The Study on Facility Accident Case and PL Recognition of the Safety Manager in Electric Installation for Private Use (자가용 전기설비 안전관리자의 PL인식 조사 및 사고사례 연구)

  • In this paper, we investigated electric safety manager's PL recognition and accident occurrence of electric power facilities in electric installation for private use. The target of questionnaire survey is 450 electric installations for private use, 154 responded the questionnaire among these and it occupied 35.8[%]. The questionnaire survey says that 58[%] of electric safety manager knew well about PL contents and 42[%] of them did not recognize its. 82.4[%] of them must applied PL law in electric installation, 5.8[%] of them did not need the PL law. Reason of PL application occupied 64[%] for stability of electrical facility and product, 19[%] for prevention of same accident. The accident of transformer occupied most and the power cable accident were much next. The questionnaire survey results, electric safety managers are interested in PL contents, they expected necessity of PL application about electrical accident strongly.

  • Purpose: Ten years after basic safety and health education in the construction industry has been implemented, no more maintenance education has been provided, and those who have completed basic safety and health education are exempt from new employee education. As soon as possible, we will seek strategic measures to prevent unstable behavior and prevent safety accidents by legislating conservative education and new recruitment education. Method: In this study, basic safety and health education, which is construction safety education, and human error, which is unstable behavior of new employees, and structural relationship between violation behavior were conducted through survey. Result: The survey analyzed that basic safety and health education in the construction industry and new recruitment education had a significant impact on the human factors of the disaster, Human Error, and the reduction of violations, and confirmed that continuous safety education could increase safety awareness. Conclusion: The insolvency of safety education shall be prevented by the revision of the Act on the remuneration of basic safety and health education and the education of new employees.

  • Purpose: Since the 1990s, Korea has been making various efforts to prevent serious industrial accidents, such as implementing a hazardous risk prevention plan and implementing a process safety management system, but a series of recent major industrial accidents have shown its limitations. Therefore, this study aims to present measures to improve safety management of PSM target workplaces. Method: Investigate previous studies and cases on improving safety management at workplaces subject to PSM, seek ways to improve safety management at workplaces subject to PSM based on existing safety management, and evaluate safety management system one of the causes of accidents. Therefore, a safety education program is established to increase employees' safety awareness and understanding, and to improve the safety level. Result: Measures such as establishing an organizational culture to improve safety management of PSM target workplaces, responding to disasters, systematic safety management, replacing and strengthening old facilities, and strengthening management's firm commitment to safety were presented. Conclusion: Through this study, it is meaningful to present a systematic plan to secure safety to reduce precious lives and damage, and it is expected to be useful in considering the direction of PSM improvement for each workplace.

  • 가정에서 사용되는 가전기기 중에서도 특히 소비전력이 높은 모발건조기는 물리적 충격에 약하고, 물기 등에 노출되는 경우 순식간에 고열이 발생해 화재로 이어질 가능성이 큰것으로 나타났다. 모발건조기의 부주의한 방치로 인한 화재의 확산 패턴 및 과학적 조사 기법을 통해 쉽게 노출되는 전기안전 사고의 방지에 경각심을 일깨워야 할 때이다.

  • Construction workers occupies only $10{\%}$ of the entire industry, but the $30{\%}$ of the critical accidents in whole industry occurs in construction industry. Since there are many dangerous process and lack of safety information measures in the construction industry, both short and long term operation in education, training, and information are necessary. Examples for existing accidents in construction sites will be very useful for preventing accidents. And if the research for individual accidents perfectly achieved, it will give valuable information to construction sites which will be very useful for preventing accidents. The object of this study is to develop construction safety information system to give safety information to construction process by work field. The data base are made from 860 major accidents mostly in buildings and apartments in year 1999 through 2000(KOSHA).

  • 산업이 발달함에 따라 대량의 화물을 빠르게 운반할 수 있는 해상운송수단의 수요가 증가하게 되고 이로 인하여 해상 선박 간 충돌사고가 빈번히 발생하게 되었다 선박 충돌은 주로 조선하는 사람들의 관습, 습관의 차이, 부주의, 판단오류 등의 이유로 발생한다. 연구자들은 선박 충돌을 방지하기 위하여 조선에 관련된 많은 부분을 지능화한 지능형 충돌회피시스템 개발에 노력을 기울이고 있다. 선박을 비롯한 자율운동체의 충돌방지 기법은 비행체, 수중운동체, 자율로봇 등 영역 특성을 달리하는 다양한 분야에서 연구되어오고 있다 기존 연구들의 충돌방지는 주로 장애물의 공간적 특성에 기반하고 있다. 이에 개체의 움직임을 예측하여 시간적 요소를 가미하면 더욱 향상된 충돌방지가 가능하다. 특히, 선박은 느린 운동 특성과 조선법, 규격화된 통신수단의 발달로 인하여 상대편 선박의 이동 예측이 용이하므로 이를 적용하여 보다 향상된 충돌방지가 가능하다. 본 연구에서는 기존의 충돌회피기법의 과정에 예측을 추가한 예측기반 충돌회피모형을 제안하고 선박운항환경을 모의실험에 의하여 해당 모형 적용시 충돌회피 경로 산출의 안전성이 크게 개선됨을 보인다.

The Effects of Near Miss and Accident Prevention Activities and the Culture of Patient Safety Management for the Patient Safety (Near Miss 사고 예방 활동과 환자안전관리 문화형성이 환자안전에 미치는 영향)

  • Purpose: Despite the rapidly changing healthcare environment, healthcare organizations have recognized the importance of patient safety management. But patient safety management has the problem of the lack of participation of members due to the process of focusing on the follow-up service and punishment. The department of nuclear medicine in Uijeongbu St. Mary's Hospital started this research to reduce the near miss and prevent patient safety accidents by both initiating the participatory near-miss-proof activities as an advance management and constructing a system without disadvantages of reporting. In addition, this research aims to establish a differentiated patient safety management system in the department of nuclear medicine. Materials and Methods: 1. Colleting cases of team members' past and present near miss and accidents(First data collection). 2. Quantifying the cases of near miss and accidents after identifying the degree of importance and urgency through surveys(Second data collection). 3. Quantifying cases and indentifying important points of contact through data analysis. 4. Making and standardizing a manual for important points of contact, and initiating participatory activities to prevent errors. 5. Activating web-based community for establishing the report system of near miss. 6. Estimating the result of before and after activities through surveys and focus group interviews. Results: 1) Quantified safety accidents and near miss in the department of nuclear medicine. About 50 near misses a month and one safety accident a year. 2) Establishing improvement measurements based on quantified data. About 11 participatory activities, the improvement of process, a manual for standardization. 3) Creating a system of safety culture and high participation rate of team members. Constructing a report system, making a check list and a slogan for safety culture, and establishing assessment index. 4) Activating communities for sharing the information of cases of near misses and accidents. 5) As the result of activities, the rate of near miss occurrence declined by 50% and the safety accident did not happen. Conclusion: The best service in the department of nuclear medicine is to provide patients with safety-guaranteed high-quality examination and cure. This research started from the question, 'what is the most faithful-to-the-basics way to provide the best service for patients?' and team members' common answer for this question was building a system with participation of all members. Building a system through the participatory improvement activities for preventing near miss and creating safety culture resulted in the 50% decline of near miss occurrence and no accident. This is a meaningful result from the perspective of advance management for patient safety. Moreover, this research paved the way for creating a culture to report and admit near miss or accidents by establishing a report system with no disadvantage of reporting. The system which sticks to the basics is the best service for patients and will form a patient safety culture system, which will lead to the customer satisfaction. Therefore, all members of the department of nuclear medicine will develop a differentiated patient safety culture with stabilizing the established system.

Study of the Introduction on the Aviation Safety Data Protection System (항공안전데이터 보호제도 도입 방안 연구)

  • To promote the aviation safety reporting system that is operated to enhance aviation safety and to utilize related information, it should first be preceded by standards for non-punishment and data protection. It is because the purpose of collection and analysis of aviation safety related data through the aviation safety reporting system is to prevent recurrence of accidents by investigating their causes through collection and analysis of diverse types of information related to aviation safety. Both mandatory and voluntary reporting systems are in operation for aviation safety under the current Aviation Safety Act. It is said that they were introduced to survey causes for accidents and to prevent recurrences. In fact, however, it is hard to expect active implementation of the reporting system for aviation safety unless the reporters are firstly exempted from punishment. Therefore, the system should be improved so that it can satisfy its purpose and the purposes of data collection concerning aviation safety through examination of the purposes of the reporting system. One of the matters that needs to be considered to promote the reporting system should be the scope of aviation safety hindrances presupposed under the current institution. The voluntary aviation safety reporting system differs from the systems of ICAO or the key advanced countries, including the USA and the UK as it limits the target accidents subject to reporting to minor aviation safety hindrances only. That being said, improvements should be made by requiring mandatory reporting of aviation safety hindrances based on their severity while recognizing a greater variety of aviation safety concerns like international standards. Safety actions and sharing of information based on collection and analysis of diverse data related to aviation safety will greatly contribute to enhance aviation safety as the purposes of the reporting system are to explore causes for accidents and to prevent their recurrences. What is most important in this regard is strict data protection and non-punishment principles; compliance with them should be secured. We can hardly expect the successful operation of the system unless the reporter is exempted from punishment and the relevant data is protected as promotion of voluntary reporting is an essential factor for enhancing the safety culture. Otherwise, the current system may induce hiding of relevant facts or data to evade punishment. It is true that the regulation for enhancing safety tends to have limitations or blind spots; nevertheless, it should still be enforced strictly and completely. Technological progresses and mistakes of operators appear in different forms based on individual cases. The consequential damages may amount to a truly severe level. Therefore, we have studied and suggested to the methods of activiation and amendments on the aviation safety reporting system, which is referred for one of the proactive safety management systems. The proposed improvement of the reporting system and introduction of non-punishment for collection of aviation safety data for deploying a preemptive prevention system would serve as the backbone for enhancing aviation safety in Korea.

A Suitability Study and Development of Low Strength Perlite Concrete as Aircraft Rapid Arresting System (항공기 과주방지 포장시스템에 적합한 저강도 펄라이트 콘크리트의 개발 및 적합성 연구)

  • More than 10 different cases of airline overrun accidents happened annually home and abroad in recent years. So the government put the guidelines to protect that kinds of accidents, which is named 'Runway End Safety Area'. However, the great part of airports are far from the standards, because most of the airports have been built before the guidelines. Moreover, in many cases natural obstacles, ambiance, and local area developments obstruct the extension of the runway to meet the criteria. For these reasons, the Federal Aviation Administration (FAA) recommends that the aviation fields construct 'Aircraft Rapid Arresting System(ARAS)' at the end of the runway. Many airdromes have been constructing the system and some airports have already completed the construction. In this research, our team performed a basic study about low strength perlite concrete to provide the proper material with 'ARAS'. As a result, the unit weight of the low strength perlite concrete was $4.5{\sim}6.4kN/m^3$ and uniaxial compressive strength was measured in the range of $400{\sim}1,470kN/m^2$. In addition, we tested penetration compressive strength by using CBR tester, and we observed that the strength was increased after around 60% of penetration rate. Also, 40% of penetration rate was measured through the penetration test with dump trucks.
