Mandibular Implant-Supported Telescopic Overdenture using Gold Electroforming System : A Case Report (Gold Electroforming System을 이용한 하악 임플란트 지지 텔레스코프 피개의치)

  • In edentulous mandible, implant supported overdenture was considered as a first treatment option. In case of a implant supported telescopic overdenture, sufficient inter-arch space needs for arrangement of artificial teeth and attachment. Passive fit of the implant prosthesis is important factor for preventing mechanical failure. Gold Electroforming System is particularly useful to achieve a passive fit of telescopic attachment and results in precision marginal fit and the small thickness of the coping provides optimal space for narrow inter-arch space. This article presents that application of Gold Electroforming System can provide excellent esthetics and function on four-implant supported telescopic overdenture.

  • It is essential to record maxillomandibular relationship accurately for the harmony of esthetic and function in complete denture. Gothic arch tracing visually demonstrates the movement of the mandible, and is useful to establish accurate and reproducible centric relation. Proper retention and stability of complete denture in patients with severe alveolar bone resorption is difficult to attain. In such case, the closed mouth impression technique might be recommended. The denture border and impression are determined by patient's physiologic movement in the closed mouth impression technique. And, denture peripheral border is entirely closed with oral mucous membrane. This report presents satisfactory complete denture restoration using closed mouth impression technique and gothic arch tracing in patients with mandibular condyle fracture and severe absorption of mandibular alveolar ridge.

  • The retention of complete dentures in patients with Parkinson's disease is often poor due to their constant dislodgement by involuntary movements of the tongue and lips. Furthermore, insertion of the overdenture is expected to be difficult due to their limited voluntary movements. This case is about the bimaxillary overdenture restoration where its retention and stability are improved, using magnetic attachment of remained roots of maxilla and two implant $Locator^{(R)}$ attachments of mandible in patients with Parkinson's disease. This report describes the fabrication of this functionally improved prosthesis.

  • When it comes to treat patient with loss of vestibule, conventional denture impression have limitation which can cause problems of excessive border extension. Suction denture with closed mouth technique which was introduced to solve this problem, forms negative pressure being sealed denture flanges by mobile mucosa when the patient swallows and chews. Also, it can decrease patient's visiting time by taking impression and gothic arch tracing at once. In this case, considering patient's chief complain which is a loose fit of present lower denture, suction dentures with closed mouth technique was planned.

  • Recently three-dimensional (3D) printed complete dentures are becoming more widely recognized as one of the treatment options for the edentulous patients. Korean National Healthcare, however, has not yet approved the application of the 3D printed dentures, and in the cases where the dentures should be refabricated, the conventional fabrication process needs to be entirely repeated. However, during the conventional process, the digital scan data of some key steps could be easily obtained. In this clinical case, using these data, a new pair of 3D printed dentures that improved the limitations of the first conventional dentures were successfully fabricated.

  • 최근 생체재료의 개발이 눈부시게 발전되고 생체적합성이 우수한 표면을 요구함에 따라 생체재료의 표면처리에 대한 연구가 활발히 진행되고 있다. Laser Deposition법은 항공기 부품제조 분야에 주로 사용되고 있으며 최근에 오하이오 주립대 타이타늄합금연구센터를 중심으로 표면처리에 관한 연구가 주로 이루어졌다. 특히 이를 이용하여 치과재료의 표면처리에 응용을 시도하였다. 치과에서 응용될 수 있는 경우는 주로 임플란트는 부분 또는 완전 무치악 환자의 보철수복에 사용되는 보철물의 제작등에 사용될 수 있으며 이중에서도 특히 생체용 임플란트의 표면처리응용으로 임플란트와 조직간의 접합성을 개선하는 표면처리법으로 연구되었다. 임플란트의 성공과 실패는 물성적인 측면에서 임플란트의 형태, 표면거칠기 및 표면처리방법, 초기하중 등에 의하여 좌우되며 임플란트 재료에 작용하는 응력차폐는 생체적합성을 좌우하는 큰 요인이 되고 있다. 이를 위하여 저 탄성계수합금을 설계하지만 하중을 버티는 강도가 낮아지는 단점이 있어 레이저증착법을 이용하여 임플란트재료인 Ti6Al4V합금에 탄성계수가 낮은 Ta, Nb등을 코팅하는 방법을 통하여 이를 해결하고자하는 시도가 이루어지고 있다. 이 방법은 최근의 3D 프린팅의 원리가 되고 있다. 따라서 발표에서는 Laser Deposition방법을 이용하여 치의학분야에서 응용되고 있는 예를 강연하고 응용 가능 분야에 대하여 토론 하고자한다. 또한 펨토레이저를 이용하여 생체합금의 표면처리는 생체활성화를 더욱 증진시키며 이를 위하여 많은 연구 수행되고 있다. 본 발표에서는 매식용 합금 표면에 펨토레이저를 이용하여 텍스춰링하여 세포가 잘 성장 할 수 있는 크기의 조절함으로써 기존의 표면처리와는 다른 효과를 얻을 수 있는 장점을 알아본다. 펨토레이저를 이용하면 여러 가지 형태의 텍스춰링이 가능하며 원형, 사각형등등 자유자제로 형태의 묘사가 가능하고 깊이 또한 쉽게 조절할 수 있는 장점이 있다. 지금까지는 표면 개질에 사용되는 레이저는 주로 Nd:YAG 레이저의 파장을 반으로 줄인 녹색레이저 (${\lambda}=532nm$)를 사용하거나, 자외선파장영역의 레이저를 사용하는 경우가 일반적으로 가장 보편화되었다. 이를 이용하여 제조된 Ti합금에 펨토 초(10-15 second) 펄스폭 대역을 갖는 레이저를 이용하여 나노크기의 미세 요철을 표면에 형성한 후, 나노튜브를 형성하여 그 표면특성의 변화를 알아보고 펨토레이저가 의료분야에 적용되고 있는 예를 살펴보고자 한다.

  • For fully edentulous patients, implant-supported overdenture can be considered to enhance chewing efficiency and denture stability. Implant planning software can be used to fabricate a surgical guide for a more precise consideration of anatomic factors and prediction of the shape of definitive prosthesis. Though there are many possible attachments for implant overdenture, milled bar can be useful due to its splinting effect of implants and rigid support of overdenture. This report presents a case of implant-supported milled bar overdenture after guided implant surgery performed with two surgical guide that was fabricated before and after bone reduction.

Patients satisfaction for implant supported overdenture with small diameter implant (작은 직경 임플란트를 이용한 임플란트 지지 피개의치에 대한 환자 만족도 조사)

  • Purpose: In cases when implant supported overdenture is made by using standard size implant, additional procedure such as bone surgery and bone grafting can be required. And it gives burden to doctor and patient in terms of cost. Therefore, it is necessary to find the implant therapy for the edentulous patients in making denture with accordable cost and simple procedure. Materials and methods: Edentulous patients with upper and lower dentures participated in this study. Before the operation, survey about patient's satisfaction to the existing dentures was carried out. Surgical procedures included four small diameter implants installation anterior area and immediate loading. One and three month after the procedure, the same survey about patient's satisfaction was carried out, and radiography was taken. Results: We are doing research to the nine patients. Survival rate is 97.2 percent. The comparison of patient's satisfaction before and after surgery is performed based on oral health impact profile 49. We analyze mainly with masticatory discomfort, retention, aesthetics, social problem, psychological discomfort problems. As a result, satisfaction level is increased at all factors. Retention is the most increased satisfactory factor followed by mastication difficulty, pronunciation, psychological discomfort, social discomfort, aesthetics in order. Marginal bone loss is 0.21 mm at 12 weeks after implant placement. Conclusion: This research reveals that the denture supported by mini dental implant increases patient's satisfaction. This study will be continued with more patients for a long time and we are scheduled for taking additional radiography to check whether peri-implant bone resorption occurs or not.

  • Alveolar bone loss and deformation can be a risk factor in removable prosthetic restoration treatment for partially or fully edentulous patients. The use of implants to solve this problem could improve the support, retention and stability of removable restoration. Attachments used in implant overdenture are versatile. The attachment should be selected according to the patients' conditions. Milled bar has been chosen when readymade bar could not be used because of the narrow distance between implants or firm stability and support of supra-structure were needed. Milled bar design is able to provide cross arch stabilization and comfortability to patients. However, it needs skilled laboratory procedures. Recently, the fabrication of milled bar has become simple and its suitability has been improved through the development of CAD/CAM system. In a 67-year-old female Alzheimer's disease patient with 8 implant fixtures on the fully edentulous site of mandible, implant overdenture with using milled bar and magnet attachment was planned. As rapid treatment was required, CAD/CAM system was used to make a simple laboratory procedure instead of a traditional fabrication process. With this system, implant overdenture with milled bar can be fabricated esthetically and functionally.

  • Teeth wear with loss of posterior support often leads to symptoms such as irregular occlusal plane and excessive wear of anterior teeth. In such environment, establishing proper posterior support and harmonious anterior guidance and occlusal plane is recommended. In this case, the patient had excessively worn dentition with loss of posterior support, while denying surgical treatment due to her(his) history of osteoporosis, ibandronate injection, and risk of medication-related osteonecrosis of the jaw (MRONJ). To recover the vertical space needed for prosthetic restoration, full mouth rehabilitation with vertical dimension increase of remaining teeth was decided. Missing teeth in the posterior area were restored with mandibular removable partial denture. The treatment plan was determined through careful diagnosis, and every step of procedures including tooth preparation, provisional phase with the increased vertical dimension, and definitive prosthetic phase were carried out accordingly. Once the treatment was completed, the patient was satisfied functionally and esthetically. Periodic examination of oral hygiene, occlusal stability was conducted.
