After global economic crisis, most countries increased the welfare expenditure as a part of stimulus package. As a However welfare expenditure was decreased radically as the crisis was transmitted into the financial crisis. Which turning point is the welfare state going through now? Although the need for the welfare and the role of the state to take responsibility of public welfare has increased because of poverty and polarization, responses of the state against the crisis had focused on the aid to the financial industry and cutting taxes and showed limitations in coordination. Financial limitation of welfare expenditure, political individualism, the change of class politics and the mixture of the welfare institutions and financial institutions make have a pessimistic prospect of the retrenchment to the minimalist welfare state. As neoliberal state is continued cash benefits mainly for the middle class is being decreased. As a result, the direction the welfare states pursue is prospected to win over the poor by strengthening selectivity in welfare provision rather than class coordination.

3년여 간 지연됐던 IPTV 법제화가 연내 이뤄질 수 있을 지 업계의 관심이 온통 국회에 쏠려 있다. 특히, 연초 방송통신위원회 설치법 및 IPTV 법제화를 위해 출범한 방송통신특별위원회 활동이 올 연말까지로 제한돼 있어 사실상 9월 정기국회가 마지막 기회로 인식되고 있다. 방송통신특위가 활동시한을 연장해 내년에도 법제화를 추진할 수는 있지만 올 연말 대선과 함께 새로운 행정조직이 들어서고, 현재 17대 국회의원의 임기가 내년 초라는 점을 감안하면 사실상 연내 처리가 불가능해질 경우 법제화가 1~2년 또 지연될 수밖에 없기 때문이다. 이 때문에 이미 IPTV 상용화 준비를 끝낸 KT는 IPTV 법제화가 될 수 있도록 최근 TF팀을 구성해 총력전을 펼치고 있으며, 지난 7월에는 러시아 블라디보스토크 내 이동통신 자회사인 NTC를 통해 러시아에서 IPTV 상용화를 먼저 시작하겠다며 국회와 정부를 압박하고 나섰다.

The breast cancer has the highest occurrence rate among the female cancers, and as the living style changes, the occurrence is increasing gradually. For the breast cancer test among women, who comprises up to 50% of the total population, the mammography is mainly used as the screening test, and the accuracy control is the most important aspect of the testing. Therefore this research divided the northern part of Kyeongsangbuk-do into 4 regions and investigated the accordance ratio of examination field and light examination field, the total focus using the optical density and compression rate, and the overall maintenance of mammography within the regions. The equipments of 11 hospitals were investigated, and the 7 hospitals passed the standard level of the accordance ratio of examination field. 6 hospitals passed the standard optical density, and 7 hospitals had the passing performance in the compression rate. Fibers, group of specks, and masses within the Mammographic Accreditation Phantom scored 10, being within the standard range. However, only 3 hospitals were equipped with private development processor and illumination. The result reflects the fact that the image quality of breast is not correctly being maintained. Moreover, only 27.27% satisfied all the three categories of compression fitting, accordance ratio of examination field, and phantom image evaluation at the same time. The accuracy control must be maintained more precisely for the accurate diagnosis of breast cancer.

Purpose: Point fixation at the margin of the glenoid is a limitation of conventional arthroscopic stabilization using suture anchors, and does not afford sufficient footprint healing, especially in glenoid bone deficiency. So, we introduce an arthroscopic suture bridge transosseous-equivalent technique for bony Bankart lesions to avoid the technical disadvantage of point contact with anchor fixation and to improve mechanical stability through cross compression of the labrum. Surgical approach: The technique was adapted from the transosseous-equivalent rotator cuff repair technique using suture bridges, which improved the pressurized contact area and mean pressure between the tendon and footprint. After preparation of the glenoid bed by removal, reshaping, or mobilization of the bony lesion, two anchors (3.0 mm Biofastak, $Arthrex^{(R)}$, Naples, FL) were inserted into the superior and inferior portion of the bony Bankart lesion. Using a suture hook, medial mattress sutures were applied around the capsulolabral portion of the IGHL complex to obtain sufficient depth of glenoid coverage. A 3.5 mm pushloc anchor ($Arthrex^{(R)}$, Naples, FL) hole was made in the articular edge of the anterior glenoid rim. distal, suture bridge was applied, and proximal was inserted to mobilize the labrum in the proximal direction. This avoided the technical disadvantage of point contact with anchor fixation and decreased the level of gap formation through cross-compression of the labrum.

Purpose : This study was undertaken to investigate clinical aspects of nutcracker syndrome in children including sexual distribution, age of onset, the ratio of peak flow velocity between the narrowed and dilated portions in the left renal vein and prevalence of hematuria or proteinuria. Materials and Method : The subjects included 30 patients diagnosed with nutcracker syndrome using Doppler sonography from September 1999 to January 2001 in Severance Hospital, Department of Pediatric Nephrology. The diagnostic criteria for nutcracker syndrome was a peak flow velocity of ratio is more than 5.0. Results : Patients consisted of 14 males and 16 females. The mean age was 9.19${\pm}$2.31 years old and the ratio of peak flow velocity was 8.52${\pm}$2.24. Age of onset us higher in males 9.68${\pm}$2.82 year old than in females, 8.76${\pm}$1.72 year old respectively(P<0.05). But there was no difference in the ratio of peak flow velocities ; 8.45${\pm}$2.31 in the male group , 8.58${\pm}$2.25 in the female group(P>0.05). There was no difference in the age of onset and the ratio of peak flow velocity between patients with hematuria and without hematuria. Among patients with nutcracker syndrome, 24 cases of hematuria, 3 cases of proteinuria only, and 3 cases of hematuria with proteinuria. Conclusion : In children of school age, early diagnosis of nutcracker syndrome and consistent follow up is worthy of consideration, especially ill the prevention of sequelae. (J. Purpose : The purpose of this study was a phantom study to measure the diffusion properties of water molecules by steady-state free precession diffusion-weighted imaging (SSFP- DWI) with a low b-value and to determine if this sequence might be useful for application to the evaluation of bone marrow pathology. Materials and methods : 1. The phantom study: A phantom study using two diffusion weighted sequences for the evaluation of the diffusion coefficient was performed. Three water-containing cylinders at different temperatures were designed: phantom A was $3^{\circ}C$, B was $23^{\circ}C$ and C was $63^{\circ}C$. Both SSFP and echo planar imaging (EPI) sequences (b-value: $1000s/mm^2$) were performed for comparison of the diffusion properties. The Signal to noise ratios (SNR) and apparent diffusion coefficient (ADC) values of the three phantoms using each diffusion-weighted sequence were assessed. 2. The Clinical study: SSFP-DWI was performed in 28 patients [sacral insufficiency fractures (10), osteoporotic lumbar compression fractures (10), malignant compression fractures (8)]. To measure the ADC maps, a diffusion-weighted single shot stimulated echo-acquisition mode sequence ($650s/mm^2$) was obtained using the same 1.5-T MR imager Results : For the phantom study, the signal intensity on the SSFP as well as the classic EPI-based DWI was decreased as the temperature increased in phantom A to C. The ADC values of the phantoms on EPI-DWI were $0.13{\times}10^{-3}mm^2/s$ in phantom A, $0.22{\times}10^{-3}mm^2/s$ in B and $0.37{\times}10^{-3}mm^2/s$. in C. The SSFP can be regarded as a DWI sequence in view of the series of signal decreases. Conclusion : Bone marrow pathologies with different diffusion coefficients were evaluated by SSFP-DWI. All benign fractures were hypointense compared to the adjacent normal bone marrow where as the malignant fractures were hyperintense compared to the adjacent normal bone marrow.

Aberrant right subclavian artery (ARSA) is an anomaly with a reported incidence of 0.5% to 2%. Most patients with an ARSA remain asymptomatic; however about to% of adult patients have compressive symptoms. A case is reported of a 64-year old female patient who had a few years of history of dysphagia and recurrent pneumonia. Angiography was performed, which demonstrated an ARSA with common origin of the right and left carotid arteries. Surgical correction was performed via right thoracotomy. The proximal aberrant artery was mobilized behind the esophagus. The distal, right subclavian artery was exposed, transected, and transposed with reimplantation into the aortic root by graft bypass.

An 11-month old girl with a feeding difficulty and recurrent aspiration pneumonia received surgical correction of complete vascular ring, which was formed by right aortic arch, aberrant left subclavian artery(LSCA) originating from Kommerell's diverticulum(KD) and ligamentum arteriosum. Through left posterolateral thoracotomy, the ligamentum arteriosum was divided to relieve the tracheo-esophageal bundle. KD was separated from the right descending aorta, and the left subclavian artery was severed from KD at its origin and trasfered to the side wall of left common carotid artery. Postoperative course was uneventful, and the patient has been followed up with a clinical improvement.

This study examined the wearing conditions of compression wear for weight training. Compression wear has recently received significant attention from the public and athletes as high-level functional sportswear. The survey was conducted on 373 men in their 20s for general information on purchase behavior, preferred brand, function, and preferred design. The majority of participants (n=181) were aware of the function of compression wear. The most selected reason for wearing compression wear was for its convenience during sports activities. Respondents had the greatest preference for designs with a round neckline, regular leggings type, upper arm-length sleeve, and thigh-length bottom. Respondents also wanted a stronger compression in the core muscle part (abdomen, back, and waist).

가락동 농수산물도매시장은 지난 1985년 개설된 국내 최초 공영도매시장이자 연간 230만톤 거래 규모의 세계 최대 도매시장으로 서울시민 농수산 먹거리의 약 50%를 담당하고 있으며 전국 공영도매시장 물량의 약 33%를 처리하는 등 전국 농수산물 유통의 중추 기능을 담당하고 있다. 가락시장은 5,000여개 유통업체가 영업 중이며 하루 이용객은 13만명, 출입차량은 6만7,000대에 달할 정도로 과부하를 겪고 있다. 또한 도시 주거환경을 저해하는 혐오시설로 치부돼 이전 압박을 받아왔으며 전반적인 시설 노후화와 유통 수요에 부적합한 물류구조, 주변 교통정체 야기 등의 문제로 시설현대화 필요성도 제기되어 왔다. 이에 따라 서울시농수산식품공사가 지난 2009년 재건축을 통한 시설현대화 사업을 발표하고 기존의 시장 기능을 유지하면서 재건축을 하는 순환식 시설현대화 사업을 3단계(당초 계획은 2009년~2018년까지 10년 예상했으나 가락시장 계획변경 타당성 검토용역 결과에 따라 2025년까지 지연될 것으로 예상됨)에 걸쳐 추진키로 했다. 이에 따라 1단계 사업은 지난 2011년 6월부터 진행되어 2015년 2월 준공 예정이다. 본지는 가락시장 시설현대화 1단계 공사에서 기계설비 및 소방공사를 담당한 (주)성아테크(대표 정해돈) 이완식 현장소장과 시공사인 대림산업(주) 박승재 현장소장을 만나 현장 이야기를 들어 보았다.
