  • REM sleep behavior disorder is parasomnia characterized by unpleasant dreams and dream-enactment behaviors associated with excessive electromyography activity in REM sleep. This may appear idiopathic or secondary to other neurological or medical conditions. REM sleep behavior disorder, which appears to be idiopathic, most often implies the possibility of later neurodegenerative diseases due to synucleinopathy, so accurate diagnosis is important in predicting prognosis. For the diagnosis of REM sleep behavioral disorder, REM sleep without atonia, which appears in the polysomnography, is essential. Obstructive sleep apnea, trauma-related sleep disorders, and vigorous periodic leg movements during sleep are known as diseases that show dream enactment behavior in elderly patients.   • Objective : This review aimed to provide information for clinical application by confirming the principles and characteristics of the tool through a review of tongue movement and measurement tools for patients with swallowing disorders. Results : We identified 15 tools used as tongue exercises and measurement tools in the field of dysphagia. According to principle, the tools were classified as either a bulb sensor, resistive sensor sheet, mouthpiece with sensor, or other techniques. The bulb sensor was easy to use but had limitations in fixing the position when measuring tongue pressure. The resistive sensor sheet could be measured at a more stable position than the bulb sensor. A mouthpiece with a sensor could be used in an individual's oral cavity such that the position was fixed when measuring the tongue pressure. Other techniques had the advantage of being wireless and capable of sensing light. Conclusion : Based on this literature review, it is necessary to facilitate the selection of the best tool for quantitative tongue measurement in dysphagia. The review can also be used to develop a Korean tongue movement tool model that can be used in hospitals and community centers.

  • A 55-year-old male was admitted to our hospital complaining of chest discomfort, dysphagia and severe pain on swallowing. Esophageal manometry showed that there was no relaxation of lower esophageal sphincter to swallowing. There was double barrelled esophagus or mucosal stripe appearance on esophagogram. Endoscopy revealed an appearance as if an esophago-tracheal fistula had been made at the level of the upper thoracic esophagus. Also, two slit-like mucosal tears was seen on the lower esophagus. There after, fasting and total parenteral nutrition for several weeks failed to bring about any changes in his symptoms. So, as treatment, primary closure of the upper opening of the false lummen was performed under general anesthesia. Soon after the surgical procedure, the patient's symptoms were improved except for mild dysphagia. He was discharged after oral intake had been juduciously commenced with fluids and soft diet subsequently. During follow-up in out-patient department, he had no specific symptoms including fever or dysphagia and massive dissection of the esophagus was improved on esophagogram. We report the experience of a case of spontaneous submucosal dissection of the esophagus which required conservative and surgical management

Effects of a Dysphagia Nursing Protocol on Reducing Aspiration Pneumonia and Improving Daily Activity Dependency in Patients with Acute Stroke (급성기 뇌졸중 환자에게 적용한 '연하 장애 간호 프로토콜'이 흡인성 폐렴 발생율과 일상생활 의존율에 미치는 효과)

  • Purpose: Aspiration pneumonia (AP) is the major complication of dysphagia in patients with acute stroke. Nursing management for dysphagia influences on patients' health outcomes. The purpose of this study was to develop and evaluate the effects of Dysphagia Nursing Protocol (DNP) for patients with acute stroke. We aimed to examine the incidence of AP and the dependency rate at the discharge. Methods: We used a non-equivalent control group posttest only design in this study. DNP was developed with content validity index > 0.75 in all items. This protocol was composed of the evaluation, education, complication prevention, and checking phases. After training the staff nurses, we applied DNP protocol to the experimental group. The control group was enrolled from June to August 2013 (n=80), and the experimental group was June to August 2014 (n=64). Results: The incidence of AP was 16.3% in control group while 4.7% in experimental group (p=.028), and the Odds Ratio (OR) was 0.26 (p=.045). The dependency ratio was 66.3% in control group while 43.8% in experimental group (p=.011), and the OR was 0.33 (p=.011). Conclusion: DNP was beneficial for the reducing the risk of AP and the improved the functional status.

  • 유비쿼터스홈 적응형 미들웨어란 디바이스/서비스의 상태 변화 혹은 장애에 의한 홈 네트워크의 Crash 및 성능저하 등을 자율적으로 감지, 억제, 복구, 치유할 수 있도록 지원해주는 유비쿼터스홈 유지 보수 프레임워크이다. 본 논문에서는 현재의 홈 네트워크 산업현황을 통하여 홈 에트워크 활성화 저해요소로 폐쇄적인 자사위주의 홈 네트워크 구조와 장애처리 및 유지보수의 부재를 지적하고, 디바이스의 지능화 서비스의 다양화, 사용자 요구사항의 다변화를 포함하는 유비쿼터스 홈네트워크 진화방향을 분석한다. 이를 지원하기 위해 본 논문에서는 적응형 홈네트워크 기술을 홈네트워크 상호운용성 제공기술, 홈네트위크 장애진단 및 복구 기술, 유비쿼터스홈 적응형 구성관리 기술로 기술하였다.

  • Astrocytomas are tumors that arise from brain cells called astrocytes. These tumors can occur anywhere in the brain and spinal cord. Clinical features and symptoms depend on the location of the tumor and the victim's age. Patients with cerebellar tumors have symptoms that include headache, vomiting and unsteadiness in walking. Tumors in the cerebral hemispheres commonly present with seizures: occasionally there is weakness of the arms and legs. We treated a 33 year-old male patient who complained of dyarthria, TMD, nausea, dysphagia, and facial numbness etc. he had frequent undergone general weakness before above symptoms occurred. He was diagnosed as astrocytoma and multifocal inf and responded well to treatment with Chukdamtang(滌痰湯). acupunture and oriental physical stimulations etc.

  • Achalasia is the disease of nonorganic obstruction of the cardia associated with dilatation of the esophagus, and of unknown etiology characterized by failure of relaxation of the inferior esophageal sphincter and lack of normal peristalsis in the body of the esophagus. This disorder is primarily one of neuromuscular dysfunction, most probably due to a central nervous system lesion with consequent nerve, ganglion and muscle degeneration. The characteristic symptoms are dysphagia, regurgitation and epigastric pain etc. We have experienced a case of Achalasia in 43 aged female with good results by daily mercury bougienage.
