The Relationship between the Quality of Life, Care Services, and Family Support of Elderly Residents in Long-Term Care Facilities (노인요양 시설 노인의 삶의 질과 시설 서비스, 가족 지지의 관계)

  • The aim of this study is to identify the influence of care services on the quality of life of elderly residents in long-term care facilities, and to determine the mediating effect of family support. The subjects of the study were 192 older adults living in long-term care facilities in the Daegu and Gyeongsangbuk-do region. The collected data were analyzed using the SPSS 22.0 program, and the methods used include descriptive statistics, t-test, ANOVA, Pearson correlation analysis, and regression analysis. The results of the study found that the subjects' quality of life differed depending on their age, education, income level of their children, reasons for admission, and length of stay. The factors that influenced their quality life included age, education, the income level of their children, reasons for admission, length of stay, and care services. The study also revealed that family support was the mediating effect in the relationship between care services and quality of life. The results of the study will provide insight in establishing a nursing intervention plan to improve the quality of life of elderly residents in care facilities.

The Effects of Communication Satisfaction of the Case Managers on Service Quality in the Long-term Care Institutions (노인요양시설 사례관리자가 인지하는 조직 내 의사소통이 서비스 질에 미치는 영향)

  • The purpose of this study is to explore the relationship between communications satisfaction and service quality perceived by the case managers in the long-term care institutions. For this, multiple regression analysis was conducted for the 104 case managers who were selected from the 48 long-term care institutions in Seoul metropolitan city and Kyungki province. The results showed that the communication satisfaction of care mangers was statistically significant variable in predicting the level of service quality. Based on the results, several practical implications were provided.

  • In this study, we analyze the relationships between long-term care workers' person-centeredness and job satisfaction, emotional labor, and service quality. Self-administered questionnaires were used to collect data from 460 long-term care workers in long-term care facilities. In the research model, person-centered care was verified using independent variables, with emotional labor (surface behavior, internal behavior) and job satisfaction applied as parameters, and service quality applied as a dependent variable. The results of this study are as follows. First, the fit of the model is good. Second, the implicit behavior of emotional labor is found to be an important factor affecting service quality. Also, emotional labor is an important mediator, improving the value of person-centered care and service quality. There is a positive correlation between internal behavior and service quality, and a negative correlation between surface behavior and service quality. Third, emotional labor (surface behavior, internal behavior) is the most influential variable in terms of service quality. The results of this study demonstrate the necessity to pay clinical and academic attention to person-centered care in terms of long-term care workers' emotional labor and service quality.

  • In this study, the effect of chronic pain on the lives of elderly people in long-term care service was analyzed based on the mediated effect of depression. The research data was sampled from elderly people in long-term care services, 204 people participated. From mediated regression analysis, depression was the most relevant factor on the quality of life, followed by chronic pain. With chronic pain and depression as independent variables and quality of life as a dependent variable, depression was proved to have had a fully mediated effect on quality of life. The result of this study suggested that convergence of various support systems should be implemented for the elderly in long-term care services.

  • This study has been undertaken to empirically test how the level of information Home Helpers have about their client and the hypothesis that home-helpers' expertise about their service provided to their clients is expected to positively affect their assessment of service provided to clients. Main results of this study are as follows : First, home-helpers' knowledge, skill, and information about clients is positively related to their assessment of service provided to clients. Home-helpers' information about clients plays a more important role than knowledge and skills in the process of assessing the services provided to clients. After maintaining that the hypothesis is empirically confirmed, the author discusses the implications of empirical findings.

A Study of Home-visit Care Agencies' Perceptions and Attitudes toward Home and Community Long-term care Agency Quality Assessment (HCLA-QA) Program (방문요양기관의 재가장기요양기관평가에 대한 인식 및 태도)

  • National Health Insurance Corporation initiated Home and Community Long-term care Agency Quality Assessment(HCLA-QA) program in 2010. This study aims to explore long-term care agencies' perceptions and attitudes toward HCLA-QA program, focused on the home-visit care agencies. Total 473 agencies completed the questionnaires(response rate 13.6%), using the e-mail and fax survey methods with high-capacity web-fax server of NHIC. On the results of this study, we found the agencies' different perceptions and attitudes on HCLA-QA program according to their quality ratings as QA results. Compared to the agencies with low quality ratings, the agencies with high quality ratings tend to have more time for preparation, better understandings about HCLA-QA program, more positive perceptions that quality indicators generally are valid, and more affirmative attitudes that the notifications of QA results are helpful for agencies. Our findings can be very useful for establishing strategies for improving the HCLA-QA program. The policy makers, the insurer, and the agencies need to consider the opinions of the agencies with high quality ratings as well as the ones with low quality ratings.

The Effect of Self-Efficacy and Job Stress on Quality of Service of Care Givers (요양보호사의 자기효능감과 직무스트레스가 서비스 질에 미치는 영향)

  • The purpose of this study was to investigate the effects of Self-Efficacy and Job Stress on Quality of Service. Data were collected through self reported structured questionnaire from Care givers using convenient sampling methods from May 1 to August 30, 2019. The data were analyzed with SPSS/WIN 25.0 using independent t-test, one way ANOVA, Pearson's correlational coefficient and multiple regression. These factors together explained 42.0% of the Quality of service in Care givers(F=13.86, p=<.001). Factor influence the Quality of Service was Self-Efficacy(β=0.56, p=<.001). The findings of this study indicate that education programs should be developed to improve the self-efficiency of caregivers and that further studies should be conducted on their effects.

The Impact of Care Workers' Employment Characteristics and Perception of Facility Directors' Transformational Leadership on Quality of Service (요양보호사의 고용특성과 시설장에 대한 변혁적 리더십 인식이 서비스 질에 미치는 영향에 관한 연구)

  • The purpose of this study is to examine the effect of care workers' employment characteristics and perception of facility directors' transformational leadership on quality of service through a hierarchical linear model. For this aim, survey data were collected amongst 240 older adults and 200 care workers who are affiliated within 45 long-term care facilities in Seoul, and analyzed using SPSS 26.0 and HLM 8.0. As a result, one's perception of transformational leadership had a positive effect, whereas, among employment characteristics, employment type and working hours had negative effects on quality of service. Regular workers with fewer working hours and higher awareness of transformational leadership toward the director provided higher quality of service. But wage, total experience and tenure didn't meaningfully affect it. Therefore, the following suggestions were presented. First, it is necessary to reorganize incentive, salary systems and budgets, changing the status of temporary workers' hourly wage system into that of regular workers' monthly one in order to strengthen employment security with acknowledging fundamental professional values through reinforcement of expertise. Reinforcement of long-term care's publicness and establishment of base facilities are also suggested. Second, maintaining appropriate hours of work and rest including annual leave under the Labor Standards Act is needed. Also, increasing the salary of and decreasing working hours for night shift workers are required. Third, education and intervention for inspiring transformational leadership of directors and strengthening qualification standards of them are required.

Effects of Positive Psychological Capitals on Service Quality and Mediating Effect of Job Satisfaction in Professional Care Workers (요양보호사의 긍정심리자본이 서비스 질에 미치는 영향과 직무만족의 매개효과 검증)

  • The purpose of this study is to provide basic data in order to improve service quality based on empirical analyses about a relationship among positive psychological capitals, service quality, and job satisfaction of professional care workers under the Long-Term Care Insurance in South Korea. In this study, total 430 numbers of the workers who were currently working in nursing homes for the elderly at the cities of Ansan, Anyang, Bucheon, and Kwangmyeong in Gyeonggi Province, South Korea, participated in self-report questionnaires. Only 393 questionnaires were used for the final analyses after excluding the ones that neither were not returned nor didn't completed it all. These are major results. First, positive psychological capitals affect positively on service quality. Second, job satisfaction works as a mediating effect in the process of positive psychological capitals affect on service quality. Based on above results, it is possible to suggest it as down below. First, it is required to have policies that will be helpful for both positive psychological capitals and capacity building through human resource management in systematic approaches. Second, it is also required to acknowledge that having positive psychological capitals is effective to improve both their job satisfaction and service quality. Therefore, it is necessary to change environmental circumstances in relevant institutions where this acknowledgement will be applied.

Literature Review on Empirical Studies of the Impact of Marketization on Quality of Care in Long-Term Care Sectors (장기요양서비스 시장화가 서비스 질에 미치는 영향에 대한 국내외 경험적 연구 리뷰(review))

  • This study reviews empirical studies that investigated the impact of marketization of long-term care services on quality of care in Korea, England, Sweden, United States, and Japan. The trend and characteristics of marketization and regulatory system in those countries were compared. Since 2008 when long-term care insurance was introduced in Korea, we selected and reviewed empirical studies which were listed in the National Research Foundation, PubMed and Web of Science and examined the relationship between marketization and quality of care. Seven Korean, three English, two Swedish, seven American studies and one Japan study were identified and there was little evidence supporting that marketization would lead to better quality of care. Specifically, competition had no consistent relationship with quality of care while there was little evidence supporting that for-profit nursing homes tended to exhibit greater quality of care compared to non-for-profit nursing homes. The chains of for-profit nursing homes were revealed to have greater deficiencies. The findings of this review imply that improvement of quality of care may not be achieved by making for-profit nursing homes have chains. Reconstruction of nursing home markets should be needed, and the number and the proportion of public and non-for-profit nursing homes should be increased.
