  • The purpose of this study was to investigate the effective penetration methods of acid etching agents in the pit and fissures on the occlusal surface. Extracted maxillary first and second premolars were divided into 4 groups : to Group I only acid etching agent was applied, to Group II ultrasonic scaler as well as acid etching agent were applied, to Group III after fissurotomy acid etching agent was applied, and to Group IV after fissurotomy the same process performed with Group II. The comparison between the 4 groups by the use of scanning electron microscope showed the results as follows : 1. Comparing each groups with regard to the penetration rate(%) of acid etching agents, Group III and Group IV showed improved penetration rate(%) compared with Group I. Group II made no significant different results from Group I, but had somewhat better penetration rate(%). 2. Comparing each groups with regard to the penetration rate(%) of acid etching agents according to regions within fissures, Group III and Group IV showed more improved penetration rate than Group I in the mesial pit and distal pit. However, no significant difference between each groups was showed in the middle fissure.

  • The objective of this study is to confirm the effect of dentine bonding primer application on penetration of sealant. Extracted permanent molars were used to compare penetration success rate of control group (sealant application only) and experimental groups (sealant application after applying the primers of $Scotchbond^{TM}$ Multi-Purpose system and $All-Bond^{(R)}$ 2 system). The following results were obtained: 1. The experimental groups using the primers showed increased sealant penetration success rate to the base of fissure when compared to control group but there was no statistically significant differences(p>0.05). 2. The depth, width and 'depth/width' value of fissure had statistically significant effect on sealant penetration success rate(p<0.05). 3. The penetration success rate decreased about 0.9 times as the depth of fissure increased every $25{\mu}m$, and increased about 1.1 times as the width of the fissure orifice increased every $25{\mu}m$ and decreased about 0.6 times as the 'depth/width' value increased every 1. From the above results, it can be concluded that assure morphology had a great effect on sealant penetration and for better penetration, use of dentine bonding primer can be helpful but it needs more study in clinical bases.

  • Mechanical preparation has been introduced to provide the sealant retention. The objective of this study was to compare the fissure penetration and the microleakage of pit and fissure sealant using mechanical preparation(mechanical preparation + acid etching) and acid etching only. An additional objective of this study was to compare the fissure penetration and the microleakage of unfilled and filled sealant in both methods. Sixty human premolars extracted for orthodontic purpose were selected. Thirty teeth were acid etched alone and remaining thirty teeth were prepared with a $\frac{1}{4}$ round bur and then acid etched. One-half of teeth in each surface treatment method were sealed with unfilled sealant and the other half were sealed with filled sealant. All of the teeth were thermocycled for 1200 cycles at $5^{\circ}C\;and\;55^{\circ}C$ and immersed in 5% methylene blue for 24 hours. Each tooth was sectioned bucco-lingually at mesial pit and distal pit and examined under a Measurescope. In the case of mechanical preparation, fissure penetration of sealant was significantly increased compared with the case of acid etching only(P < 0.05). The filled and unfilled sealant using mechanical preparation showed significantly decreased microleakage when compared with the unfilled sealant using acid etching only(P < 0.05). No differences were found in fissure penetration and microleakage between unfilled and filled sealant in both methods. Taken together, the results of this study suggest that mechanical preparation and filled sealant are recommended when placing pit and fissure sealant. However, further clinical studies should be performed in regard to microleakage.

  • The purpose of this study is to compare microleakage and penetration depths of sealants applied in carious and sound fissures. Extracted premolars(n=80) were divided into 4 groups according to caries status. 1; sound, 2; stained, 3; initial caries, 4; enamel caries. Sealants were applied to the occlusal groove as per manufacturers' instructions, and specimens were thermocycled, stained, sectioned, and examined for microleakage and penetration ability. The results of the present study are as follow: 1. It showed significantly higher microleakage scores in group I than group Two, Three, Four(p<0.05), but no significant difference was found between group Two, Three, and IV(p>0.05). 2. It showed significantly higher penetration scores in group Four (p<0.05), followed by group One, Two, Three, but no significant difference was found between group One, Two, and Three(p>0.05). Based on the results of present study, when the borders of the fissure sealant are on carious enamel, a significantly higher microleakage must be expected. It is considered that depth of enamel caries in the fissure should be taken into account when applying a fissure sealant.

  • This study aimed to evaluate the microleakage and penetration of two hydrophilic sealants, Embrace $Wetbond^{TM}$ and Ultraseal $XT^{(R)}$ $hydro^{TM}$, when applied on dry and moist enamel, as compared to a conventionally used hydrophobic sealant, $Clinpro^{TM}$. A total of 60 sound human third molars were randomly divided into 5 groups according to the enamel moisture control and the sealant material used. After sealant application, the teeth were thermocycled and immersed in 1% methylene blue dye. Subsequently, the teeth were sectioned twice and the sections were examined using an optical microscope and image analysis software. Application of Embrace $Wetbond^{TM}$ on either dry or moist enamel resulted in more microleakage than $Clinpro^{TM}$. Application of Ultraseal $XT^{(R)}$ $hydro^{TM}$ on dry enamel showed a similar level of microleakage to $Clinpro^{TM}$, but application on moist enamel resulted in more microleakage. There were no significant differences between the groups in penetration. In conclusion, application of hydrophilic sealants on moist enamel did not improve the sealing ability and showed lower sealing ability than that of $Clinpro^{TM}$ applied on dry enamel.

  • The aim of this study was to compared the penetration scores and microleakage levels of flowable resin in comparison to resin-based pit and fissure sealant. A total 120 extracted premolars were used and classified with group I (resin-based pit and fissure sealant), group II (flowable resin), group III (bonding agent + flowable resin), IV (fissurotomy + bonding agent + flowable resin) depending on the materials and the application methods. The penetration scores of the group treated with flowable resin following fissurotomy were similar to those of the group treated with resin-based sealant, but higher compared to those of the other groups treated with flowable resin without fissurotomy or bonding agent. The group treated with flowable resin following fissurotomy also exhibited the lowest microleakage levels. The group treated with resin-based sealant and the one treated with a bonding agent prior to the application of flowable resin showed similar microleakage levels. In conclusion, flowable resin may be applied as a pit and fissure sealant, and its application along with fissurotomy could increase the penetration scores.

  • Pit and fissure sealants has been proven as safe and effective method of caries prevention. But improper application of pit and fissure sealant may masking the caries process in occlusal fissure. Dentists have to understand the correct and exact application method of pit and fissure sealant for he good result of caries prevention. A key factor to enhanced the effectiveness of caries prevention is retention of pit and fissure sealant. Deep penetration of material into fissure and least marginal leakage around the fissure orifice are the major concerns for dentists to achieve the successive application of pit and fissure sealants. This paper reviewed the literature on the pit and fissure sealants under the following subtitles for enhanced retention : (1) application timing, (2) Indication, (3) Occlusal prophylaxis, (4) Materials, (5) Penetration, (5) Recall check Dental profession must perform the exact application of pit and fissure sealant because this procedure is one of the most technique-sensitive one in dental field.

  • The purpose of this study was to evaluate the effect of fissurotomy on the penetration and microleakage of flowable resins for carious fissures. A total of 250 extracted premolars with early fissure caries were selected and divided into five groups according to the fissurotomy; no fissurotomy (n = 50), fissurotomy with $Fissurotomy^{(R)}$ original bur (n = 50), fissurotomy with $Fissurotomy^{(R)}$ Miro NTF bur (n = 50), fissurotomy with SF104R tapered diamond bur (n = 50), fissurotomy with 1/2 round carbide bur (n = 50). Two types of flowable resins ($UniFil^{(R)}Flow$, $Filtek^{(R)}Flow$) were used as sealing materials. All samples were sectioned and observed using a stereoscopic microscope after thermocycling and immersing in methylene blue solution. The adaptation of flowable resin to the fissure wall was observed using scanning electron microscopy. The penetration of flowable resin into the carious fissure was significantly increased by fissurotomy, which also decreased microleakage. Fissure preparation using different burs showed a significantly different in penetration, but did not show any difference in microleakage. $Unifil^{(R)}Flow$ showed better penetration than $Filtek^{(R)}Flow$, but there was no significant difference in microleakage. Fissurotomy can be used to increase the penetration of flowable resin into carious fissures and decrease microleakage.
