Mediastinal Parathyroid Cyst -1 case report - (종격동 부갑상선 낭종 -1례 보고-)

  • Mediastinal parathyroid cyst is a very rare disease and is usually found incidentally. Surgical excision is the treatment of choice and recurrence is very rare when complete excision is done. A 71-year-old man was referred to our department because of 6$\times$5cm sized right superior mediastinal mass found incidentally on chest X-ray Surgical excision was performed and pathologic findings were confirmed as mediastinal parathyroid cyst. The patient has been followed up postoperatively without recurrence for 4 months up to now. We report a case of mediastinal parathyroid cyst.

Primary Mediastinal Liposarcoma -1 Cases Report- (원발성 종격동 지방육종 -1예 보고-)

  • The primary mediastinal liposarcoma is a very rare tumor. It mainly causes respiratory symptoms, but can be asymptomatic. The most favorable treatment of primary mediastinal liposarcoma is a surgical removal, whether it is complete or not, regardless of the size or histologic type of the tumor. The survival is determined by the histologic type of tumor. A 34-year- ld man was admitted because of asymptomatic mediastinal tumor which was progressively growing over 6 years. The tumor was completely resected and the microscopic findings of the tumor were compatible with well-differentiated liposarcoma. The patient recovered and discharged without complication on the 7th postoperative day.

Evaluate Utility of Thyroid Incidentaloma Discrimination by $^{18}F$-FDG PET/CT Delay Scan Images ($^{18}F$-FDG PET/CT검사에서 지연영상을 이용한 갑상선 우연종 감별의 유용성 평가)

  • Purpose: To evaluate the degree of malignancy of incident thyroid lesion found in $^{18}F$-FDG PET/CT findings and the usefulness of the method suggested in this study, we applicate the Delay Scan Method that differentiate a false positive benign tumor, inflammation and malignancy, as well as make the criteria of SUV. Materials and Methods: A retrograde study was conducted of 800 patients who were admitted in E hospital to receive $^{18}F$-FDG PET/CT examination. One patient who was diagnosed as primary thyroid cancer and received $^{18}F$-FDG PET/CT examination was excluded. The number of final patients of this study was 799, the reasons of $^{18}F$-FDG PET/CT examination of these patients were follow-up of old cancer or suspicious tumorous lesion in 696 and disease screening in 103. $^{18}F$-FDG PET/CT image photographing was taken in Biograph-Duo made by SIEMENS, after taking normal $^{18}F$-FDG PET/CT image (1 hr) and then 1 hr later we took the thyroid 1 bed-delayed image for the patients who showed abnormal thyroid $^{18}F$-FDG uptake and above 2.0 SUV for 2 minutes every 1 bed. For the patients who showed abnormal thyroid uptake and above 2.0 SUV, 1 hr later, we took a 1 bed-delayed image and then made a comparative study between measured $SUV_{max}$ of 1 hr-abnormal uptake image and that of 2 hr-delayed image. Results and Conclusion: In this $^{18}F$-FDG PET/CT study among the patients who showed incidental $^{18}F$-FDG thyroidal uptake the number of thyroid incidentaloma was 5 (0.63%), all of then showed benign findings. And in the case of incidental $^{18}F$-FDG uptake in thyroid, $SUV_{max}$ variance obtained from 2 hr delayed image can be a indirect criteria in differentiating benign tumor from malignancy and decrease finding error. In the cases found thyroid incidentaloma when 1) $SUV_{max}$ of focal thyroid lesion is above 5.0 and 2) $SUV_{max}$ variance between normal $^{18}F$-FDG PET/CT exam and 2 hr delayed is $1.0{\pm}0.5$, they are suspected as malignancy and confirming biopsy is to be followed. Otherwise, I also think that distinct follow-up PET or CT image study is a reasonable diagnostic method.

  • Multiple primary malignant neoplasms refer to two or more malignancies in an individual that are not related. We report a case of a 78-year-old male with concurrent breast mucinous carcinoma and extramammary lymphoma. The patient initially presented with palpable masses in the left breast and the right groin, which were pathologically confirmed after a surgical biopsy as breast mucinous carcinoma and diffuse large B-cell lymphoma, respectively. He underwent whole-body 18-fluorine deoxyglucose PET/CT before surgery, and an enhancing nodular lesion in the left lingual tonsil was found incidentally. It was later confirmed as a diffuse large B-cell lymphoma, a pathology of the same type as the right inguinal mass. Unspecified lymphadenopathies in breast cancer patients may easily be considered as metastatic lesions. However, this case suggests that lymphomas should be included in the differential diagnoses to avoid misdiagnosis and treatment delay, especially in older adult patients.

  • Cavernous hemangiomas of the mediastinum are rare tumors. A 3 year and 8 month-old female patient was referred because of an abnormal chest radiograph. Chest X-ray revealed abnormal shadow occupying nearly the entire left thoracic cavity Surgical excision was performed and pathologic diagnosis was confirmed as 15 $\times$ 10 cm sized cavernous hemangioma. On the eighth postoperative day, the patient was discharged without any complications and has been followed up without any problems.

  • Cavernous hemangioma in mediastinum is a rare tumor. A 13 year old girl was referred becaused of abnormal mediastinal shadow in simple chest X-ray. Chest CT scan revealed a somewhat inhomogenous cystic legion arround the whole length of SVC. Surgical excision was done through the right posterolateral thorachotomy. The pathology was confirmed as cavernous hemangioma.

  • An adenomyoma of the gastrointestinal tract is a rare, benign, tumor-like condition histologically characterized by glandular structures embedded within a smooth muscle stroma. An adenomyoma has been considered to be as an abortive variant of an ectopic pancreas. An ectopic pancreas is defined as the presence of pancreatic tissue that lacks anatomic and vascular continuity with the main body of the pancreas. Common sites of an ectopic pancreas are the stomach, duodenum, and upper jejunum. An adenomyoma may rarely undergo a malignant transformation or cystic dystrophy. Recently, the authors an experienced a case of an incidental adenomyoma of the stomach associated with early gastric carcinoma. We report that case here.

  • Mediastinal teratoma is one of the most common lesions found in the anterior mediastinum, accounting for $8\sim13%$ of all mediastinal tumors. This tumor is incidentally detected by routine chest roentgengography, but pericardial perforation or pleural effusion occurs rarely. In our patient cardiac tamponade was developed due to anterior chest wall contusion, we confirmed the anterior mediastinal tumor. Vital signs were stabilized after the pericardiocentesis, and the patient underwent the tumor resection in the anterior mediastinum for a definite treatment. On histologic examination, the tumor revealed cystic structures composed of mature squamous epithelium, pilosebaceous glands, mature fat tissue, gastrointestinal mucosa, respiratory epithelium, and pancreatic tissues.

  • 지방종은 아주 흔한 양성종양이지만 흉막에서 발생한 지방종은 드물다. 대부분 무증상이며 우연히 발견되는 경우가 많다. 단순 흉부 X-선 사진상 양성종괴의 모습을 보이며, 전산화 단층촬영에서 특징적인 균질한 지방으로 구성된 종양의 모습을 보인다. 흉막의 지방종의 경우 다른 종양이나 악성 종양을 배제하기 위하여 개흉술을 통한 종양의 제거를 해왔으나 수술 후 통증이 적고 회복이 빠른 흉강경으로 지방종을 제거한 53세 여자환자 1례를 경험하였기에 보고하는 바이다.
