  • Sella turcica contains pituitary gland that has influence on craniofacial growth. So, if the volume of sella turcica correlate to the function of Pituitary gland, we can assume that the volume of sella turcica in skeletal Class III patients has some difference to that of normal occlusion group. The purpose of this study was to evaluate the difference of shape and volume of sella turcica between normal occlusion group and Class III patients. The shape of sella turcica was Classified by Inaba method and the volume of sella turcica was measured in lateral and P-A cephalograms by Di Chiro method. To find out the possibility of the volume of sella turcica as diagnostic aid to predict Class III growth pattern, the correlation coefficients between the volume of sella and cephalometric variables were calculated. The results were as follows. 1. The volume of sella turcica in Class III patients is larger than that of normal occlusion groups 2. The volume of sella turcica in female was larger than that of male in Class III patients 3. The volume of sella turcica has close correlation with APDI, ANB, SNA, SNB, ODI, gonial angle, post. cranial base length 4. Sella Index (volume of sella / ant. cranial base length) can be a more accurate indicator that represent Class III growth pattern than volume of sella itself. 5. The morphologic pattern of sella turcica had no significant difference between two groups.

  • Russell-Silver syndrome is a type of intrauterine growth retardation, characterized by short stature noted at birth, hemiatrophy or asymmetry, variation in sexual development and other abnormalities, including cafe-aulait pigmentation and clinodactyly. Facial features commonly associated with this syndrome are a small triangular face, decreased facial height, down-turned corners of the mouth(shark's mouth), a small mandible, and occasionally asymmetry. The major intra-oral features of the syndrome that have been reported are a high-arched palate, delayed tooth eruption, microdontia, hypodontia, and crowding. These cases were diagnosed at birth as Russell-Silver syndrome by clinical features such as prenatal growth retardation, short stature, low body weight, et al., and have been treated with growth hormone. The purpose of this paper is to report the dental findings of two patients and review the pertinent literature through the two cases.

  • The aim of this study was to evaluate the morphology of the mandibular symphysis and anterior alveolar and skeletal relationship under the influence of the rotational growth pattern of mandible in skeletal Class III malocclusion. A total of U untreated adult subjects were divided into two groups-forward rotational growth pattern group, backward rotational growth pattern group-according to the suggestion of Skieller et al.. The antero-posterior position, vertical relationship, mandibular symphysis and anterior alveolar and skeletal relationship were assessed on lateral cephalometric radiographs. Mandibular symphysis and anterior alveolar and skeletal relationship in each subject were studied and the following conclusions were drawn : 1. Concerning the antero-posterior position, forward rotational growth pattern group showed significantly larger SNA, SNB. Conceming the vertical relationship, all measurements showed statistically significant differences. 2. Forward rotatioal growth pattern group showed significantly larger IMPA, MnAD, backward rotational growth pattern group showed significantly larger MxABH. 3. There was no statistically significant difference in symphysis ratio to mandibular plane between forward and backward rotational growth pattern group. 4. In the correlative analysis of rotational growth pattern of mandible and mandibular symphysis, anterior alveolar and skeletal relationship, statistically significant correlations in overbite, IMPA, MnAD, symphysis width were showed.

  • The purpose of this study was to analyze the growth changes of ODI and APDI with age on the three facial growth patterns. The biennial serial cephalometric radiographs of 19 male and 14 famale samples with normal occlusion during 10 years were used in this study. The samples were divided into three groups - drop type, neutral type, forward type - by the total change of the Y-axis during the periods of the study. The findings in this investigation indicated the following: 1. The mean values and standard deviations of each age group in each facial type of male and famale were obtained. 2. The difference of ODI and APDI among the 3 facial types was not significant through all observed ages(P)0.05). The size of ODI appeared large consistently in order of the drop, neutral, and forward type in both male and female through all observed ages. 3. The ODI and APDI were maintained without any charges with age during the periods of the study(p>0.05). 4. In correlation analysis between the total change of the Y-axis and 6 measurements, the AB plane angle and facial angle showed correlation in both male and female(p<0.05), the APDl in only female(p<0.001), but the ODI not appeared correlation in either male or female(p>0.05).

  • A 14-year-old male patient was referred to us for evaluation of Right TMJ clicking sound and mandibular deviation during mouth opening. On examinaaation, protrusion and lateral excursion was also restricted. Radiographs showed hook-like curved elongation of Right coronoid process with limitation o right condylar translation. His past medical and dental history was noncontributory. This patient underwent an extraoral coronoidectomy and the specimen revealed coronoid process malformation mimicking condylar head. Microcoically, the specimen discloses mature bony trabeculae. At postoperative 5 days, physical therapy was instituted. At present, this patient could open his mouth well with any clicking sound and mandibular deviation. The asymmetric face of this patient is expectated to be corrected by periodic observation and remained growth porential. If not to be corrected, orthognathic surgery will be done.

  • Treacher Collins syndrome is inherited as an autosomal dominant trait with variable penetrance. It shows a marked variability even in the same family. This syndrome is developmental defect affecting the branchial arches. It is not usually associated with acute respiratory distress, but has symptoms of microtia, hypoplastic zygomatic bones, hypoplastic mandibular rami, and bilateral coloboma. It usually requires an emergency operation immediately after the birth. We experienced an infant with Treacher Collins syndrome who showed retrognathia, glossoptosis, microtia, and cleft palate. Intermittent cyanosis, depression of the chest, respiratory difficulty associated with airway obstruction, and swallowing difficulty were also observed. To relieve severe upper airway obstruction caused by retrognathia and glossoptosis, we simultaneously performed tongue-lip adhesion and subperiosteal release of the floor of the mouth. The respiratory and swallowing difficulties were relieved and the tongue repositioned anteriorly. We report the present case with a review of the literature.

  • Condylar hyperplasia is a pathologic condition showing 3-dimensional skeletal hyperplasia of the mandible. The reason for condylar hyperplasia is not yet known, but the effects of hormone, trauma, infection, genetics, fetal condition, and hypervascularity are known as possible reasons. When we diagnose a patient as having condylar hyperplasia, it is important to decide if it is in progress or not. Treatment for facial asymmetry due to condylar hyperplasia are decided accordingly, including condylectomy, that is removal of growth site of the affected condyle, and conventional orthognathic surgery only or condylectomy with orthognathic surgery after the completion of growth. Therefore, it is important to determine the growth state of condylar hyperplasia in treatment stability. This is verified through bone scan and regular check-ups with 3D CT or PA cephalogram. This case report introduces an improved case of facial asymmetry with condylectomy together with orthognathic surgery.

  • True midfacial deficiency is defined as a hypoplasia of various components of midface such as maxilla, orbit, zygoma and nasal bone. For treatment of these anomalies Le Fort III osteotomy and its modifications have been used traditionally. Le Fort III osteotomy is the method which advances maxilla with nasal bone and zygomatic bone at a time. At first midfacial osteotomy was introduced by Gillies to treatment of dentofacial deformity in 1950. In 1967 Tessier designed Le Fort III osteotomy according to Le Fort III midfacial fracture line and popularized to treat midfacial deficiency using coronal incision to appoach osteotomy sites. This is a case of patient who had mandibular prognathism with midfacial deficiency with severe discrepancy in maxillomandibular interrelation. First we performed Le Fort III osteomomy for zygomaticomaxillary advancement, and then carried out simultaneous two jaw surgery with Le Fort I osteotomy and BSSRO three months after first surgery.
