A Study on the Types of Public Hospitals in the Region by Cluster Analysis (군집분석을 통한 지역거점공공병원의 유형화)

  • This study selected indicators that can represent the characteristics of general hospitals, including local medical centers and Red Cross hospitals, which are representative public health institutions, and analyzed clusters. And we present to benchmark in each cluster. According to the analysis, 276 general hospitals were classified into 13 clusters, and local medical centers and Red Cross hospitals were classified into clusters between 1 and 7 of the total 13 clusters because of their small size. Local medical centers and Red Cross hospitals, selected as excellent hospitals in each cluster, showed significant differences in management performance despite similar regional environment and medical performance, and among them, surgical consultation and internal medical care rates, inpatient and outpatient rates. In order for local medical centers and Red Cross hospitals to play their role as secondary acute hospitals in the region, inpatient care services and surgical functions must be activated.

  • In the Soviet Union School health services are provided as an integral part of the health care delivery system, which is under the Ministry of Health. This paper presents an overview of the Soviet Union's health care delivery system, the model for the delivery of school health service, the role and training of school personnel involved in school health services and implications the Soviet model may have for the countries. 1. School health services are a part overall Soviet health system under the Ministry of Health. 2. Municipal and rural health departments implement programs at the local level. Diagnosis and treatment are conducted through "polyclinics" that are outreach divisions of a district hospital. 3. Education institutions for the development of health manpower, including medical schools and nursing schools, are under the Ministry of Health, as are medical and scientific search institutes.

  • The practice pattern of pediatric dentistry in the university dental hospital has been changing for the last several decades, due to socioeconomic changes, developments in dental materials and increasing number of private pediatric dental clinics. The purpose of this study is to survey the changing patterns of pediatric dental treatment in the university dental hospital and to present the future direction for pediatric dentistry. Patient distribution and practice trends were reviewed based on the OCS (order communication system) records of the department of pediatric dentistry, Seoul National University Dental Hospital from 2001 to 2008. Results were as follows; 1. The number of patients increased up to 2005 and decreased in 2006, and has remained constant up to 2008. 2. The proportion of preventive and orthodontic treatment increased and the proportion of restorative and pulpal treatment decreased. 3. In restorative treatment, amalgam restoration decreased and resin restoration increased. 4. Frequency of sedation decreased in 2006, 2007 and increased in 2008. 5. Frequency of general anesthesia increased up to 2008.

  • 문제: 소화기병검사실에서 대장내시경 검사의 부도건수가 최근 월 200명 정도로 예약인원의 16%에 달한다. 이는 본원 외래진료 부도율이 평균 10%인 것과 비교할 때 상당히 높은 결과이므로 대장내시경 예약환자를 대상으로 부도율 관리를 위한 활동이 필요하였다. 목적: 외래 및 입원환자의 부도건수 및 부도환자의 특성을 분석하고, 개선활동을 통하여 대장내시경 검사의 부도율을 줄인다. 의료기관: 서울 아산병원. 질 향상 활동: 부도관리를 위한 개선활동으로 SMS 횟수를 2주전, 4일전, 1일전 3회로 변경하고, 내용도 각 시기마다 차별화하였다. 대장내시경 검사 예약을 하고 기다리는 환자를 대상으로 처방일 순, 예약 날짜순서 등 필요한 항목 순으로 볼 수 있는 리스트가 필요하여 예약대기 프로그램을 개발하였고, 처방일로부터 2주 이내 자동예약을 금지하였다.(7월 10일부터 시작) 예약대기명단에 있는 환자를 대상으로 전화로 예약 변경을 하여 2주 이내에는 예약의 빈자리가 없도록 하였다. 미리 부도를 예측하여 추가로 예약하던 방법에서 정원조정을 하여 정원 외 추가 예약을 하지 않았다. 입원환자 일정관리를 위하여 일정관리 프로그램을 개발하여 타과의뢰 회신을 하는 전임의가 직접 날짜를 전산에 입력하여 회신과 동시에 검사실, 담당주치의, 담당 간호사가 모두 일정을 확인할 수 있도록 하였으나 프로그램이 늦어져 9월 12일부터 시행하였다. 개선효과: 부도건수가 개선 전 8주 동안 385건(예약인원 대비 16%) 에서 개선 후 6주의 평가기간 동안 256건(예약인원대비 14.1%)으로 129건이 감소하였으며, 검사 1-2일전 일정 변경 연락 환자 감소 및 검사 비용 수납 환자의 부도건수가 감소하였다. 대장내시경 검사 예약대기 프로그램 운영 및 예약 변경 전화 활동으로 2달씩 기다리던 환자의 예약일을 약 한 달 정도 단축하여 줄 수 있었다. 검사 정원 관리의 결과로 검사 예약 시 예약기준이 명확해지고, 부적절한 예약으로 인한 검사지연 등이 확실히 줄었다. 또한 입원환자 일정관리 프로그램 적용으로 입원환자의 검사일정과 관련된 전화업무는 확실히 줄어들 것으로 예상된다.

Comparison of Clinical Characteristics by Types of Enuresis in Children Utilizing Questionnaires (병력취득 설문지를 이용한 야뇨증의 형태에 따른 임상양상의 비교)

  • Purpose : It is important to take a precise history of patients to diagnose enuresis. However, this is difficult to perform in the outpatient department, because the consultation time is limited. So by taking the medical histories with questionnaires, we classified the enuresis patients into monosymptomatic and non-monosymptomatic groups and compared the clinical characteristics between the two groups in order to determine whether this would aid in the diagnosis and treatment of enuresis. Methods : Thirty-four enuresis patients seen in the Department of Pediatrics at Yeungnam University Hospital from 2004 January to 2005 July were enrolled in this study. Two to three weeks before starting medical treatment, questionnaires about the patients' urination habits and past medical history were taken from the Parents with consents. Results : There were 21 patients in the monosymptomatic group and 13 patients in the non-monosymptomatic group. There was no difference in the delivery mode, birth weight, birth order, sleep habit, constipation, and treatment results between the two groups. Urinary infection was more frequently observed in the non-monosymptomatic group. There was a higher incidence of daytime incontinence and a past medical history of allergy in the non-monosymptomatic group. Oriental medicine was the most common treatment of enuresis before visiting the hospital, but the treatment effect was minimal. Conclusion : Comparison between the monosymptomatic and non-monosymptomatic group with the questionnaires was helpful for detailed diagnosis and treatment of enuresis. A more effective education for enuresis patients is needed, since before visiting the hospital many of them wasted their efforts with ineffective oriental medicine treatment.   • Purpose: This study is to derive the behavioral characteristics of the users appear in the outpatient department of psychiatry. Methods: By referencing John Zeisel's Tools for environment-behavior research, observation of user behavior and expert interviews as a method of the study has been carried out. Results: The results of this study are as follows. First, adjacent placement among children, youth and regular the outpatient department of psychiatry is necessary and easy access to internal medicine department and family medicine department is required in order to allow cooperated treatments. Secondly, depending on the characteristics of the outpatient department of psychiatry treatment, adjustment of space requirement or the change of kinds of necessary space is required. Third, considering the psychological and behavioral characteristics of mental illness and guardians, various search for the environment that can support this. Implications: This study is a basic research for space improvement and evaluation of the outpatient department of psychiatry, and need to be followed by further study using various perspectives and methods.

Difference in Outpatient Medical Expenditure and Physician Practice Patterns between Medicaid and Health Insurance Patients (건강보험환자와 의료급여환자 간 의원 외래 의료이용 차이와 공급자 진료행태)

  • The purpose of this study was to examine the role of provider practice patterns in the difference in health expenditure between the two types of patients: Health Insurance and Medical Aid type 1. The study used the outpatient claim data for all Medicaid and health insurance patients of hypertension who received medical services from 8,454 primary care physicians during the first half of 2006. The data were stratified by patient's gender and age for the two groups of patients who received care from the same physician. The dependent variables were the differences in medical expenditure per case, patient days per case and medical expenditure per patient day between Medicaid patients and health insurance patients. Empirical results showed that physician characteristics, such as physicians under age 50, greater proportion of pediatric Medicaid patients, lower proportion of new Medicaid patients and the greater number of comorbidity of Medicaid patients are associated with the greater difference between the two types of patients (i.e., greater expenditure of Medicaid patients relative to health insurance patients). This study shows that factors associated with provider practice patterns need to be taken into account in Medicaid policy.

The Relationships among Waiting Time, Patient's Satisfaction, and Revisiting Intention of Outpatients in General Hospital (일 종합병원 외래환자의 진료대기시간 및 환자만족도, 재이용의도와의 관계)

  • Purpose: The purpose of this study was to explore the degree of delay in waiting time, and the relationships of waiting time, patient satisfaction, and revisiting intention of outpatient in general hospitals. Methods: The data were collected from June 22 to July 4, 2009. A total of 536 outpatients who visited 21 clinics of a general hospital were subjected to evaluate the waiting time. The survey tools used were the Korea Health Industry Development Institutes (2008) tool for patient satisfaction and Reichheld & Sasser (1990) for revisiting intention. The data were analyzed by SAS version 9.1, descriptive statistics, t-test, ANOVA, and Pearson correlation coefficient. Results: The mean patient's waiting time was $28.3{\pm}30.7\;min$, the revealed mean score of patient's satisfaction was 2.92, and the revisiting intention showed was 4.56. The waiting time was negatively correlated with patient's satisfaction (r=-.10, p<.019). Patient's satisfaction was positively correlated with revisiting intention (r=-.51, p<.001). Conclusion: Waiting time management is an important factor of increasing patient's satisfaction and revisiting intention in general hospitals. It is mandatory that reservation management systems take into account the patient's characteristics of visiting outpatient department in order to shorten the real waiting time.

A Survey of the Outpatient General Anesthesia and Dental Treatment in Chungnam Dental Clinic for the Disabled (충남장애인구강진료센터에서 시행된 외래전신마취와 치과치료에 대한 조사연구)

  • Background: Dental treatment under outpatient general anesthesia is an option for disabled person, children and dentally disabled person who have the difficulty of cooperating voluntary with dental care. The aim of this study was to access the patients who had dental management under general anesthesia at Chungnam dental clinic for the disabled. Methods: This survey is consisted of 114 severely disabled and dentally disabled person who undergone dental treatment under general anesthesia from January to June 2011. Data were collected by individual dental and anesthesia records. Results: Of the 114 patients studies, the 45 patients was the dentally disabled and the 69 patients was the severely disabled. Rate of dental treatment among male patients was higher than among female patients. The dentally disabled was an similar age distribution. In the severly disabled, above 13-year-old was higher than below 12-year-old. The most popular induction method of general anesthesia was sevoflurane inhalation after induced sedative effect of nitrous oxide inhalation. The severly disabled who had definitely negative behavior was applied inhalation of high concentration sevoflurane. Conclusions: Chungnam Dental clinic for the disabled provide dental management for the disabled and the dentally disabled.

A Space Analysis Model of Evacuation Overcrowding at Hospital Outpatient Department (병원 외래진료부에서의 피난 과밀 공간 분석 모델에 관한 연구)

  • This study aimed to suggest a quantitative model analyzing overcrowding area under emergency evacuation situation in the outpatient department of hospital. Overall study process included the review on legal conditions of an emergency evacuation, the investigation of precedent research documents and the analysis of spatial configuration. The user movement with considering exit gates and the one without considering exit gates were analyzed for routine activity condition. An agent-based simulation was applied for the analysis. Also, user movement for the emergency condition was tested with evacuation simulation. The variation of simulation conditions revealed the difference between overcrowding spaces from situation change. At all nodes, visit frequencies derived from different conditions and situations were compared. The overcrowding spaces are to increase the risk of delaying emergency evacuation time which is critical for user safety. It suggests the need for dispersing overcrowding spaces under evacuation situation. The suggested analysis model can evaluate overcrowding spaces in the outpatient department of hospital and provide locational data for distributing evacuation design resources.
