The Role of Bone Scans in Routine Preoperative Evaluations of Non-Small Cell Lung Cancer Patients. (비소세포 폐암의 병기에 있어 통상적인 골 스캔의 역할)

  • The objective of this study was to assess the usefulness of bone scans in routine preoperative examinations of patients with newly diagnosed non-small cell lung carcinoma. Material and Method: We reviewed the medical records of 258 patients who were newly diagnosed with non-small cell lung cancer in our hospital between January 2000 and December 2000. More than half of the patients (132) were deemed to be inoperable due to their advanced stage based on the CT scans. The remaining 126 patients were considered potentially operable. For these patients, clinical evaluation including the presence of bone pain, serum alkaline phosphatase, and calcium levels was used as clinical predictors of bone metastasis. All patients received bone scans. Bone X-rays, MRI or bone biopsy were performed to confirm the presence of bone metastasis. The usefulness of the bone scan was evaluated by comparing its power of predicting bone metastasis to that of the clinical information. Result: In all patients, the positive and negative predictive values of bone scans for the bone metastasis were 44%, and 99%, respectively. Those of the clinical information were 38% , and 94%. However, in potentially operable patients, the negative predictive value of the clinical information was as high as 99%. Conclusion: If newly diagnosed non-small cell lung cancer patients are presented as potentially operable on the basis of CT scan with no clinical evidence of distant metastases, curative resection could be considered without performing routine bone scans because of the low probability of bone metastasis. However, if there are positive clinical findings, further evaluations, including bone scan should be followed as metastasis will be documented in more than 30% of patients.

Estimation of Fish Species Diversity of Small and Medium Rivers of Korea with Fish Species-Habitat Relationship Models od GAP (GAP기법을 이용한 종소하천의 어류종다양성 예측기법 연구)

  • The objectives of this research were to develop fish-habitat relationship models which can be used to estimate fish species riclmess of small and medium rivers in Korea, and test the accuracy of the models. The models are based on the Aquatic GAP Analysis model in the New York Cooperative Fish & Wildlife Research Unit (19%), and they employ three habitat factors; river size, physical habitat, and water quality of each river segment. Model 1 and model II are based on the water quality standard for life support of EP A and the water quality class of Korea, respectively. Test sites for this study include one urban stream and three less spoiled tributaries of the Han River. The results of this research can be summarized as follows. First, the number of habitat types identified by model I and model II are nine and 14, respectively. Second, the average accuracy of the three distribution maps of rare or endangered fish species are 80.6% (model 1) and 81.2% (model II). Third, the accuracy of fish species richness are 94% (model 1) and 95% (model II), and the water quality is the most important factor affecting fish species richness. Fourth, the accuracy of fish species list are 50.5% (model 1) and 68.7% (model II), but the accuracy of less spoiled stream segments and that of polluted stream segments are 67.1% and 86.5%, respectively. Finally, it can be concluded that the overall performance of model II is better than that of model I at our test sites.

  • 급변하는 e비즈니스 환경과 IT기술 발전으로 인해 e비즈니스 인력의 수급예측은 매우 어려운 실정이다. 이는 e비즈니스 인력수급 불균형, e비즈니스 산업분야별 인력양성 정보제공의 어려움 등의 원인으로 작용하고 있다. 이에 본 연구에서는 e비즈니스 인력수급 실태조사를 통해 e비즈니스 산업에서 실제로 활용 가능한 인력양성 방안을 제시하고자 한다. 이를 위해 기업체, 교육기관, 취업자 등 조사대상을 3개로 하여 조사를 실시하였으며, 결과를 바탕으로 e비즈니스 산업 분야별 인력수급 실태와 기존 e비즈니스 인력양성 프로그램의 문제점을 분석하였다. 본 연구의 결과를 통하여 정부의 e비즈니스 인력 관련 정책수립에 도움을 줄 수 있으며, 기업체, 교육기관에게는 인력양성을 위한 지침으로서의 활용이 기대된다.

Usefulness of Tuberculin Test in Adult Patients with Suspected Pulmonary Tuberculosis (폐결핵이 의심되는 성인 환자에서 투베르쿨린 검사의 유용성)

  • Background : The tuberculin skin test has been used to diagnose latent tuberculosis infection, but is not widely used to diagnose or exclude pulmonary tuberculosis. The objective of this study was to evaluate the diagnostic utility of the tuberculin test in diagnosing and excluding pulmonary tuberculosis, and differentiating pulmonary tuberculosis from nontuberculous mycobacteria (NTM) pulmonary disease, when a sputum acid-fast bacilli (AFB) smear was positive. Material and Methods : From October 2002 to August 2003, among all the inpatients of the Division of Pulmonary and Critical Care Medicine at Samsung Medical Center, 258 patients with clinical suspicion of pulmonary tuberculosis were enrolled and underwent a tuberculin test. Results : 156 males and 102 females were included, with a mean age of 57.5 years. The final diagnoses included lung cancer in 89 cases (34.5%), pulmonary tuberculosis in 59 cases (22.9%), bacterial pneumonia in 33 cases (12.8%) and NTM pulmonary disease in 24 cases (9.3%). The positive tuberculin test rate was higher in the tuberculosis than non-tuberculosis group; 81.4 (48/59) vs. 42.4% (81/199). (p<0.001). In 208 patients with a negative sputum AFB smear, the result of the tuberculin test was positive in 69.4% (25/36) of the tuberculosis group and in 44.8% (77/172) of the non-tuberculosis group (p=0.007), so a positive result of the tuberculin test could predict pulmonary tuberculosis with 69.4% sensitivity, 55.2% specificity, a 24.5% positive predictive value and a 89.6% negative predictive value. In 50 patients with a positive sputum AFB smear, the positive rates of the tuberculin test were 83.9% (26/31) in tuberculosis group and 21.1% (4/19) in NTM pulmonary disease group (p<0.001), so a positive result of the tuberculin skin test could predict pulmonary tuberculosis with 83.9% sensitivity, 78.9% specificity, a 86.7% positive predictive value and a 75.0% negative predictive value. Conclusion : The tuberculin test could be useful in excluding pulmonary tuberculosis when the sputum AFB smear is negative, and to differentiate pulmonary tuberculosis from NTM pulmonary disease when the sputum AFB smear is positive.

Prediction of Amyloid β-Positivity with both MRI Parameters and Cognitive Function Using Machine Learning (뇌 MRI와 인지기능평가를 이용한 아밀로이드 베타 양성 예측 연구)

  • Purpose To investigate the MRI markers for the prediction of amyloid β (Aβ)-positivity in mild cognitive impairment (MCI) and Alzheimer's disease (AD), and to evaluate the differences in MRI markers between Aβ-positive (Aβ [+]) and -negative groups using the machine learning (ML) method. Materials and Methods This study included 139 patients with MCI and AD who underwent amyloid PET-CT and brain MRI. Patients were divided into Aβ (+) (n = 84) and Aβ-negative (n = 55) groups. Visual analysis was performed with the Fazekas scale of white matter hyperintensity (WMH) and cerebral microbleeds (CMB) scores. The WMH volume and regional brain volume were quantitatively measured. The multivariable logistic regression and ML using support vector machine, and logistic regression were used to identify the best MRI predictors of Aβ-positivity. Results The Fazekas scale of WMH (p = 0.02) and CMB scores (p = 0.04) were higher in Aβ (+). The volumes of hippocampus, entorhinal cortex, and precuneus were smaller in Aβ (+) (p < 0.05). The third ventricle volume was larger in Aβ (+) (p = 0.002). The logistic regression of ML showed a good accuracy (81.1%) with mini-mental state examination (MMSE) and regional brain volumes. Conclusion The application of ML using the MMSE, third ventricle, and hippocampal volume is helpful in predicting Aβ-positivity with a good accuracy.

The Utility of $^{18}F$-FDG PET/CT for Following Up Head and Neck Cancer (두경부암 환자의 추적관찰에서 $^{18}F$-FDG PET/CT의 유용성)

  • 서 론 : 두경부암의 예후에 가장 중요한 인자는 진단시 병기와 치료 후 재발의 유무이다. 따라서, 재발 병변을 조기에 발견하는 것이 매우 중요하다. 본 연구에서는 두경부암 환자의 추적 관찰에서 $^{18}F$-FDG PET/CT의 유용성에 대해 알아보고자 하였다. 대상 및 방법 : 2004년 4월부터 2008년 8월까지 고려대 안암병원에서 두경부암으로 치료를 완료한 후 추적관찰이 가능하였던 88 예의 환자를 대상으로 하였다. 환자는 본원의 두경부암 추적관찰 양식에 따라 술 후 6, 12개월째 $^{18}F$-FDG PET/CT를 시행하였으며, 원발부위 및 경부 재발, 원격전이와 중복암의 진단에 있어 $^{18}F$-FDG PET/CT의 민감도, 특이도, 양성예측율, 음성예측율을 평가하였고, 기존의 추적관찰 방법과 이를 비교하였다. 결 과 : 원발부위 재발의 진단에 있어 민감도, 특이도, 양성예측율, 음성예측율은 각각 95%, 97%, 90%, 98% 였다. 경부전이 재발의 진단에 있어서는 각각 94%, 98%, 83%, 99%였으며, 원격전이 및 이차암 진단에 있어서는 94%, 100%, 100%, 99%였다. 원발부위 재발은 고식적인 진단 방법, 원격전이와 이차암은 $^{18}F$-FDG PET/CT가 진단에 의미있게 유리하였다. 결 론 : $^{18}F$-FDG PET/CT는 두경부암의 추적관찰 과정에서 재발 여부를 판정하는데 있어 매우 유용하며, 특히 원격전이 및 이차암의 발견에 있어 유용하다. 그러나, 위양성 및 위음성의 가능성으로 고식적인 진단 방법과 병행해야 진단의 효과를 높힐 것으로 생각된다.

노동력수요의 중장기 전망-지식기반경제로의 이행-

  • 세계화 추세의 가속, 급속한 기술진보 및 정보화의 확산 등은 국제간의 분업구조 뿐만 아니라 국내의 산업구조 및 취업구조를 급격히 변화시키고 있다. 국내 취업구조의 변화는 산업간 직업간 취업비중을 모두 변화시키고 있어 고용의 양적 측면은 물론이고 질적 측면에서도 큰 변화가 예상된다. 그러므로 중장기적인 노동력수요 예측은 향후 인력양성의 방향과 체계를 결정하는 데 중요한 지표가 될 뿐만 아니라 노동시장 정책에 있어서도 반드시 필요한 자료가 되고 있다. 따라서 본 연구에서는 향후 노동력에 대한 중장기 수요를 일반균형적인 접근을 통하여 산업별 직업별로 예측하고 이 예측결과에 따른 향후 노동시장 및 인력정책의 방향을 제시하는 데 있다. 본 연구에서는 기존의 연구들이 사용한 방법의 이론적 약점을 보완하면서 실증 결과의 예측성을 높일 수 있는 개선된 방법을 사용하였다. 산업별 취업자를 전망하는데 있어 기존 연구결과들은 주로 노동시장의 단일방정식만을 추정함으로써 생산요소의 수요가 비탄력적이지 않음에도 불구하고 생산요소간의 대체가능성을 무시하고 있으며, 인력수요에 중요한 결정 요인인 기술변화를 고려하지 못하고 있거나, 취업계수의 변화를 통해 기술진보의 효과를 고려한다고 하여도 기술진보가 생산요소간에 비중립적(factor non-neutral technology changes)으로 일어날 경우 요소간 대체가능성을 간과하고있다. 이러한 단점을 보완하기 위해 본 연구에서는 산업별 취업자 전망에서 노동시장의 단일방정식 추정이 아닌 일반균형에 입각한 경제 모형을 사용하였다. 또한 직종별 취업자 전망을 위해서는 일반적으로 이용되는 필요 인력량에 대한 고정계수(fixed coefficient manpower requirement)를 추정할 때 이중비례행렬모형(二重比例行列模型)을 이용하여, 산업구조의 변화로 인한 직업별 인력수요 변화가 충분히 고려되도록 하였다. 전망의 결과에 따르면 향후 우리 경제는 지식기반경제(knowledge-based economy)로 이행하고 있다고 볼 수 있다. 우선 산업구조면에서 지식집약적산업으로의 구조조정이 일어나게 되고 이에 따라 산업별 취업구조에서도 고기술산업의 취업준비중이 급속히 증가하게 된다. 직업별 취업분포에 있어서도 전문기술직 행정관리직 등의 고숙련 사무직의 비중은 크게 증가하는 반면 생산관련직과 농림어업직의 비중은 감소하게 된다. 이처럼 경제가 지식집약화되어 감에 따라 고학력자에 대한 수요는 지속적으로 증가하지만 현재 적절한 인력양성과 공급이 이루어지지 않고 있어 향후 기술이나 기능에 따른 수급부일정(需給不一政)(skill mismatch)현상이 매우 심해질 것으로 보인다. 따라서 앞으로의 인력정책에서 가장 주안점을 두어야 할 부분은 첨단기술산업과 관련된 인력의 양성에 있다고 하겠다.

Effective Ways of Performing Surveillance Surface Cultures in Extremely Low Birth Weight Infants

  • Purpose: The rationale for skin surface cultures is that bacterial colonization precedes infection and, as a result, that identification of a potential pathogen is predictive of later systemic infection in preterm infants. We aimed to analyze results of surveillance surface cultures in extremely low birth weight (ELBW) infants and seek for effective ways of performing surveillance surface cultures. Methods: We analyzed the surveillance surface cultures of 113 ELBW infants over a 4-year period. Surveillance cultures were obtained routinely from five sites: axilla, external ear canal, nasopharynx, throat (or tracheal aspirate if intubated) and anus. Each surface culture obtained during the 13 days, prior to the date of the blood culture, was compared with the blood culture obtained when sepsis was suspected. The sensitivity, specificity and positive predictive value (PPV) of the surveillance cultures were calculated among 1894 blood-surface culture pairs by surface sites, recovered organisms and interval between surveillance samples and blood cultures. Results: The overall sensitivity, specificity and PPV of surface cultures were 45.9, 22.4 and 6.8%, respectively. The PPV was highest for the throat/tracheal cultures (11.0%) and lowest for the anal cultures (2.3%). As the time of culturing progressed toward the day of blood culturing, the sensitivity and specificity of the surface cultures significantly increased. Only axillary and throat/tracheal cultures were useful in predicting the microorganisms causing sepsis. Conclusion: Surface cultures could help to predict sepsis pathogens and infection surveillance in preterm infants could be continued with a reduced number of cultured sites focusing on the axilla and throat/trachea.

Rapid Detection of Mycobacterium tuberculosis Complex in Tissues by Using the Nested PCR (Nested PCR을 이용하여 조직으로부터 Mycobacterium tuberculosis Complex 신속검출)

  • Due to the increase in incidence of infection of Mycobacterium tuberculosis complex (MTC), it is imperative that a rapid diagnosis accompanies the handling of MTC. This is due to the three to eight weeks it takes to culture Mycobacteria, and the lack of sensitivity of microscopic examination of AFB. Recently, nested PCR has been used to detect and diagnose mycobacteria. It is especially useful in complementing diagnosis by histological extra pulmonary. After culturing all the specimens and practicing the nested PCR, we did comparison analysis between nested PCR and culture. There were 76 specimens, 31 of which were positive. Of the 31 positive specimens in culturing, only 22 were positive in nested PCR. Of the 45 negative specimens, 36 were negative in nested PCR. As a result, Sensitivity was 71% and specificity was 80%. Furthermore, the positive predictive value was 71% and negative predictive value was 80%. These results indicate that nested PCR based techniques are sensitive, specific, and rapid methods for the detection of MTC.

Usefulness of Clinical Scoring System in the Diagnosis of Group A Streptococcal Pharyngitis (인두염 환아에서 A군 연구균 검출의 임상적 점수제의 유용성)

  • Purpose : Proper diagnosis of group A streptococcal pharyngitis that may cause chronic diseases in childhood is not easy because its signs and symptoms would be nonspecific. Because results of classical throat culture delays for one to two days, we'd like to determine whether early antibiotics would be introduced with according to the clinical score system. This study was undertaken to evaluate of clinical usefulness of scoring system based on the clinical and laboratory findings. Methods : From Jan. 1998 to Dec. 2000, 10 clinical items based on modified 9 items by Breese in 1977 were checked in patients with pharyngitis who visited on outpatients clinic of pediatrics, Kyunghee University Hospital. We compared the results of throat culture with the points of clinical score system. Results : Out of 45 cases, the positive culture for Group A Streptococcus was 20 and negative culture was 25. When we applied more than 30 points of score, which correspond to 70 percentile of study population, the sensitivity and specificity were 35.0% and 96.0%, respectively. Conclusion : Although sensitivity was relatively low this scoring system, but the high specificity may be useful diagnostic tool in the areas where the rate of isolation of Group A Streptococcus is low.
