  • 2000년 후반부터 국내에 도입된 MDCT는 한번 숨을 참고 검사할 수 있는 volume coverage speed를 향상시킴으로 복부의 검사시간을 단축시켰고 보다 얇은 단면영상획득, isotropic resolution에 의한 reformation의 활용, 다중시기검사, 균일하게 조영된 3차원 혈관영상 획득이 가능해져 미세한 조기 복부암 진단에 주도적 역할을 수행하고 있다. 이에 CT의 기술적 발전과정과 MDCT를 이용한 복부암 진단에 대하여 소개하고자 한다.

  • 현재 백혈병 진단에서 사용중인 방법은 골수영상을 획득하고 이를 관찰하여 비정상의 백혈구의 형태, 백혈구 핵의 크기와 추출된 골수에서의 백혈구가 차지하는 비율을 이용하여 진단하고 있다. 비정상적인 모양을 띠고 있는 백혈구의 검출은 백혈병 진단에 있어 중요한 정보로 사용된다. 백혈구의 이상 형태중 다수의 구멍이 있는 백혈구는 검출하기 위해 골수영상에서 백혈구 영역을 추출하고 이에 대해 UNL transform을 이용하여 모양 특징을추출하였다. UNL Fourier transform은 원영상의 이동(translation), 회전(rotation), 확대/축소(scale)에 대해 불변인 성질을 지니므로 이를 이용해 백혈구의 모양 특징을 추출하고 유사도 검색을 통해 비정상의 백혈구를 검출하였다.

Analysis of Medical Images Using EM-based Relationship Method (EM기반 관계기법을 이용한 의료영상 분석)

  • The integrated medical information system is an effective medical diagnosis assistance system which offers an environment in which medial images and diagnosis information can be shared. Because of the large-scale medical institutions and their cooperating organizations are operating the integrated medical information systems, they can share medical images and diagnosis information. However, this system can only stored and transmitted information without other functions. To resolve this problem and to enhance the efficiency of diagnostic activities, a medical image analysis system is necessary. In this paper, the proposed relationship method analyzes medical images for features generation. Under this method, the medical images have been segmented into several objects. The medical image features have been extracted from each segmented image. Then, extracted features were applied to the Relationship Method for medical image analysis. Several experimental results that show the effectiveness of the proposed method are also presented.

  • Malignant lymphoma typically presents with homogeneous enhancement of enlarged lymph nodes without internal necrotic or cystic changes on multiphasic CT, which can be suspected without invasive diagnostic methods. However, some subtypes of malignant lymphoma show atypical imaging features, which makes diagnosis challenging for radiologists. Moreover, there are several lymphoma-mimicking diseases in current clinical practice, including leukemia, viral infections in immunocompromised patients, and primary or metastatic cancer. The ability of diagnostic processes to distinguish malignant lymphoma from mimicking diseases is necessary to establish effective management strategies for initial radiological examinations. Therefore, this study aimed to discuss the typical and atypical imaging features of malignant lymphoma as well as mimicking diseases and discuss important diagnostic clues that can help narrow down the differential diagnosis.

  • Various sequences have been developed for MRI to aid in the radiologic diagnosis. Among the various MR sequences, susceptibility-weighted imaging (SWI) is a high-spatial-resolution, three-dimensional gradient-echo MR sequence, which is very sensitive in detecting deoxyhemoglobin, ferritin, hemosiderin, and bone minerals through local magnetic field distortion. In this regard, SWI has been used for the diagnosis and treatment of various neurologic disorders, and the improved image quality has enabled to acquire more useful information for radiologists. Here, we explain the principle of various signals on SWI arising in neurological disorders and provide a retrospective review of many cases of clinically or pathologically proven disease or components with distinctive imaging features of various neurological diseases. Additionally, we outline a short and condensed overview of principles of SWI in relation to neurological disorders and describe various cases with characteristic imaging features on SWI. There are many different types diseases involving the brain parenchyma, and they have distinct SWI features. SWI is an effective imaging tool that provides complementary information for the diagnosis of various diseases.

The Usefulness of Cone Beam Computed Tomography in Diagnosis of Temporomandibular Joint Osteoarthritis (측두하악관절 골관절염 환자의 진단에서 Cone Beam 전산화 단층촬영의 유용성)

  • This study is designed to assess Relationship between clinical diagnosis of Temporomandibular Joint Disorder and diagnostic finding of Cone Beam Computed Tomography(CBCT) The author performed clinical examination for TMD Patients who visited Orofacial pain clinic, Jin-ju ooo Dental office. CBCT(Cone beam computed tomography) was taken for 190 joints in 95 subjects. A Oral medicine and Oral radiologist evaluated CBCT each other. then we compared with that result, Condyle bony changes were classified by no bone change, flattening, erosion and osteophyte. The obtained results were as follow: 1. The Kappa index of the diagnosis between oral medicine and oral radiogist were high, the index of diagnosis by degenerative joint disease were more higher. 2. The Kappa index of panoramic view and CBCT was low, more condylar bone chages were observed by CBCT diagnosis 3. Condylar bone changes of the 54.2% of non-DJD group clinicaly was observed by CBCT diagnosis and no bone changes of the 15.3% of DJD group.was observed by CBCT 4. TMJ pain was associated with erosion of condyle bone change of TMJ. Crepitation and longest duration of TMD were associated with osteophytic bone change.

Image Analysis Using Digital Radiographic Lumbar Spine of Patients with Osteoporosis (골다공증 환자의 Digital 방사선 요추 Image를 이용한 영상분석)

  • This study aimed to propose an accurate diagnostic method for osteoporosis by realizing a computer-aided diagnosis system with the application of the statistical analysis of texture features using digital images of lateral lumbar spine of patients with osteoporosis and providing reliable supplementary diagnostic information by model experimental research for early diagnosis of diseases. For these purposes, digital images of lateral lumbar spine of normal individuals and patients with osteoporosis were used in the experiments, and the values of statistical texture features on the set ROI were expressed in six parameters. Among the texture feature values of the six parameters of osteoporosis, the highest and lowest recognition rates of 95 and 80% were shown in average gray level and uniformity, respectively. Moreover, all the six parameters showed recognition rates of over 80% for osteoporosis: 82.5% in average contrast, 90% in smoothness, 87.5% in skewness, and 87.5% in entropy. Therefore, if a program developing into a computer-aided diagnosis system for medical images is coded based on the results of this study, it is considered possible to be applied to preliminary diagnostic data for automatic detection of lesions and disease diagnosis using medical images, to provide information for definite diagnosis of diseases, to diagnose by limited device, and to be used to shorten the time to analyze medical images.

  • Bladder cancer is a relatively common cancer type, with a high recurrence rate, that can be often encountered in the imaging study. Accurate diagnosis and staging have a significant impact on determining treatment and evaluating prognosis. Bladder cancer has been evaluated by transurethral resection of bladder tumor for clinical staging and treatment, but it is often understaged when compared with final pathologic result by radical cystectomy. If the location, size, presence of muscle invasion, lymph node metastasis, distant metastasis, and presence of upper urinary tract cancer can be accurately diagnosed and evaluated in an imaging study, it can be treated and managed more appropriately. For an accurate diagnosis, radiologists who evaluate the images must be aware of the characteristics of bladder cancer as well as its types, imaging techniques, and limitations of imaging studies. Recent developments in MRI with functional imaging have improved the quality of bladder imaging and the evaluation of cancer. In addition, the Vesical Imaging Reporting and Data System was published to objectively assess the possibility for muscle invasion of cancer. Radiologists need to know the types of bladder cancer treatment and how to evaluate the changes after treatment. In this article, the characteristics of bladder urothelial carcinoma, various imaging studies, and findings are reviewed.

Recognition for Lung Cancer using PCA in the Digital Chest Radiography (디지털 흉부영상에서 주성분분석을 이용한 폐암인식)

  • Risk of lung cancer among lung-related diseases has gradually increased during last decades. The chest digital radiography is the primary diagnosis method for lung cancer. Diagnosing lung cancer using this method requires doctors of ripe experience. Despite their experience there are often wrong diagnoses, which decrease early diagnosis and survival rates of patients. The aim of this study was intended to establish the base on the Computer Aided Diagnosis (CAD) by analyzing Image Recognition Algorithm using Principle component Analysis (PCA) and diagnosing patient's chest X-ray image. The database obtained through this approach enables a doctor to significantly reduce misdiagnosis during the early diagnosis stage, if he or she utilizes it as the preliminary reading step. Case studies were carried out using normal organ, and organs suffering from bronchogenic carcinoma and granuloma. A normal image and unique disease images were extracted after PCA analysis, and their cross-recognition efficiency were compared each other. The result revealed that the recognition rate was much high between normal and disease images, but relatively low between two disease images. In order to increase the recognition efficiency among chest diseases the related algorithms have to be developed continuously in the future study, and such effort will establish the resolute base for CAD.
