Power Spectral Analysis in Anterior Temporal and Masseter Muscles at Sustained Submaximal Isometric Contraction Close to Chewing Force Level (저작 수준의 등척성 수축시 전측두근 및 교근의 근전도 스팩트럼 분석에 관한 연구)

  • 저자는 저작 수준의 등척성 수축시 하악 폐구근에서의 근전도 스펙트럼의 특성을 규명하기 위하여, 정상 성인 25명을 대상으로 최대수의적 수축의 40% 수준으로 근활성을 2분간 유지하게 하고, 이때 전측두근과 교근에서 얻어진 근전기 신호를 컴퓨터를 이용한 근전도 시스템을 이용하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 저작 수준의 등척성 수축 지속시 시작과 마지막 구간 모두에서 전측두근과 교근의 평균중간주파수와 중간주파수 기울기는 성별 및 연령의 영향을 받지 않았다. 2. 저작 수준의 등척성 수축 지속시 시작과 마지막 구간 모두에서 전측두근과 교근의 평균중간주파수와 중간주파수 기울기는 좌우사이에 통계적으로 유의한 차이가 없었다. 3. 저작 수준의 등척성 수축 지속시 전측두근과 교근의 마지막 구간 평균중간주파수는 시작 구간 평균 중간주파수보다 통계학적으로 유의하게 낮았다.(전측두근 : p<0.01, 교근 : p<0.01). 4. 전측두근과 교근에서 저작 수준의 등척성 수축이 지속됨에 따라 시작 구간과 마지막 구간 모두에서 power spectrum은 저주파 영역으로 이동하였으나, 중간주파수 기울기는 시작 구간과 마지막 구간 사이에 통계적으로 유의한 차이가 없었다.

  • 안면 특정 근육의 움직임으로부터 기록되는 Electronic Sensing System(ESS)신호는 인간의 관능검사에 사용할 수 있다. 본 연구는 ESS를 이용하여 저작패턴에 따라 나타나는 결과들을 분석, 분류하고 각 그룹별로 같은 식품에 대한 관능 감지도의 차이를 검사하기 위하여 각 그룹별 씹는 특성을 파악하여 이에 따라 패널요원들을 선발하였다. 패널들을 젊은 남$.$여 및 노년 남$.$여 총 4그룹으로 구분하여 껌 및 당근의 저작과정에 대한 패턴 즉 속도, 시간 및 총 에너지를 비교한 결과, 젊은 남$.$여 그룹(n=47, 18∼42세, 남성 27명, 여성 20명) 및 노년 남$.$여 그룹((n=44, 65세 이상, 남성 33명, 여성 31명)은 저작 특성들이 서로 달라 같은 식품에 대하여 서로 관능감지도가 달라질 수 있음을 나타내었다. 또한 ESS에 의한 각 패널들의 검사치를 분석한 결과 껌을 씹을 때의 에너지에 대한 표준편차는 2.0 이하(n=71)그리고 씹는 속도의 경우는 0.1 이하로 반복실험의 재현성이 있었다. 91명의 패널들을 저작 에너지 및 씹는 속도에 따라 4그룹(낮은 에너지를 사용하며 천천히 씹는 그룹, 낮은 에너지를 사용하면서 빨리 씹는 그룹, 높은 에너지를 사용하며 천천히 씸는 그룹, 그리고 높은 에너지를 사용하며 빨리 씹는 그룹)으로 분류하고 각 그룹별 평균값에 가장 비슷한 양상을 나타내는 6명씩 총 24명의 관능검사 요원들을 선발하였다.

  • The aim of this study is to evaluate the relative signal intensity of TMJ retrodiscal tissue in T2-weighted MRI as diagnostic marker of temporomandibular disorder(TMD). 58 temporomandibular joints from 29 TMD patients (14 men & 15 women) were evaluated. The relative signal intensity of retrodiscal tissue in T2-weighted MRI was referenced to brain gray matter same size of the region of interest(ROI). The collected data was compared to disc positions (normal, anterior disc displacement with reduction, anterior disc displacement without reduction), the presence of joint effusion. The relative signal intensity of retrodiscal tissue was significantly increased when the disc was displaced without reduction. And the relative signal intensity of retrodiscal tissue was significantly increased when joint effusion was present. The results suggest that evaluating the relative signal intensity of TMJ retrodiscal tissue using a T2-weighted MRI is valuable as a non-invasive tool for diagnosing the procession of TMD.

  • Temporomandibular joint disorders (TMD) often show muscular disorder, internal derangement and degenerative change of articular surface of the condyle. It is important to choose adequate diagnostic method for obtaining data for the diagnosis and treatment. Magnetic Resonance Imaging (MRI) has some advantages, because it shows good resolution image for the soft tissue and it is not invasive to the human body. We have performed serial research about the retrodiscal tissue of the TMJ according to the progress of the disease, using T2 weighted image, since 2002. In this study, we applicated resized observation area to compare signal intensity between the retrodiscal tissue of the TMJ and cerebral gray matter, using T2 weighted image.

  • Botulinum toxin type A (BoNT-A) has been applied successfully to treat chronic migraine, dystonia, spasticity and temporomandubular disorders(TMDs) as well as frontal wrinkle and glabella wrinkle. Recently it has been reported that BoNT-A, reversibly blocks presynaptic acetylcholine release, also inhibits the release of substance P, CGRP(calcitonin gene related peptide) and glutamate related to peripheral sensitization and neurogenic inflammation in sensory nerve, In this study we reviewed animal nerve injury model such as rat and rabbit and identify the analgesic effect and mechanism of nerve injury pain after dental treatment.

Feasibility of Pediatric Low-Dose Facial CT Reconstructed with Filtered Back Projection Using Adequate Kernels (필터보정역투영과 적절한 커널을 이용한 소아 저선량 안면 컴퓨터단층촬영의 시행 가능성)

  • Purpose To evaluate the feasibility of pediatric low-dose facial CT reconstructed with filtered back projection (FBP) using adequate kernels. Materials and Methods We retrospectively reviewed the clinical and imaging data of children aged < 10 years who underwent facial CT at our emergency department. The patients were divided into two groups: low-dose CT (LDCT; Group A, n = 73) with a fixed 80-kVp tube potential and automatic tube current modulation (ATCM) and standard-dose CT (SDCT; Group B, n = 40) with a fixed 120-kVp tube potential and ATCM. All images were reconstructed with FBP using bone and soft tissue kernels in Group A and only bone kernel in Group B. The groups were compared in terms of image noise, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR). Two radiologists subjectively scored the overall image quality of bony and soft tissue structures. The CT dose index volume and dose-length product were recorded. Results Image noise was higher in Group A than in Group B in bone kernel images (p < 0.001). Group A using a soft tissue kernel showed the highest SNR and CNR for all soft tissue structures (all p < 0.001). In the qualitative analysis of bony structures, Group A scores were found to be similar to or higher than Group B scores on comparing bone kernel images. In the qualitative analysis of soft tissue structures, there was no significant difference between Group A using a soft tissue kernel and Group B using a bone kernel with a soft tissue window setting (p > 0.05). Group A showed a 76.9% reduction in radiation dose compared to Group B (3.2 ± 0.2 mGy vs. 13.9 ± 1.5 mGy; p < 0.001). Conclusion The addition of a soft tissue kernel image to conventional CT reconstructed with FBP enables the use of pediatric low-dose facial CT protocol while maintaining image quality.

  • We developed an orofacial pains diagnosis/therapy system which can enlarge a therapy effect through a new stimulation. Contrary to its established method, this therapy system(DDTS-1) is a treating method which absorbs a surplus ion charge- the cause of an outbreak of a pain-of the human body using a capacitor. Absorbing a surplus ion charge of the human body with effect, it obtained optimal parameters which is control signal to control charging/discharging a capacitor to be 6Hz, and capacitance to be $0.1\~0.33{\mu}F.$ Through clinical demonstration, experimented on among patients-namely dental pain patients, joint disorders patients, and trigeminal neuralgia patients to verification of system. In result, an EAST stimulater had a very low change of abnormal potential against normal potential before/after being placed under medical care. DDTS-1 showed obvious differences that two potentials are one and the same potential or one potential is similar to the other one. DDTS-1 comparing with EAST showed the remedial value of the comparative advantage in all the medical treatment of pains. Therefore, stimulation of DDTS-1 is more effective than the existing electric stimulation. We verified its validity of ion charge absorption in the human body using capacitor which presented the present thesis. That is, we verified theoretical adequacy of control action in a pain, and its efficiency as well as confidence.

A STUDY ON THE CHANGES OF MR SIGNAL INTENSITY OF POSTERIOR ATTACHMENT OF THE TEMPOROMANDIBULAR JOINT (악관절 후방부착부에 있어서 MRI 신호변화에 대한 고찰)

  • Magnetic resonance imaging has been used in the temporomandibular joint(TMJ) primarily to define morphology and positional relationship of associating structures. This report examines signal intensity characteristics of the posterior attachment as they related to the severity of internal derangement. Fifty six joints in 35 patients with a history of TMJ dysfunction were imaged writ MR using $T_1$-weighted spin echo sequence. According to disk position, ability to reduction, and the presence of osteoarthritis, the joints were categorized into three groups. A group 1 was anterior disk displacement with reduction; a group 2 was anterior disk displacement without reduction; a group 3 was anterior disk displacement without reduction and condyle had osteoarthritic change. The control group was determined by the clinical absence of any signs or symptoms of current or past TMJ pain and dysfunction. Calculated the relative value of MR signal intensity in posterior attachment and disk to cerebral cortex of temporal lobe by means of computer program, we have compared them with each groups. The result showed statistically little significant difference of disk signal intensity among each groups. but, signal intensity from posterior attachment in group 2 and 3 were significantly(p<0.05) decreased than control group. this might reflect an fibrosis or hyalinization of posterior attachment, which was part of remodeling process that occurs in disk displacement without reduction. However, this study could not demonstrate histologic confirmation of the decreased signal intensity in the posterior attachment. So, further investigation could be needed to understand the association between them.

IMMUNOHISTOCHEMICAL ASSAYS FOR THE EXPRESSION OF ANGIOGENIC SIGNALING MOLECULES AND MICROVESSEL DENSITY IN ADENOID CYSTIC CARCINOMAS OF HUMAN SALIVARY GLANDS (타액선 선양낭성암종에서 혈관형성 신호전달 물질의 발현과 미세혈관농도에 관한 면역조직화학적 연구)

  • Adeonoid cystic carcinoma (ACC) is one of the most common malignant tumors of salivary glands. It is characterized by a relentless regrowth especially around nerve tissues and a high rate of hematogenous distant metastasis. Clinically most deaths from salivary ACC are caused by delayed lung metastases that are resistant to conventional chemotherapy. So, knowledge of cellular and molecular properties that influence the dissemination of metastatic tumor cells, is important for new treatment strategies of metastatic lesions. We determined expressions of angiogenic signaling molecules microvessel density (MVD) using surgical specimens of human salivary ACC. Protein expressions of vascular endothelial growth factor (VEGF), VEGF receptor (VEGFR)-2, activated VEGFR-2, and human CD31 were assessed in 20 cases of salivary ACC by immunohistochemical staining. Most of the tumors, especially ACC with a tubulocribriform pattern, were positive for antibodies of VEGF, VEGFR-2, and activated VEGFR-2. The overall percentages of the 20 specimens expressing VEGF, VEGFR-2, activated VEGFR-2 were 90, 95, and 95%, respectively. Immunoreactivities of the biomarkers in salivary ACC were higher than those in normal salivary gland. Furthermore, immune-related cells as well as tumor cells expressed VEGF/VEGFR-2. Microvessel density of salivary ACC was higher than that of normal salivary gland (P<0.05). Taken together, angiogenic signaling molecules are actively expressed in salivary ACC. And we suggest that these molecules may have critical role in the hematogenous spread of salivay ACC, which has a propensity for delayed lung metastasis. Therefore, these biomarkers can be molecular targets for therapy of metastasis of salivary ACC.

Cervicofacial Actinomycosis with Orbit Involvement (안구 침범을 동반한 두경부 방선균증)

  • Actinomycosis is caused by filamentous Gram positive anaerobic bacteria from the Actinomycetaceae family, and known as a rare cause of the infection at the eyeball. We report magnetic resonance findings of a 60-year-old Korean man with cervicofacial actinomycosis, including cellulitis in the eye and central nervous system actinomycosis. On orbital magnetic resonance imaging, gadolinium-enhanced T1-weight images showed multiple abnormal enhancing lesions in head and neck including right eye, and some include low signal intensities which considered as abscesses. The lesions was diagnosed as actinomycosis by incisional biopsy, and since then was cured by using antibiotics of penicillin family.
