Physical and Mental Health Status and Its Related Factors among Centenarians in Korea (100세 이상 고령자의 신체적·정신적 건강수준 및 관련 요인)

  • This study was performed to determine the level of physical health(ADL, IADL) and mental health (cognitive impairment, dementia) among centenarians and to find the related factors. The study subjects were 268 centenarians who received medical check-ups from the National Health Insurance Corporation during 2011-2014. From the results, the factors that significantly influenced the ADL of the study subjects were gender, residential area, IADL, cognitive impairment and dementia. ADL and cognitive impairment were selected as the influencing factors of IADL, smoking status, ADL and IADL were selected as the influencing factors of cognitive impairment, and gender and ADL were selected as the influencing factors of dementia. These results suggested that the levels of physical and mental health of the centenarians were significantly affected by socio-demographic characteristics and health-related variables. Especially, the levels of physical and mental health were decreased with the poor group of health-related variables such as smoking, alcohol drinking and regular exercise.

Characteristics of Premenstrual Changes in Patients with Affective Disorder (정서장애 환자의 월경전기변화 특성)

  • Objectives : A large of studies have found an association between premenstrual syndrome and affective disorder, in particular, depression. Some studies have reported that women with depressive disorders may experience menstrual cycle-associated changes in the severity of their symptoms. This study was designed to compare the characteristics of premenstrual changes between control group and affective patient group, and to assess possible risk factors for premenstrual changes in patients. Methods : Eighty normal controls and eighty outpatients given maintenance therapy with fixed dose for at least more than four weeks were asked to complete questionnaires on menstrual history, obstetric-gynecological history, and functional impairment. In addition, to compare the characteristics of premenstrual changes, 16 items based on DSM-IV criteria A for premenstrual dysphoric disorder were rated on the following scale : 0(no change), 1(mild), 2(moderate), 3(severe). Moderate or severe change in each item was considered as premenstrual change and the subjects who reported more than one premenstrual change were defined as premenstrual change group. Results : The results were as follows : 1) The percentage of premenstrual change group was 32.6% in patient group and 50% in control group. 2) Frequently reported premenstrual changes were as follows in control group : breast tenderness; anger ; affective liability ; lethargy, easy fatigability, or marked lack of energy ; abdominal pain or discomfort. In patients, the mood or behavioral changes were frequently reported. The changes were as follows : lethargy, easy fatigability, or marked lack of energy ; change in appetite; affective liability ; sense of difficulty in concentrating : hypersomnia or insomnia. 3) In the premenstrual change group, the patients with only mood or behavioral changes were significantly more than those with only physical changes or both changes. 4) The severity of functional impairment was significantly correlated with the frequency of mood or behavioral changes in patients. 5) There were no significant differences in menstrual characteristics between patients with premenstrual changes and patients without them except the severity of dysmenorrhea. And the severity of dysmenorrhea was correlated with the frequency of premenstrual change. Conclusion : The proportion of patients with affective disorder, who reported moderate-to-severe premenstrual changes, experiencing mood or behavioral changes larger than those experiencing physical changes during premenstrual period. It is possible that some patients with affective disorder, who reported premenstrual mood or behavioral changes, suffer from coexisting premenstrual syndrome with affective disorder or premenstrual exacerbation of affective disorder. Since the more premenstrual changes, the severer functional impairment, the patients reporting mood or behavioral disturbance in premenstrual period should be carefully evaluated, and appropriate therapeutic stategies might be considered.

The Effect of the Shift Work on Drinking and the Mediating Effect of Sleep (교대근무가 음주에 미치는 영향 및 수면의 매개효과)

  • Objectives : Shift work has been known to cause various health problems by making it difficult for humans to adapt to their natural circadian rhythms. In particular, shift workers tend to complain of sleep difficulties associated with work schedules, and sometimes use alcohol as a self-medication to induce sleep. To date, no clear mechanism has been identified regarding the link between shift work and sleep, between shift work and drinking. This study aims to confirm the relationship between shift work and sleep, and to analyze whether the change in sleep caused by shift work causes drinking. Methods : This study included 11360 people (5704 men and 5656 women) among the Korean National Health and Nutrition Survey data in 2014, 2016 and 2018. The work type between 6am-6pm was defined as day work, and other work types were defined as shift work. Using logistic regression analysis, the relationship between shift work and sleep quality, shift work and high risk drinking, drinking amount at 1 time, drinking frequency were analyzed. In addition, we analyzed whether sleep mediates the relationship between shift work and drinking using mediated analysis. Results : Shift work showed a significant negative relationship with sleep quality in men and women (male OR=1.37, 95% CI 1.11-1.70, female OR=1.26, 95% CI=1.05-1.50). There was no significant relationship between shift work and alcohol in the case of men, but in the case of women, there was a significant positive relationship between shift work and the number of alcohol consumption (OR=1.34, 95% CI=1.04-1.72). When mediation analysis was conducted, it was found that women's sleep quality partially mediated the relationship between shift work and the frequency of drinking. Conclusions : The results of this study suggest that shift work causes sleep difficulties, and in the case of women, drinking can be induced through the partial mediating effect of sleep. Considering that women are more likely to choose drinking as a coping method for sleep problems, more active interventions for female shift workers are needed.

Characteristics of PMS and PMDD in Female College Students (여대생에서의 월경전기증후군 및 월경전불쾌장애의 특성)

  • Objectives : We investigated the prevalence and functional impairment of premenstrual dysphoric disorder (PMDD) and premenstrual syndrome(PMS) in young women. Methods : A total of 1063 female college students were recruited from two urban areas(Seoul and Suwon) of Korea. Questionnaires for sociodemographic data and risk factors of PMDD, attitude about menstruation, and the Premenstrual Symptoms Screening Tool(PSST) were applied. Results : The prevalence of severe moderate to severe PMS and PMDD were 16.9% and 11.7%, respectively. There were differences in the alcohol and coffee consumption, severity of menstrual cramp, and family history of PMS among the moderate to severe PMS, PMDD, and no/mild PMS groups. Although some participants did not fulfill diagnostic criteria for PMDD, they showed significant functional impairment. Participants with negative attitude about menstruation reported premenstrual symptoms more frequently than those with positive or ambivalent attitude about menstruation. Conclusion : These results suggest that PMS and PMDD were prevalent and associated with functional impairment in young females. Some participants reported significant functional impairments although they did not meet the full DSM-IV diagnostic criteria for PMDD. Negative attitude about menstruation was associated with more premenstrual symptoms experiences.

Fragile X Syndrome and it's Association with Autism (약체X염색체 증후군과 자폐증과의 연관)

  • The fragile X syndrome, which is considered to be synonymous with the Martin-Bell syndrome, is a relatively common form of X-linked mental retardation. The syndrome seems to occure in many different ethnic groups and its prevalence among mentally retarded males has been estimated to be in the order of 2 to 6%. The karyotypic hallmark of the syndrome is made up with a pronounced constriction near each tip of the long arm of the X chromosome(fragile site), shown in vitro only under conditions in which thymidylate production is blocked(lowered folate levels). Special culture media are needed to demonstrate this constriction site. Major clinical features associated with the syndrome include macroorchidism, large or prominent ears, significant emotional and behavioral dysfunctions such as hyperactivity, self-injury, lack of eye contact and social interaction, schizophrenia, autism, etc., and speech and language dysfunctions ranging from nonverbal to verbal speech with moderate to severe expressive language delays. Some have minor clinical features in common such as an increase in birth weight high forehead, prognathism, increased head circumference in infancy and childhood which did not persist into adult life. The recent research findings have shown that the fragile X syndrome is associated with infantile autism. Many patients with the fragile X syndrome fulfill the diagnostic criteria for infantile autism. Therefore it is recommendable that any patient with developmental delays and autism or autistic manifestations should have a chromosomal analysis, including fragile X examination. In the present review, historical aspects, incidence, and clinical features are presented. Recent anecdotal reports of the association with autism and the clinical improvement following high dose folic acid treatment will be discussed.

A Study on the Clinical Characteristics of Burning Mouth Syndrome (구강내 작열감 증후군의 임상적 특징에 관한 연구)

  • 구강내 작열감 증후군은 객관적 징후없이 구강점막에 작열감을 나타내는 만성동통장애이다. 다양한 원인요소들이 제시되어 있지만, 이러한 요소들의 관련성에 대해서는 아직 분명하지 않은 실정이다. 그러므로, 이러한 구강내 기능이상의 근본적이고 효과적인 치료를 위해서는 다양한 임상적 관찰과 원인요소의 분석등이 계속 연구 조사되어져야 할 것이다. 저자는 구강내 작열감 증후군의 증상을 호소하는 112명의 환자의 임상적 특징에 대해 조사하고자, 환자군과 대조군에 대해 구강내 작열감 증후군에 관한 설문조사 및 구강검사와 혈액학적 검사를 시행하였으며, 점도계를 사용하여 구강내 작열감 증후군 환자의 자극이 전타액의 점도를 측정하였다. 또한, 환자의 심리적 요인을 파악하고자 간이정신진단검사를 실시한 결과 다음과 같은 결론을 얻었다. 1. 구강내 작열감 증후군은 주로 페경기 전후의 여성에서 많이 나타났으며, 호발부위는 혀, 치주 및 치조점막, 구개, 협점막의 순이었다. 2. 구강내 작열감 증후군 환자에게서 대조군 비해 구강건조감, 미각 및 수면장애, 빈혈, 소화기 장애, 두통과 기타 다른 신체불편감의 호소 등이 더 많이 나타났다. 3. 혈액학적 검사결과, 구강내 작열감 증후군 환자중의 26%와 12%에서 각각 혈중 엽산농도와 철분농도의 저하를 나타냇다. 4. 자극시 분비된 전타액의 점도는 40대와 50대이상의 연령군에서 구강내 작열감 증후군 환자가 정상인에 비해 높았다.(p<0.01). 5. 구강내 작열감 증후군 환자군의 간이정신진단검사의 각 증상차원 및 전체지표의 T점수의 평균치는 정상범위내에 있었으며, 신체화(SOD)와 우울 (DEP)차원의 평균치가 가장 높았다.

Effects of Foot Bath Therapy on Menopausal Symptoms and Sleep in Women in Their 50s (족욕요법이 50대 여성의 갱년기 증상 및 수면에 미치는 영향)

  • Purpose: Purpose: This study was to study the effect of foot bathing on menopausal symptoms and sleep disorders in female subjects in their 50s with menopausal symptoms. Methods: After 12 weeks of foot bath therapy three times a week, pre- and post-menopausal signs were measured. Results: Subjects with hot flashes, heart discomfort, sleep problems, depression, irritability, anxiety, physical and mental fatigue, sexual problems, urination problems, vaginal dryness, and joint and muscle discomfort significantly improved after than before(p < .001). Subjective sleep quality, sleep incubation period, sleep duration, sleep disturbance score, sleep drug use, and sleep dysfunction significantly decreased after foot bath than before(p < .001). Habitual sleep efficiency increased considerably. Conclusions: The subjects showed overall improvement in menopausal symptoms and sleeping quality after a foot-bath. Therefore, foot bath therapy is evaluated as a natural healing therapy suitable for improving menopausal symptoms and sleep.

Life Stress, Social Support and Suicidal Ideation of North Korean Refugee Women in South Korea (탈북여성의 일상생활 스트레스가 자살생각에 미치는 영향과 사회적 지지의 조절효과)

  • The purpose of this study is to identify the moderating effect of social support between daily life stress and suicidal ideation of North Korean refugee women in South Korea. We survey with self-report questionnaires for 156 refugee women, conduct descriptive and regression analysis. As results, (1) almost half of the surveyed experience suicidal ideation in a year, (2) daily life stress affects suicidal ideation controlling with depression, PTSD, alcohol use, income, and age, (3) social support which North Korea refugee women perceive buffers the relationship between life stress and suicidal ideation. From these results, implications are suggested. Gender cognitive policies are needed in North Korea refugees in South Korea. Also, community mental health services and sustainable community program for North Korea refugee women are important to prevent their suicidal attempts.

Psychiatric Symptoms Among Female Adult Victims of Sexual Molestation : Comparison with Rape Victims (성인 여성 성추행 피해자들의 정신적 증상 : 강간 피해자들과의 비교)

  • Objectives : According to the recent Korean National Police Agency report, rape victims were 17.1%, but sexual molestation victims were more than the rape victims by 78.0%. Despite many international reports about the occurrence of severe psychiatric symptoms in sexual molestation victims, there is no domestic research. Therefore in this study, we investigated psychiatric symptoms of sexual molestation victims, and we also compared it with psychiatric symptoms in rape victims. Methods : 58 women who visited Busan Smile Center within 3months after sexual violence were the study subjects. Questionnaire about sociodemographic and sexual violence related characteristics were retrospectively investigated. Of the psychiatric symptoms, Beck Depression Inventory(BDI), Beck Anxiety Inventory(BAI) for severity of depression and anxiety, and Impact of Event Scale-Revised(IES-R) to check the presence of posttraumatic stress disorder(PTSD) symptoms were used. Results : Of 58 sexual violence victims, sexual molestation victims were 36(62.1%) and rape victims were 22(37.9%). In sexual molestation victims, 80.6% had more than moderate severity of depression, 83.3% had more than moderate severity of anxiety, and 94.4% had significant scores at PTSD screening test. Compared with rape victims' psychiatric symptoms(each 95.5%, 95.5%, 95.5%) there were no significant difference. Conclusions : The majority of sexual molestation victims were also accompanied by depression, anxiety and PTSD symptoms as rape victims. There results suggest that appropriate assessment and early treatment for psychiatric symptom must be made in the early stage of injury in sexual molestation victims.

Factors Affecting Medical Service Utilization of Disabled (장애인의 의료이용에 영향을 미치는 요인)

  • This article examined the condition of medical service utilization and the usage degree for the disabled in Korea, and analyzed factors affecting medical service utilization. This paper offers data for improving the health of disabled Korea residents and for enhancing medical service utilization. We analyzed data for disabled residents aged 19 from the 6th Korea National Health and Nutrition Examination Survey. Results found significant effects depending on gender, age, average monthly income, types of disorders, disability rating, and status of smoking. Concerning gender, men had a higher probability of lacking medical treatment compared with women, had increased probability of having a disability, and smokers had a higher probability of lacking treatment compared with nonsmokers. Therefore, for resolving medically untreated disabled Korean residents, government needs to improve the policy system and to managethe inequality of handicapped welfare work.
