  • Evaluation of the methods of processing allogenic bone must be considered in order to make an effective choice of graft materials in oral surgery. Allograft materials processed by the tissue banking industry have varying capacities of bone reconstruction. The biological function of processed bone can be affected by many factors, like particle size, processing parameters, and inclusion or exclusion of mineral and moisture. For example, freeze drying step offers a safe and economical means for packaging, shipping, storage, and preservation of homologous bone. Demineralization of cortical bone using hydrochloric acid can produce a uniform demineralized surface with a capacity for osteoinduction. The objectives of this review were to evaluate the processing methods for allogenic bone and to characterize processed materials for grafting. It is important to understand the biological, biomechanical healing of different types of allografts to make the right choice for allogenic bone on each clinical application and to achieve a successful outcome for alveolar bone reconstruction in oral surgery.

  • Herpes Zoster is an acute viral infection characterized by the appearance of vesicles that occur on the skin and mucous membrane along the pathway of an involved sensory nerve. Although Herpes zoster is usually as benign viral infection, complications may occur especially when cranial nerves are involved. There are few reports of bony and dental complications by Herpes Zoster infection, all of which were isolated in a single quadrant. These include devitalized teeth, abnormal development of permanent teeth, internal resoption and spontaneous exfoliation of teeth with osteonecrosis of the alveolar bone. No agreement has been reached concerning the pathogenesis of osteonecrosis and tooth exfoliation associated with herpes zoster infection. We recently experienced series of maxillary osteonecrosis and spontaneous teeth exfoliation in patients with Herpes Zoster infection and present two cases with review of literature.

Effects of Baicalin on the Proliferation and Activity of Osteoblastic Cells (Baicalin이 조골세포의 생성 및 활성에 미치는 영향)

  • Baicalin is a flavonoid compound isolated from the medicinal plant Scutellaria baicalensis. It is known to affect multiple biological functions, including of antibacterial, anti-viral, anti-inflammatory and analgesic effects. Baicalin can inhibit nuclear factor-kappaB activation. It has been reported that some flavonoids possess the effects of bone metabolism. The present study was undertaken to determine the possible cellular mechanism of action of baicalin in osteoblasts. The effects on the osteoblast were determined by measuring cell proliferation, cell viability, alkaline phosphatase activity, and osteoprotegerin secretion. Baicalin has no effect on the osteoblastic cell proliferation and cell viability. Baicalin treatment showed increase in alkaline phosphatase activity and osteoprotegerin secretion of osteoblasts. Thus, baicalin may be a regulatory protein within the bone.

  • Bisphosphonates are compounds widely used in the treatment of various metabolic and malignant bone disease. Recently, an association between bisphosphonate use and a rare dental condition termed 'osteonecrosis of the jaw(ONJ)' has been reported. Bisphosphonate-related osteonecrosis of the jaw(BRONJ) is rare, but serious, side effect of bisphosphonate therapy in affected patients. It is characterized by poor wound healing and spontaneous intra-oral soft tissue break down, which lead to exposure of necrotic maxillary and mandibular bone. We reviewed 11 patients of BRONJ visited Ajou University Hospital Dental clinic from May 2007 to November 2008. The management of the patients included cessation of bisphosphonate therapy and various surgical restorative procedures and conservative care there after. Aggressive debridement is contraindicated. A new complication of bisphosphonate therapy administration, osteonecrosis of jaws, seems to be developing. The improved results after cessation of the medication should make clinicians reconsider the merits of the rampant use of bisphosphonates, while further investigation is needed to completely elucidate this complication.

  • The maxillary posterior area is the most challenging site for the dental implant. After missing of teeth on maxillary posterior area due to periodontal problems, the remaining alveolar ridge is usually very thin because of not only pneumatization of maxillary sinus but also destruction of alveolar bone. The maxillary sinus bone graft procedure is one of the most predictable and successful treatments for the rehabilitation of atrophic and pneumatized endentulous posterior maxilla. But, in case of severe destruction of alveolar bone due to periodontal problems, very long crown length is still remaining problem after successful sinus graft procedures. We performed vertical augmentation of maxillary posterior alveolar ridge using mandibular ramal block bone graft with simultaneous sinus graft. After this procedures, we could get more favorable crown-implant ratio of final prosthodontic appliance and more satisfactory results on biomechanics. This is a preliminary report of the vertical augmentation of maxillary posterior alveolar ridge using mandibular ramal block bone graft with simultaneous sinus graft, so requires more long-term follow up and further studies.

  • If patients complain about sudden ill-fitting occlusal sensation(acute malocclusion), a dentist has to consider many possible causes about that. Acute malocclusion is characterized its sudden onset and mainly can be caused by teeth and its related structure problem. But we always keep on mind that acute malocclusion also can be caused by temporomandibular disorders In this 3 cases of acute malocclusion, evaluating by patient history, clinical and radiographic findings, we diagnosed them as 1. Lateral pterygoid muscle spasm, 2.. Retrodiscitis, 3. Osteoarthritis. Through this non-odontogenic acute malocclusion cases, we emphasize the key of diagnosis is on comprehensive and careful patient history taking and clinical examination.

SINUS BONE GRAFT USING COMBINATION OF AUTOGENOUS BONE AND $BIOOSS^{(R)}$: COMPARISON OF HEALING ACCORDING TO THE RATIO OF AUTOGENOUS BONE (자가골과 $BioOss^{(R)}$를 이용한 상악동골이식술: 자가골 함량에 따른 치유 비교)

  • We performed sinus bone graft using some amount of autogenous bone and $BioOss^{(R)}$ and covered the sinus window with $Ossix^{(R)}$ membrane in these case series. After 4 to 6 months after operation, histopathologic examinations of trephine core biopsy showed following results. 1. There were bone density of 39.2% to 41.2% four months after operation, and we could observe the favorable early new bone formation. 2. Active bony remodeling of woven and lamellar bone was observed during 4 to 6 months healing period. 3. There were no significant differences between two groups. And also there were no significant differences between 4 months and 6 months.

Clinical Experience of Maxillary Fibrous Dysplasia: Shaving, Partial Maxillectomy & Maxillary Sinus Formation (상악골에 생긴 섬유성이형성증의 치료 경험: 안면윤곽술, 부분적 상악골 절제술과 상악동 형성)

  • Fibrous dysplasia(FD) of the bone is a slowly progressive, benign disease of unknown cause where normal architectures are replaced with fibrous and osteoid tissue. FD of the maxilla usually manifests as a bony enlargement with painless swelling and bone deformity, contouring to facial asymmetry. The lesion may involve the nasal fossae, orbits, or alveolus bone, causing diverse functional disturbance. Treatment options range from shaving to total maxillectomy and reconstruction depending on the presenting symptoms. Shaving, partial maxillectomy and maxillary sinus formation was performed in 5 patients with fibrous dysplasia in the past 2 years. Follow up period ranged from 1 month to 11 months. Aesthetic appearance, CT findings, and relief from symptoms were compared. In all patients, facial asymmetry was restored to symmetry and nasal obstructive symptoms were improved. With this procedure, expansion of the lesion will be controlled until puberty, preventing the development of new functional disturbances. After puberty, no further treatment can be anticipated due to the growth arrest inherent to the disease.

  • The therapeutic methods and follow - up prognosis of subcondylar fractures in adults have always been sources of controversy. To improve the therepeutic results in subcondylar fractures with displacement, and especially, the bicondylar ones, the auther employ the surgical reduction using of sagittal split and oblique subcondylar osteotomy. This report is illustrated by six clinical cases.

  • Osteoma is defined as a benign neoplasm of bony tissue. Osteomas are divided into two groups: central and peripheral by origin, compact and cancellous by histopathologic findings. Solitary osteoma of jaw bone, not related with Gardner syndrom, is relatively uncommon. We present two cases of huge solitary peripheral osteomas: compact type exophyting osteoma on the inferolingual side of mandibular angle and cancellous type on the residual ridge of posterior maxilla, which is extremely rare.
