Effects of Silkworm Powder on Blood Glucose and Lipid Levels in NIDDM(Type II) Patients (인슐린 비의존형(Type II) 당뇨환자에서 누에분말 섭취가 혈당 및 혈중지질농도에 미치는 영향)

  • The purpose of this study is to investigate the effects of the silkworm powder on blood levels of glucose, Hb Alc, insulin, and lipids. Thirty-one NIDDM patients from Kyung Hee Medical Center were divided into two groups : patients with silkworm powder supplements and drug treatments(Drug diabetes) and patients with silkworm powder supplement only(Non-drug diabetes). For the control group, age-matched subjects were recruited. During the 4 weeks of the experimental period, silkworm powder(500mg/mea1) was given to the subjects right after each meal. Nutritional assessments and dietary education were carried out periodically, and body weight and blood pressure were measured when patients visited the hospital. Overnight fasting and 2-h postprandial blood glucose levels were measured at 2 week intervals. The blood levels of insulin, Hb Alc, and lipids were measured before and after the supplements. The mean ages of the three groups were 56.7-59.6 years old. The height, weight, and BMI did not differ among the groups. The fasting blood glucose levels were 138.1$\pm$22.0mg/dl for the Drug treated diabetes group, 175.0$\pm$32.0mg/dl for the Non-drug diabetes group, and 108. 3$\pm$16.gmg/dl for the control group at the begining of the supplement. After 4-wks of supplements, the blood levels of glucose tended to decrease in all three experimental groups. Before the supplements, the 2-h postprandial blood glucose levels of the Drug diabetes, Non drug diabetes, and control groups were 244.7$\pm$62.6mg/dl, 272.4$\pm$40.1mg/dl, and 147.7$\pm$28.0mg/dl, respectively. After the supplement, the levels were 197.2$\pm$30.gmg/dl, 208.6$\pm$ 56.6mg/dl, and 151.3$\pm$30.3mg/dl, respectively. This shows that silkworm powder tended to lower blood levels by 19.4% and 23.4% in NIDDM patient groups. However, the changes in the blood levels of insulin, Hb Alc, ind lipids were not observed after the supplement. In conclusion, the present study has demonstrated that silkworm powder has a tendency to decrease 2-h postprandial blood glucose levels, but it should be used with caution in controlling the diabetes.   • 최근 소셜미디어 리뷰 데이터를 활용한 약물 이상 반응 탐지 연구가 활발히 진행되고 있지만, 약물을 복용하기 전 증상과 약물 이상 반응을 구분하지 못한다는 한계가 있다. 본 논문에서는 약물 이상 반응 탐지에서 약물 복용 전의 증상을 구분할 수 있는 Filtering Clinical BERT(FC-BERT) 모델을 제안하였다. FC-BERT 는 약물 복용 전 증상과 다른 약물에 대한 부작용 표현을 제거하기 위해 약물명이 나오기 전 모든 문장을 제거하는 필터링과 약물-부작용 쌍을 추출하는 모델을 사용했다. 성능 평가 실험을 위해 문장에 대한 ADE(Adverse Drug Event) 여부가 들어있는 ADE Corpus V2 데이터를 활용하였고 SPARK NLP 라이브러리에서 제공하는 ADE Pipeline 모델과 비교하여 성능 평가를 실시하였다. 실험 결과 필터링을 활용한 FC-BERT 모델이 기존 모델보다 정확도, 평균 정밀도, 평균 재현율, 평균 F1-score 가 모두 높은 결과를 보여주었다. 본 논문에서 제시한 모델은 기존 연구의 한계점을 보완하여 보다 정확한 약물 부작용 시그널을 탐지하는데 기여할 수 있을 것이다.

  • 잘 쓰면 약이고 못 쓰면 독이 된다는 '약'에 대한 금언처럼 필요할 때만 적정 용량을 최적의 시간에 복용해야 하는데 실상은 그렇지 못하다. 수요가 고급을 창출한다는 경제 원리에 따라 의약품이 20~30년 전에 비해 값싸게 공급되다보니 의약분업 시행에도 불구하고 불필요한 약을 먹게 되는 경우가 많다. 약의 폐해와 안전하고 비용 대비 효율이 높은 약물 복용법에 대해 알아본다.

  • 약 먹는 것을 좋아하는 김간난 할머니 (72세, 중구 신당동)는 하루에 먹어야 하는 약만 해도 몇 가지나 된다. 당뇨약, 고혈압약, 고지혈증약, 관절염약을 복용하고 있으며 그 외에도 글루코사민, 종합 영양제, 홍삼 액기스도 챙겨먹고 있다. 양로원이나 친구를 만날 때면 박카스 등 각종 드링크제도 자주 마시고 있다. 그러나 이 아플 때만 약을 먹고 약 먹는 것을 잊어버릴 때도 많아 꾸준하게 복용하고 있지는 않다.

AN OBSERVATIONAL MULTI-CENTER STUDY FOR EVALUATION OF EFFICACY, SAFETY AND PARENTAL SATISFACTION OF METHYLPHENIDATE-OROS IN CHILDREN WITH ADHD (주의력결핍과잉운동장애 아동에게 Methylphenidate-OROS 투여시 효용성과 안전성 및 부모 만족도를 평가하기 위한 다기관관찰연구)

  • Objectives : This study was performed to evaluate the efficacy and safety of MPH-OROS and parental satisfaction for treatment of children with ADHD Method : The 569 participants were clinically diagnosed for ADHD using DSM-IV criteria. We switched current medication to MPH-OROS or introduced MPH-OROS for treatment of ADHD. We assessed the clinical global impression severity of illness (CGI-S), the clinical global impression severity of improvement(CGI-I). And the parents of participants measured the Korean version of Conners rating scale at baseline, the 1 st week and the 3rd week after MPH-OROS trial. At the 3rd week, the parents measured the parent satisfaction questionnaire. Results : $13\%$ of participants dropped out because of several causes including side effects. The change of CGI-S was significantly decreased. Using CGI-I, the improvement was $72.3\%$ at the 1st week and $87.4\%$ at the 3rd week. The total score of the Korean version of Conners parent rating scale was significantly decreased. The participants complaining one or more of side effects were 119$(20.7\%)$, and the most common side effect was anorexia. The $94\%$ of parents replied that they were overall satisfied with MPH-OROS trial. Also the advantages of MPH-OROS of parental report were the long duration of the drug, the improvement of schoolwork and attitude, the improvement of home behavior and homework and the improvement of overactivity. Conclusion : MPH-OROS is effective and well-tolerated in actual clinical use for ADHD.

Association of Liver Dysfunction with Self-Medication History in Korean Healthy Male Adults (건강한 한국 성인남성의 자가약물복용력에 따른 간기능 장애 발생여부 조사)

  • Background: Korean people could abuse healthy foods as well as medications, which might cause serious side effects. The aim of this study was elucidating liver dysfunction due to the self-medications of hepatotonics, healthy foods and herb medications by nested case-control study. Methods: Study subjects were drawn from male members of Seoul Cohort Study who were recruited by self-administered structured questionnaire survey through mailing to the healthy men between the age of 40 and 59 years through the program of biennial health check-up offered by Korea Medical Insurance Corporation(KMIC). The liver dysfunction was defined as the level of serum AST and ALT above 40 IU/L and increased in more than one hunderd per-cent during the 2 year follow-up period. To estimate the odds ratio between self-medication and liver dysfunction after controlling for potential confounders, logistic regression was performed. Results: During the follow-up period, 30 members were identified to fit into case criteria and 2,625 members were selected as control. In logistic regression analyses, history of healthy foods intake, age under 45 years, obesity, and habit of regular exercise were significantly associated with liver dysfunction. The following factors exhibited no statistical significance: intake of hepatotonics, of herb medicine; history of disease in family, of operation, and of radiologic examination; smoking habits and drinking amounts. Conclusion: The significant association between the intake of healthy foods and the liver dysfunction illustrates that chronically optional overuse of healthy foods might bring to hazards to health. As the increasing trend of the size of purchasing healthy foods in Korea, pharmacoepidemiologic studies evaluating the safety and efficacy of the widely used healthy foods should be performed in the near future.

  • 한국인이면 누구나 건강을 위해 비타민이나 영양제를 하나쯤 복용하고 있을 것이다. 게다가 여름철이면 보양식을 즐겨 먹고 몸에 좋다는 음식을 물불 가리지 않는다. 한국인만큼 건강식품을 사랑하는 민족이 또 있을까? 그러나 무작정 먹기만 한다고 몸에 다 좋지는 않다. 아무리 좋은 약이라도 약과 약, 약과 음식이 만나면 우리 몸에 독이 되기도 한다. 약의 올바른 복용법을 숙지해서 건강을 해치는 일이 없도록 하자.

  • 의료기관에서 접근이 용이한 마약류 약물로 오남용하는 사례가 지속적으로 증가하고 있다. 이를 해결하고자 본 논문은 블록체인 프레임워크인 하이퍼레저 패브릭을 기반으로 구축된 약물 오남용 관리 시스템을 제안한다. 하이퍼레저 패브릭의 분산 원장 기능을 사용하면 의약품 거래를 투명하게 기록하고 처방 기록을 안전하게 보존하여 모든 거래 세부 정보를 변조할 수 없게 된다. 또한 약물 사용 기록을 추적하고 남용을 방지하기 위해 과다 복용 사용자를 규제하는 기능을 제안한다. 본 논문이 약물 오용을 크게 완화하고 과다 복용 사용자 보호가 가능할 것이라 기대한다.

A Study on Sedation, Delirium and Fall in Mental Illness Taking Psychiatric Drugs (정신약물을 복용하는 정신질환자의 진정, 섬망, 낙상에 관한 연구)

  • This study is a descriptive research study to understand the relationship between sedation, delirium, and fall of people with mental illness taking psychiatric drugs. From March10, 2019 to November 10, 2019, four psychiatric nurses were evaluated on 149 people with mental illness in two psychiatric hospitals located in D city. The collected data were analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficient using SPSS 22.0. Sedation and delirium(r=.587, p=<.001), sedation and fall(r=.273, p=.001), delirium and fall(r=.390, p=<.001)were all significant positive correlation. This study identified the degree of sedation, fall, and delirium of patients taking psychiatric drugs and confirmed the relationship between sedation, fall, and delirium of people with mental illness. It is thought that it will be able to provide basic data to prevent falls and delirium that may occur when taking psychiatric drugs and to prepare a nursing intervention strategy.
