  • The purpose of this study is to find out the characteristics of depressive episode about major depression and bipolar disorder in child and adolescent. The subjects of this study were 34 major depression patients and 17 bipolar disorder patients hospitalized at child and adolescent psychiatry in OO university children's hospital from 1st March 1993 to 31st October 1999. The method of this study is to review socio-demographic characteristics, diagnostic classification, chief problems and symptoms at admission, frequency of symptoms, maternal pregnancy problem history, childhood developmental history, coexisting psychiatric disorders, family psychopathology and family history and therapeutic response through their chart. 1) The ratio of male was higher than that of female in major depressive disorder while they are similar in manic episode, bipolar disorder. 2) Average onset age of bipolar disorder was 14 years 1 month and it was 12 years 8 months in the case of major depression As a result, average onset age of major depression is lower than that of bipolar disorder. 3) The patients complained of vegetative symptoms than somatic symptoms in both bipolar disorder and depressive disorder. Also, the cases of major depression developed more suicide idea symptom while the case of bipolar disorder developed more aggressive symptoms. In the respect of psychotic symptoms, delusion was more frequently shown in major depression, but halucination was more often shown in bipolar disorder. 4) Anxiety disorder coexisted most frequently in two groups. And there coexisted symptoms such as somartoform disorder, mental retardation and personality disorder in both cases. 5) The influence of family loading was remarkable in both cases. Above all, the development of major depression had to do with child abuse history and inappropriate care of family. It is apparent that there are distinctive differences between major depression and bipolar disorder in child and adolescent through the study, just as in adult cases. Therefore the differences of clinical characteristics between two disorders is founded in coexisting disorders and clinical symptoms including onset age, somatic symptoms and vegetative symptoms.

  • Purpose: The purpose of this study was to investigate the factors affecting depression in mothers of children with disabilities and to identify the relationships between the depression, parenting-stress and self-esteem. Methods: The participants were 100 mothers of children with disabilities lving in G city. Data were collected using a structured questionnaire and analyzed using descriptive statistics, t-test, ANOVA and Pearson Correlation Coefficients with SPSS/PC+ 21.0. Results: There was a significant relationship between depression and parenting stress (p<.001). Depression was negatively correlated with self-esteem (p<.001). Variables affecting depression were parenting-stress, self-esteem and family income. The three factors accounted for 38.7% of the depression. Conclusion: The results of the study show that parenting-stress and selfe-steem are the most significant factors affecting depression. There is an urgent need to develop nursing interventions to reduce levels of depression in mothers of children with disabilities and to help them lead a healthier life.

  • Objectives: Depression, sleep complaints and cognitive impairments are commonly observed in the elderly. Elderly subjects with depressive symptoms have been found to show both poor cognitive performances and sleep disturbances. However, the relationship between sleep complaints and cognitive dysfunction in elderly depression is not clear. The aim of this study is to identify the association between sleep disturbances and cognitive decline in late-life depression. Methods: A total of 282 elderly people who underwent nocturnal polysomnography in a sleep laboratory were enrolled in the study. The Korean version of the Neuropsychological Assessment Battery developed by the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) was applied to evaluate cognitive function. Depressive symptoms were assessed with the geriatric depression scale (GDS) and subjective sleep quality was measured using the Pittsburg sleep quality index (PSQI). Results: The control group ($GDS{\leq}9$) when compared with mild ($10{\leq}GDS{\leq}16$) and severe ($17{\leq}GDS$) depression groups, had significantly different scores in the Trail making test part B (TMT-B), Benton visual retention test part A (BVRT-A), and Stroop color and word test (SCWT)(all tests p<0.05). The PSQI score, REM sleep duration, apnea-hypopnea index and oxygen desaturation index were significantly different across the three groups (all indices, p<0.05). A stepwise multiple regression model showed that educational level, age and GDS score were predictive for both TMT-B time (adjusted $R^2$=35.6%, p<0.001) and BVRT-A score (adjusted $R^2$=28.3%, p<0.001). SCWT score was predicted by educational level, age, apnea-hypopnea index (AHI) and GDS score (adjusted $R^2$=20.6%, p<0.001). Poor sleep quality and sleep structure alterations observed in depression did not have any significant effects on cognitive deterioration. Conclusion: Older adults with depressive symptoms showed mild sleep alterations and poor cognitive performances. However, we found no association between sleep disturbances (except sleep apnea) and cognitive difficulties in elderly subjects with depressive symptoms. It is possible that the impact of sleep disruptions on cognitive abilities was hindered by the confounding effect of age, education and depressive symptoms.

  • Objectives : Stress induces alteration of gastrointestinal function in which interleukin (IL) and hypothalamo-pituitary-axis are involved. Depression again is associated with functional gastrointestinal disorders (FGID) and interleukins. The author attempted to look into the role of interleukins in the FGID also the association of depression and stress in this context. Methods : Entered were 20 patients with FGID, diagnosed by Rome IT criteria and 20 healthy controls. Depression was measured by Hamilton Rating Scale for Depression (HAM-D), and stress of the last one year by Schedule of Recent Experience (SRE). Serum levels of IL-$1\beta$, IL-2, IL-6 were measured by Enzyme-linked Immunosorbent Assay and serum cortisol by Fluorescence Polarization Immunoassay. The results were as follows : Results: 1) Serum levels of IL-$1\beta$, IL-2 were significantly lower in the patients with FGID than those in the controls, but level of IL-6 did not differ between two groups. 2) The patient group showed significantly higher level of serum cortisol as well as higher degrees of depression and stress. 3) Positive correlation was noted between depression and serum cortisol, and between depression and IL-2. A trend of negative correlation was seen between depression IL-$1\beta$. Positive correlation was noted between SRE and IL-6 and IL-6 and serum cortisol. Conclusions : It was concluded that FGID might be related to depression and stress. Changes of the interleukins might be involved with elevated cortisol level.

  • This study was performed to determine the levels of depressive symptoms and to reveal its relationships between ADL and IADL among rural elderlies. The interviews were performed, during the period from April 1st, to June 30th, 2010, to 412 elderlies in rural areas. As a results, the distribution of ADL among all subjects were 94.7% in normal-range group, and 5.3% in impaired group. In terms of IADL, 87.4% of the subjects were normal-range group, 12.6% were impaired group. The distribution of depression group among all subjects were 38.2% in normal-range group of ADL, and 90.9% in impaired group of ADL, but it was 35.8% in normal-range group of IADL, and 76.9% in impaired group of IADL. In logistic regression analysis, the adjusted odds ratio of the depression was significantly increased in impaired group of ADL than in normal-range group of ADL(OR=23.21, 95% CI=4.38~123.05). Also it was significantly increased in impaired group of IADL than in normal-range group of IADL(OR=7.76, 95% CI=2.99~20.08). In conclusion, the depression of rural elderlies was significantly increased in impaired group than in normal-range group of ADL and IADL. Thus, strategic effort needs to be prevented to depression in impaired group of ADL and IADL.

  • Depression is the most common and widespread mood disorder. About 20% of the population might suffer a major, incapacitating episode of depression during their lifetime. This disorder can be classified into two types: major depressive disorders and bipolar disorder. Since pharmaceutical treatments are different according to types of depression disorders, correct and fast classification is quite critical for depression patients. Yet, classical statistical method, such as minnesota multiphasic personality inventory (MMPI), have some difficulties in applying to depression patients, because the patients suffer from concentration. We used electroencephalogram (EEG) analysis method fer classification of depression. We extracted nonlinearity of information flows between channels and estimated approximate entropy (ApEn) for the EEG at each channel. Using these attributes, we applied two types of data mining classification methods: decision tree and possibilistic support vector machines (PSVM). We found that decision tree showed 85.19% accuracy and PSVM exhibited 77.78% accuracy for classification of depression, 30 patients with major depressive disorder and 24 patients having bipolar disorder.

  • This study examined the relationships among fear of falling, ADL/IADL disability, and depressive symptoms among Korean community-dwelling middleaged and older adults based on the disablement process model. The panel data from the Korean Longitudinal Study of Aging(KLoSa) were used to test whether fear of falling affects disablement process as a risk factor and an exacerbator. The results of path analysis showed that middle-aged and older adults who reported fear of falling were more likely to experience ADL/IADL disability and depressive symptoms, and fear of falling influenced depressive symptoms through increasing the risk of ADL/IADL disability. In addition, fear of falling exacerbate the risk of ADL/IADL disability among middle-aged and older adults who had chronic illnesses. The results indicated that fear of falling is not only risk factor but also excerbator in predicting ADL/IADL disability and depressive symptoms prospectively.
