  • Traumatic intrusive luxation, an occurrence common in the primary dentition but one that occurs rarely in the permanent dentition, has a poor prognosis. There have been many treatment approaches such as allowing the tooth to reerupt spontaneously, surgical repositioning and immediate luxation, surgical luxation, and orthodontic repositioning; but all have their own drawbacks. Meanwhile, Turley et al. (1987) have proposed surgical and orthodontic combination therapy to treat intrusion. Surgical and orthodontic combination therapy means to apply the orthodontic traction force immediately after surgical luxation. If ankylosis occurs, orthodontic force may be applied after re-luxation repeatedly. But in cases of complete intrusive luxation, it would be not feasible to bond an orthodontic button or bracket on the tooth directly. Thus, in this case, traction of the tooth was attempted after surgically repositioning it close to the probable original socket site to promote better healing.

Traumatic Posterior Dislocation of the Shoulder with Ipsilateral Humeral Surgical Neck Fracture in a Child - A Case Report - (소아에서 동측 상완골 외과적 경부 골절을 동반한 외상성 견관절 후방 탈구 - 증례 보고 -)

  • Purpose: Traumatic posterior dislocation of the shoulder in a child is extremely rare, and posterior dislocation of the shoulder concomitant with ipsilateral humeral surgical neck fracture has not been reported in a child previously in Korea. Materials and Methods: The authors treated a 10-year-old with posterior dislocation of left shoulder and an ipsilateral humeral surgical neck fracture, that occurred during Taekwondo practice, by open reduction of the shoulder and pin fixation under general anesthesia. Results: A normal range of motion with complete union and good remodeling was achieved without redislocation or avascular necrosis of humeral head at 1 year after surgery. Conclusion: The authors report a successfully treated case of traumatic posterior dislocation of the shoulder with an ipsilateral humeral surgical neck fracture in child.

  • A traumatic dislocation of the sternoclavicular joint is a rare injury, and among them, anterior dislocation is more common than a posterior dislocation. Posterior dislocation is a potential risk by compressing the mediastinal structures, but an anterior dislocation has not been considered a risk. Traumatic sternoclavicular joint anterior dislocation associated with anterior angulation of a sternal fracture can develop mediastinal compression and have a risk in the same way as a posterior dislocation. This case report is about a traumatic sternoclavicular joint anterior dislocation with a sternal fracture accompanied by mediastinal compression that was treated surgically using a plate and showed relatively good clinical results. This rare case is reported along with a review of the relevant literature.

  • Unstable pelvis ring trauma can accompany urologic injuries such as bladder, ureter, or urethral injuries. In very rare cases, testicular dislocation can accompany a pelvic ring injury. Testicular dislocation causes pain, but can be regarded as the pain caused by pelvic ring injury. If not detected early, testicular dislocation may result in gangrenous testes. The authors report a case of pelvic ring injury with testicular dislocation.

  • Most acute patella dislocations can be reduced closely using a spontaneous or simple maneuver, but it may not be possible if accompanied the by anomalies, such as rotation of the patella, osteophyte ridge protruding from the femur, or anatomical variations. This case report outlines irreducible acute patella dislocation with anatomical variations, a notched patella. If the reduction of an acute patella dislocation fails, an additional radiological examination and reduction under general anesthesia may be necessary with the possibility of an anatomical variant in mind.

  • A traumatic sternal segment dislocation in children is very rare. Of the few cases reported, some recommended surgical treatment, while others reported good results by conservative treatment. Therefore, the treatment remains unclear. This paper reports a case of completely remodeled traumatic sternal segment dislocation in a 13-year-old boy treated by the restriction of activity and periodic observations.

  • In general, most knee dislocations are reduced easily by simple traction. In rare cases, closed reduction of the knee dislocation is attempted, but ruptured ligaments or muscles are stuck in the joints and cannot be reduced. The cases of this irreducible knee dislocation have sometimes been reported. The authors encountered a case of irreducible knee posterolateral rotational dislocation that was not reduced by simple traction and report it along with a review of the literature. This case provides an opportunity for clinicians to examine the clinical considerations when experiencing an irreducible knee dislocation.

  • Although uncommonly encountered, knee dislocation is frequently associated with popliteal artery injury. It has been reported that all detected arterial injuries warrant intervention and open surgical management is the standard of treatment. We report here on a case of endovascular intervention for popliteal artery injury in knee dislocation, and then this was treated with ligament surgery with arthroscopic and open procedure.

슬개 대퇴 불안정성에 대한 진단적 접근 방법

  • 슬개 대퇴 불안정성의 진단은 정확한 해부학적 지식을 바탕으로 역동적인(dynamic)기전을 이해하여야 한다. 슬개골의 저명한 탈구는 쉽게 진단되지만, 경미한 외상으로 슬개골이 탈구되어 정복된 후에는 올바른 진찰(dynamic study)없이 방사선 검사(static study)만으로는 진단을 놓치는 경우가 많다. 그러므로 정확한 진단을 위해서는 병력, 이학적 검사, 방사선 검사, 전산화 단층 촬영, 자기 공명 영상 그리고 관절경 검사까지 다양하게 필요할 수 있다.
