알코올중독자의 신체적 건강과 삶의 만족도: 종교적 대처의 완충효과 (Physical Health Problems and Life Satisfaction among Alcoholics: Moderating Effects of Religions Coping)

  • Although the relationship between physical health problems and life satisfaction is well documented, less is known about the role of coping strategies, in particular strategies for coping via religious or spiritual means. To investigate the relationships among these factors, data collected with questionnaires completed by 326 adult alcoholics at a rehabilitation agency. Significant Physical Health Problem $\times$ Religious Coping buffer interactions were found in the relationship between physical health problem and life satisfaction. Moderated regression analysis showed that religious coping reduced the impact of physical health problem on life satisfaction. Implications for practice are discussed.

익명의 알코올중독자(AA) 모임 참여자의 삶의 질에 영향을 미치는 요인 (Factors Influencing Quality of Life of Alcoholics Anonymous Members in Korea)

  • Purpose: The purpose of this study was to determine quality of life (QOL) related factors in Alcoholics Anonymous (AA) members based on PRECEDE Model. Methods: A cross sectional survey was conducted with participants (N=203) from AA meeting in 11 alcohol counsel centers all over South Korea. Data were collected using a specially designed questionnaire based on the PRECEDE model and including QOL, epidemiological factors (including depression and perceived health status), behavioral factors (continuous abstinence and physical health status and practice), predisposing factors (abstinence self-efficacy and self-esteem), reinforcing factors (social capital and family functioning), and enabling factors. Data were analyzed using t-test, one way ANOVA, Tukey HSD test and hierarchical multiple regression analysis with SPSS (ver. 21.0). Results: Of the educational diagnostic variables, self-esteem (${\beta}=.23$), family functioning (${\beta}=.12$), abstinence self-efficacy (${\beta}=.12$) and social capital (${\beta}=.11$) were strong influential factors in AA members' QOL. In addition, epidemiological diagnostic variables such as depression (${\beta}=-.44$) and perceived health status (${\beta}=.35$) were the main factors in QOL. Also, physical health status and practice (${\beta}=.106$), one of behavioral diagnostic variables was a beneficial factor in QOL. Hierarchical multiple regression analysis showed the determinant variables accounted for 44.0% of the variation in QOL (F=25.76, p<.001). Conclusion: The finding of the study can be used as a framework for planning interventions in order to promote the quality of life of AA members. It is necessary to develop nursing intervention strategies for strengthening educational and epidemiological diagnostic variables in order to improve AA members' QOL.

분노를 경험하는 동안에 나타난 알코올 사용 장애자의 자율신경계 반응 특성 (Characteristics of Autonomic Nervous System Responses to Anger in Individuals with Alcohol Use Disorders (AUDs))

  • 분노는 알코올 사용 장애자들이 재발하기 전에 가장 흔하게 경험하는 정서 중 하나이다. 본 연구는 알코올 사용장애자의 분노 경험에 따른 자율신경계 반응 특성을 규명하고자 하였다. 본 연구에는 알코올 사용 장애로 진단받고 치료 중인 남성 환자 12명과 이들과 연령 및 학력수준이 유사한 일반인 남성 14명이 참여하였다. 분노를 유발하기 위하여 시청각 동영상을 제시하고, 이때 나타난 심리 및 자율신경계 반응(피부전기 활동)을 측정하였다. 연구 결과, 참여자들이 경험한 정서의 강도에서는 알코올 집단과 일반인 집단에서 차이가 나타나지 않았다. 반면, 알코올 집단은 안정 상태에 비해 분노 상태에서 비중독자에 비해 피부전도 수준, 피부전도 반응 그리고 피부전도반응 수가 유의하게 낮았다. 이 결과는 알코올 중독자들이 분노를 처리하는 동안에 생리 반응에서 이상이 나타남을 시사하였다. 알코올 사용 장애자들의 분노 경험 동안에 나타난 생리 반응 이상을 완화할 수 있는 개입방법을 개발한다면 알코올 사용 장애자들의 정서 처리 문제를 해결할 수 있어 그들의 재발 방지에 도움을 줄 수 있을 것이다.

알코올중독자의 A.A.몰입과 회복과의 관계에서 단주자기효능감의 매개효과: 성별 및 연령의 차이를 중심으로 (Mediating Effect of Alcohol Abstinence Self-efficacy in the Relationship between the Alcoholics Anonymous Affiliation and Alcohol Recovery)

  • 본 연구의 목적은 알코올중독자의 A.A.몰입이 회복에 어떻게 영향을 미치며 단주자기효능감의 매개효과에 대해 검증하는 것이다. 이를 위해 전국의 A.A.에 참여하고 있는 알코올중독자 554명을 대상으로 구조방정식 모형을 활용하여 분석을 실시하였다. 그 결과 첫째, A.A.에 참석하는 사람들의 A.A.몰입도가 높아질수록 그들의 회복 정도도 큰 것으로 나타나 A.A.몰입이 회복에 미치는 직접 효과가 존재하였다. 둘째, A.A.몰입도는 회복에 영향을 미치는데 있어 단주자기효능감의 부분매개효과가 존재하였다. 셋째. A.A.몰입도가 단주자기효능감을 매개로 회복에 이르는 경로에서 성별 및 연령에 따른 차이가 있는지를 살펴보았을 때, 남녀 집단의 차이는 나타나지 않았으며 연령에 따른 차이가 존재하였다. 본 연구는 알코올중독자의 A.A.참여를 통해 회복을 도울 수 있는 다양한 사회복지적 방안들에 대해 제시하고 특히, 성별과 연령에 따라 차별화된 개입전략을 논하였다는데 의의가 있다.

알코올 중독자 부모를 둔 청소년의 자녀역할과 학교 적응과의 관계에서 부모애착과 교사관심의 조절효과 (Moderating Effects of Parental Attachment and Teacher's Concerns in the Relationships between Children's Roles and School Adjustment among Children's of Alcoholics)

  • The purpose of this study is to examine the relationships between children's roles, parental attachment, teacher's concerns and school adjustment among Children's Alcoholics(COAs). Participants were 2,803 middle and high school students in Seoul. The regression analysis results showed that hero role was positively associated with school bonding and academic performance but increased the level of anxiety/depression. Meanwhile, scapegoat and lost children's roles were negatively associated with school bonding and also increased the level of anxiety/depression. Mascot role were positively associated with school bonding and academic performance but had no relation with anxiety/depression. Regarding moderating effects, maternal attachment moderated the relationship between scapegoat role and school bonding while teacher's concerns moderated the relationship between hero role and anxiety/depression, and the relationship between scapegoat role and anxiety/depression. These findings suggested that practitioners need to consider the contributions of children's roles on school adjustment and moderating effects of maternal attachment or teacher's concerns when intervention programs are developed to improve school adjustment among COAs.

알코올 중독 환자의 재발과 치료 경험 (The Lived Experience of Relapse and Treatment Among the Alcoholics)

  • Purpose: The purpose of this study is to explore the subjective experiences of relapse and treatment among the alcoholics. Methods: Phenomenological methodology was used for the study. Participants were seven people with inpatient alcoholics at the alcohol treatment center at K hospital in K province. Data were collected via in-depth interviews from March to September, 2012 and analyzed using Colazzi's framework. Results: Six themes and seventeen meanings were formulated for the relapse and treatment experiences of participants with alcoholism. Six themes were 'Lack of true insight', 'Lack of strength to live a life', 'Unable to overcome an urge to drink', 'Being drawn away from others', 'Falling into the powerlessness and despair', and 'Being foothold of self-growth'. Conclusion: The results from this study revealed the relapse experiences among alcoholic patients. Therefore, the findings can provide the framework and direction for developing the relapse prevention program for alcoholic patients. In addition, practitioners in the treatment center or hospital need to consider their relapse and treatment experience.

알코올 중독자 성인자녀의 극복력에 대한 통합적 문헌고찰: 조작적 정의와 조정변수 중심으로 (An Integrative Literature Review of Resilience in Adult Children of Alcoholics: Focusing on Operational Definitions and Moderators)

  • Purpose: The objective of this study was to explore operational definitions and moderators of the resilience of adult children of alcoholics (ACOA) and its associated variables. Methods: This integrative review was conducted based on a literature review method suggested by Whittemore and Knafl. Databases including PubMed, CINAHL, PsycINFO, and RISS were searched using the combinations of search terms such as; 'adult children of alcoholics,' 'parental alcoholism,' 'resilience,' and 'children of alcoholics.' Results: Seventeen studies were selected for this review. Findings of integrative reviews reported operational definitions for the resilience of ACOA and moderators. The operational definition of ACOA's resilience is: (1) having no behavioral and emotional problem, (2) meeting social and developmental expectancies, (3) having the ability to cope with adversity, (4) having high self-concept, and (5) having a meaningful relationship. In addition, the varying environmental moderators include family communication, qualities of caring environment, parental communication, parentification, and social support. Several moderators were identified as positive effect, self-esteem, alcohol-specific coping style, locus of control, spirituality, the image of God, and dyadic cohesion in marital communication. Conclusion: Our study findings emphasize comprehensive understanding of diverse definition of resilience and moderators in developing nursing interventions to promote resilience in ACOA.

가상현실에서의 아바타 음주권유가 갈망감 유발에 미치는 영향에 관한 연구 (A Study of the Effect of an Avatar Encouraging Drinking in Virtual Reality on Alcohol Craving)

  • 본 연구는 음주거절훈련을 위한 전 단계로 가상현실을 통한 자극 제시가 알코올 중독 환자의 갈망감에 어떠한 변화를 주는 지를 알아보고자 하였다. 본 연구결과를 통해 향후 음주 거절 훈련 프로그램의 개발 뿐만 아니라 알코올 중독 환자들을 위한 새로운 치료 및 훈련 도구로서의 가상현실기법의 활용 가능성을 알아보기 위한 기초적인 자료 제공을 목적으로 한다. 가상현실 자극은 알코올 중독자들의 음주유발 상황으로 알려진 음주권유(사회적 압력)상황과 알코올 노출 여부 그리고 긍정적 상황 부정적 상황으로 구성하였다. 대상자는 2006년 12월부터 2007년 9월까지 입원중인 알코올 중독 환자 12명이었으며 수집된 자료는 SPSS/WlN 11.5 프로그램으로 통계처리 하였으며, Wilcoxon Matched-pairs Signed-Ranks Test로 분석하였다. 분석 결과 배경에서 아바타의 음주권유 여부가 일반적인 배경만 제시되는 경우보다 더 높은 갈망감을, 알코올이 노출된 장면이 그렇지 않은 장면보다 더 높은 갈망감을 나타내었고 긍정적 부정적이냐는 상황은 유의미한 결과를 나타내지 못하였다. 이러한 결과를 토대로 가장현실을 활용한 훈련이 실제의 사회적 압력과 유사한 음주 압력 상형과 환경을 느끼도록 하며, 가상현실 기법이 기존의 알코올 치료 기법들과 연합되어 효과적인 치료 도구로써의 활용가능성을 많이 가지고 있다는 것을 확인 할 수 있었다.

  • Medications or cognitive-behavior methods have been mainly used as a treatment of alcoholism. lately the virtualy technology has been applied to the kink of alcoholic disorders. A virtual environment makes him having ability to over come the drink. In this study, we were implemented by making panorama images and 3D object modules using 3D Studio MAX. VRML, JAVA Applet. And the BAR stimulator that composed with a position sensor head mount display, and audio system, is suggested. To illustrate the physiological difference between a person who has a alcoholism and and without a liquor bottle, heart rate was measured during experiment, and also measured a Person's HR after the virtual reality training. we demonstrated the subjective effectiveness of virtual reality psychotherapy through the clinical experiment.

Experiences of Overcoming Alcoholism in Alcoholics' Female Spouses Using Al-Anon

  • Purpose: The aim of this study is to investigate and elucidate actual experiences of alcoholics' spouses using familial social gatherings. Methods: A total of nine women whose alcoholic husbands were attending Al-Anon meetings held in Y Cathedral in S District of Seoul were included in this study. Data collection of this study was conducted through in-depth interviews with participants from September 24 to November 30, 2015. Data analysis was conducted at the same time as the data collection, and analysis was performed according to the method suggested by Colaizzi(1978). Results: We categorized the participants' experiences into five themes including 'Trapped in a Deep Dark Tunnel', 'Life of Walking in an Endless Tunnel', 'Reaching a Dead End', 'Walking towards a Ray of Light', 'Finding Oneself on a Strange Path'. Conclusion: The results of this study provide an understanding of the essential structure of the life experiences of alcoholic spouses using familial social gatherings.
