정형외과 수술의 임상적 결과에 영향을 미치는 요소는 여러 가지가 있지만 수술적 수기가 가장 중요한 요인이며 로봇공학은 술기를 보다 정확하고 일관되게 수행하기 위해 도입되었다. 정형외과 영역에서는 20년 전부터 로봇수술을 사용하였으나 실용성 문제와 합병증의 발생으로 일부 로봇은 도태되었으며 단점을 개선한 로봇이 소개되고 있다. 대부분의 로봇수술은 인공관절수술 분야에서 시행되었고 치환물의 정렬과 위치 선정에서 우수한 것으로 보고되고 있다. 그러나 임상적 결과의 우수성, 실용성, 가성비 등의 측면에서 제한점이 있고 일부 로봇수술에서는 합병증 발생 빈도가 높다는 보고가 있었다. 초기 로봇수술은 수술자의 개입 없이 계획된 수술이 진행되는 능동형 로봇수술이 주를 이루었으나 최근에는 수술과정에서 실시간 피드백을 통해 술자가 술 전 계획을 변경할 수 있고, 햅틱 경계 내에서 제한된 골절제를 함으로써 주요 혈관, 신경, 인대 등의 손상을 최소화할 수 있는 반능동형 로봇수술이 소개되고 있다.

A 17-year-old male patient of aortic stenoinsufficiency with a small aortic annulus underwent aortic valve replacement[St. Jude Medical valve 21mm] with Rittenhouse-Man-ouguian procedure[patch enlargement of the area of the noncoronary sinus and anterior mitral leaflet] The used patch was made from tubular dacron graft[the longitudinal length was 35mm, the width 20mm]. The length of incision into anterior mitral leaflet was 10mm and the aortic annulus was enlarged in 10mm, The patient was evaluated with two-dimensional echocardiography at 4 month after operation and the result was excellent.

To evaluate risks, complications and mortality of reoperations on heart valve prosthesis, we reviewed clinical records of 53 patients who underwent reoperation because of prosthetic valve failure[PVF], from Jan 1959 through Jun. 1991. They had undergone 48 mitral, 10 aortic valve rereplacement Primary tissue failure was the main cause of reoperation : it occurred in 51 valves at a mean postoperative interval of 58 months. Calcification and collagen disruption of prosthesis were main causes of primary tissue failure in macro and micropathology, In 3 failing mechanical prostheses, paravalvular leak was in 2 cases, another one case had the thrombi at the hinge portion. If conditions such as emergency operation with or without endocarditis, thromboembolism and advanced NYHA functional class are prevented, we think that reoperative valve replacement has similar morbidity and mortality to initial valve replacement surgery. But our sturdy represents higher mortality [22.6%] because of late surgical intervention failing the prevention of conditions leading to myocardial damage. In conclusion if the tearing, calcification, and a new murmur were detected the early reoperation should be considered to increase late survival.

최근 학문간의 장벽이 무너지고 융합학문이 큰 이슈가 되면서 다양한 결합이 시도되고 있다. 의학분야중 특히 정형외과 분야는 기계공학의 고체 및 바이오공학과의 융합연구로 매우 매력도가 높은 분야이다. 이 글을 통해 정형외과 분야 중 슬관절 분야가 기계공학도에게 왜 흥미로울 수밖에 없는가에 대해서 몇 가지 내용들을 소개하고자 한다.
