TREATMENT OF HORIZONTAL AND VERTICAL ROOT FRACTURE IN IMMATURE PERMANENT TEETH - A CASE REPORT (미성숙 치근의 수직 파절과 수평 파절의 치험례)

  • Traumatic injuries in the young peranent dentition are common, but root fractures, defined as fractures in volving dentin, cementum and pulp, are relatively uncommon. Case 1 is a 9-year-old boy who had a horizontal root fracture of his maxillary right central incisor in the apical third. Root canal therapy was performed in coronal segment and calcium hydroxide therapy was initiated. Six months after treatment, a periapical radiograph showed calcific tissue formation and normal root development. 1 year and 3 months later, the canal was permanently obturated with gutta-percha. Case 2 is a 7-year-old girl who had a vertical root fracture of her maxillary right central incisor. Fractured tooth was intentionally extracted atraumatically, and then the separated fragments are bonded with resin cement. the restored tooth was replanted into the original socket. Recalls up to 8 months showed normal mobility and no periapical pathosis. In these cases, we performed conservative treatment. Clinical and radiographic examination showed no pathosis or abnormality of the teeth and periodontal tissue.

  • Tooth impaction is a frequently observed eruption anomaly in pediatric dental practice. Young patients with impacted or unerupted teeth have more prediction for dentigerous cyst formation. Dentigerous cyst presents radiographic features, unilocular or multilocular radioluscency. Cysts occur most frequently in the premolar region except third molar. Dentigerous cysts can grow to a considerable size, and large cysts may be associated with a painless expansion of the bone in the involved area. Extensive lesions may result in facial asymmetry, osseous destruction, root resorption of proximal teeth and displacement of associated tooth. The nature of the causative tooth influences the type of surgical treatment required for the dentigerous cyst. If the cyst is associated with a supernumerary or wisdom tooth, complete enucleation of the cyst along with extraction of tooth may be the first treatment choice. Otherwise, preservation of the associated teeth should be considered to prevent a young patient from psychological and mental trauma because of the loss of tooth. We should consider the degree of tooth displacement, osseous destruction and growth pattern of oromaxillofacial area when planning treatment. Thus a proper and logical treatment planning can help a proper growth and development of oromaxillofacial area and can save the patient from a psychological and mental trauma. This report describes 4 cases of the management of impacted premolars and molars associated with dentigerous cysts in children.

  • It is commonly acknowledged that bone morphogenic protein (BMP-2) functions as a potential osteogenic inducer in bone formation. Recently, several papers reported that bone marrow-derived stem cell (BMSC) from human is not responsive to BMP-2 in comparison to high capacity of BMP-2 in the osteoinduction of stromal cell derived from bone marrow of rodent animals such as rat or mouse. In this study, we characterized BMSC derived from 11 years old donor for the responsiveness to rhBMP-2, dexamethasone (Dex) and 1,25-dihydroxyvitamin D (vitamin D), in order to analyze their function in the early osteogenesis. The effect of over mentioned agents was evaluated by means of assessing alkaline phosphatase (ALP) activity/staining, RT-PCR analysis and von Kossa staining. In addition, we analyzed the meaning of expressed several osteoblastic markers such as alkaline phosphatase, collagen typeI, osteopontin, bone sialoprotein and osteocalcin with relation to either differentiation or mineralization. Only in the presence of Dex, human BMSC could commit osteoblastic differentiation and matrix mineralization, and either BMP-2 or vitamin D treatment was not able to induce. But BMP-2 or Vitamin D showed potential synergy effect with Dex. ALP and bone sialoprotein were clearly expressed in response of Dex treatment compared to weak expression of osteopontin in early osteogenesis. Therefore, we expect that this study will contribute partly to elucidiating early osteogenesis mechanism in human, but variations among bone marrow donors must be considered through further study.

Guided Bone Regeneration Using Mineralized Bone Allograft and Barrier Membrane Derived from Ox Pericardium (골유도재생술 시 비탈회 동종골와 우심막유래 차단막의 임상적 활용)

  • Purpose: This study evaluated the clinical applications of implant placement and guided bone regeneration using a mineralized bone allograft and a barrier membrane derived from ox pericardium Methods: From January 2007 to June 2009, among the patients who received an implant at Chosun University Dental Hospital, patients were selected if they were treated with guided bone regeneration (GBR) with simultaneous implant placement or GBR prior to implant placement. The selected patients were sorted according to the materials and membranes used in GBR, and the implant survival rate was recorded by clinical examination and reviewing the medical records and the radiographs. Each study list was analyzed by SPSS (version 12.0, SPSS Inc., USA) software and the survival rate was verified by Chi-square tests. $P$ values less than 0.05% were deemed significant. Results: 278 implants were placed on a total of 101 patients and 8 implants resulted in failure. Three implants failed among 15 implants with only a mineralized bone allograft. No failure was shown among the 74 implants placed with mineralized bone allograft and a barrier membrane derived from ox pericardium. One group of 4 implant placements showed failure among the 102 implants placed with a mineralized bone allograft and another bone graft material. The group that had a barrier membrane derived from ox pericardium with a mineralized bone allograft or other bone materials showed no implant failure. Three failures were shown among the 21 implants placed with only bone graft and not using a membrane. The group with membranes other than a barrier membrane derived from ox pericardium showed 5 failures among 170 implants. Conclusion: The implant survival rate of the group with GBR using a mineralized bone allograft was 96.3%, which meant there was little difference compared to the groups of another bone graft materials (98.9%). The implant survival rate of the group without a membrane-was 85.7% and it showed a significant difference compared to the group using a barrier membrane derived from ox pericardium (100%) and the group using another membrane (97.1%).

The Effect of Silk Fibroin/Nano-hydroxyapatite/Corn Starch Composite Porous Scaffold on Bone Regeneration in the Rabbit Calvarial Defect Model (가토 두개골 결손 모델에서 실크단백과 나노하이드록시아파타이트, 옥수수 녹말 복합물을 이용한 골 이식재 개발)

  • Purpose: This study evaluated the capability of bone formation with silk fibroin/nano-hydroxyapatite/corn starch composite scaffold as a bone defect replacement matrix when grafted in a calvarial bone defect of rabbits $in$ $vivo$. Methods: Ten New Zealand white rabbits were used for this study and bilateral round-shaped defects were formed in the parietal bone (diameter: 8.0 mm). The silk fibroin 10% nano-hydroxyapatite/30% corn starch/60% composite scaffold was grafted into the right parietal bone (experimental group). The left side (control group) was grafted with a nano-hydroxyapatite (30%)/corn starch (70%) scaffold. The animals were sacrificed at 4 weeks and 8 weeks. A micro-computerized tomography (${\mu}CT$) of each specimen was taken. Subsequently, the specimens were decalcified and stained with Masson's trichrome for histological and histomorphometric analysis. Results: The average ${\mu}CT$ and histomorphometric measures of bone formation were higher in the control group than in the experimental group at 4 weeks and 8 weeks after surgery though not statistically significant ($P$ >0.05). Conclusion: The rabbit calvarial defect was not successfully repaired by silk fibroin/nano-hydroxyapatite/corn starch composite scaffold and may have been due to an inflammatory reaction caused by silk powder. In the future, the development of composite bone graft material based on various components should be performed with caution.

Silk Fibroin and Substance P Combination Graft for the Reconstruction of a Bone Defect (실크 피브로인 지지체와 Substance P를 이용한 골 이식재)

  • Purpose: Substance P is a well known neurotransmitter and has been known to mediate pain. Recently, it has been unveiled that substance P is involved in the recruitment of mesenchymal stem cells to wound sites. The purpose of this study was to exam bone formation when a combination of substance P and silk fibroin was used in a bone defect model. Methods: Twenty rabbits were used and 40 calvarial defects were formed. They were divided as 4 groups (unfilled control, silk only, silk+$10{\mu}g$/ml substance P; Sub10, and silk+$100{\mu}g$/ml substance P; Sub100). All animals were humanely sacrificed 4 or 8 weeks after grafting. The specimens were analyzed by micro-computerized tomography and histological analysis. Results: When compared to the unfilled control to silk only group, there was significant difference in bone mineral density (BMD) and the attenuation coefficient (AC) at 4 weeks ($p$=0.037 and 0.038, respectively). When compared Sub10 group to Sub100 group, there was significant difference in BMD and AC at 8 weeks ($p$=0.004 for all). Residual graft amounts were $52.1{\pm}15.8$%, $15.2{\pm}9.2$% and $9.0{\pm}3.3$% for silk only, Sub10, and Sub100 groups, respectively. When comparing the residual graft amount of silk only to sub10 or sub100, the differences were statistically significant ($p$ <0.001). Conclusion: The silk fibroin scaffold showed higher BMD and AC than the unfilled control. The combination graft with substance P and silk fibroin scaffold showed a faster graft degradation than with a silk fibroin scaffold only.

The Measurement of Vertical Length from the Root Tip of Maxillary Anterior Teeth to Nasal Floor, When Maxillary Anterior Implants Placed (상악전치부 임플란트 식립 시 상악전치부 치근첨에서 비강저까지 치조골의 수직적 길이 측정)

  • Purpose: Length measurement from maxillary anterior teeth root tip to nasal floor is incorrect in panoramic radiography. Comparison of cone beam computed tomography (CBCT) and panoramic radiography should be helpful in determining the vertical length. Methods: Sixty nine patients were taken CBCT and panoramic radiography of the maxillary anterior. First measurement is length of parallel lines from the central incisor and lateral incisor root tip to nasal floor on CBCT. Second measurement is length of perpendicular lines from the central incisor and lateral incisor root tip to nasal floor on CBCT. Third measurement is length from the central incisor and lateral incisor root tip to nasal floor on panoramic radiographys. Results: In the first measurement of the maxillary central incisors, an average of $11.36{\pm}2.49$ mm in males and $10.49{\pm}2.17$ mm in females. In the second measurement of the maxillary central incisors, $10.41{\pm}2.42$ mm in males and $9.18{\pm}2.08$ mm in females. In the third measurement of the maxillary central incisors, $10.71{\pm}2.20$ mm in males and $9.27{\pm}2.10$ mm in females. In the first measurement of the maxillary lateral incisors, an average of $13.40{\pm}2.04$ mm in males and $11.96{\pm}2.20$ mm in females. In the second measurement of the maxillary lateral incisors, $11.25{\pm}1.82$ mm in males and $10.06{\pm}1.68$ mm in females. In the third measurement of the maxillary lateral incisors, $11.47{\pm}2.01$ mm in males and $10.13{\pm}1.70$ mm in females. Conclusion: The vertical length from root tip to nasal floor was longer in male than female (P>0.05). First measurement was longer than third measurement (P<0.05). This is the actual placement of the implant site measured on the vertical length than the length of the panorama means that there are a few more free. The first measurement was longer than 114% over the third measurement.

Osteogenic Response of Human Osteoblasts Derived from Mandible and Maxilla: A Preliminary Study (상, 하악골 유래 조골세포의 골형성 능: 일차 연구)

  • Purpose: Maxilla and mandible have different patterns of cortical and trabecular bone and different bone mineral densities, even though both are components of the jaw bone. However, cellular differences between maxilla- and mandible derived osteoblasts (OBs) have rarely been studied. We hypothesize that maxilla- and mandible-derived OBs show different responses to $17{\beta}$-estradiol (E2), which is one of the critical factors for bone formation. This study compares skeletal site-specific cell responses between maxilla- and mandible-derived human OBs to E2. Methods: Maxilla- and mandible-derived OBs derived from an identical donor were separately isolated from a total of five normal healthy subjects aged 18~44 years old, cultured with a treatment of 100 nM estrogen. The responses between maxilla- and mandible-derived OBs to E2 were evaluated and compared using cell proliferation, alkaline phosphatase (ALP) activity and gene expression of osteoprotegerin (OPG), ALP, insulin-like growth factor-1 (IGF-1), and estrogen receptor ${\alpha}$ ($ER{\alpha}$). Results: E2 did not have any distinct effects on the proliferation of both types of OBs. Mandible-derived OBs exhibited higher ALP activity than maxilla-derived OBs in the non-treated condition, which was common in all tested individuals. ALP activities of both types of OBs showed a minor increasing tendency with the treatment of E2, even though there was no statistical significance in some specimens. The gene expression of OB by E2 was diverse, depending on the individuals. There was increased expression of OPG, IGF-1, or $ER{\alpha}$ gene in the part of subjects, which was more repeated in maxilla-derived OBs. In particular, OPG or ALP induction by E appeared less frequently in mandible-derived OBs. Conclusion: Current results revealed that E2 affects maxilla- and mandible-derived OBs into facilitating the osteogenic process despite individual differences. Mandible-derived OBs are less sensitive to bone-forming gene expression by E2.

The Bone Formation Potency on the Titanium Cap According to the Pore on the Rabbit Calvarium (가토의 두개골에서 Pore의 유무에 따른 티타늄 반구에서의 골형성 능)

  • Purpose: This study is performed to determine the effects of titanium cap with various sizes of pores on bone formation during guided bone regeneration (GBR). Methods: Calvaria from 10 adult male rabbits were chosen as the recipient sites. A trephine bur with a diameter of 10 mm was used to form one round groove on each side of sagittal suture of the cranium, and a round bur with a diameter of 1.5 mm was used to form 6 small holes on the inner circles of round grooves to induce bleeding. In the control group, bone graft was not conducted, and closed titanium cap was fixed in the round groove. Bone graft was not performed in groups 1 and 2, but fixed on titanium caps with 0.2 mm, and 0.5 mm sized pores, respectively. For groups 3, 4, and 5, a synthetic bone graft material (${\beta}$-tricalcium phosphate, Cerasorb$^{(R)}$, Germany) was transplanted, and titanium caps without pore, with 0.2 mm and 0.5 mm sized pore were fixed, respectively. The animals were sacrificed 4 weeks after, and clinical, radiographical, and histomorphometrical evaluation of bone regeneration was performed. Results: In all groups, there were no clinical signs of infection, inflammation or wound dehiscence. Radiographic evaluation revealed well-defined semi-circular radiopacity inside the titanium cap of groups 3, 4, and 5. Histologically, the inner surface of the hemisphere was evenly lined with newly formed bone tissue, as well as grafted bone material in the group 3. In groups 4 and 5, the insertion of connective tissue was observed along the inner surface. However, the overall surface area between the grafts with different holes yielded no statistical significance in the histomorphometrical evaluation. Conclusion: Although the total area of newly formed bone showed no significant difference, excellent bone formation tendency was observed histologically when closed caps were used with bone graft was accompanied.

Effect of Electron-beam Irradiaton on the Artificial Bone Substitutes Composed of Hydroxyapatite and Tricalcium Phosphate Mixtures with Type I Collagen (수산화인회석과 인산삼칼슘 및 1형 콜라젠 혼합골의 전자빔 조사 효과)

  • Purpose: The aim of this study is to evaluate the effect and potential of electron beam (E-beam) irradiation treatment to the synthetic bony mixtures composed of hydroxyapatite (HA; Bongros$^{(R)}$, Bio@ Co., Korea) and tricalcium phosphate (${\beta}$-TCP, Sigma-Aldrich Co., USA), mixed at various ratios and of type I collagen (Rat tail, BD Biosciences Co., Sweden) as an organic matrix. Methods: We used 1.0~2.0 MeV linear accelerator and 2.0 MeV superconductive linear accelerator (power 100 KW, pressure 115 kPa, temperature $-30{\sim}120^{\circ}C$, sensor sensitivity 0.1~1.2 mV/kPa, generating power sensitivity 44.75 mV/kPa, supply voltage $5{\pm}0.25$ V) with different irradiation dose, such as 1, 30 and 60 kGy. Structural changes in this synthetic bone material were studied in vitro, by scanning electron microscopy (SEM), elementary analysis and field emission scanning electron microscope (FE-SEM), attenuated total reflection (ATR), and electron spectroscopy for chemical analysis (ESCA). Results: The large particular size of HA was changed after E-beam irradiation, to which small particle of TCP was engaged with organic collagen components in SEM findings. Conclusion: The important new in vitro data to be applicable as the substitutes of artificial bone materials in dental and medical fields will be able to be summarized.
