Full-mouth rehabilitation of a patient with severe tooth wear using a gothic arch tracer and stabilization splint. (비기능적 습관에 의한 전반적인 마모 환자의 고딕아치 기록장치 및 교합안정장치를 통한 완전 구강 회복 증례)

  • A patient with para-functional habits can cause widespread teeth wear, along with temporomandibular joint disorders and myofascial pain syndrome in the masticatory muscles. Prolonged teeth wear is associated with a decrease in vertical occlusal dimension, leading to issues such as changes in facial morphology, decreased masticatory efficiency, and temporomandibular joint disorders. To achieve a three-dimensional full-mouth rehabilitation in patients with decreased vertical occlusal dimension, accurate diagnosis, analysis, and proactive treatment planning are essential. Prosthetic treatment accompanied by the restoration of physiological vertical occlusal dimension and the re-establishment of a normal occlusal plane is necessary. This case report presents a full-mouth rehabilitation case involving a patient with overall teeth wear, showing decreased vertical occlusal dimension which results in discomfort in the temporomandibular joint and aesthetic dissatisfaction. The report highlights the successful outcome achieved through the use of occlusal stabilization splint for temporomandibular joint stability and Gothic arch tracing devices for the re-establishment of intermaxillary relationships. Also, through adjustments and adaptation assessment using provisional prostheses, favorable outcomes were achieved both functionally and aesthetically.

  • When wearing complete dentures, patients want to function naturally within a physiologically stable range. To do this, recovery of esthetics, biologically stable arrangement and contour, and occlusal contacts with denture stability are necessary. In this case report, a complete denture patient of adverse conditions was presented. To increase stability of the dentures, functional impression was made by border molding using the neutral zone. The dentures were checked for physiological centric relations and stable occlusion. The clinical results showed satisfactory results on function and esthetics.

  • Ectodermal dysplasia is a hereditary disease characterized by a congenital dysplasia of one or mote ectodermal structures. Characteristic manifestations Include scanty hair and eyebrows, pigmented and hyperkeratinized skin around the eyes and mouth, frontal bossing with prominent supraorbital ridges, nasal bridge depression and dental anomalies. Hyperthermia or unexplained high fever as a result of the deficiency of sweat glands is common medical history. Findings of intraoral structures are anodontia or oligodontia with conical crowns. Consequently, generalized spacing and loss of vertical dimension of occlusion. Interdisciplinary approach has been performed to treat a 10-year old boy with ectodermal dysplasia. Orthodontists and a prosthodontist worked together on this case, and the result was satisfactory.

Soft tissue cephalometric analysis of Aesthetic Korean female (심미적 측모에 관한 두부계측방사선학적 연구)

  • The purpose of this study was to provide data on the normative values of some clinically important soft tissue dimensions for adult Korean females with aesthetically beautiful facial profiles. Lateral cephalograms of 18 Korean female models, who were selected for their well balanced and aesthetic facial profiles, were evaluated. All cephalograms were taken with the subjects in a natural head position with the teeth in occlusion and the lips at rest. The means and standard deviations were determined and presented. In addition, comparisons with the previous studies were performed The results of the present study were as follows: 1. The upper and lower lips were posteriorly located in relation to the Ricketts' E line (Upper lip to E line: -2.08, Lower lip to E line: -0.04). 2. Both lips were more posteriorly located than those in the results of previous studies on Korean females selected by normal occlusion, but more anteriorly located than in the results of studies selected on an aesthetic basis. 3. The nasolabial angle for this sample was 101.03 degrees with a standard deviation of 8.47 degrees.

A Cephalometric Analysis on Esthetic Facial Soft Tissue of Korean Young Adult Female (한국인 젊은 여성의 심미적인 안면 연조직 형태에 관한 두부 X-선 계측학적 연구)

  • Cephalometric radiographs, frontal photographs and profile silhouette phogographs of 68 young adult female who were model or were recommended to have esthetic face were used in this study. 7 Students in department of Art of Kyungpook national university and 15 orthodontists estimated profile slides which were made of 3 Profile silhouettes in parallel with FH plane. Profile silhouettes were made of soft tissue profile line of cephalometric radiograph. Only orthodontists estimated frontal photographs. Students and orthodontists score 9 in excellent case, score 7 in good case, score 5 in average case, score 3 in poor case. Correlation analysis between orthodontists' esthetic concept and Artists' esthetic concept, between frontal view esthetics and profile view esthetics which estimated by orthotontists, between profile view esthetics and profile measurements which consisted of measurements of 38 female who were scored above 5 mean score in profile silhouette by orthodontists were done. And the finding in this study indicated the following 1. Correlation between orthodontists' esthetic concept and Artists' esthetic concept in profile silhouette was significant (r=0.67,P=0.0001). 2. Correlation between frontal view esthetics and profile view esthetics which estimated by orthodontist was significant (r=0.26,P=0.0381). 3. Measurements which had significant correlation between profile measurements and profile view esthetics wer Na-Pog, to N', BNV to Pog', BNV/B' -Pog', Ls-Li-Pog', Li-B'-Pog' Z angle(P<0.05). 4 Mean and standard deviation of profile measurements of 38 female were obtained.

  • Fall is the most common cause of trauma in the elderly and a major reason of dental injury. Maxillary anterior region is the most vulnerable area to the traumatic fall. Loss of teeth and adjacent tissue in this area results in maxillofacial defects and inappropriate maxillomandibular relationship. Reconstruction of the loss requires comprehensive and careful planning to fulfill not only functional but also esthetic demand. Prosthetic approach can avoid surgically extensive intervention, reducing both treatment cost and time. This clinical report describes a 78-year-old woman with the loss of teeth and alveolar bone in maxillary anterior region due to a traumatic fall, rehabilitated with a metal-ceramic fixed dental prosthesis using gingival porcelain.

A study on the characteristics of attractive profiles of Korean young women to orthodontists (교정의사가 선호하는 측모의 유형에 따른 특징적 양상에 관한 연구)

  • The perception of facial esthetics is critically important to orthodontists. A viewpoint to facial esthetics is influenced by various factors and dependent on the perception of observer. The purpose of this study was to examine the differences regarding esthetic viewpoints among orthodontists, to identify attractive profiles preferred to orthodontists and to present the characteristic aspects of attractive profiles upon the degree of facial convexity. 35 persons whose faces were judged as attractive one by S orthodontists were selected out of 133 young Korean women. Soft tissue profiles Identified as a good-profile group were measured and analyzed. And then according to the facial convexity, good-profile group was subdivided to convex (G-Sn-Pg$9^{\circ}$) and straight (G-Sn-Pg<$9^{\circ}$) groups for the purpose of this study. There were statistically no significant differences regarding esthetic viewpoints among S orthodontists(p<0.05), even if there exists prevailing concept that the standard for facial esthetics is substantially subjective. The frontal characteristics of esthetic lips and lips after anterior segmental osteotomy in Korean females (정면에서 평가한 한국인 여성 입술의 심미성과 전방 분절 골절단술 후 입술의 평가)

  • The aim of this study was to determine firstly the characteristics of esthetic lips in Korean females and secondly to measure the changes of the lips before and after anterior segmental osteotomy in bimaxillary protruded patients. Methods: Samples consisted of 30 models and 26 nonmodels, and 10 patients who had received anterior segmental osteotomy. Twelve linear measurements, 5 angular measurements, and the lip perimeter and area were measured. These results were compared for each group using unpaired and paired t-tests. Results: Full face width, nose to chin, upper vermilion height and angle, lateral heights at the point of the tips of Cupid's bow, central bow angle, and the lower lip to chin lengths were significantly greater in nonmodels than in models. However, overall lip width, lower vermilion height and angle, tip-to-tip of Cupid's bow lip perimeter, and lower vermilion area were greater in models than in nonmodels. Comparison of before and after anterior segmental osteotomy revealed that the values for the upper vermilion and lateral heights, the angle to Cupid's bow tip lengths, and upper vermilion area of post-operative patients had become similar to those of Korean female models. Our findings demonstrated that Korean female models have a fuller lower vermilion & thinner upper vermilion compared with nonmodel controls, as determined by the vermilion heights, angles, and areas. Conclusions: It will be helpful for clinicians to use these measurements as guidelines for improving patients' facial esthetics.

  • Defects due to mandibulectomy often cause hard and soft tissue loss and result in esthetic problems and functional disorders such as mastication, swallowing, and pronunciation. After the mandibular reconstruction, several complications including loss of alveolar bone can cause limitations in maintenance or supporting of removable prosthesis. For these patients, implant-supported fixed restorations have been an appropriate prosthetic restorative method. In this case report, we report the patient who underwent mandibulectomy and mandibular reconstruction owing to oral cancer, and then restored the current dentition functionally and aesthetically by applying zirconia frameworks and monolithic zirconia crowns by computer-aided design and computer-aided manufacturing.

Analysis of esthetic factors and evaluation of esthetic perception for maxillary anteriors of dental students (치과대학생의 상악 전치부의 심미적 요소 분석 및 인식도 평가)

  • Purpose: This research is to investigate factors that affect dental aesthetics and analyze whether individual perception affects and difference of genders affects the esthetic factors. Materials and methods: Seventy dental students of Chonnam University aged from 25 to 35 years old without periodontal problems were included. Maxillary dental cast for participants were made, and standardized photo was taken with a digital camera. Maximum visual width and the position of gingival zenith of maxillary anterior teeth and maximum height of the maxillary central incisors were measured by a measurement program and the shape of central incisor was categorized. Questionnaire was conducted to evaluate esthetic perception. SPSS program was used to perform statistical analysis. Results: The average visual width ratio of right maxillary anteriors was 1.38:1:0.78 and 1.41:1:0.81 for the left which differ from the golden ratio. The width to height ratio for right and left central incisor was 0.84 and 0.83 respectively. The gingival zenith position was determined to be: mostly, central incisors and canines were located in the distal side, lateral incisors were located in the center. The visual width ratio of right maxillary anterior teeth, ratio of width to height of central incisor, gingival zenith position, crown morphology and amounts of gingival exposure upon smiling were not significantly different between genders, and facial patterns and these dental esthetic factors were not related to aesthetic perception. Conclusion: Even though participants had the visual width ratio of maxillary anteriors or ideal width to ideal height of central incisors that did not agree with the golden ratio, they had high satisfaction for dental esthetics. Esthetic perception depends more on subjective judgements of participants than objective indices.
