Stiffness and Elasticity of the Masticatory and Facial Expression Muscles in Patients with the Masticatory Muscle Pain (저작근통 환자에서 저작근 및 안면표정근의 경도와 탄성도 평가)

  • This study aimed to assess stiffness and elasticity of the masticatory muscle in the patients with the masticatory muscle pain using a tactile sensor and to investigate whether the masticatory muscle pain affects the facial expression muscles. From those who visited Department of Oral Medicine in Dankook University Dental Hospital, 27 patients presenting with unilateral muscle pain and tenderness in the masseter muscle (Ms) were selected (mean age: $36.4{\pm}13.8$ years). Exclusion criterion was those who also had temporomandibular joint (TMJ) disorders or any neurological pain. Muscle stiffness and elasticity for the muscles of mastication and facial expression was investigated with the tactile sensor (Venustron, Axiom Co., JAPAN) and the muscles measured were the Ms, anterior temporal muscle (Ta), frontalis (Fr), inferior orbicularis oculi (Ooci), zygomaticus major (Zm), superior and inferior orbicularis oris (Oors, Oori) and mentalis (Mn). t-tests was used to compare side difference in muscle stiffness and elasticity. Side differences were also compared between diagnostic groups (local muscle soreness (LMS) vs myofascial pain syndrome (MPS) and between acute (< 6M) and chronic ($\geq$ 6M) groups. This study showed that Ms and Zm at affected side exhibited significantly increased stiffness and decreased elasticity as compared to the unaffected side.(p<0.05) There was no significant difference between local muscle soreness and myofascial pain syndrome groups and between acute and chronic groups. The results of this study suggests that masticatory muscle pain in Ms can affect muscle stiffness and elasticity not only for Ms but also for Zm, the facial expression muscle.

Effects of Red or Black Ginseng Extract in a Rat Model of Inflammatory Temporomandibular Joint Pain (흰 쥐의 턱관절 염증성 통증모델에서 홍삼 및 흑삼추출물의 효과)

  • Temporomandibular joint (TMJ) pain is characterized by persistent jaw pain associated with dysfunction and tenderness of the temporomandibular muscles and joints. The aim of this study was to investigate whether treatment with red or black ginseng extract helps in the modulation of inflammatory TMJ pain. Male Sprague-Dawley rats weighing 220~260 g were used. The experimental group was subdivided into 4 groups based on the treatment method (n=6, each group): formalin (5%, $30{\mu}l$), formalin after distilled water (vehicle), formalin after red or black ginseng extract (per oral, single or repeated, respectively). To induce TMJ pain, $30{\mu}l$ of formalin was injected into the articular cavity under ether inhalation anesthesia. The number of noxious behavioral responses of scratching the facial region proximal to the injection site was recorded for 9 successive 5-min intervals following formalin injection. Repeated treatment with red or black ginseng extract reduced the nociceptive responses in the second phase (11~45 min). Nuclear factor erythroid 2-related factor 2 (Nrf2) is an oxidative stress-mediated transcription factor. Both ginsengs significantly down-regulated the increased Nrf2 level compared to the vehicle group. In the test for liver and kidney functions, repeated treatment with red or black ginseng was not different compared to the vehicle group. These results indicate that red and black ginseng extract might be promising analgesic agents in the treatment of inflammatory TMJ pain.

The Effects of Transcutaneous Electrical Nerve Stimulation and Electroacupuncture Stimulation Therapy on the Current Perception Threshold of Orofacial Region (구강안면영역에서의 경피성 신경자극과 전기침자극요법이 전류인지역치에 미치는 영향)

  • 구강안면동통 환자의 치료법으로 널리 쓰이는 전기요법은 연조직과 신경계 구조물에 대한 치료 시 중요한 역할을 담당하고 있다. 저자는 현재 구강안면동통의 치료법으로 널리 쓰이고 있는 전기요법들 중 경피성 신경자극(Transcutaneous electric nerve stimulation)과 전기침자극요법(Electroacupuncture stimulation therapy)이 각각의 신경섬유에 미치는 효과를 평가하고자 정상 성인 남녀 29명에게 경피성 신경자극 및 전기침자극을 시행하고 시행 전 및 시행 후 삼차신경 영역의 3가지 종류($A{\beta}$, $A{\delta}$, C fiber)의 신경섬유의 전류인지역치(CPT) 변화를 측정하여 그 차이점을 분석하였으며 이를 대조군과 비교하였다. 경피성 신경자극 및 전기침자극 모두에서 대조군에 비해 삼차신경 영역의 모든 신경섬유에 걸쳐 고른 전류인지역치의 증가를 나타내었으며, 경피성 신경자극과 전기침자극 후의 전류인지역치 변화량은 서로 유의할만한 차이를 나타내지 않았다. 이는 경피성 신경자극 과 전기침자극 모두 3가지 종류($A{\beta}$, $A{\delta}$, C fiber)의 감각신경섬유의 전류인지역치에 영향을 미치며, 구강안면동통의 감소에 효과적으로 사용될 수 있으리라 생각된다.

  • A Case of pemphigus bulgaris in buccal mucosa of 48 years old Korean female was reported. Final diagnosis was determined by evaluation of clinical and histopathological finding. The patient could be treated successfully by administration of prednisolone, injection of bethametasone, application of oramedy and irradiation of soft laser.

Effect of Masticatory Muscle Pain Control by Morphine (Morphine에 의한 저작근 통증의 조절 효과)

  • This study was designed to evaluate the pain control effect by morphine injection to masticatory muscle pain patients. Patients with masticatory muscle pain visited the Department of Oral Medicine, Kyung Hee University Dental Hospital were recruited to this study and diagnosed by RDC/TMD. Experimental group were divided into three group; saline injection group(n=10), lidocaine injection group(n=10) and morphine injection group(n=10). Evaluation list was the subjective pain evaluation(visual analogue scale, Mc Gill pain questionnaire, pain drawing) and the objective pain evaluation(pressure pain threshold, pressure pain tolerance) and evaluation time was injection before, after 10min, 30min, 60min and then it was analyzed statistically. The results were as follows : 1. The subjective pain evaluation and the objective pain evaluation were significantly different statistically in within subject effects(p<0.001). 2. The subjective pain drawing evaluation(p<0.001) were significantly different statistically in between subject effects. 3. The objective pressure pain threshold evaluation(p=0.025) were significantly different statistically in between subject effects. 4. The morphine injection group(p=0.001) were more significantly different than the saline injection group statistically in the subject pain drawing evaluation. Therefore, it was considered that the morphine injection was effective to pain control for masticatory muscle pain patients within 60 minute.

The Effect of topical EMLA Cream for venipuncture on Patients' Pain and Anxiety (엠라크림이 정맥천자 시 통증과 불안에 미치는 영향)

  • The purpose of this study was to examine the effect of topical EMLA Cream on venipuncture related pain and anxiety of surgery patient from oct. 27 to nov, 13, 2011, EMLA Cream was applied to the experimental group(n=40) 1hour before the procedure, but was not applied to the control group (n=37). The collected data were analyzed with the SPSS WIN 15.0 program. The percentage, mean and standard deviation were figured statistic, ${\chi}^2$-test, t-test, ANOVA, Correlation analysis were performed. Consequently, Pain in the EMLA applied experimental group was significantly lower than that of the control group. also state anxiety in the experimental group was lower than that of the control group. correlation of pain and anxiety after EMLA was related. EMLA cream was considered to be an effective local anesthetic for preoperative venipuncture on pain and anxiety.

Effects of Laughter Therapy on Recovery Status from Facial Paralysis, Pain and Stress in Bell's palsy Patients (웃음치료가 구안와사환자의 안면마비 회복정도, 통증 및 스트레스에 미치는 효과)

  • Purpose: This study was done to identify the effects of Laughter therapy on recovery status from facial paralysis, pain, and stress in Bell's palsy patients. Methods: This research was a nonequivalent control group non-synchronized pre-posttest quasi-experimental research design. Data were collected from August 8, to October 15, 2013 at D university Oriental Medical Center in B city. Participants were 60 patients with Bell's palsy who were assigned to one of two groups: an experimental group of 30 patients and a control group of 30 patients. t-test, $x^2-test$, Fisher's exact test and Welth-Aspin test were used to test the homogeneity between the two groups and t-test, Welth-Aspin test and ANCOVA were used to test the research hypotheses. SPSS program was used for statistical analysis. Results: There were significant increases in recovery status from facial paralysis and decreases in stress after Laughter therapy in the experimental group. Conclusion: The results indicate that Laughter therapy is effective in facilitating recovery status from facial paralysis and decreasing stress. Therefore, Laughter therapy can be used as an effective nursing intervention for patients with facial paralysis and to relieve stress in these patients.
