Thirty patients with nonresectable hepatocellular carcinoma (HCC) due to either locally advanced lesion or association with liver cirrhosis, treated with combined radiotherapy and hyperthermia between April 1988 and July 1988, at Dept. of Radiation Oncology, Yonsei university College of medicine, were analysed. External radiotherapy of a total dose of 3060cGy/3.5 wks was given. Hyperthermia was given twice a week with a total of 6 treatment sessions using 8MHz radiofrequency capacitive type heating device, i.e., Thermotron RF-8 and Cancermia. In all cases hyperthermia was given within 30 minutes after radiotherapy for 30~60min. Temperature was measured by inserting thermocouple into the tumor mass under the ultrasonographic guidance only for those who had not bleeding tendency. As a result, partial response (PR) was achieved in 12 patients (40%), and symptomatic improvement was observed in 22 patients (78.6%) among 28 patients who had suffered from abdominal pain. The most significant factor affecting the tumor response rate was the type of tumor (single massive: 10/14, 71.4%; diffuse infiltrative: 2/10, 20%; multinodular:0/6, 0%; p<0.005). There were not any significant side effects relating to combined treatment. The overall 1 year survival rate was 34%, with 50% in the PR group and 22% in the no response group (NR), respectively. Median survival was 6.5 months and longer for those of PR than of NR (11 mos. vs 5, p<0.05). In conclusion, combined radiotherapy and hyperthermia appeared to be effective in local control and symptomatic palliation of HCC. Further study including a larger number of the patients to confirm its effect in survival and detrimental side effect should be urged.
This paper proposes a new temperature control algorithm and system configuration of the pTMS(personal Thermal Massage System). By controlling the pulse width of the PWM(Pulse Width Modulation), the temparature of the heating lamp can be controlled stably, which is indispensable to the massage function. This technology is also adapted to the 'thermal massage', 'thermal acupressure', 'thermal moxibustion' functions of medical equipments. The temperature could be set at between $40^{\circ}C{\sim}70^{\circ}C$ by increments of $5^{\circ}C$, the control could be made in real time by increments of $1^{\circ}C$, and the temperature is displayed on the monitor by triggering every 2 seconds. when the present temperature is equal to the preset temperature, the PWM signal is minimized, and when the present temperature is higher than the preset temperature, overheating is prevented by interrupting the PWM output signal. When the difference of temperature exceeds $4^{\circ}C$, the PWM control is maximized in order for the system to reach the target temperature within a short period of time.

Purpose In 18F-FDG PET/CT, the absorption of 18F-FDG due to the activation of Brown Adipose Tissue (BAT) greatly interferes with the discrimination of lymph node malignant metastasis. Warming the patient's body temperature before and after injection of 18F-FDG to prevent FDG absorption by BAT is a safe and non-pharmacological approach. The purpose of this study was to identify and select patients with a high potential for BAT activation in advance, and to investigate whether BAT can inhibit FDG absorption when the body temperature is raised for a short time by directly applying heat to the target patient. Materials and Methods Among the patients who underwent 18F-FDG PET/CT at the National Cancer Center from January 2020 to December 2020, 825 female patients (415 in the thermal group, 410 in the non-thermal group) under 50 years old were included. The thermal group was administered heat for 10 minutes before injection of 18F-FDG. For statistical analysis, the Z test comparing the ratios between the two groups was used, and logistic regression analysis was performed to correct for important variables (BMI, outdoor temperature, blood sugar) according to the results of the previous retrospective study. Results Among 825 patients, 19 patients with BAT activated (Thermal group: 5(1.2%), Non-thermal group: 14(3.41%)) accounted for 2.3% of the total. As a result of performing the Z test to compare the ratios between the two groups, the activation of BAT in the thermal group was significantly decreased (P=0.034). In the univariate logistic regression analysis, the activation of BAT was also decreased in the thermal group (OR: 0.34, P<0.05). In the multivariate results, BAT activation increased in patients younger than 45 years old (OR: 4.46, P<0.05) and outdoor temperature less than 13.2 degrees (OR: 9.97, P<0.05). BAT activation tended to decrease in the thermal group, but there was no significant difference (OR: 0.37, P=0.066). Conclusion We confirmed that the activation of BAT tends to decrease by 62.5% in the group subjected to the thermal method, and it will be of great help in preventing FDG absorption of BAT more effectively in the future.
Purpose: The purpose of this study was to identify the effects of heating on the non-affected hand on blood flow velocity, wound healing, and pain for hand microsurgery patients. Methods: This study was designed using the nonequivalent control group pretest-posttest design. Thirty-nine patients were assigned either to the experimental group (20 patients) or control group (19 patients). Data were analyzed with $x^2$-test, Fisher's exact test, t-test, and repeated measure ANOVA using SPSS/WIN 17.0 program. Results: After treatment in this program, blood flow velocity (F=5.13, p=.008) and wound healing (F=4.11, p=.020) improved significantly in the experimental group compared to the control group. But there was no significant improvement in pain in the experimental group compared to the control group (F=2.40, p=.097). Conclusion: Based upon these results, the non-affected side hand heating was recommended as an independent nursing intervention for the patients who need improvement in blood flow velocity and wound healing such as patients who have microsurgery. As the heating was effective even when applied on the non-affected side, it is the applicable to patients who cannot tolerate any therapy on affected side.
This study was conducted to investigate the effects of wild ginseng complex (WGC) pharmacopuncture combined with hyperthermia on abdominal obesity in post-menopause women. Two post-menopausal women were treated 5 times in 2 weeks with WGC pharmacopuncture on abdominal acupoints and 30 minutes of hyperthermia on abdominal area. Before treatment and after 5 treatments were finished, body weight (BW), body mass index (BMI), body fat ratio, waist circumference (WC), waist hip ratio, thickness of abdominal subcutaneous fat (ASF) were measured. In case 1, BW, BMI, WC, and ASF were reduced by 0.6 kg, $0.2kg/m^2$, 6 cm, and 18 mm respectively. In case 2, BW, BMI, WC, and ASF were reduced by 0.4 kg, $0.17kg/m^2$, 4.6 cm, and 12 mm respectively. This suggest that combined therapy of WGC pharmacopuncture and hyperthermia might be an effective treatment for abdominal obesity in post menopause women.
근막동통은 발통점과 중추흥분효과가 나타나는 국소적 근육성 동통이다. 발통점은 근육의 일부나 건 부착점에 나타나는 부위로서 단단한 띠로서 느껴지고 촉진시 통증을 유발한다. 통증은 관련된 발통점의 위치에 따라 연관된 부위에 나타난다. 이러한 근막 발통점에 따른 증상은 약한 기능이상부터 일상생활이 어렵게 되는 정도의 심한 통증까지 다양하다. 이의 발생에는 국소적 근육이상, 심부 동통, 심리적 스트레스의 증가, 수면장애와 전신적인 요인들도 관여한다. 일반적으로 치료는 보존적인 처치로써, fluoro-methane 같은 vapocoolant spray를 사용하는 사용하여 분사신장하는 방법이 있으며, 국소적인 주사, 마사지, 초음파, 온열요법등이 있다. 여러 환자에서 근막동통을 가지는 환자에 분사신장요법을 시행한 결과 대부분 환자에서 증상의 개선을 보였다. 근막동통의 치료시 분사신장요법은 매우 보존적인 처치로 임상가의 적용과 접근이 쉬우며, 환자로 하여금 치료에 따른 불안감을 줄이면서 만족을 느낄 수 있는 치료로 사료된다. 그러나 만성 환자의 경우 분사신장요법의 치료결과에 대한 만족도가 높지 않았다. 분사신장 용법은 근막동통을 가진 환자 모두에서 효과적이지는 않으나 치료의 도입으로, 근막동통을 진단시에 일차적으로 사용할 수 있는 안정한 보존적인 용법으로 사료된다.
Intracavitary brachytherapy combined hypertermin for utero-conical cancer seems to be a promising method for salvage treatments in persistent tumors and inoperable or previously irradiated cervical recurrences. In order to heat the vaginal apex and uterus, powerfull conical antennas which are suitable for afterloading cervical applicator have been designed for use in conjuction with intracavitary radiation therapy. The antennas were constructed with conical conductive material to feed line and the effective lenght were designed proportional to microwave length, Power deposition profiles of 2450 MHz of conical antennas were studied in both phantom models and muscle tissue and compared to those of commonly used dipole antenna. Improvement of the heating pattern was found in both phantom and muscle tissue. The heating pattern produced by the conical antenna resembles an ellipsoid and then the temperature distribution in depth was extended to $2\~3\;cm$ from the effective antenna axis.
The purpose of this study was to investigate the effects on the bio signal of brain waves and pulse waves using acupressure thermal bed combined with acupressure and thermal therapy. Forty healthy adults were divided into 20 experimental groups using combinatorial stimulation beds and 20 non-irritating controls. As a result of the experiment, the EEG was significantly increased in the delta wave and theta wave in the experimental group, and the alpha wave and beta wave decreased significantly. The pulse wave increased the parasympathetic activity index in the experimental group and the heart rate decreased. There was no difference in the control group. This change in EEG can be seen as a combination stimulation bed having a sleep-inducing effect, and a change in pulse wave can seen as having a relaxing effect. This study is expected to be used as basic data for the development of various combination stimulators and functional bed products.
