욕창발생위험요인과 영양상태가 중환자실 입원환자의 욕창발생에 미치는 영향 - 욕창발생위험집단을 중심으로 - (The Influence of the Risk Factors and Nutritional Status on the Development of Pressure Sores for the Risk Patients in ICU)

  • Pressure sores are a serious concerns in that respect to increasing risk of medical complications and medical costs. Prevention and care of pressure sores is an essential area of nursing practice. The nurse at ICU should be more careful of maintaining the skin integrity of patients especially than at any other place. This study was conducted to determine if the risk facotrs of pressure sores and nutritional status of the patients at risk for pressure sores is related the occurance of pressure sore. The risk group refers the patients having the below 14 scores of the braden scale. The 100 subjects were recruited from the ICU ward at an university hospital in Choongnam. The parameters for nutritional status are the blood chemistry including plasma protein, albumin, hemoglobin and the anthropometric measurements consisting of weight, BMI, LBM, the proportion of body fat, body fluid and triceps skin fold using bioimpedence analizer and caliper. The results are as follows : 1. The subjects were 55 years and stayed 8 days on average. Of the 100 subjects, males were 61%, neurologic/neurosurgical diseases were 68% and the incidence of pressure sores was 17% mainly occuring within 3days after the admission. 2. The present paralysis(or paraplegia) and edema(arm, leg, trunk) were showed more significantly the subjects with pressure sores than those without pressure sores. 3. Regarding with the nutritional status, the subjects with pressure sores had significantly lower the weight, BMI, LBM, body fluid, albumin than the ones without pressure sores. This results were supported the reports of previous studies that the decreased weight and albumin could be the important predictors of pressure sores. Thereafter we should encourage these factors to be utilized in predicting pressure sores for a comprehensive assessment. Nurse should identify patients at risk of the development of pressure sores, assess their nutritional status and dietary intake at regular intervals.

  • 본 연구의 목적은 중환자실 환자를 대상으로 케어비데로봇을 이용한 배설간호가 실금관련 피부염, 욕창위험도, 생리적 지표(혈청 알부민, 단백질, 헤모글로빈, 림프구 변화)에 미치는 효과를 확인하기 위해 시행되었다. 연구대상은 종합병원 중환자실에 입실한 환자들을 대상으로 케어비데로봇 ((주)큐라코)을 이용하여 실험군에게는 케어비데로봇을 1인당 1대씩 장착하고 실금이 발생 될 시 작동하게 하여 건조 완료하였으며, 대조군에게는 실금이 발생될 시 물티슈로 닦고 자연 건조를 시키는 방법으로 1주간 진행하였다. 수집된 자료는 기술통계, t-test, repeated measured ANONA로 분석하였다. 연구결과 실금 관련 피부염, 욕창 위험도, 알부민, 림프구는 집단간에 통계적으로 유의한 차이가 있었다. 본 연구는 케어비데로봇을 이용한 배설간호가 피부 점막에 미치는 효과를 과학적 근거인 생리적 지표로 검증하였다는 점에서 의의가 있으며, 대상자 확대를 통한 추후 연구가 더욱 필요하다.

  • 본 연구는 와상환자에게 자주 발생하는 낙상, 욕창, 불면증을 영상기술을 통해 인식하여 섬망의 조기진단 모형을 구현한다. 실시간 모니터링을 통해 섬망 잠재환자를 선별하고 집중적인 관리와 치료로 이어질 수 있도록 간호인력을 보조하는 데 주된 목적을 두고 있다. 과활동형 섬망은 파생위험 중 하나인 낙상과, 저활동형 섬망은 원인 요소인 욕창과 묶어 자세인식을 통해 판정한다. 또한 주로 밤에 악화되는 섬망의 특성을 고려해 눈 깜빡임을 통한 불면증 검사를 추가로 반영하였다. 낙상과 욕창을 섬망과 묶어 융복합적인 위험예측 시스템을 구축함과 동시에, 기존의 섬망 사정도구들이 지니는 시공간적 제약을 개선함으로써 간호인력의 부담을 덜어줄 것으로 기대된다.

노인요양기관의 욕창발생 위험요인과 욕창예방 관리: 혼합연구 (Pressure Ulcer Risk Factors and Preventive Intervention in Long-Term Care Facilities : A Mixed method study)

  • 본 연구의 목적은 노인요양기관의 욕창발생 위험요인과 욕창예방 관리에 대한 내용을 비교. 분석하여 노인요양기관에 적합한 욕창예방 관리 방법을 위한 기초자료를 제공하고자 하였다. 본 연구는 체계적 문헌고찰과 포커스그룹 인터뷰를 활용한 혼합연구로 체계적 문헌고찰은 PRISMA(Preferred reporting items for systematic reviews) 지침에 따라 자료를 수집하고 분석하였으며, 포커스그룹 인터뷰는 노인요양기관에서 3년 이상 근무한 경력직 간호사를 대상으로 시행하였다. 쳬계적 문헌고찰은 2010년부터 2019년 6월까지 국내외에서 발표된 논문을 본 연구의 논문 선정기준에 따라 총 8편 논문이 선택하였다. 논문검색어는 'pressure ulcers', 'bedsore', 'decubitus ulcers', 'intervention','prevention', 'elderly', 'long term' 단어를 조합하여 검색하였다. 포커스그룹 인터뷰는 노인요양기관에 3년 이상 근무한 간호사를 대상으로 반 구조화된 설문지를 통해 내용분석을 하였다. 연구결과 노인요양기관의 경우 특히 체위변경과 압력 감소 장치를 적용한 중재가 욕창 예방관리에 중요한 중재임이 확인되었고 기관마다 표준화된 욕창간호 관리프로토콜 및 프로그램의 활용이 필요하다고 제시되었다. 본 연구는 혼합연구방법을 통해 욕창 예방관리 방법을 통합적으로 제시하였으며 특히 다학제간 협력이 노인요양기관의 중요한 이슈임이 확인되었다.

  • 본 논문에서는 MIMIC-IV(Medical Information Mart for Intensive Care) v2.0 데이터를 이용한 시계열 데이터의 정제 및 가공 방법을 제안한다. 더불어 해당 가공법을 기반으로 정제한 데이터셋을 활용하여 구축한 기계학습 기반의 욕창 조기 경보 시스템을 통해 해당 가공 방법의 유의성을 검증하였다. 구현된 욕창 조기 경보 시스템은 병변이 발생하기 전 12, 24시간에 미리 의료진에게 경보를 주는 시스템이다. 전자의무기록(Electronic Medical Record; EMR) 시스템과 연동하여 실시간으로 환자의 욕창 발생 위험도를 의료진에게 알려 중환자 의사결정을 지원하고, 나아가 효율적인 의료 자원 배분을 가능하게 한다. 여러 기계학습 모델 중 GRU 모델을 사용하였을 때, AUROC 평가지표를 기준으로 발생 전 12시간이 0.831, 24시간이 0.822로 가장 좋은 성능을 보였다.

Braden scale을 이용한 신경외과 중환자의 욕창 위험 요인 사정과 욕창 발생과의 관계 (The Relationship of Risk Assessment Using Braden Scale and Development of Pressure Sore in Neurologic Intensive Care Unit)

  • Purpose: The purpose of this study was to evaluate the applicability of braden scale to assess pressure ulcer risk patients and to identify additional risk factors of pressure sores in an neurologic intensive care unit. Method: The subjects of this study were 66 patients in neurologic intensive care units. Data was prospectively collected from Sep. to Dec., 2002. Data were analyzed by mean, percentage, t-test, chi-square, discriminant analysis using Spss pc+. Result: The results of this study were as follows: 1) There was a significant difference between scoring of braden scale and pressure ulcer development. The subscales that predicted pressure ulcer development using braden scale only were sensory perception, moisture, mobility, friction & shear. By using these subscales, sensitivity was 86.7%, and specificity was 61.1%, and total hit ratio was 72.7%. 2) Additional pressure ulcer risk factors which showed significance for discriminating two group were protein, albumin, gender, level of consciousness, pattern of bowel elimination. By using the combination of these additional risk factors in addition to the braden scale, total hit ratio increased to 84.8%. Conclusion: This data suggest that albumin, protein, gender, level of consciousness, pattern of bowel elimination in addition to the braden scale should be included in the pressure sore assessment tool.

욕창발생위험사정도구의 타당도 비교 (A Comparative Study on the Predictive Validity among Pressure Ulcer Risk Assessment Scales)

  • Purpose: This study was to compare the predictive validity of Norton Scale(1962), Cubbin & Jackson Scale(1991), and Song & Choi Scale(1991). Method: Data were collected three times per week from 48~72hours after admission based on the four pressure sore risk assessment scales and a skin assessment tool for pressure sore on 112 intensive care unit(ICU) patients in a educational hospital Ulsan during Dec, 11, 2000 to Feb, 10, 2001. Four indices of validity and area under the curve(AUC) of receiver operating characteristic(ROC) were calculated. Result: Based on the cut off point presented by the developer, sensitivity, specificity, positive predictive value, negative predictive value were as follows : Norton Scale : 97%, 18%, 35%, 93% respectively; Cubbin & Jackson Scale : 89%, 61%, 51%, 92%, respectively; and Song & Choi Scale : 100%, 18%, 36%, 100% respectively. Area under the curves(AUC) of receiver operating characteristic(ROC) were Norton Scale .737, Cubbin & Jackson Scale .826, Song & Choi Scale .683. Conclusion: The Cubbin & Jackson Scale was found to be the most valid pressure sore risk assessment tool. Further studies on patients with chronic conditions may be helpful to validate this finding.

폼매트리스 사용 중환자의 욕창발생위험군별 적정 체위변경시간 (Optimal Time Interval for Position Change for ICU Patients using Foam Mattress Against Pressure Ulcer Risk)

  • Purpose: This study was done to identify the time interval to pressure ulcer and to determine the optimal time interval for position change depending on pressure ulcer risk in patients using foam mattress in intensive care units. Methods: The Braden scale score, occurrence of pressure ulcers and position change intervals were assessed with 56 patients admitted to an intensive care unit from April to November, 2011. The time to pressure ulcer occurrence by Braden scale risk group was analyzed with Kaplan-Meier survival analysis and log rank test. Then, the optimal time interval for position change was calculated with ROC curve. Results: The median time to pressure ulcer occurrence was 5 hours at mild or moderate risk, 3.5 hours at high risk and 3 hours at very high risk on the Braden scale. The optimal time interval for position change was 3 hours at mild and moderate risk, 2 hours at high and very high risk of Braden scale. Conclusion: When foam mattresses are used a slight extension of the time interval for position change can be considered for the patients with mild or moderate pressure ulcer risk but not for patients with high or very high pressure ulcer risk by Braden scale.

신경외과 수술 환자의 수술 관련 욕창 발생 위험요인 (Risk Factors of Pressure Injury related to Surgery in Neurosurgery Patients)

  • Purpose: The purpose of this study was to evaluate risk factors of pressure injuries in patients after neurosurgery. Methods: A retrospective case-control study was conducted using 273 patients undergoing neurosurgery admitted to a general hospital from 2015 to 2021. Data were collected from September 1 to 30, 2021. The data were analyzed using the SPSS/WIN 26.0 program. Results: Risk factors significantly influencing the occurrence of pressure injuries in the patients undergoing neurosurgery were hypertension (OR=3.12, p=.024), postoperative hypoalbuminemia (OR=0.30, p=.028), and prolonged operative duration (OR=1.00, p=.001). The regression model explained 86.0% of the variance of the outcome variable. Conclusion: In order to prevent surgery-related pressure injuries in patients undergoing neurosurgery, thorough blood pressure management, avoidance of hypoalbuminemia, and preventive nursing intervention considering operative duration are required.

전자의무기록을 이용한 욕창발생 예측 베이지안 네트워크 모델 개발 (Predictive Bayesian Network Model Using Electronic Patient Records for Prevention of Hospital-Acquired Pressure Ulcers)

  • Purpose: The study was designed to determine the discriminating ability of a Bayesian network (BN) for predicting risk for pressure ulcers. Methods: Analysis was done using a retrospective cohort, nursing records representing 21,114 hospital days, 3,348 patients at risk for ulcers, admitted to the intensive care unit of a tertiary teaching hospital between January 2004 and January 2007. A BN model and two logistic regression (LR) versions, model-I and .II, were compared, varying the nature, number and quality of input variables. Classification competence and case coverage of the models were tested and compared using a threefold cross validation method. Results: Average incidence of ulcers was 6.12%. Of the two LR models, model-I demonstrated better indexes of statistical model fits. The BN model had a sensitivity of 81.95%, specificity of 75.63%, positive and negative predictive values of 35.62% and 96.22% respectively. The area under the receiver operating characteristic (AUROC) was 85.01% implying moderate to good overall performance, which was similar to LR model-I. However, regarding case coverage, the BN model was 100% compared to 15.88% of LR. Conclusion: Discriminating ability of the BN model was found to be acceptable and case coverage proved to be excellent for clinical use.
