 -뇌영상검사 이상유무에 따른 비교-)

  • Objectives : This study was undertaken to identify the clinical and psychological characteristics in patients of mental disability assessment following traumatic brain injury who had the finding with or without abnormal brain imaging study. Methods : A consecutive series of 59 patients were assessed in hospital from January 1994 to December 1998. Patients were divided into 2 groups based on normal or abnormal brain imaging and the two groups were compared in demographic characteristics, psychiatric symptoms, type of head injury, clinical psychological findings. There were 27 patients with abnormal findings and 32 with normal findings in brain imaging study. Results : Abnormal finding group in brain imaging study had significantly higher incidence of psychosis, decreased memory, decreased appetite, increased nihilistic idea, and intracranial hemorrhage. Also, abnormal finding group showed significantly lower level of performance on the block design subtest of K-WAIS and had significantly lower scores on F, hypochodriasis, depression, hysteria, psychopathic deviate, psychasthenia and schizophrenia subscale of the MMPI. Conclusion : The findings suggest that the patients undergone brain surgery due to intracranial hemorrage at that time of brain injury may have higher frequency of abnormal findings in brain imaging study, complain more cognitive and affective symptoms, and have lower the abstract concept formation and perceptual organization abilities.

Psychological Characteristics Related to Subjective Satisfaction Level of Oral Malodor Treatment Outcome (구취 환자의 주관적 치료 만족도와 인성적 특성의 상관관계에 관한 연구)

  • 구취를 주소로 내원한 환자 중 구취의 치료 후 객관적인 구취의 감소가 인정되는 데도 불구하고, 계속해서 구취를 호소하며 만족해 하지 못하는 환자들의 심리적 특성을 알아보고자 하였다. 구취를 주소로 서울대학교 치과병원 구강진단과 구취클리닉에 내원하여 검사 및 치료를 시행한 환자 중, 치료 전 객관적 구취의 소견을 보였으나, 치료 후 할리미터 수치의 객관적인 감소를 보이는 40명의 구취 환자를 대상으로 하였다. 치료 전, 모든 환자들은 자신의 주관적 구취 심도 및 그로 인한 불편감 정도를 표시하였고, 간이정신진단검사 (SCL-90-R)를 시행하였으며 할리미터를 이용하여 구강내 휘발성황화합물의 농도를 측정하였다. 치료 후, 구강내 휘발성황화합물의 농도, 주관적 구취 심도 및 불편감 정도를 재측정하였다. 주관적 구취 심도의 감소 정도에 따라 환자들을 두 군으로 분류하여 할리미터 수치와 불편감 정도 및 간이정신진단검사항목을 비교, 분석하였다; A군-주관적 구취 심도가 평균값 이상으로 감소한 환자군; B군-주관적 구취 심도가 평균값 이하로 감소한 환자군. 1. 치료 전후 모두에서 주관적 구취 심도와 휘발성황화합물 농도 사이에는 유의한 상관관계가 없었으며 주관적 구취 심도의 감소 정도 또한 휘발성황화합물의 감소 정도와 상관관계가 없었다. 2. 치료 전 휘발성황화합물의 농도는 치료 후 휘발성황화합물의 감소치와 유의한 상관관계를 보였다 (p < 0.01). 3.구취로 인한 불편감 정도는 주관적 구취 심도와 유의한 상관관계를 보였으며 (p < 0.01), 치료 후 A군에서는 유의하게 감소 (p < 0.01) 하였으나, B군에서는 유의한 감소를 보이지 않았다. 4. B군은 A군에 비하여 간이정신진단검사 항목 중 대인예민성과 우울증의 항목에서 유의하게 높은 수치를 보였다 (p < 0.05).

The Comparison of Perceived Stress, Quality of Life between Patients with HIV Positive and Patients with Pulmonary Tuberculosis (HIV감염자와 폐결핵환자 간의 스트레스지각과 삶의 질의 비교)

  • Objectives : The object of this study was to compare perceived stress and quality of life among patients with HIV infection, patients with pulmonary tuberculosis and normal controls Methods: Stress Response Inventory(SRI) and Symptom checklist 90-Revised(SCL-90-R) were used to measure perceived stress responses and psychopathology. Smithklein Beecham quality of life scale was used to measure quality of life. Results : Patients with HIV infection scored significantly higher on scores of tension, anger, depression, fatigue and frustration subscale of the SRI than those with pulmonary tuberculosis and normal controls. Scores of the SCL-90-R, somatization, depression, anxiety, hostility, phobic anxiety, paranoid ideation and psychoticism subscale were also significantly higher in patients with HIV infection than those with pulmonary tuberculosis and normal controls. Patients with HIV infection scored significantly lower in quality of life than those with pulmonary tuberculosis. In patients with HIV infection, age had a significantly negative correlation with scores of somatization, obsessive-compulsive, interpersonal sensitivity, phobic anxiety, paranoid ideation and psychoticism sub scale of the SCL-90-R. but the level of education had a significantly positive correlation with somatization, obsessive-compulsive, interpersonal sensitivity, phobic anxiety, paranoid ideation and psychoticism sub scale of the SCL-90-R. Conclusion : The results suggest that patients with HIV positive were likely to have higher levels of perceived stress response and psychopathology, and lower quality of life than those with pulmonary tuberculosis and normal controls.

Emotional State and Personality Characteristics in Patient with Panic Disorder (공황장애 환자의 정서상태와 성격특성)

  • Objectives: The purpose of the present study was to investigate the emotional state and personality characteristics of patient with panic disorder. Methods: Twenty patients with panic disorder, 21 patients with somatoform disorder, and 20 normal healthy controls were studied. Korean version of Beck Depression Inventory(BDI), State and Trait Anxiety Inventory(STAI) and Korean standardized edition of Cattell's 16 Personality Factors Questionnaire(16-PF) were used for assessment. Statistically, One-way ANOVA with Scheffe test were used by SPSS/PC for windows. Results: 1) Total score of BDI was higher in the panic group than the somatoform group and normal control group(p<0.001). 2) Total score of state anxiety and trait anxiety in the panic group was higher than the somatoform group and normal control group(p<0.001). 3) In 16 PF, there were no definitely abnormal range of scores. But in first-stratum source traits of 16PF, the panic group was higher than the somatoform group and normal healthy group in O-factor (p<0.01) and Q4-factor(p<0.001). In second-stratum source traits of 16PF, the panic group was higher than the somatoform group in ANX-factor(p<0.05), but lower than normal control group in TOUfactor(p<0.05). 4) There were no differences in the panic subgroup according to sex and cutoff points of BDI score 16 and STAI-T score 54. 5) According to cutoff point of STAI-S score 52, the panic subgroup above 52 was higher than the panic subgroup under 52 in G-factor(p<0.001), Q3-factor(p<0.05) and SUP-factor(p<0.001), but lower in L-factor(p<0.05). Conclusion: These results suggest that emotional state of patients with panic disorder are depressive and anxious as compared with patients with somatoform disorder and normal controls. Patients with panic disorder did not show any abnormal personality characteristics but were more guilt-prone, anxious, emotionally sensitive. We propose that the understanding of panic patients' emotional state and personality characteristics will helpful to treat and manage in patient with panic disorder.

Psychological Characteristics of Alopecia Areata and Androgenetic Alopecia in Women (원형 탈모증과 안드로겐성 탈모증 여성의 정신적 특성에 관한 연구)

  • Objectives : We have studied female patients with either alopecia areata or androgenetic alopecia to evaluate psychological aspects, such as anxiety, depression, alexithymia, and characteristic personalities. In addition, we tried to examine the differences in psychological characteristics between these two types of alopecia, where the alopecia areata has been cotroversial on the role of stress in its etiology and the androgenetic alopecia seems to be more influeced by genetic and biological factors. Methods : All participated patients were females with alopecia for more than 1 you. Among them, 52 were with alopecia areata and 33 were with androgenetic alopecia. They were compared with 54 normal healthy controls by using MMPI, BDI, STAI-S, STAI-T, and TAS-20K. Results The average scores of F, Hs, D, Pd, Pa, Sc, Si in MMPI of alopecia groups were significantly higher than that of normal controls, and the androgenetic alopecia group had highest Hy and Pt scores. The average scores of BDI, STAI-S, and STAI-T in alopecia groups were higher than the normal controls. 94.2% of alopecia areata patients and 97.0% of androgenetic alopecia patients had severe depression, who scored higher than 23 in BDI. In TAS-20K, the average total scores of alopecia groups were higher than the normal control group, and the average Factor 3 score in androgentic alopecia was higher than the other groups. The alopecia groups scored higher than normal control group in STAI-S and STAI-T. Conclusion : Females with chronic alopecia were more depressed, had higher levels of anxiety, and more alexithymic than normal healthy females. In spite of arguments about etiological role of stress to alopecia, psychiatric interventions are needed for depression, and considerations for personality and psychological defense mechanism were needed in both types of alopecia.

A Comparative Analysis of Mental Status, Dietary Life and Nutritional Status among Senior Citizens With or Without Leisure Activities in Urban Area (수도권 거주 노인들의 여가활동 유무에 따른 정신건강 수준, 식생활 실태 및 영양상태 비교 분석)

  • The study aims to examine that regular leisure activity of the elderly has a beneficial effect to enhance their health condition. The elderly subjects of 164 (82 of elderly who do leisure and 82 of elderly who do not leisure) aged over 60 years in Suwon area were employed. Survey questionnaires were administered to investigate general characteristics, life style, mental status, dietary life and nutritional status. From the findings, the elderly who do leisure were highly educated, better in living status and higher employment rate. The mental status of the elderly without leisure activity was very much inferior to that of the elderly with leisure activity, representing higher prevalence rate of depression. Furthermore, the elderly who do not leisure showed higher drinking and smoking rates, irregular eating habits, poor teeth condition and chewing style as well as having eating obstacles. Mean caloric intake of both the elderly was also lower than that of RDA. The elderly who do not leisure have shown lower intakes in most of essential nutrients in contrast to the elderly who do leisure. In conclusion, the elderly with regular leisure activity seemed to have better life style and mental status, dietary life and nutritional status. Therefore, it is critically important to establish the health program combined with various leisure activity for the seniors to promote and energize the later years of life.
