Determinants of Long-Term Care Service Use by Elderly (노인장기요양서비스 이용형태 결정요인 연구)

  • This study examined the factors affecting forms of long-term care service use by elderly and the forms of use are classified facility care service, home care service, and unused. It is used data from the 2nd pilot program for the Long Term Care Insurance scheme and it is analysed 5,497 cases. Multi-nominal regression is used. According to the results, women use formal service more than man do, and wowen use facility care than home care. Those who eligible for National Basic Livelihood Security System(NBLSS) are shown to have higher use of formal care(especially facility care) than the middle income class, and the low income class than the middle income class has lower use of formal care. In addition, higher the family care is available, lower the taking part in the service. The big cities and mid sized cities than rural are used the formal service and moreover mid sized cities are used facility care than home care. Furthermore, the level of care need is determinants of service use and function of ADL, IADL, and abnormal behavior is also determinants of formal service(especially facility care). But nursing need and rehabilitation need are not determinants of formal service use. Based on the results, the recommendations are developed and implemented for the improvement the elderly long-term care insurance.

A Study on Satisfaction of Service by the Models of Informal-formal Resources Linkages for Long-term Care Elder (장기요양대상노인의 비공식적·공식적 자원연계 유형에 따른 서비스 만족도 연구)

  • This study is to examine the effective system for providing services for long-term care elders, despite the expansion of formal care, informal care still is needed. Thus, this study's purpose is to classify the informal-formal resources linkages types of long-term care elders and looks into service satisfaction by these types and discover effective informal-formal resources linkages models. For that, this study is to divide informal-formal resources linkages types by the degree of providing services of informal and formal caregivers and discover the informal-formal resources linkages types using cluster analysis and explores the effectiveness of service satisfaction using multiple regression. The study's results is to suggest four models, such as family care, complementary, separation, formal service and complementary type was founded to be the most effective and then based on the result, we discuss as follows. First, we must strive to combine informal resources into formal service systemto the system for providing of service is made up the complementary type. Second, the system for providing services of long-term care elders is required integrated care system to alining of medical and long-term care services. Third, we have to consider a measure to improve of formal service type, for the satisfaction of formal service appears low relatively. Based on research findings, this study propose that the informal-formal resources linkages models are subdivided into the dimension of quantity and quality of care for improving the effectiveness of long-term care services.

Effects of Self-Efficacy in the Relationships between Job Stress and Mental Fitness of Care Provider (요양보호사의 업무스트레스가 마음건강에 미치는 영향 -자기효능감의 매개효과를 중심으로-)

  • The purpose of this study was to find whether job stress and self-efficacy influence mental fitness of care providers. The intervening variable was self-efficacy. The subjects for this study were 211 care providers. The data was conducted by structural equating model. The results of the study were as follows: The significant result out of this research model was that job stress influenced self-efficacy and self-efficacy influenced mental fitness. Job stress did not affect mental fitness, but job stress had a significant influence on mental fitness through self-efficacy. According to the results of this study, in order to improve the positive mental fitness and increase self-efficacy of care providers, we suggested that we would be required to improve working conditions of care providers.

Mediated Effect of Empowerment in the Influence on the Job Satisfaction and Job Involvement of Care Worker Self-efficacy (요양보호사의 자기 효능감이 업무만족과 업무몰입에 미치는 영향에서 임파워먼트의 매개효과)

  • The study originated in finding the way to improve the job satisfaction and job involvement of care worker, who work in the Elderly Care Facilities, by researching and analysing the influences on the job satisfaction and job involvement of their self-efficacy and empowerment directly or indirectly. The result from the influences on empowerment of care worker self-efficacy was that the higher the self-confidence and self-control efficacy were, the higher the empowerment was. And the influence on job involvement of self-efficacy was that the higher the self-control efficacy and job preference were, the higher the job involvement was. The influence on job satisfaction of care worker was that the higher the self-confidence, self-control efficacy, and job preference were, the higher the job involvement was. The higher care worker individual empowerment and environmental empowerment were, the higher their job involvement and job satisfaction were. We can find out that there is a mediated effect as an influence on job involvement and job satisfaction of self-efficacy.

A Study on the Effect of Job-stress on Job Satisfaction of Nursing Care Workers in cSeoul-type Day Care Center : Focusing on the Moderating effect of Ego-resiliency (서울형 데이케어센터 요양보호사의 직무스트레스가 직무만족에 미치는 영향에 관한 연구 : 자아탄력성을 조절변인으로)

  • In order to verify that the ego-resiliency regulates the effect of nursing care workers job stress on the job satisfaction, this research ascertained the effect of job stress of the nursing care workers working at Seoul-type Day Care Center and confirmed the moderating effect of ego-resiliency on the process where nursing care workers job stress affects the job satisfaction. A survey of 256 employees at 28 Seoul-type Day Care Centers were conducted on job stress, job satisfaction, and ego-resiliency, and 138 nursing care workers were analyzed. The data analysis was conducted using SPSS PROCESS Macro, which was presented by SPSS 25.0 and Hayes. The results of the study are as follows. First, nursing care workers had higher job stress and lower job satisfaction compared to other job groups. Second, job stress identified the negative effect job satisfaction. Third, ego-resiliency confirmed positive moderating effect between job stress and job satisfaction.

  • National long-term care insurance started in July, 2008. We try to make up for weak points and develop a long-term care insurance system. Especially, it is important to upgrade the rating model of the category of need for long-term care continually. We improve the rating model using the data after enforcement of the system to reflect the rapidly changing long-term care marketplace. A decision tree model was adpoted to upgrade the rating model that makes it easy to compare with the current system. This model is based on the first assumption that, a person with worse functional conditions needs more long-term care services than others. Second, the volume of long-term care services are de ned as a service time. This study was conducted to reflect the changing circumstances. Rating models have to be continually improved to reflect changing circumstances, like the infrastructure of the system or the characteristics of the insurance beneficiary.

The Effect of the HYO Consciousness on Job Satisfaction in Care Worker : The Mediation Effect of the Elderly Image (요양보호사의 효의식이 직무만족에 미치는 영향 : 노인이미지 매개효과)

  • The purpose of this study is to verify the effect of the elderly image in the relationship of HYO consciousness and job satisfaction in care worker. In this study, the HYO consciousness of care workers is the consciousness of taking good care of the elderly. The research subjects were selected by 10 long-term care institutions belonging to Seoul and Gyeonggi Province, and a total of 450 nursing care providers were selected for the survey using a questionnaire. The final 429 questionnaire was used for the analysis. The analysis shows that first, HYO consciousness and the elderly image have a defining effect on job satisfaction. Second, HYO consciousness were found to have a positive influence on the elderly image. Finally, the elderly image played a role in the relationship between HYO consciousness and job satisfaction. The government should seek ways to foster a sense of HYO consciousness and a positive image of the elderly by utilizing educational opportunities for care worker in the future.

Perception Types on the Training System with regard to long-term Care Worker (노인 장기요양인력 양성체계에 관한 인식 연구)

  • The purpose of this study is to find out subjective perception types and needs of interested group on manpower training system with long-term care service by Q methodology, then to search for political measures to improve long-term care system in the implication of the sorted subjective perception types. The result of the study showed that there are five major sorts of subjectivities on manpower training system with long-term care service: emphasizes retraining of existing care worker(Type1), emphasizes ethics of public service provider(Type2), emphasizes practice-oriented education(Type3), emphasizes policy judgement of government(Type4), emphasizes training of long-term care worker in demand(Type5). And to try quantity approach methods, this study was developed Q-Block as a assessment tool to enhance validity and reliability of Q-types. The results of the survey showed high conformity.

Effect of Long-term Care Utilization on Health Care Utilization of the Medicaid Elderly (국민기초생활보장수급자의 장기요양 서비스 이용 여부가 의료 이용에 미치는 영향)

  • This study examined the effect of long-term care utilization on the health care utilization of Medicaid elderly. The subjects were 5,834 long-term care insurance with the level 1 Medicaid elders, who received either service or non-service. This study examined the impact of long-term care service on the probability of health care utilization and the costs of health care utilization. The total medical cost and inpatient day between 2009-2007 were significant factors affecting long-term care utilization (${\beta}=.29$, p< .001, ${\beta}=.33$, p< .001 ) and this variable explained 22.6% of the total medical cost and 22.4% of the inpatient day. The results showed that non-service in long-term care was associated with an increase in health care utilization. The current long-term care insurance system should place higher priority and more resource allocation on long-term care utilization to increase the efficiency of the insurance system.

  • The purpose of this study is to suggest implications for the development of the senior care industry in Korea and how to apply it to the Korean market. For this study, a case analysis study was conducted on the successful business model of 'Driving Miss Daisy®'. For the development of Korea's senior care industry, understanding of senior, establishing infrastructure for community care, and improving the treatment of professional personnel care should be preceded. In order to apply 'Driving Miss Daisy®' to the Korean market, it was suggested that it is necessary to reduce regulations, develop differentiated services, establish linkage systems with various service institutions in the community, establish cooperative models between large and small businesses and startups.
